Abstract

Appropriate management of difficult airway is imperative, particularly in cases with anticipated difficult airway. It customarily requires deliberate perioperative assessment. In this article we present a case of limited mouth opening along with restricted mobility of the temporomandibular joint secondary to mandibular abscess which requires surgical intervention. Preoperatively, difficult airway intubation was expected (based on LEMON mnemonic on preoperative evaluation). Therefore, Macintosh video laryngoscope and fiberoptic bronchoscope was prepared to aid in intubation. Due to the patient’s anxiety towards the procedure, the intubation was done while the patient is asleep. Preoxygenation was uneventfully. Following induction with general anesthesia, several attempts to conduct nasotracheal intubation using Macintosh video laryngoscope were made but all were abortive. This failed attempt is primarily due to the limited mouth opening and jaw mobility. Hence, fiberoptic bronchoscope-assisted nasotracheal intubation was conducted and was able to achieve airway patency. In conclusion, preoperative management holds great importance in selecting airway management strategy. To add, fiberoptic bronchoscope-assisted intubation is a safe option for intubation

Abstract

Breast cancer cell begins in the ductal epithelium and its lobes, where abnormal cell growth that is excessive, uncoordinated, and susceptible to metastasis accumulates. This study set out to find out how far early estrogen exposure influence the risk of having breast cancer in women. This systematic literature review was done by collecting sources related to the investigated variables which are menarche age, age at first birth and hormonal contraception use. We collect from variety of sources such as Google Scholar, PubMed, and Proquest to draw conclusions by grouping the odds ratio (OR) and their 95% CI. Data of the research articles, including author, year, title, and research results in the form of p-value, OR, and 95% CI, were collected from 35 articles published between 2011 and 2022 for this study. The data analysis revealed woman with menarche age at less than 12 years old have more likely to have breast cancer (range of OR 0.80 – 26.8), meanwhile woman who give birth more than 30 years old also have more likely to have breast cancer ( range of OR 1.26 – 6.47) and duration of  hormonal contraception use > 5 years were also give more chance to breast cancer development (range OR 0.51 – 9.06). This review analysis highlighted the increased probability in the incidence of breast cancer in woman related to the duration of hormonal contraception use, the age of menarche, and the age at first birth.

Abstract

Physical growth during adolescence will occur at more than 20% of total height growth, and 50% of bone mass has also been achieved during this period. Body Mass Index (BMI) to control normal body weight is achieved by adjusting body height. BMI can be used to interpret the possibility that the person is at risk of developing risk of weight-related diseases. The human feet are the most important part of supporting our body and can be correlated with body weight. The aim of this research is to determine the relationship between Body Mass Index (BMI) and the shape of the longitudinal arcus pedis angle in medical students in Surabaya. An analytic observational method with a cross-sectional research design was used with a sample size of 53 students. The results of this study conclude that there is a significant relationship between Body Mass Index and the shape of the longitudinal curve of the pedis in students (p-value: 0.009). An increased BMI will cause the pedis angle to widen which will result in the feet becoming flatter, thus affecting the body's balance when standing for a long time. Strong legs are associated with a normal Body Mass Index.

Abstract

In postmenopausal women, sexual function may worsen with increasing menopausal status. Dyspareunia is a common complaint that is experienced by women who have experienced menopause and this also causes a decrease in the quality of sexual intercourse in women who have experienced menopause. The aim of this study is to determine the relationship between dyspareunia in menopausal women and the quality of sexual intercourse. This research method uses a literature review. Literature review means reviewing various types of literature, both national journals and international journals obtained from an academic database on Google Scholar, PubMed NCBI, and Science Direct from 2013 to 2023, without language restrictions, and full-text research journals with the keywords that related between “Dyspareunia”, “Quality of Sexual Intercourse”, and “Menopause Women”. Data were obtained from twenty-two research journals that met the inclusion criteria. Results showed that overall the article supports the effect of dyspareunia in postmenopausal women on the quality of sexual intercourse. All of the 22 articles show a relationship between dyspareunia in postmenopausal women and the quality of sexual intercourse. As a conclusion, dyspareunia experienced by postmenopausal can lead to a decrease in the quality of sexual intercourse in postmenopausal women. These findings may contribute to the development of a valid association of dyspareunia in menopausal women with the quality of sexual intercourse.

Abstract

Stroke is a cerebrovascular disease that causes sudden neurological deficits due to the interruption of blood supply to the brain, resulting in the disruption of brain function. It is ranked as the second most common cause of death worldwide and leads to the highest number of disabilities globally. The incidence of this disease is 10-20 cases per 100,000 population, reaching 15% of all strokes worldwide yearly. The highest mortality rate was 20.3% after 48 hours and 18.3% less than 48 hours. This number is greater than the death rate in stroke infarction. This study aims to analyze the factors influencing mortality in patients with intracerebral hemorrhage (ICH). This type of research is observational analytic research. The design used is cross-sectional. This study was approved by the Health Ethics Committee of the Jemursari Islamic Hospital in Surabaya. The sampling technique uses total sampling. The total sample of this study was 65 patients. The sample in this study were all ICH stroke patients who were treated at RSI JS Surabaya from January 2019 to December 2021. Statistical analysis in this study used the chi-square correlation test. The results showed that the duration of hospitalization and bleeding volume had a significant correlation with the mortality in stroke patients (p-value<0.05) with OR (95% CI) are 0.243 (0.082-0.723) and 0.154 (0.048-0.500) respectively. Bleeding volume has a correlation with mortality in this study. However, this study has only fewer samples and it can’t be generalized.

Abstract

Obesity is a health problem with an increasing prevalence every year in many countries including Indonesia.  Obese patients are at risk for complications related to some non-communicable diseases. The difficulty of losing weight in obese patients is due to low awareness of a good lifestyle, appropriate dietary patterns, and discipline in taking drugs to prevent complications. However, the need for drug consumption takes a long time. The risk of side effects arising from long-term drug use needs to be considered. Using herbal plants as an additional dietary intervention could be a better choice. This review described the benefits of the butterfly pea (Clitoria ternatea) flower as antiobesity. This study is a literature review by searching preceding published articles in Pubmed, ScienceDirect, Cochrane, and Google Scholar journals. The keywords used are (Clitoria ternatea) OR (Bunga Telang) OR (Butterfly pea) AND (Obesity) OR (Antiobesity). Articles are limited by publication period from 2012 to 2022 and then selected based on the criteria. This literature review concluded that the administration of Clitoria ternatea extracts (CTE) can become an alternative antiobesity by inhibiting weight gain, increasing adipose lipolysis, and decreasing the expression of adipogenic and lipogenic proteins.

Abstract

Occupational risk factors have a significant effect on osteoarthritis (OA). It has been studied before. However, no one has looked into occupational risk for the prevalence of hip OA in agricultural communities of Jember. This study aims to determine the relationship between risk factors (gender, age, body mass index, work history, work posture, length of work, and length of work) and hip OA incidence. This analytical observational study uses a cross-sectional design at Doctor Soebandi Jember's Regional Hospital and Jember Clinic Hospital, East Java-Indonesia. This study included all hip OA patients who visited the orthopedic clinic from January 2020 to December 2021. Thirty-six patients and 18 samples had hip OA (50%). Of eighteen subjects with hip OA, 14 were female (77.8%), 12 were aged ≥55 years old (66.7%), 12 were pre-obese with BMI >25 (66.7%), 14 had a history of lifting heavy weights (77.8%), 16 related to bending working posture (88.9%), 13 had a long working year >10 years (72.7%), 11 had a long working duration >44 hours (61.1%), 9 worked as a housewife and farmers (50%). %). A significant relationship of hip OA was observed with gender (p-value 0.003), age (p-value 0.019), work posture (p-value 0.002), length of work (p-value 0.003), and duration of work (p-value 0.018) based on contingency coefficient statistical analysis.  We can conclude that the significant risk factors related to hip osteoarthritis in agricultural areas are gender, age, work posture, work length, and work duration.

Abstract

Hyperlipidemia is one of the main risk factors for heart disease caused by the formation of atherosclerosis. Eucheuma spinosum contains a natural antioxidant flavonoid, which has an antiatherogenic effect that can inhibit the formation of cholesterol in the liver so that it can prevent the development of atherosclerosis. This study aims to determine the effect of prophylactic administration of Eucheuma spinosum on the total cholesterol levels of Rattus novergicus induced by dexamethasone. This study was an experimental design with a post-test-only control group design. This study would use 24 white male Rattus norvegicus divided into three groups; a negative control group (K-) without treatment, a positive control group (K+) induced dexamethasone 8 mg/Kg BW on day 14 until day 20, treatment group (KP) given Eucheuma spinosum extract 200 mg/Kg BW on day 8 until day 20 as Prophylactic and on day 14 until day 20 induced dexamethasone 8 mg/Kg BW. After completing the treatment period, all groups were terminated and taken total calculation of cholesterol levels through the aorta of rats. Data was analyzed statistically computerized. The results of the study of total cholesterol levels that used Kruskal-Wallis test showed differences between groups (p<0.001). There was a decrease in total cholesterol levels in the treatment group (p<0.024) compared with the positive control group. The administration of Eucheuma spinosum extract as a prophylactic dose of 200 mg/Kg BW for 14 days could reduce the total cholesterol level of Rattus norvegicus induced by dexamethasone compared to those not given Eucheuma spinosum extract.

Abstract

One of the causes of pediatric eye vision problems in the pandemic era is the increased use of digital screen devices to play online games. This is due to an increase in accommodation and convergence in the eyes when doing activities through digital screen devices at close range. The purpose of this study was to determine the relationship between screen time duration and visual acuity in children aged 9-12 years in the Kalongan and Sidokerto Malang areas during the Pandemic era. This research is a descriptive observational quantitative research with a cross-sectional research design. The research sample was 62 children with a saturated sampling method or a number of population members. The research was conducted in the community of school-age children in the Kalongan and Sidokerto Malang areas. The data used in this study were primary data giving questionnaires to children through parents and checking children's visual acuity. The analysis method used in this research is a univariate and bivariate analysis using the chi-square test.  Based on the results of the study, the majority of respondents play online games. Some respondents experienced decreased visual acuity. There is a correlation between the duration of playing online games and the visual acuity of the right and left eyes. Children who play online games for longer periods of time (more than 2 hours) are more likely to experience decreased visual acuity than children who play online games for less than 2 hours or do not play online games.
