Abstract

Latar belakang: Ulkus kaki diabetik (UKD) adalah salah satu komplikasi terbesar dari DM yang mengalami ulserasi pada eskremitas bawah yang berhubungan dengan abnormalitas neurologis, kelainan vaskuler atau peripheral artery disease (PAD). Penggunaan electrical muscle stimulation (EMS) dapat meningkatan aliran darah dan meningkatkan proses penyembuhan luka. Tujuan: Tujuan penelitian ini untuk mengidentifikasi efek dari electrical muscle stimulation (EMS) pada otot betis terhadap peningkatan nilai Ankle Brachial Index (ABI)  pada pasien Ulkus Kaki Diabetik. Metode Penelitian: Penelitian ini akan menggunakan desain true-experiment dengan randomised controlled trials. Total sampel yang terlibat 60 responden yang akan dibagi dalam kelompok pada EMS terapi dengan menggunakan alat veinoplus arterial selama 30 menit (kelompok Intervensi) dan EMS terapi dengan menggunakan alat veinoplus arterial selama 20 menit (kelompok kontrol). Penelitian ini menggunakan alat pengkajian penyembuhan luka dengan skore MUNGS (maceration, undermining/ tunnelling, necrotic tissue, granulation, signs or symptoms). Bivariat analisis yang  digunakan adalah dengan menggunakan Paired T Test. Hasil: Electrical Stimulation (EMS) pada otot betis dengan menggunakan alat Veinoplus terjadi peningkatan nilai ABI yang signifikan pada pasien dengan ulkus kaki diabetik di Klinik Kitamura Pontianak. Kesimpulan: Terdapat perbedaan yang signifikan nilai ABI pada kelompok intervensi dan kontrol sebelum dan setelah tindakan terapi EMS pada pasien UKD. Tidak terdapat perbedaan yang signifikan selisih nilai rerata pada kelompok kontrol dan intervensi.

Abstract

  Abstract: Diabetic Foot ulcer (UKD) is a chronic complication of Diabetes Mellitus with incidence rate of 15%, to increase the wound healing process one of the methods that can be done is to use Electrical Muscle Stimulation (EMS) therapy which is a procedure by producing short electric waves aimed to imitate the movement normal body and stimulates wound healing. The aim of this study was to identify the effect of EMS on calf muscle on wound healing in UKD patients. This research uses pure experimental design (true-experiment) with randomized controlled trials (RCT). The total sample involved 60 respondents divided into two groups respectively 30 respondents. Analysis of data by using Wilxocon Test analysis on mean comparison of Ankle Bracial Index (ABI) and Dependent T-test analysis on mean comparison of MUNGS score. Multivariate data analysis used was MANOVA with GLM-RM method on repeated measurement of dependent variable. EMS therapy on calf muscle using Veinoplus Arterial tool obtained P value 0.00 <0.05, which means there is significant difference between before and after treatment (control and intervention) to ABI value and MUNGS score. Multivariate test results were obtained by p value 0.00 <0.05, which means that there are significant differences between multiple measurements of ABI and MUNGS scores, but the plot graph shows no optimal time in EMS therapy. The researchers advise that EMS therapy can be applied as one of the management of UKD patients. Further research needs to tighten the inclusion criteria of respondents with ABI <0.9, and add time> 7 days to be able to assess the optimum time of EMS therapy. Keywords: Diabetic foot ulcers, electrical muscle stimulation, ankle brachial index, wound healing   Abstrak: insiden 15%, untuk mempercepat proses penyembuhan luka salah satu metode yang dapat dilakukan adalah menggunkan terapi Electrical Muscle Stimulation (EMS) yang merupakan prosedur dengan menghasilkan gelombang elektrik yang pendek bertujuan meniru pergerakan tubuh yang normal dan menstimulasi penyembuhan luka. Tujuan penelitian ini untuk mengidentifikasi efek dari EMS pada otot betis terhadap penyembuhan luka pada pasien UKD. Penelitian ini menggunakan desain Eksperimen murni (true-experiment) dengan randomised controlled trials (RCT). Total sampel yang terlibat 60 responden dibagi dalam dua kelompok masing-masing 30 responden. Analsis data dengan menggunakan analisis Wilxocon Test pada perbandingan rerata nilai Ankle Bracial Index (ABI) dan analisi T-tes Dependent pada perbandingan rerata Skor MUNGS. Analisis data multivariat yang digunakan adalah MANOVA dengan metode GLM-RM pada pengukuran berulang variabel dependen. Terapi EMS pada otot betis dengan menggunakan alat Veinoplus Arterial diperoleh hasil P value 0.00 < 0.05, yang artinya terdapat perbedaan yang signifikan antara sebelum dan sesudah perlakuan (kontrol dan intervensi) terhadap nilai ABI dan Skor MUNGS. Hasil uji multivariat di dapatkan p value 0.00 < 0.05, yang artinya terdapat perbedaan yang signifikan antara beberapa kali pengukuran terhadap nilai ABI dan Skor MUNGS, namun pada grafik plot menunjukkan belum ditemukan waktu yang optimal dalam pemberian terapi EMS. Peneliti memberikan saran agar terapi EMS dapat diterapkan sebagai salah satu penatalaksanaan pasien UKD. Penelitian lebih lanjut perlu memperketat kriteria inklusi responden dengan ABI < 0.9, serta menambahkan waktu > 7 hari untuk dapat menilai waktu optimum terapi EMS. Kata kunci: Ulkus kaki diabetik, electrical muscle stimulation, ankle brachial index, penyembuhan luka.

Abstract

Abstract Diabetic Foot Ulcer is one of the biggest complications of Diabetes Mellitus who experience ulceration of the lower extremities associated with neurological abnormalities, vascular abnormalities or peripheral artery disease (PAD). The use of Electrical Muscle Stimulation (EMS) can increase blood flow and improve the process of wound healing. The purpose of this study was to identify the effect of electrical muscle stimulation (EMS) on calf muscles on increasing the value of the Ankle Brachial Index (ABI) in diabetic foot ulcer patients. This study used a true-experiment design with randomized controlled trials. The total sample involved 60 respondents which were divided into groups on EMS therapy using arterial veinoplus devices for 30 minutes (Intervention Group) and EMS therapy using arterial veinoplus devices for 20 minutes (Control group). Bivariate analysis used is to use Paired T Test. Electrical stimulation (EMS) in the calf muscles using the Veinoplus tool showed a significant increase in ABI values ??in patients with diabetic foot ulcers at the Kitamura Clinic in Pontianak. Abstract

Abstract Congestive Heart Failure (CHF) is a chronic disease that causes a pathological condition, because heart cannot pump enough blood to sucifient the body's metabolic needs, the cause of this is impaired cardiac contractility in systolic dysfunction and cardiac  (diastolic). Home based exercise walking is one of the interventions that can be given to control blood pressure values ??and increase the adaptation of CHF patients to activities. This study aims to determine the effect of home base exercise walking on blood pressure of patients with CHF in Dr. Soedarso Pontianak Hospital. The research method uses the quasi experimental design and the non-randomized pretest-posttest approach without control method. The sampling technique used was consecutive sampling with 15 respondents. The place Research at  Dr. Soedarso Pontianak hospital, on period November 2018-January 2019. Analysis of the data used is Wilxocon test because data not normally distributed. The results showed a decrease in systolic blood pressure values ??before and after the intervention with (p-value 0.01) and no decrease diastolic (p-value 0.29). Conclusion there is influence of home based exercise walking on the value of CHF blood pressure in Dr.Soedarso Pontianak Hospital. Home based exercise walking can be an alternative for CHF sufferers to increase the functional capacity of the heart. Keyword: Congestif Heart Failure, Home Based Exercise Walking and Blood Presure       Abstrak Congestif Heart Failure (CHF) merupakan penyakit kronis yang menyebabkan kondisi patologis yaitu jantung tidak dapat memompakan darah yang cukup untuk memenuhi kebutuhan metabolik tubuh, penyebab hal ini adalah adanya gangguan kontraktilitas jantung disfungsi sistolik maupun pengisian jantung (diastolik). Kondisi tersebut tentu akan mempengaruhi tekanan darah di dalam tubuh penderita. Home based exercise walking merupakan salah intervensi yang dapat diberikan guna mengontrol nilai tekanan darah dan meningkatkan adaptasi pasien CHF terhadap aktifitas. Penelitian ini bertujuan untuk Mengetahui pengaruh home base exercise walking terhadap tekanan darah pasien dengan CHF di RSUD Dr. Soedarso Pontianak. Metode penelitian menggunakan desain metode quasi eksperimental design melalui pendekatan pretest-posttest non randomized without control. Tekhnik pengambilan sampel yang digunakan adalah  consecutive sampling dengan 15 responden. Tempat penelitian di RSUD Dr. Soedarso Pontianak, waktu penelitian November 2018-Januari 2019. Analisa data yang digunakan adalah wilxocon test data tidak berdistribusi normal. Hasil penelitian menunjukkan terjadi penurunan nilai tekanan darah sistolik sebelum dan setelah diberikan intervensi (p-value 0,01) dan tidak terjadi penurunan nilai diastolik (p-value 0,29). Kesimpulan ada pengaruh home based exercise walking terhadap nilai tekanan darah penderita CHF di RSUD Dr.Soedarso Pontianak. Home based exercise walking dapat menjadi salah satu alternatif bagi penderita CHF guna meningkatkan kapasitas fungsional jantung. Abstract

One of the factors that affect wound healing is maceration which can increase wound healing time. However, the results of observations revealed that many maceration wounds were treated with a washing process and an inappropriate selection of dressings. This community service was carried out by transferring knowledge and skills to wound specialist nurses to detect early maceration using flir one smartphone thermography. Based on the results of the evaluation, the nurse can explain the definition and signs of maceration and can demonstrate using a maceration detection tool in the wound with appropriate and correct steps.

Abstract

Diabetes Mellitus (DM) is a non-communicable disease whose prevalence is increasing every year in the world, including in Indonesia. DM patients are very susceptible to ulcers caused by uncontrolled blood sugar due to the wrong diet. The low level of knowledge related to eating patterns is a problem that must be addressed so that the incidence can be suppressed. In this case, education is needed to increase the understanding of patients and families in terms of eating patterns. The right diet or eating pattern will certainly be one aspect in improving the quality of life of type 2 DM patients. The purpose of this activity is to increase knowledge, especially in the right diet in type 2 DM patients, it is hoped that it can improve the quality of life of type 2 DM patients by controlling blood sugar due to a good diet. The method used is to provide counseling with lecture, discussion and question and answer methods. The targets of this community service program are patients suffering from type 2 DM and their families in Sungai Ambangah village, Kubu Raya district. The results of this activity indicate that the effectiveness of counseling has an effect on increasing knowledge of type 2 DM patients and their families. This can be seen from the evaluation results that people with type 2 DM and their families understand the right diet for people with DM.

Abstract

The number of diabetic foot ulcer (DFU) sufferers increases from year to year. Many studies have been conducted to examine the appropriate therapy to overcome DFU. One type of therapy that has been widely studied is related to the use of herbal products in treating DFU. So that currently there are many scientific articles that inform about the efficacy of herbal products in accelerating wound healing both acute and chronic. The activity aimed to socialize the use of herbal products in the treatment of foot ulceration. This activity was carried out in February 2022 in collaboration with the Public Health Center of Sungai Raya District as a health service that oversees the location of the activity. The target population in this activity are those who have a history of diabetes mellitus or diabetic foot ulcers or have a family with diabetic foot ulcers. Participants who participated were given health education about the management of diabetic foot wounds and the use of herbal products in the treatment of diabetic foot wounds.

Abstract

There is an increase in diabetic complications every year, therefore prevention is prioritized before complications such as diabetic foot ulcers occur. Many studies have been conducted related to the prevention of diabetic foot ulcers with Buerger Allen exercise which has a significant impact on improving peripheral circulation. The activity aimed to socialize the prevention of diabetic foot ulcers with Buerger Allen exercise. This activity was carried out in March 2021 in collaboration with the Public Health Center of Sungai Raya District as a health service that oversees the location of the activity. The target population in this activity are those who have a history of diabetes mellitus or diabetic foot ulcers or have a family with diabetic foot ulcers. The participants were given knowledge about preventing diabetic foot ulcers by means of the Buerger Allen exercise.
