Abstract

Background: One components of the quality of health care which is given by hospitals is the availability of data or information from the medical record. According to Permenkes 269 of 2008 concerning medical records, recording of patient data is a compulsion, and a legal obligation. Methods: This research is a non-experimental descriptive study. The design of the study uses a cross-sectional. This research was conducted in the inpatient medical record installation of RSUD DR Moewardi Surakarta. The research’s method is a combined method approach (mixed methods) that combines qualitative and quantitative approaches. A qualitative approach is as the primary while the quantitative approach plays as a support. Samples were taken in purposive sampling, is the five informants as medical records executant. Data collected through in-depth interviews, observation, and analysis of documents. Qualitative data analysis is done through a tangle analysis and interactive analysis, while quantitative data were collected and analyzed by using a check list to determine the output quality of medical records. Results: The associated factors of the medical records quality of inpatients in RSUD DR Moewardi are the input, processes and the output of medical records. Input include energy, funds, materials, machines, methods, socialization, time and information. Process include the implementation process when the medical records in medical services and medical records at the time of file handling, namely the processing, storage, lending, and the return of the medical record file. Output include stuffing completeness, accuracy, timely refund, and compliance aspects of the legal requirements. The result of the total quality of medical records was 93.05%. The qualities of the medical records are categorized as good. Conclusion: The associated factors of the medical records quality of inpatients in RSUD DR Moewardi are the input factors, process factors and the output factors. Input factors are workforce factors, fund factors, material factors, engineering factors, methods factors, socialization factors, the time factor, information factors. Process factors include the implementation process when the medical records in medical services and medical records at the time of file handling, namely the processing, storage, lending, and the return of the medical record file. Output factors include stuffing completeness, accuracy, timely refund, and compliance aspects of the legal requirements. Keywords: medical records, medical records quality.

Abstract

Mahasiswa kedokteran saat ini telah didominasi oleh Generasi Milenial ( Generasi Y) yang lahir antara tahun 1982-2000. Generasi tersebut telah terpapar teknologi sejak usia dini, salah satunya adalah jejaring sosial yang termasuk Web 2.0. Dilakukan penelitian observasional deskriptif dengan sampel 46 mahasiswa kedokteran yang mengisi kuesioner online di situs http://elearning.fk.uns.ac.id; 33% pria dan 67% wanita, 43% berusia 18 tahun. Sejumlah 57% responden mulai mengenal internet saat SD. Semua responden (100%) memiliki akun di berbagai situs jejaring sosial terutama Facebook® (46 orang) dan Twitter® (43 orang). Sebanyak 67% responden mengakses jejaring sosial hampir setiap hari, 57% sejak usia SD. Mereka mengaksesnya melalui laptop (38 orang), ponsel (34 orang), tablet (15 orang) dan melalui PC (5 orang). Sebanyak 78% responden setuju bahwa jejaring sosial dapat membantu mereka dalam belajar, 17% ragu-ragu dan hanya 4% yang tidak setuju. Disimpulkan bahwa jejaring sosial mempunyai potensi sebagai media pembelajaran bagi mahasiswa kedokteran generasi Y.Medical students have been dominated by the millennial generation known as Y Generation. This generation was born between 1982-2000 and has been exposed to technology since early age, including social network in web 2.0. This research was descriptive observational study on 46 medical students, 33% male and 67% female, 43% in the age of 18 years. They filled online questionnaire at http://elearning.fk.uns.ac.id. All respondents have access to social networks, 57% since in elementary school. All have accounts in various social network sites, especially Facebook® (46 respondents) and Twitter® (43). Accessing is almost every day (67%), using laptops (38), mobile phones (34), tablet (15) and personal computer (5). 78% respondents agreed that social network could help in learning, 17% undecided, and 4% disagree. Social networks is potentially used as a learning medium among medical students of Y generation.

Abstract

Latar belakang: Pemanfaatan virtual reality (VR) di bidang kedokteran, khususnya sebagai wahana simulator bedah, psikoterapi, hingga terapi non-farmakologi sudah menunjukkan hasil keluaran yang baik. Google cardboard (GC) merupakan teknologi VR berbasis smartphone Android yang baru dan relatif terjangkau dibanding simulator VR konvensional. Penelitian ini bertujuan untuk mengetahui potensi penerapan teknologi VR berbasis smartphone tersebut bagi pendidikan kedokteran untuk mencapai kompetensi di tengah keterbatasan wahana pada mahasiswa kedokteran. Metode: Penelitian observasional deskriptif dengan kuesioner online pada 110 responden mahasiswa preklinik Fakultas Kedokteran Universitas Sebelas Maret (UNS). Hasil: Didapatkan 88% responden menggunakan smartphone Android, 10% iOS Apple, dan 2% Blackberry OS. Versi Android yang digunakan 43% Kitkat, 30% Jellybean, 17% Lollypop, 6% Ice Cream Sandwich, dan 4% Gingerbread. Berdasarkan ukuran layar, sensor Gyroscope dan sensor magnetometer, hanya 24% smartphone respoden yang kompatibel menjalankan VR. Sejumlah 100% responden mengaku terbantu dengan smartphone Android dalam aktivitas belajar meliputi membaca jurnal/ebook (92 orang), menonton video (50 orang),menggunakan aplikasi kedokteran (41 orang), berkomunikasi lewat instant messaging (79 orang), dan mencari informasi di media sosial (62 orang). Simpulan: Berdasarkan data penggunaan smartphone, teknologi VR berbasis smartphone seperti GC tampaknya memiliki potensi sebagai media belajar mahasiswa kedokteran.

Abstract

Rekam Medis Elektronik (RME) berbasis cloud telah berkembang di Indonesia. Layanan tersebut memiliki berbagai keuntungan seperti biaya lebih rendah, fitur lebih mudah digunakan, potensi data sharing antar layanan kesehatan, serta mempermudah pengambilan keputusan klinis. Meskipun demikian, implementasi RME berbasis cloud berpotensi menimbulkan masalah etika dan hukum seperti masalah keamanan data dan jaringan, kepastian penyedia layanan cloud, prosedur data sharing, dan rahasia medis. Indonesia belum memiliki regulasi spesifik mengenai RME maupun RME berbasis cloud. Regulasi diperlukan agar implementasinya tidak bertentangan dengan etika dan hukum yang berlaku di Indonesia.Cloud based-Electronic Medical Record (Cloud based-EMR) has been developing in Indonesia. These services have some benefits such as lower costs, more user-friendly features, potential for data sharing, and support to clinical decision-making. Nevertheless, the implementation has implicationsin ethical and legal issues such as data and network security, cloud service provider, data sharing procedures, and medical privacy. Indonesia has no specific regulation on EMR or cloud-based EMR. EMR and cloud based-EMR regulation are needed so that its implementation does not contradict with Indonesian ethics and laws.

Abstract

Background: Vaccination is very important because it can help prevent diseases that can be prevented by immunization (PD3I) such as polio, tetanus, diphtheria, measles, mumps, and rubella. During the Coronavirus Disease -19 (COVID-19) pandemic, the role of vaccination is very crucial, especially to reach herd immunity. However, there are many problems related to the implementation of vaccination, including needle phobia, which is a phobia of needles because it is considered painful. Virtual reality is a technology that develops in the form of a virtual image that can be used in vaccination services. This study aims to determine the potency of virtual reality to solve the needle phobia in vaccination services.Methods: The method used in this study is a literature review with sources from journals in the Scopus, ProQuest, and ScienceDirect database that was published in 2000 until 2020, and also books and articles with the keywords "virtual reality" "AND" "immunization," virtual reality " AND " vaccination " , "Needle phobia" "AND" "vaccination". This research was conducted from January to February 2021.Results: Based on the results obtained, virtual reality can be used in vaccination services, especially in the treatment of needle phobia. Virtual reality can become a distraction so that the patient will be distracted from fear and can lower the pain threshold. In addition, virtual reality can also be used as a means of education about immunization.Conclusion: Product development and clinical research need to be developed to assess the use of virtual reality in vaccination services during the COVID-19 pandemic. The use of virtual reality for other medical procedures that use needles can also be further investigated

Abstract

Background: Personalized medicine is one of the health treatment approaches that has developed in the last ten years. This treatment is specific because it considers the genomics of patients and requires a complete database or medical record covering clinical and genomic data. Nevertheless, there are ethical and legal challenges in the implementation of electronic medical records in personalized medicine.Objective: This study aimed  to determine the ethical and legal challenges in the use of electronic medical records in personalized medicine based on previous literature.Methods: The method used in this study was literature review with sources from journals, books, and articles on electronic medical records, personalized medicine, as well as legal and ethical aspects related both domestically and abroad. Results: Ethical and legal challenges that can occurred in the use of electronic medical records in the personalized medicine era include security and ownership of data, legal responsibility, genomic discrimination, and changes in the relationship between doctors and patients.Conclusion: Indonesia does not yet have specific regulations regarding data security, data ownership, and data sharing in the health sector. Further regulations regarding the use of electronic medical records in the personalized era are needed so that their implementation does not conflict with the ethics and laws that apply in Indonesia

Abstract

Latar Belakang: Kebijakan Study from Home (SfH) dan Work from Home (WfH) selama pandemi Coronavirus Disease-19 (Covid-19) berdampak pada proses pendidikan mahasiswa kedokteran, salah satunya dalam penyelenggaraan evaluasi keterampilan klinis atau Objective Structured Clinical Examination (OSCE). Diperlukan sebuah model OSCE berbasis dalam jaringan (daring) agar uji kompetensi tetap berjalan dengan optimal.Tujuan: Mengembangkan model OSCE berbasis daring yang disesuaikan pada kondisi pandemi Covid-19.Metode: Metode observasional deskriptif digunakan melalui dua tahapan. Tahapan pertama meliputi proses pelaksanaan OSCE berbasis daring. Tahap kedua adalah evaluasi pelaksanaan melalui survei kuesioner online kepada 149 mahasiswa Fakultas Kedokteran Universitas Wahid Hasyim yang mengikuti OSCE berbasis daring  pada periode Agustus 2020.Hasil: Proses pelaksanaan OSCE berbasis daring meliputi tahap pra-OSCE, OSCE, dan pasca-OSCE. Pada evaluasi selama  pra-OSCE, 79,5% mahasiswa menyebutkan bahwa sarana telah dipersiapkan dengan baik dan 46,1% menyebutkan bahwa informasi yang disampaikan saat technical meeting jelas. Pada tahap pelaksanaan,  91,3% mahasiswa  menyebutkan soal OSCE jelas, 65,9% memiliki koneksi internet dan komunikasi berjalan lancar, 53% menyebutkan dukungan technical support baik, dan 88% menyatakan waktu pengerjaan cukup.Kesimpulan: Pelaksanaan OSCE berbasis daring dapat diimplementasikan. Meskipun demikian, dukungan teknis dan persiapan yang matang diperlukan sehingga dalam pelaksanaannya tetap berlangsung secara efektif.

Abstract

The contact tracing policy strategy implemented during the COVID-19 pandemic requires data on COVID-19 patients to be accessible to the public so that people who have been in contact with the positive patient can do self-isolate. However, this is contrary to medical secrets and the confidentiality of COVID-19 patient data as a form of private law. This research is normative research with a statutory approach (statue approach) and conceptual (conceptual approach). Based on ethical and legal aspects, the confidentiality of COVID-19 patient data must be maintained. This data can be disclosed with the patient consent or disclosed to the Health Office to conduct epidemiological investigations. Suppose the contact investigation involves several parties: the neighborhood unit, the village head, Village Community Empowerment Service, police, and other parties. In that case, further regulation is needed to ensure the commitment of these parties in maintaining COVID-19 patient data. Disseminating patient data openly or publicly can make harm to the patient, especially in the not good environment with risk of stigmatization. There is also a need for harmony between existing regulations and the programs or policies implemented by the government.

Abstract

Online medical consultation or often known as teleconsultation is part of telemedicine. The technology began to develop in Indonesia in line with the development of information technology. Flexibility of time, lower consultation costs, wide coverage, and improved health services are the benefits gained from implementing the technology. Nevertheless, the implementation of online medical consultation has potency to create ethical and legal issues in Indonesia such as legality of healthcare, legality of practice licenses, therapeutic transactions, and the issue of confidentiality and security of patient data. Indonesia does not yet have a specific regulation on the implementation of online doctor consultations. Regulations are required for the implementation of online medical consultation so that not to conflict with ethics and law. In addition, information technology in the health sector should be developed to support the implementation of online medical consultation services.
