Abstract

Objective: This study proves differentiation of the combination of dutasteride and green tea, dutasteride, green tea, and placebo alone and their association with differences in hematocrit levels and the expression of hypoxia induced factor-1 alpha (HIF-1α) in patients with Benign Prostate Hyperplasia (BPH) were performed Trans urethral resection of the prostate (TURP) surgery. Material & method: Experimental study with the draft "randomized control trial". Comparing angiogenesis changes between groups of BPH patients who underwent TURP surgery to assess the expression of HIF-1α and Δ Ht (Hematocrit) after administration of dutasteride, green tea with combination of dutasteride and green tea for 14 days. Results: The combination of dutasteride and green tea was not significant in reducing the expression of HIF-1α. Mean P1 group (59.32 ± 14.69); P2 group (59.11 ± 20.73); P3 group (64.21 ± 14.95); K group (58.16 ± 16.00). Kruskal test results obtained p=0.491 walis which means the difference percentage of HIF-1α among the 4 groups was not significant. The mean Δ Ht P1 group (0.61 ± 0.204); P2 group (0.54 ± 0.250); P3 group (0.41 ± 0.275); group K (0.41 ± 0.275). In statistical test with Mann Whitney test comparing the percentage reduction obtained Ht levels dutasteride group against group of green tea obtained p=0.213 means that there is no significant difference. Where a significant difference to the other groups. Conclusion: The combination of dutasteride and green tea for 14 days before TURP surgery does not reduce the expression of HIF-1α in BPH patients who underwent TURP surgery. Δ Ht significant decline in the combination group compared with other groups and might be influenced by several factors during TURP surgery.

Abstract

Swallowing irritating agent can cause chemical burn, which can develop complications such as stricture. Correctional procedures may cause further tissue damage with additional loss of function. Colon interposition is a therapeutic option for severe stricture cases.Menelan agen iritatif mengandung bahan kimia seperti asam atau basa dapat menyebabkan luka bakar kimiawi, pada beberapa kasus menyebabkan komplikasi striktur ringan hingga berat. Beberapa prosedur koreksi esophagus dapat menyebabkan kerusakan jaringan lebih lanjut dengan kehilangan fungsi. Interposisi colon merupakan pilihan terapi kasus striktur berat.

Abstract

Background: Inguinal hernias are one of the most often cases in surgery. Inguinal hernias are usually found in males rather than females. There are two types of meshes in inguinal hernia repairs: heavyweight and lightweight mesh. There were still a lot of differences in the outcome of each mesh that was used. Therefore, with various postoperative pain results from past research, the author expresses an interest to research this topic for geriatric patients.Methods: In this cross-sectional comparative study, 45 patients using heavyweight mesh and 45 patients using lightweight mesh herniorrhaphy with Lichtenstein technique in inguinal hernias at RS Siloam Karawaci Lippo Village and RSU were reviewed retrospectively. Patient data were taken from the medical records using the purposive sampling method. Early Postoperative pain from day 1 until 7 was classified following the Visual Analogue Scale (VAS). Incidence of postoperative infection and seroma were assessed in both groups. Collected samples will be processed using Pearson's Chi-Squared test.Result: This study shows that there is a significant difference between the use of lightweight mesh in reducing moderate early postoperative pain for men above the age of 60 years old (p = 0.025) rather than the use of heavyweight mesh. Incidence of other postoperative complications such as infection and seroma have no statistical difference between both groups.Conclusions: The use of lightweight mesh has the advantage of reducing early postoperative pain from herniorrhaphy using the Lichtenstein technique rather than heavyweight mesh in men above 60 years old.

Abstract

Introduction: Colorectal cancer is one of the most common cancer found in the gastrointestinal tract. This tumor is less commonly found on the descending part of colon and about 15 - 30 % can progress to obstruct the passage of the colon and invade other organs and may cause perforation which may show sign of peritonitis. The majority of perforation cases occur in patients with acute obstruction, only in a rare setting of colonic cancer cases due to penetration of the tumor mass through the intestinal wall. Clinical Presentation: A 34-year-old male came with pain on his whole abdomen area for 1 day and keeps on getting worse. On clinical examination There were sign and symptoms of peritonitis and during emergency laparotomy, we found perforation on descending colon caused by tumor infiltration and performed Left Hemicolectomy and Stoma at emergency settings.Conclusion: Perforation of descending colon caused by tumor infiltration is a rare case, Left hemicolectomy and stoma shows good results in emergency settings.

Abstract

Introduction: Pancreatic injury is a rare case, caused by blunt or sharp trauma. Difficulty in making diagnose on pancreatic trauma cases are associated with high mortality, and the treatment can be either operative or conservatively. However, It is still unclear which treatment is more favorable.Case: We present 2 cases of Grade III pancreatic injury with stable hemodynamic who suffered bicycle accident. First case, 12-year-old boy complaining severe pain on the upper left abdomen (VAS 9-10) and get worsening by time, with vomiting. The patient underwent distal Pancreatectomy-Splenectomy. Second case, 8-year-old boy complaining of pain on the upper left abdomen (VAS 6-7) without extension on whole abdominal region with vomiting and fever. The patient was treated conservatively. In both cases, patient was discharged with improvement. However, about 3 months later patients who were treated conservatively developed into a pseudocyst.Conclusion: The selection of management in grade III pancreatic injury can be operative or conservative depending on clinical findings such as hemodynamic condition and the quality of abdominal pain. But the occurrence of pseudocysts pancreas is another surgical challenge.

Abstract

The background of this research is that there is intense competition in the mobile industry and the still limited research examining brand displacement. This research was conducted on 110 mobile phone users in Purwoketo. Analysis tools used in this study is Stuctural Equation Modelling (SEM). The results of this study is the variable quality of features, design and advertising have a positive influence on satisfaction. Variables positively influence satisfaction of the needs for information and consumer loyalty. Variable needs to find information in a positive impact on customer loyalty.
