Abstract

Sebagai badan otonom Ikatan Dokter Indonesia (IDI) yang bertanggung jawab dalam penerapan etika kedokteran, ketertiban administratif adalah sangat penting bagi Majelis Kehormatan Etik Kedokteran (MKEK), terutama karena banyak berurusan dengan masalah sensitif dan rahasia yang menyangkut hajat hidup teman sejawat dan kewibawaan MKEK maupun IDI. Oleh karena itu, MKEK sudah sepatutnya memiliki aturan tentang pengelolaan surat menyurat untuk melancarkan komunikasi dan koordinasi dengan berbagai pihak dan agar tertib secara administratif. Setiap MKEK wilayah/cabang hendaknya memiliki aturan internal tentang pengelolaan surat menyurat, yang dapat dibuat dengan merujuk ke Standar Prosedur Operasional yang telah dibuat oleh MKEK Pusat.

Abstract

Perawatan pasien yang melibatkan banyak disiplin ilmu dan perjumpaan dokter-pasien yang terpisah-pisah berpotensi menimbulkan perbedaan pendekatan dan rekomendasi antardokter. Kondisi ini dapat menyebabkan kebingungan pasien dan tumpang tindih dalam tatalaksana pasien. Prinsip layanan medis mencakup patient-centered care dan layanan berkesinambungan (continuity of care). Ketimpangan prinsip etik dapat dijumpai dalam layanan medis multidisiplin meliputi prinsip etik beneficence, non-maleficence, autonomy, dan justice. Tulisan ini bertujuan untuk mengingatkan kembali tujuan dasar pelayanan medis multidisiplin, serta potensi ketimpangan dalam praktiknya.

Abstract

Sebagai profesi yang menjunjung tinggi hubungan luhur dengan pasien, dokter wajib menjunjung tinggi nilai-nilai etik yang terkandung dalam Kode Etik Kedokteran Indonesia (KODEKI) tahun 2012. Pelanggaran etik kedokteran perlu disikapi dengan pemberian sanksi yang sesuai, yang bertujuan sebagai pembinaan terhadap teman sejawat. Prinsip, tujuan, dan ketentuan pemberian sanksi tersebut diatur oleh Majelis Kehormatan Etik Kedokteran PB IDI.

Abstract

Sejak awal tahun 2020, dunia telah dikejutkan dengan pandemi COVID-19. COVID-19 telah menyebabkan perubahan yang fundamental terhadap segala segi kehidupan, termasuk dalam pendidikan kedokteran. Apabila kita menilik sejarah pendidikan kedokteran di Indonesia, peristiwa wabah juga menjadi tonggak dimulainya pendidikan kedokteran di Indonesia. Di dunia, pandemi flu Spanyol juga telah mengubah pendidikan kedokteran di kala itu. Dengan kemajuan teknologi, pendidikan kedokteran di masa pandemi ini dilaksanakan dalam bentuk pendidikan jarak jauh (PJJ). Meskipun demikian, tidak dipungkiri dampaknya terhadap pemenuhan kompetensi yang semakin sulit. Memperpanjang durasi pendidikan kedokteran tampaknya menjadi solusi yang ideal, namun juga menambahkan beban staf pendidik dan peserta didik. Dalam menentukan pilihan yang akan diambil oleh pembuat kebijakan pendidikan kedokteran perlu dipertimbangkan risk-versus-benefit serta konsekuensinya masing-masing.

Abstract

Di tahun politik ini, dokter sebagai warga negara tidak mungkin dilepaskan dari hasrat dan haknya untuk berpolitik. Namun, dalam ekspresi politiknya seorang dokter harus selalu berhati-hati dan mengingat tradisi luhur profesi kedokteran. Tulisan ini akan membahas bagaimana sikap dokter dalam memelihara etika kedokteran dan profesionalisme di tahun politik khususnya dalam konteks hubungan dokter-pasien, serta bagaimana pihak rumah sakit/klinik dan organisasi profesi kedokteran membuat regulasi terkait ekspresi politik tersebut, ditinjau dari Sumpah Dokter dan peraturan-peraturan lain yang berlaku.

Abstract

Cases of pulmonary mycosis or pulmonary fungal diseases continues to increase in frequency along with the expanding population with impaired immune systems, including patients with pre-existing pulmonary diseases. Changing profile of underlying diseases might cause altering diseases profile as well. In previous decades, Pneumocystis pneumonia was the most common pulmonary mycosis in HIV-infected patients. As the increasing number of TB cases, pulmonary malignancy, chronic obstructive pulmonary disease (COPD) and certain chronic diseases, other pulmonary mycoses also increase such as chronic pulmonary aspergillosis, cryptococcosis, histoplasmosis, and other filamentous fungal infection. Furthermore, the airborne fungal particles of Aspergillus and other fungi could seriously worsen asthma or allergic respiratory diseases. In low- and middle-income countries, including Indonesia, lack of diagnostic facilities may lead to inadequate treatment. It will contribute to poor clinical outcomes with high mortality rates. The awareness among clinicians and other health workers of this epidemiology changes is the important step in early diagnosis and better managemenet of pulmonary mycosis in the future. Abstract

Background: Adverse event studies of favipiravir use in treating COVID-19 have been ongoing since it was established as a treatment option. A better understanding of the side effects of favipiravir from recent studies is important in developing and assessing the recognition of effective treatments for COVID-19. Method: This was a systematic review based on studies and case reports on favipiravir monotherapy in COVID-19. Access to the included studies was gained via PubMed, SCOPUS, Science Direct, SpringerLink, and MedRxiv. Results: Twelve studies consisting of eight studies and four case reports were reviewed. The most common side effects were diarrhea, elevated liver enzyme levels, and hyperuricemia. None of which were significantly different from the comparison. Currently, various adverse event were reported in case reports such as drug fever,acute generalized exanthematous pustulosis (AGEP), and transient increase in viral load. The side effects would mostly be subsided after the treatment was discontinued.Conclusion: The use of favipiravir to treat COVID-19 caused dose-related side effects such as diarrhea, changes in liver enzymes, and increased level of uric acid. There were no serious side effectscompared to other antiviral drugs. To improve the efficacy and safety of COVID-19 therapy, it is important to prepare an incidence report of antiviral adverse events in special populations such as children, pregnant women, and  patients with organ dysfunction.

Abstract

Background: Asthma is a chronic inflammation of the bronchial tree that emerges as a response to exogenous factors, such as allergens, irritants, and infections. Some asthmatic patients had been reported having symptoms of asthma due to house-dust mites (HDM) allergen exposure. It is associated with immune responses which were increased in the form of specific Immunoglobulin E (IgE) production against HDM allergens. This case-control study aimed to determine the HDM profiles in persistent asthmatic patients, including density of mites, as well as its relationship with specific IgE anti-HDM serum levels.Materials and Methods: A total of 13 patients with persistent asthma and 12 control patients had their specific anti-HDM IgE levels examined using Immulite 2000 xpi. The house dust samples were taken and analyzed with the Fain method.Results: The results have shown that 69% of patients in the persistent asthma group and 25% of normal patients were positive for IgE anti-HDM. Dermatophagoides pteronyssinus is a predominant species with a total of 120 mites (83.9%) of 143 mites. Correlation analysis indicated a positive relationship between IgE anti-HDM levels within the serums of patients and the density of mites in the dust obtained from bedroom spaces (Spearmen Rho, R=0.35, p=0.04).Conclusion: Positive IgE anti-HDM patients in the persistent asthma group were higher (69%) than those in the clinically normal group (25%). The density of mites were dominated by D. Pteronyssinus. The bedroom-dust mites density revealed a positive correlation with serum IgE anti-HDM levels in persistent asthma patients.Keywords: asthma, density, Dermatophagoides spp.,IgE
