Abstract

About 30% of occupational diseases in farmers are contact dermatitis. Contact dermatitis due to work in the agricultural sector is the main commodity in Lampung Province at 35,82%. Farmers are generally always exposed to chemicals in the form of pesticides and fertilizers which can be irritants and allergens. This research was an observational descriptive using a cross-sectional method with 379 respondents. The research data were taken from interviews, questionnaires, and physical examinations by dermatologists and genitals. Data were analyzed using the chi-square test. Results: 13,5% of farmers experienced contact dermatitis. The analysis results of this study indicate that there is a statistical relationship between contact dermatitis with the use of the following factors, personal protective equipment (p-value=0,000), contact time (p-value=0,000), personal hygiene (p-value=0,000), total types of pesticides (p-value=0,000), and number of types of fertilizer (p-value=0,000). While the factors that were not statistically significant in this study were the relationship between contact dermatitis and length of service (p-value=0,919), atopic history (p-value=0,784), and type of occupation (p-value=0,115). There is a relationship between the following factors such as the use of personal protective equipment, duration of contact, personal hygiene, the number of pesticides, and the number of types of fertilizers with the incidence of contact dermatitis in farmers in the District of Punduh pedada.

Abstract

Prevalensi hipertensi di Indonesia mengalami peningkatan setiap tahunnya. Provinsi Lampung sendiri memiliki jumlah penderita hipertensi sebanyak 24,7% dari jumlah penduduk. Selain hipertensi, penyakit degeneratif lain yang banyak terjadi dan diperberat oleh keadaan obesitas adalah gout arthitis. Salah satu cara yang dapat digunakan untuk mengontrol kedua penyakit degeneratif tersebut adalah menghilangkan faktor risiko yang dapat dimodifikasi. Penerapan pelayanan dokter keluarga berbasis evidence based medicine pada pasien dengan mengidentifikasi faktor risiko, masalah klinis, serta penatalaksanaan pasien berdasarkan kerangka penyelesaian masalah pasien dengan pendekatan patient centred dan family approach. Studi adalah Case Report. Data primer diperoleh melalui anamnesis (autoanamnesis dan alloanamnesis), pemeriksaan fisik dan tes laboratorium di klinik. Kunjungan rumah, melengkapi data keluarga dan psikososial serta lingkungan. Penilaian berdasarkan diagnosis holistik dari awal, proses dan akhir studi secara kuantitatif dan kualitatif.Pasien yang memiliki derajat fungsional 2 dengan artritis gout akut dan hipertensi grade II memiliki faktor resiko internal yaitu usia 62 tahun, faktor keturunan, pola pengobatan kuratif, tidak pernah berolahraga dan kurangnya pengetahuan tentang penyakitnya. Lalu dilakukan edukasi terhadap pasien dan keluarganya tentang penyakit, obat, gaya hidup, serta diet yang benar. Setelah dilakukan evaluasi didapatkan penurunan kadar asam urat daan tekanan darah pada pasien.Proses perubahan perilaku pasien untuk mengontrol tekanan darah dan kadar asam uratnya terlihat setelah pasien diberikan intervensi dan akhirnya mencoba mengubah gaya hidupnya dengan mengurangi makanan mengandung tinggi purin, garam dan makanan berlemak, serta pasien juga mulai rajin untuk kontrol. Peran keluarga amat penting dalam perawatan dan pengobatan anggota keluarga yang sakit Kata Kunci: Artritis gout, Hipertensi, Pelayanan Kedokteran Keluarga

Abstract

Pestisida merupakan bahan yang digunakan untuk mengendalikan populasi jasad yang dianggap sebagai hama yang dapat merugikan manusia baik secara langsung maupun tidak langsung. Penggunaan pestisida dapat menyebabkan terjadinya keracunan pestisida. Pestisida yang sering digunakan antara lain golongan organofosfat (OP). Akumulasi pestisida pada batas ambang tertentu, bisa menimbulkan dampak buruk pada tubuh antara lain, muncul gangguan pada otak, penyakit tumor, kanker, bahkan, pada ibu hamil bisa mengakibatkan bayi lahir cacat. Paparan OP pada masa kehamilan diketahui dapat menyebabkan gangguan terhadap perkembangan anak. Perkembangan anak adalah suatu proses perubahan menuju tingkat integritasi yang lebih tinggi. Perubahan-perubahan yang terjadi pada perkembangan anak meliputi berbagai aspek, antara lain aspek gerakan (motorik), kecerdasan, bahasa atau komunikasi, dan aspek sosial. Paparan OP dilihat dengan pengukuran kadar metabolitnya dalam urin ibu pada masa kehamilan atau pada urin anak. Selain itu dapat pula dideteksi melalui cairan amnion dan darah tali pusat. Metabolit OP dalam urin yang dapat dideteksi antara lain kadar dialkilfosfat (DAP) dan dietilfosfat (DEP). Terdapat beberapa faktor terjadinya gangguan perkembangan anak pada ibu yang terpapar pestisida saat masa kehamilan yaitu penggunaan alat pelindung diri (APD), frekuensi penyemprotan, lama paparan, pencampuran pestisida, lokasi tempat tinggal, dan penyimpanan pestisida.Kata kunci: metabolit organofosfat, perkembangan anak, pestisida

Abstract

Obesity (overweight) is a fundamental disease from non-communicable diseases such as diabetes, hypertension, and cardiovascular disease who still becomes a major health issue in Indonesia and the world. The teenager is a transition period that involves changes in physiological, psychological, social, and can develop into overweight or obesity. The fear of being obese is more common in girls than boys, and this explains why an effort to lose weight is mostly found in girls. This study aimed to determine the relationship between obesity and teenagers' behavioral changes in 14-16 years old of SMPN 4 Bandar Lampung students. It was a comparative analytic with a cross-sectional study. The sampling method used a case-control and using a questionnaire as a measuring tool. Data were then analyzed with a chi-square test. The study was conducted against 77 respondents with 48 respondents (62.3%) who were obese and 29 respondents (37.7%) not obese. While as many as 20 respondents (26.0%) had problematic behavior and 57 respondents (74.0%) did not have problematic behavior. The result of the chi-square test with a p-value was 0.015. There was a significant association between obesity with the teenager's behavior change in SMP Negeri 4 Bandar Lampung.

Abstract

Tinea versicolor is an infection of superfisial fungal caused by Malassezia furfur, chronic, mild and usually without inflammation. Tinea versicolor occurs because of the condition that affect the balance between host and the fungal. Personal hygiene influenced by body image, social practic, social economic status, knowledge, culture, personal choices and physical condition. Poor personal hygiene levels is a risk factor for tinea versicolor. This study aimed to determine the relationship between personal hygiene and the incidence of tinea versicolor on male students in islamic boarding school. This study is an analytic observational with quantitave research method and cross-sectional approach. The sample in this study were 68 male students at the islamic boarding school. The diagnosis of tinea versicolor confirmed by clinical and microscopic findings on skin scrapings. Bivariate data analysis using chi square test (????=10%; CI=90%). The results of this study showed that 58,6% of respondents have a poor personal hygiene and 21,4% of the respondents had tinea versicolor. From chi sqaure test analysis, it was found that the relationship of personal hygiene and the incidence of tinea versicolor statistically significant with p=0,013. From this study we can conclude that there was a relationship between personal hygiene and the incidence of tinea versicolor.

Abstract

Abstract Moist Exposed Burn Ointment (MEBO) is an ointment which can accelerate burn healing. Wharton's jelly mesenchymal stem cell (WJMSc) extract in human umbilical cord has the ability to potentially assist the burns healing process. This research was a laboratory experimental study with the use of 27 white rat induced with second degree burns and was divided into a control group (K1), Moist Exposed Burn Ointment (MEBO) group (P1) and WJMSc (P2) group. Observation of burn healing rate used healing time and burn diameter was conducted for 28 days. Data were analyzed using One way ANOVA and Kruskal wallis statistic test with 5% of α value. Average burn healing rate of K1 group: 26,78 days, P1: 24,56 days, P2: 19,67 days with the result of burn healing rate data analyzed test is p:0,005. Differences in burn diameter occurred on day 8, 12, 16, 20 and 24 on the ploriferation and remodeling stage with the result of burn diameter data analyzed test is p<0,05. Conclusion: There was a significant difference on the speed of burn healing and burn diameter between treatment group with p: 0,001. WJMSc is the fastest therapy and the largest diameter cutback.

Abstract

It is estimated that by 2045, the number of DM sufferers will reach 630 million of people. Indonesia,based on the 2018 Basic Health Survey, the prevalence of DM reached 10.9%. During the pandemicof Covid-19, people with pre-existing medical conditions such as Diabetes Mellitus (DM) were moresusceptible to becoming seriously ill due to covid-19. Health services were also hampered duringthe pandemic of Covid-19. The purpose of this activity is to increase knowledge about DM, self-careskills in patients or care-givers. Health Education was carried out through Whatsapp Group onlineeducation in the form of providing material knowledge about DM disease, self-care skills for DMpatients in the new normal era, and prevention of DM patients from Covid-19, educational videos,and flyers. This activity was attended by 168 participants. The results of the activity show thatonline education can increase knowledge about self-care skills in DM patients and care-givers. Thecontinuation of this activity was followed up through providing information on WhatsApp Groupand online consultation (telemedicine)

Abstract

Informal sector workers are workers who are most vulnerable to various risks that cause health problems. Health and safety issues related to their work can interfere with their productivity. Tapis craftsmen in the Negeri Katon Village,Pesawaran Regency, Lampung is part of the Tapis Jejama UMKM. Based on interviews with the UMKM coordinator, it was found that the craftsmen complained of back, neck, arm, and hand pain and symptoms of anemia. This interferes with the productivity of Tapis craftsmen. This service aims to improve the occupational health status of filter craftsmen in Negeri Katon Village to support work productivity in both health and economic aspects. The benefits of this activity are to reduce the incidence of anemia, musculoskeletal pain and help strengthen Negeri Katon Village as a Creative Tourism Village. The activity was carried out with a target of 30 tapis craftsmen. The solutions offered are occupational health services for tapis craftsmen, health education about ergonomics, occupational nutrition, and the stretching benefits. The results obtained that 100% of workers complained of musculoskeletal pain, as many as 5 people (8.3%) got hypertension and as many as 12 people (40%) got anemia. Before education, the knowledge of filter craftsmen about health risks in their work was 68% and after education is carried out, there is an increase of 97%. This activity needs to be carried out with the establishment of the Pos Upaya Kesehatan Kerja (UKK) as an effort to maintain the health of tapis craftsmen in Negeri Katon Village, Pesawaran Regency, Lampung.

Abstract

Asthma is a chronic respiratory disease with a complex multifactorial disorder and its etiology is associated with the interaction between genetic susceptibility, host factors, and environmental exposures. Asthma can be considered a significant public health problem. Recurrent asthma symptoms often cause difficulty, daytime fatigue, reduced activity and productivity, and increase absenteeism from school and work. Asthma management must be with continuous monitoring to achieve controlled asthma, so it is necessary to manage patients holistically and comprehensively to improve patients’ quality of life. This study is a case report of asthma exacerbation in a woman who has various internal and external risk factors that influence the patient's disease. Primary data were obtained through history taking, physical examination and support during home visits. Secondary data obtained from the patient's medical record. Management of patients with asthma exacerbations is carried out comprehensively with the concept of a patient-centered, family-focused, and community-oriented approach. Medical management is in the form of relieving drugs and asthma control drugs. Non-medical management is in the form of education about asthma, including explaining and asking patients to identify and avoid asthma triggers. The results of the evaluation showed changes in attitudes in reducing the risk of exacerbations and improvements in asthma control as assessed by the Asthma Control Test.

Abstract

Hypertension is an abnormal increase in blood pressure which can be a major cause of cardiovascular disease. It is a condition characterized by systolic blood pressure> 140 mmHg and / or diastolic blood pressure> 90 mmHg. The prevalence of hypertension in Indonesia occurs in the age group 31-44 years (31.6%), age 45-54 years (45.3%), age 55-64 years (55.2%). One of the risk factors associated with hypertension is obesity. Every 1 kg of increase in body weight has a HR for suffering from hypertension of 1.36 (95% CI: 1.29-1.45). In addition, based on the results of previous studies, it shows that every kilogram of weight loss can reduce systolic blood pressure by 1.05 mmHG and diastolic by 0.92 mmHg. Hypertension can be controlled by improving modifiable risk factors. This is the reason for comprehensive management of hypertensive patients with risk factors for obesity, in order to prevent serious complications in patients that lead to decreased quality of life of patients both within the family and community.
