Abstract

PT XYZ merupakan perusahaan jasa operasi & pemeliharaan pada sistem kelistrikan di Regional Sumatera. Performansi suatu unit pembangkit dapat dilihat dari salah satu indikator Equivalent Forced Outage Rate (EFOR). Jika nilai EFOR terlalu tinggi menandakan bahwa unit pembangkit tersebut sering mengalami gangguan. PT XYZ terhadap pencapaian EFOR pada tahun 2019 telah melebihi batas EFOR maksimal dengan target maksimal hanya sebesar 9,66% sedangkan pada realisasi 10,62%. Dampak perusahaan mengalami kehilangan daya total sebesar 362.225 Mw, dengan kerugian sebesar 367 Milyar. Atas temuan tersebut dilakukan pengukuran kinerja pemeliharaan dan tindakan perbaikan. Metode yang digunakan yaitu Maintenance Scorecard yang diintegrasikan dengan Objective Matrix. Hasil penerapan dari metode terdapat 2 Indikator yang kritis dengan kategori poor diantaranya KPI EFOR dan KPI Schedule Compliance. Dengan usulan perbaikan antara lain: meningkatkan kompetensi karyawan, menerapkan sistem KANBAN, melakukan Failure Mode and Effect Analysis atau Root Cause Failure Analysis.

Abstract

A B S T R A C TNeuroendocrine neoplasm (NEN) of gastric is a term that includes all tumour types withneuroendocrine differentiation of gastric, well differentiated or poorly differentiatedtumour. NEN is a new term in 2019 World Health Organization (WHO) classification ofdigestive system tumours. In WHO 2019 had many updates, one of them isneuroendocrine tumours (NET) grade 3 have been included in tumour category of welldifferentiated tumour or NET. Previously, NET G3 in WHO 2010 are included as poorlydifferentiated tumour or neuroendocrine carcinoma (NEC). This neoplasm is geneticallywell differentiated and because of that, WHO 2019 classification included them as a welldifferentiated tumour. For NEC, WHO 2019 subdivided them as NEC with small cells(SCNEC) and NEC with large cells (LCNEC). In WHO 2010, mixed type neuroendocrineneoplasms with other components of carcinoma are called mixed adenoneuroendocrinecarcinoma (MANEC). But, not all of non-neuroendocrine components areadenocarcinoma and it is possible that one of the components in not carcinoma. Becauseof that, in WHO 2019 the term has been changed to mixed neuroendocrine-nonneuroendocrine neoplasm (MiNEN).

Abstract

Background. Ceruminous adenocarcinoma is rare, accounting for less than 2,5% of all external auditory canal neoplasms. The tumours occur in the outer half of the external auditory canal, excluding direct extension from parotid gland. Adenoid cystic carcinoma is the most common subtypes. Adenoid cystic carcinoma needs to be distinguished from its primary origin from the salivary glands and extends to ear, because they share the same histopathology features and immunohistochemistry. Therefore, combinations of clinical features and radiology is needed to diagnose this tumour correctly.Case description. A female patient, 60 years old, admitted to RSMH Palembang with complaint ofyellow discharge coming out from ear. She also havehearing loss and pain since 6 months before admission to the hospital. One week later, the patient develops facial weakness with no mass on face. CT-Scan examination was performed, with the results showed that the mass in right external auditory canal and extends to the mastoid cavity, maxillary sinus wall and brain parenchym. Biopsy was performed in mastoid region and revealed tumour mass with cribriform pattern, few solid pattern, consists of bilayered neoplastic cells with inner luminal and outer abluminal cells. Immunostaining showed CK7 and CD117 positive in luminal cells, p63 positive in abluminal cells, S100 showed positivity in myoepithelial cells.Discussion and conclusion. Based on clinical symptoms, radiology, histopathology, and imunohistochemical staining, the mass in right mastoidis consistent with a ceruminous adenocarcinoma, adenoid cystic carcinoma subtypes, originated from external auditory canal.

Abstract

Fuzzy logic control is one application that has been able to provide a sulition for system traffic problem on higway. The funcition of the fuzzy logic control the speed of the vehicle. A traffic light system that is controlled by using a fuzzy logic control application to adjust trafic indicators to reduce congestion problem on the higway, because it takes too long to wait for the trafic light to trun green. To control traffic light, it requires tools called photoelectrik sensors at every intersection and an arduino mega 2560 microcontroller to run a fuzzy system into a programming language that can be accepted by hardware. And researching using a simulation of four intersection there must be at least 30 vehicles with a time of 60 seconds. The test results have proven changes in vehicles density at crossroads when the green lightis on.
