Abstract

Introduction: The readmission rate due to heart failure increases every year in South Sulawesi. A Frequently used method for assessing readmission is an echocardiographic examination by measuring ejection fraction. However, recent studies show that the speckle tracking parameters are more sensitive to predict readmission of heart failure patients. In this study, we assess the role of Global Longitudinal Strain (GLS) and Global Circumferential Strain (GCS) parameters on the Echo Heart Failure Score (EHFS) as a 60-days readmission predictor in heart failure patients. Methods: We included 175 patients admitted through the inpatient installation of Integrated Heart Center Dr. Wahidin Sudirohusodo General Hospital. In addition, we reviewed up to 60 days after outpatient prospectively by collecting data through medical records. The correlation between echocardiographic parameters and readmissions was analyzed using the Spearman Rank Correlation. To determine each echocardiographic parameter's cut-off point, sensitivity, and specificity, we use Receiver Operating Characteristic (ROC) curve. Results:GLS and GCS are very sensitive and specific parameters in predicting 60-days readmission with an area under the curve (AUC) value > 0.7. This study shows that adding GLS and GCS parameters to the EHFS increases the predicted value (AUC 0.850 vs. 0.820) and sensitivity to 79%. Conclusion: GLS and GCS parameters additional on EHFS can help predict 60-days readmission of heart failure patients with sensitivity and specificity of 79% and 83%, respectively. The present study shows that with higher GLS and GCS scores on the EHFS, the risk of readmission in heart failure patients will increase.

Abstract

Pendahuluan: Berbagai studi sebelumnya yang melihat outcome pada pasien PJK multivessel stabil baik yang menjalani CABG, PCI, maupun yang hanya mendapat terapi obat tidak mempertimbangkan gambaran EKG yang mungkin berpengaruh pada outcome. Studi ini bertujuan untuk melihat outcome jangka pendek pasien PJK multivessel stabil kandidat untuk operasi CABG dengan gambaran EKG normal. Metode: Studi Kohort. Outcome primer yang dinilai berupa angina berulang, kejadian SKA, stroke dan kematian karena berbagai sebab dalam 6 bulan. Data dianalisis menggunakan SPPSS versi 16. Data dinyatakan signifikan jika nilai p<0.05. Hasil: Didapatkan 79 pasien (69 pria dan 10 wanita) dikelompokkan pada kelompok yang menjalani CABG(n=13), PCI (n=12), maupun terapi obat (n=54). Kejadian bebas SKA dalam 6 bulan pada kelompok CABG sebanyak 100%, kelompok PCI sebanyak 75%, dan kelompok terapi obat sebanyak 85.1%. Dalam ikutan selama 6 bulan, kejadian bebas angina pada kelompok CABG sebanyak 87.4%, kelompok PCI sebanyak 67% dan kelompk terapi obat sebanyak 42.9% (p=0.015 & OR=7.413). Kesimpulan: Terapi obat untuk PJK multivessel berhubungan dengan kejadian SKA yang lebih rendah dibandingkan PCI dalam 6 bulan. CABG lebih superior dibandingkan terapi obat dalam menghilangkan gejala angina. Ketiga strategi terapi berhubungan dengan tingkat kematian yang rendah dalam 6 bulan.

Abstract

Beruwas laut leaf (Scaevola taccada (Gaertn.) Roxb has pharmacological effects such as anti-inflammatory, analgesic, antioxidant, antidiabetic and anticancer. Although beruwas laut leaf have many advantages, safety is the main requirement that herbal medicine must have. This research was aimed to prove sebacute of beruwas laut leaf (Scaevola taccada (Gaertn.) Roxb.) about the function of kidney and liver. This research used 20 rats were devided into 4 groups. Group 1 as a control group and groups 2,3, and 4 as an experimental group by administration of beruwas laut leaf extract with dose 200 mg/kgBB, 400 mg/kgBB, and 600 mg/kgBB. The extract was made using maceration and sebacute toxicity testing was carried out for 14 days. After giving the extract, some of rats had diarrhea. The results showed significant effect to increase levels of ureum and CGT after administration of dose 400mg/kgBB. Moreover, administration of dose 600 mg/kgBB caused significant effect in liver biomarkers and kidney (GGT and ureum). It was concluded that ethanol extract of beruwas laut leaf (Scaevola taccada (Gaertn.) Roxb.) with dose 200mg/kgBB showed safe but toxic to kidney and liver with dose 600mg/kgBB.

Abstract

Penggunaan parasetamol secara berlebihan atau melebihi dosis dapat mengakibatkan peningkatan metabolit reaktif yang mengganggu integritas membran sel dan berlanjut menjadi kerusakan ginjal. Penelitian ini bertujuan untuk mengetahui efek ekstrak rimpang kunyit hitam (Curcuma caesia) dalam mencegah terjadinya gangguan fungsi dan kerusakan struktur ginjal yang disebabkan oleh toksisitas paracetamol. Penelitian ini dilakukan secara eksperimental laboratorium yang menggunakan 20 ekor tikus yang terbagi dalam kelompok kelompok kontrol negatif yang hanya diberikan plasebo, kelompok perlakuan yang diberi ekstrak dengan dosis 100 mg/kg, 200 mg/kg dan 300 mg/kg, serta kelompok kontrol positif yang diberi sediaan ekstrak curcuma. Berdasarkan pengukuran kadar ureum dan kreatinin, ekstrak kunyit hitam pada semua dosis mampu mencegah peningkatan biomarker ginjal, namun dosis 200 mg/kg yang secara signifikan mencegah kenaikan kadar kreatinin dan ureum, dimana hasil histopatologi menunjukkan kerusakan ginjal sangat minim dibandingkan dengan kelompok plasebo. Disimpulkan pemberian ekstrak etanol kunyit hitam terutama dosis 200 mg/kg-BB berpotensi mencegah kerusakan fungsi dan struktur jaringan ginjal yang diinduksi dengan parasetamol dosis toksik.
