Abstract

Hormonal contraception is related to change in the metabolism of some nutrients that may lead to an increase in body weight. The aims of this study is to assess the risk of obesity in hormonal contraceptive users in the District of Kulon Progo, Jogjakarta, Indonesia. A cross sectional study was used to determine the prevalence of obesity among users of contraception. A systematic cluster random sampling, using villages as clusters, was used to choose 647 users of contraception in Kulon Progo.  A hundred and two obese cases and 102 control, matched-for-age and socioeconomic status, controls were included in the case control study used to evaluate the risk of obesity among users of hormonal contraception. The prevalence of obesity among users of contraception in Kulon Progo was 15.9%. Users of hormonal contraception has a increased risk for obesity, OR: 9.4 (95% CI: 1.1 – 81.5). Users of combination pills faced the highest risk, followed by users of injected progesterone depot, while the risk in implant users was the same as that in users of non-hormonal contraception. The risk of obesity was significantly higher after 7 years of hormonal contraception use. The risk of obesity was neither related to energy intake nor expenditure. The increased risk of obesity in users of hormonal contraception was still significant after controlling for age, parity, initial weight, socioeconomic status, energy intake and expenditure, and parental obesity. We conclude that the risk of obesity is higher in users of hormonal contraception compared to the non-hormonal ones. Abstract

Antioksidan merupakan salah satu zat gizi yang dapat menurunkan mual dan muntah. Antioksidan ini dapat ditemukan pada bahan makanan seperti jahe, kacang kedelai, dan kacang hijau. Bahan makanan tersebut dapat dikonsumsi dalam bentuk minuman fungsional. Penelitian ini bertujuan untuk mengetahui aktivitas antioksidan pada minuman fungsional berbasis jahe dan kacang-kacangan yang dapat membantu mengatasi mual dan muntah. Desain dari penelitian ini adalah eksploratif deskriptif. Sampel pada penelitian ini yaitu 16 formula yang didapatkan dengan metode d-optimal Mixture Design pada software Design Expert 7®. Software design expert ini mampu menentukan proporsi sari jahe, sari kacang hijau, dan sari kacang kedelai. Aktivitas antiosidan diukur menggunakan metode DPPH yang disajikan dalam bentuk nilai aktivitas antioksidan IC50. IC50 didefinisikan sebagai konsentrasi antioksidan yang dapat menangkap 50% radikal bebas DPPH. Hasil penelitian ini menunjukkan bahwa nilai aktivitas antioksidan IC50 formula 1 hingga 16 adalah 109,20; 104,08; 102,66; 147,88; 99,42; 129,89; 132,53; 115,87; 136,94; 109,78; 139,78; 99,33; 102,26; 135,46; 116,07; 120,45 mg/ml. Semakin kecil nilai IC50 menunjukkan bahwa aktivitas antioksidannya semakin baik. Terdapat perbedaan aktivititas antioksidan yang signifikan dari 16 formula (p = 0,0048).  Kesimpulan dari penelitian ini yaitu terdapat aktivitas antioksdan pada formula minuman fungsional berbahan dasar jahe dan kacang-kacangan sebesar 99,33 mg/ml hingga147,88 mg/ml.   Kata kunci: aktivitas antioksidan; mual; muntah;  minuman fungsionalAbstractAntioxidant is one of the nutrients that can reduce nausea and vomiting. It can be found in ginger, soy bean, and green beand. They can be consumed as functional drinks. This study aimed to determine the antioxidant activity on functional drinks based on ginger and beans that can reduce nausea and vomiting. Sample in this study is 16 formula which got by d-optimal Mixture Design method in software Design Expert 7®. That design was used to decide the proportion of ginger extract, green bean extract, and soybean extract. Antioxidant activity was measured by DPPH. It was dercribed by IC50 score of antioxidant activity. Result of this research showed that IC50 Score of antioxdant activity formula 1-16 were 109.20; 104.08; 102.66; 147.88; 99.42; 129.89; 132.53; 115.87; 136.94; 109.78; 139.78; 99.33; 102.26; 135.46; 116.07; 120.45 mg/ml respectively. The smaller score of IC50 indicated that antioxidant activity was better than the higher score.  Statistical analysis indicated a significant different (p < 0,005) on antioxidant activity among formulas (p = 0,0048). From this study could be concluded there were antioxidant activity in ginger formula about 99.33 mg/ml until 147.88 mg/ml. Keywords: antioxidant activity; nausea; vomiting; functional drink

Abstract

Background : Nutrient intakes may influence nutritional status of patients during health care. It was reported that nutritional status of 75% of hospitalized patients declined during hospitalization.Objective: The objectives of this study was to assess factors that may influence malnutrition among adult patients in hospital.Methods: This study was done using a nested case control study design. Subjects were inpatients of internal and neurology departments of Sanglah, Sardjito and M. Jamil hospitals. They were all given oral feeding, inconsciousness condition, cooperative, and non-ascites. Data on food consumption were obtained using comstock methods, while nutritional status were measured using Subjective Global Assessment.Results: Low energy intake, length of stay, infection, and special diet were all associated with a higher risk of hospital malnutrition. Patients with low energy intakes were 2.4 (OR=2.4, 95%CI= 1.17-4.92) times more likely to be malnourished than those with sufficient energy intakes. Patients with length of stay >= 7 days were 8 (OR=8.15, 95% CI =1.87-35.51) times more likely to be malnourished than those with length of stay < 7 days. Patients with infectious diseases were 3 (OR= 0.33, 95% CI= 0.17-0.64) times less likely to be malnourished than those with non infectious diseases. Lastly, patients with special diet were 2 (OR=1.96, 95% CI= 1.05-3.68) times more likely to be malnourished than those without special diet.Conclusions: Low energy intake, length of stay, non infectious diseases, and special diet are risk factors of malnutrition in adult hospitalized patients.

Abstract

Background: in Indonesia diarrhea is still a major cause of morbidity and mortality among children especially those under five years of age. The Household Health Survey 2000 and 2003 shows an increase of diarrhea morbidity rate from 300 to 374 per 1,000 people. At the inpatient ward of Dr. Sardjito Hospital in 2005, diarrhea was found as the major cause of patients being hospitalized. One of the causes of high diarrhea morbidity rate is poor nutritional status. In 2002, the prevalence of undernourished children in Indonesia was 27.3% and in the Province of Yogyakarta Special Territory was 11.39%. Ultimately, nutritional status and diarrhea are related to each other, which lead to very famous "vicious cycle", diarrhea causes malnutrition and malnutrition causes diarrhea. Objective: To identify relationship between nutritional status and the duration of diarrhea. Method: This observational study was conducted with retrospective cohort design using the data of medical records and surveillance of diarrhea at Dr. Sardjito Hospital from September 2005 to September 2006. Subject of the study were 138 children of 6 months to 5 years old suffering from acute diarrhea. Data analysis used Fisher's exact test and one way ANOVA.Result: The average duration of diarrhea among undernourished children was 101.0 + 28.28 hours, wasted children was 96.31 + 16.69 hours, normal nourished children was 65.06 + 6.90 hours, and well nourished children was 64.52 + 11.70 hours. There was a significant relationship between nutritional status and the duration of diarrhea (p < 0.05). Conclusion: There was a significant relationship between nutritional status and the duration of diarrhea. This means that children with poor nutritional status would likely suffer longer from diarrhea.

Abstract

Background: Anemia among pregnant women may increase abortion, premature, low birth weight and even mortality before and after delivery. Factors causing anemia in Indonesia are lack of Fe, inadequate intake, demanding needs, and lack of nutrient facilitating Fe absorption. Besides, infection such as worm and malaria may cause anemia as well. Result of household health survey 2001 stated that prevalence of pregnant women with anemia was 40% in Indonesia. In Jambi Province, it was 39% in 2003 and 42.3% for Batang Hari Regency while infection of malaria was 50.0% in all age groups and many other districts that its environment were woods, humid, and wasted that may increase infection of hookworm.Objective: The study was proposed to know the correlation of hookworm, malaria with anemia among pregnant women by analyzing correlation of hookworm, malaria, and anemia and correlation of hookworm, malaria and Fe status among pregnant women.Method: This was cross sectional study. It held from September until December 2005 in Batang Hari Regency, Jambi Province. Subjects were 132 pregnant women. Examination of blood for Hb, malaria and ferritin were taken together while feces were later.Results: There were no significant correlation of Ascaris lumbricoides infection with anemia (p = 0.36; OR = 2.43), Trichiuris thrichiura infection with anemia (p = 0.30), 2 kinds of worm Ascaris lumbricoides + Trichiuris trichiura with anemia (p = 0.08; OR = 4.87), and infection of Ascaris lumbricoides + Necator americanus with anemia (p = 0.30). There was significant correlation of 3 kinds of worm Ascaris lumbricoides + Trichiuris trichiura + Necator americanus with anemia (p = 0.018; OR = 7.3). There were no significant correlation of Ascaris lumbricoides with ferritin (p = 0.17; OR = 3.23) and Trichiuris trichiura with ferritin (p = 0.25). There was significant correlation of 2 kinds of worm Ascaris lumbricoides + Trichiuris trichiura with ferritin (p = 0.04; OR = 6.4). There was no significant correlation of Ascaris lumbricoides + Necator americanus with ferritin p = 0.25. There were significant correlation of 3 kinds of worm (Ascaris lumbricoides + Trichiuris trichiura + Necator americanus) with ferritin p = 0.007; OR = 9.69 and ferritin with anemia p = 0.0001; OR = 17.45.Conclusion: There was no significant correlation of hookworms Ascaris lumbricoides, Trichiuris trichiura, Ascaris lumbricoides + Trichius trichiura, Ascaris lumbricoides + Necator americanus with anemia. There were significant correlation of infection of Ascaris lumbricoides + Trichiuris trichiura + Necator americanus with anemia and malaria with anemia. There was no correlation of hookworms Ascaris lumbricoides, Trichiuris trichiura, Ascaris lumbricoides + Neca- tor americanus with anemia. There were correlation of infection Ascaris lumbricoides + Trichiuris trichiura and Ascaris lumbricoides + Trichiuris trichiuris + Necator americanus with ferritin, malaria with ferritin, and ferritin with anemia.

Abstract

Background: Malnutrition is one common problem for hospitalized patient, estimated between 20-60%. American Dietetic Association (ADA) recommends a standardized Nutrition Care Process (SNCP) that ensures services and outcomes in the quality management of nutrition care to all patients based on an individual basis and scientific facts. In Indonesia was developed nutrition screening tool, namely the Simple Nutrition Screening Tool (SNST) which was valid and reliable.Objective: The study was to determine the effect of SNCP based on screening to improve nutrient intake and nutritional status.Method: The method using a 2x2 factorial design, with 4 groups which are: screened with Nutritional Risk Screening (NRS) and get hospital-based SNCP (A1B1); screened with NRS and algorithm SNCP (A1B2); screened with SNST and get hospital-based SNCP (A2B1); screened with the SNST and get algorithm SNCP (A2B2).Results: Nutritional screening interventions using NRS and SNST method can increase the energy and protein intake also Mid-Upper Arm Circumference (MUAC). There is no relationship between algorithm SNCP with nutrient intake and nutritional status, although the average intake of energy, protein, MUAC and albumin was better in the algorithm SNCP than in hospital-based SNCP. The interaction between the SNST and algorithm SNCP have better mean intake of energy, protein and MUAC changes than other groups. The size of the effect of energy intake, protein intake, and MUAC was 129 kcal; 4.6 g; and 0.35 cm, respectively.Conclusion: Interaction between an SNST and algorithm SNCP have a better average intake of energy, protein, and MUAC compared with other groups.

Abstract

Background: The prevalence of obesity is markedly increasing, both in developed and developing countries. Whether physical activity contributes to the obesity in Indonesian adolescents is still unknown.Objective: To assess whether physical activity is associated with obesity in junior high school adolescents in Yogyakarta. Methods: A case control study was conducted in Yogyakarta Province in 2003. Subjects were 140 obese and 140 non- obese junior high school adolescents in Yogyakarta and Bantul, randomly chosen from an obesity survey performed previously. Pattern and duration of activity were assessed using IPAQ modified questionnaire. The activities were than sorted into light (sedentary), moderate and vigorous activities.Results: There was significant difference in the distribution sedentary activity between adolescents of Yogyakarta and Bantul (p<0.0001). The mean duration of sedentary activity in Yogyakarta was 12.4 hours/day while in Bantul was 11.0 hours/day. Obese adolescents spent longer time in sedentary activity than non-obese adolescents (p=0.002). The odds of being obese in adolescent whose sedentary activity was longer than 13 hours/day were almost doubled.Conclusions: Urban adolescents spent more time for sedentary activities than rural adolescents, so were obese adolescents. The association of sedentary activities to obesity is independent from other factors such as calorie intake and parental obesity status.

Abstract

Background: The high prevalence of anemia pregnant women anemia in Indonesia reaches 50%–70%, and it is caused by many factors. First, it may be caused by low compliance of iron supplementation and second, it may be caused by bad distribution of iron supplementation. This prevalence results some effect: high maternal mortality rate, neonatal mortality rate, anemia in infancy, and low productivity. One solution to overcome this prevalence is by giving iron supplementation to pregnant women with husband’s supervision.Objective: This study aims to examine the relationship between iron supplementation with compliance, Hb value, and average of age gestation under husband’s supervision.Methods: This was quasi experimental non-equivalent control group design. Subjects were pregnant women aged 20–28 weeks whose hemoglobin level 8 gr/dl to 15 gr/dl. Subject were taken from three sub-districts of Bantul District, Special Teritory of Yogyakarta. Subject were divided into two groups, the first group (n=55) received 60 mg iron supplementation with husband’s supervision and the second group (n=65) received 60 mg iron supplementation without husband’s supervision.Results: The compliance of first the group was higher than the second group (x2=19,48 p<0,01). In the first group, iron supplementation was effective to increase Hb level it was statistically insignificant (p>0,05). In the second group, iron supplementation was also effective to increase Hb level and it is statistically significant (p<0,05). The difference of Hb level between two groups was insignifi cant. After the predictor variable was controlled using multivariate regression test, it showed that iron supplementation with husband’s supervision was effective to increased Hb level 0,8 gr/dl and statistically signifi cant (p<0,05). The gestation average of two groups was similar, but after predictor variable was controlled using multivariate test, it showed that iron supplementation with husband’s supervision had one week gestation longer than those group without husband’s supervision.Conclusion: Iron supplementation with husband’s supervision are effective to lengthen one week age gestation.

Abstract

Background: Diabetes Mellitus (DM) is a chronic disease with its increasing prevalence worldwide, including in Indonesia and in Bali province especially among adult group. The increasing prevalence of DM is followed by the increasing prevalence of obesity affected by changes in lifestyle and unhealthy dietary patterns.Objective: The study was conducted to investigate relations between dietary patterns and obesity with type 2 DM in Sanglah Hospital, Denpasar.Method: The study was observational analytic with matched case control study. Subject of the study was divided into two groups, i.e. case and control with comparison case and control 1:2 which matched to age and sex. The cases were type 2 DM patients who had recently diagnosed on first visited in Sanglah Hospital. Control 1 was outpatients of non-type 2 DM in Sanglah Hospital and control 2 were taken from the case-neighboring household. Total subject were 147, taken by consecutive method. Collected data were subject identity, dietary patterns, physical activity, weight, height, waist circumference and fasting blood sugar. Dietary patterns were collected using Food Frequency Questionnaire (FFQ), physical activity was collected using modified International Physical Activity Questionnaire (IPAQ), and fasting blood sugar were estimated using biosensor of glucose. Multiple logistic regression models were used to identify risk factors associated with type 2 DM.Result: This study showed that there were 5 variables significantly related with type 2 DM, i.e. interaction of overall obesity with the dietary fiber intake (OR:10.7; 95% CI: 3.5-33.7), family history (OR:5.5, 95% CI: 2.2-13.3), source of high fat animal protein (OR:4.9, 95% CI:1.5-16.1), source of low fat animal protein (OR:0.1, 95% CI: 0-0.5), hypertension (OR:3.7, 95% CI:1.4-9.9)Conclusion: The interaction between overall obesity with dietary fiber intake, family history, source of high fat animal protein, source of low fat animal protein and hypertension were the risk factors of type 2 DM occurrence in Sanglah Hospital, Denpasar.
