Abstract

Background Ovarian carcinoma is the second most common malignancy of the female genital tract, following endometrial carcinoma. Highly mortality of ovarian carcinoma is primarily due to difficulties in diagnosing early stage disease. Metastatic process is influenced by many factors, including molecular factors p53, Bcl-2 and nm23- H1. p53 is a tumor suppressor gene that acts to control cell proliferation, while Bcl-2 is an oncoprotein that inhibit apoptosis, which is also associated with response to chemotherapy. Mutation of p53 gene might inhibit Bcl-2 gene therefore stimulate cell proliferation and metastasis. nm23-H1 gene is metastatic inhibitor and decrease the level or expression of these genes will increase the risk of metastasis. p53, Bcl-2 and nm23-H1 expression of ovarian carcinoma need to be investigated in order to obtain more accurate prognosis. This study aimed to investigate p53, Bcl-2 and nm23-H1 expression in Serous and Mucinous ovarian carcinoma and to determine their association with Staging and Grading . Methods Design of this research was a cross sectional design. Subjects used in this study were 50 paraffin-embedded tissues of epithelial ovarian carcinoma, which had been taken from Dr. Sardjito General Hospital (Pathology Laboratory archive). Samples were processed with immunohistochemical staining (IHC) methode using monoclonal antibody anti p53, bcl-2, and nm23-H1. Different expression of p53, Bcl-2, nm23-H1 among samples and their association with staging and grading was analyzed with chi-square. Results p53 expression was associated with grading but not with staging of serous ovarian carcinoma and this protein was not expressed in mucinous ovarian carcinoma. Both Bcl-2 and nm23-H1 expression was not associated with grading and staging of serous ovarian carcinoma. In mucinous ovarian carcinoma, bcl-2 and nm23-H1 expression was not associated with staging. Conclusion Bcl-2 expression was not different among early stages of serous and mucinous ovarian carcinoma. nm23-H1 expression was not significantly different with grading and staging of both ovarian carcinoma. Abstract

ABSTRAK Latar belakang Ekspresi ALDH-1 mempunyai peran penting dalam identifikasi sel yang diduga cancer stem cell sedangkan ekspresi EGFR-1 pada karsinoma kolorektal dinyatakan meningkat dan hubungan ekspresi keduanya dengan prognosis karsinoma kolorektal masih kontroversial. Tujuan Mengetahui hubungan antara ekspresi ALDH-1 dan EGFR-1 dengan karakteristik klinikopatologi karsinoma kolorektal. Metode Studi potong lintang retrospektif (retrospective cross sectional), dengan 50 sampel karsinoma kolorektal tahun 2009-2010 di RSUP Sardjito, Yogyakarta. Data klinikopatologi yang meliputi usia, jenis kelamin, lokasi tumor, derajat diferensiasi histopatologi, dilihat dari data pasien dan hasil evaluasi histopatologi di Instalasi Patologi Anatomik RSUP. Sardjito, Yogyakarta. Dilakukan pewarnaan imunohistokimia dengan antibodi monoklonal anti ALDH-1 dan EGFR-1. Hubungan antara ekspresi ALDH-1 dan EGFR-1 dan tiap variabel klinikopatologi akan dievaluasi menggunakan metoda analisis Pearson Chi-Square,Fisher dan Kruskal Wallis. Hasil Tidak didapatkanhubungan yang bermakna antara ekspresi ALDH-1 dengan usia (p=0,697), jenis kelamin (p=0,615), lokasi tumor (p=0,525), EGFR-1 (p=0,279), namun didapatkan hubungan yang bermakna dengan derajatdiferensiasi histopatologi (p=0,021). Ekspresi EGFR-1 tidak didapatkan hubungan yang bermakna dengan usia (p=0,074), jenis kelamin (p=1,0), dan lokasi tumor (p=0,293), tetapi didapatkan hubungan yang bermakna dengan derajat diferensiasi sel tumor pada histopatologi (p=0,014). Kesimpulan Adanya hubungan ekspresi ALDH-1 dan EGFR-1 dengan derajat diferensiasi histopatologi menunjukkan peran sel punca dan EGFR-1 dalam perkembangan diferensiasi karsinoma kolorektal. The correlation between ALDH-1 and EGFR-1 expression and each variable will be evaluated using Pearson Chi-Square analysis, Fisher and Kruskal Wallis. Results There are no significant correlation between ALDH-1 with age (p=0.697), gender (p=0.615), location of tumor (p=0.525), and EGFR-1 (p=0.279), but there is a significant correlation with differentiation of histopathological grading (p= 0.021). There are no correlation between ekspresion EGFR-1 with age (p=0.074), gender (p=1.0), location of tumor (p=0.293), but there is a significant correlation with differentiation of histopathological grading (p=0.014 ). Conclution There are significant correlations between expression ALDH-1 and EGFR-1 with differentiation of histopathological grading indicate the role of stem cells in development of differentiation of cancer cells in colorectal carcinoma.
