Abstract

Latar belakang Kanker ovarium merupakan kanker ginekologi yang dijumpai hampir 30% dari semua kanker organ reproduksi. Tipe paling umum dijumpai adalah kanker ovarium yang berasal dari epitel permukaan ovarium. Differensiasi menjadi jenis sel-sel epitel pada kanker ovarium jenis epitel berada pada kontrol yang sama dan nantinya akan menentukan jenis epitel pada organ sistem reproduksi wanita selama perkembangan embrionik. Protein gene product (PGP9.5) merupakan suatu peptide spesifik pada neuron yang menghilangkan ubiquitin dari protein-protein ubiquinated dan dapat berperan sebagai promoter hipermetilasi yang penting dalam gen transkripsi pada kanker. Tujuan penelitian ini untuk melihat hubungan ekspresi imunohistokimia PGP9.5 pada berbagai derajat histopatologi kanker ovarium tipe adenokarsinoma serosum dan adenokarsinoma musinosum. Metode Penelitian ini bersifat deskriptif analitik dengan pendekatan penelitian cross sectional. Penelitian ini dilakukan dengan pengambilan dan pengumpulan blok parafin jaringan ovarium sebanyak 45 sampel terdiri dari 22 sampel adenokarsinoma serosum dan 13 sampel adenokarsinoma musinosum, dipulas dengan imunohistokimia PGP9.5 dan dilakukan interpretasi dengan menilai intensitas kualitas tampilan warna pada sel tumor. Hasil Berdasarkan analisa dengan uji Kruskal-Wallis didapati 26 sampel yang terpulas negatif dengan imunohistokimia PGP9.5 terdapat 13 kasus dengan derajat differensiasi well differentiated (50%), 6 kasus (23,1%) moderately differentiated dan 7 kasus (26,9%) poorly differentiated. Pada 5 sampel yang terekspresi lemah tedapat 4 kasus (80%) dengan well differentiated dan 1 kasus (20%) dengan poorly differentiated. Pada 4 kasus terekspresi sedang, terdapat 2 kasus (50%) pada well differentiated, 1 kasus (25%) pada moderately differentiated dan 1 kasus (25%) pada poorly differentiated. Pada 10 sampel yang terekspresi kuat, terdapat 2 kasus(20%) pada well differentiated dan 8 kasus (80%) pada poorly differentiated Kesimpulan Tidak ada hubungan ekspresi PGP9.5 dengan derajat histopatologi kanker ovarium tipe adenokarsinoma serosum dan adenokarsinoma musinosum (p-value >0,05). Kata kunci: adenokarsinoma musinosum, adenokarsinoma serosum, imunohistokimia, kanker ovarium, protein gene product (PGP9.5).

Abstract

BackgroundTend to have adenoid cystic carcinoma (AdCC) is one of malignant epithelial tumor of the salivary gland poor prognosis, localrecurrence and metastases. Histopathological grading system according Weert, et al. observed from presence or absence solidcomponent of AdCC. CD117 is a transmembrane tyrosine kinase receptor usually strong expression in AdCC. EGFR belongs to theErbB family of receptor tyrosine kinases associated with cell proliferation, angiogenesis and metastases. Patients with advanceddisease may benefit from targeted therapy besides conventional management. Expression of CD117 and EGFR may give proposaltherapy through difference histology pattern of AdCC based on histopathological grading.MethodsThis is an analytical descriptive study with cross sectional approach. Formalin-fixed paraffin-embedded sections from 35 adenoidcystic carcinomas were obtained for histopathological grading and for immunostained of CD117 and EGFR using monoclonalantibodies. The staining intensity and percentage of cells with membranous and cytoplasmic of CD117 and EGFR expression werescored and then grouped according to the parameters of the Allred scoring system.ResultsChi-square test between CD117, EGFR expression and histopathological grading of AdCC showed no sigficant differences.Besides, McNemar test between CD117 and EGFR expression and histopathological grading AdCC showed significant differences,p-value=0.03 (p<0.05).ConclusionsCD117 and EGFR could be used to detected salivary gland AdCC

Abstract

Background. While carrying out its duties, doctors certainly cannot be separated from the act of communicating with patients. Doctor-patient communication is defined as ongoing communication between doctors, as a medic, with the patient as the treated person. This communication skills are important and therefore must be mastered by doctors. But before that, doctors must have adequate knowledge of how to communicate properly and effectively with patients. Lack of this communication skill will be a serious problem so it should not be ignored. Objective. To determine the doctors level of knowledge about doctor-patient communication. Methods. This study was conducted in descriptive with cross sectional approach. The data were collected by distributing the questionnaire of 15 questions to 71 doctors who were still actively teaching at Faculty of Medicine, Universitas Sumatera Utara. The collected data then analyzed using SPSS computer program. Results. The results showed that most of the doctor that work on Faculty of Medicine, Universitas Sumatera Utara, 83.1% of the samples were in good category of knowledge, 14.1% of the samples were in sufficient category of knowledge, the rest 2.8% of the samples were in low category of knowledge. Conclusion. Majority of the doctor have a good level of knowledge about doctor-patient communication. Keyword: Communication, Doctor-Patient, Knowledge, Skills

Abstract

Background Incidence and mortality of colorectal carcinoma (CRC) worldwide varies widely (10 times). Ninety percent colorectal malignancies are adenocarcinomas. Expression SOX2 is associated with high tumor grade, invasion, metastasis as well as a prognostic predictor of colorectal adenocarcinoma. Objective: Analyze the association of SOX2 immunohistochemistry expression with histopathological grading, intramural and extramural vascular invasion in colorectal adenocarcinoma. Methods Analytical study using a cross-sectional approach on 35 samples from the Anatomic Pathology Unit, H. Adam Malik Hospital, Medan. Immunohistochemical staining of SOX2 mouse monoclonal antibody (Bioassay Technology Laboratory) was performed and expression assessed as low (0-3) and high (4-6). Differences in expression were analyzed by Chi-square and Mann-Whitney correlation tests. Results The most patients with colorectal adenocarcinoma were at age of 51-60 years (34.3%), male sex was more common (60%), the most tumor location was in the rectum (40%), the most depth of invasion was T3 (74.3%) , low grade was more common (60%), more intramural vascular invasion was found (40%), and the highest expression of SOX2 was found to high expression which mostly in the nucleus and cytoplasm (84.6%) There was no significant association between SOX2 expression and histopathological grading (p= 0.884) and there was no significant association between SOX2 expression and vascular invasion, both intramural and extramural (p= 0.390). Conclusion There was no significant association between SOX2 expression and histopathological grading and vascular invasion, both intramural and extramural.
