Abstract

Healthcare facilities such as hospitals, public healthclinics, and private practices are places that peoplevisit to obtain thorough medical care. The state billno.44 of 2009 stipulates that hospitals areinstitutions that provide complete healthcare servicefor individuals. The service included is providinginpatient facilities, home care, and medicalemergency service. According to the bill, among thefunctions of a hospital is to establish and maintainthe quality standard of healthcare service within thehospital, which is then used as a blueprint or areference in day to day patient care. The general public are becoming evermoreconscientious on choosing which hospital to go towhen they become ill. Which is why for everyhospital that offers medical service, the challengethen arises on how to implement various strategiesin order to develop and not merely survive in thegrowing healthcare industry. Among the strategiesthat needs attention is sustaining the qualitystandard of the hospital care service. The quality standard of the hospital care service canbe measured using the SERVQUAL model (ServiceQuality model), which gives rate to the indicators ofvarious qualities of service (Tjiptono, 2011). Thereare 5 main aspects that are measured and arrangedbelow according to the relativesignificance/importance (Tjiptono dan Chandra,2011):A. Reliability, which signifies the institution's abilityto provide error-free and accurate medical careupon patient arrival, within the agreed upon timeconstraint.B. Responsiveness, signifies the healthcarepersonnel's availability and capability to provide fastresponse aid and cater to their needs and request. Italso involves the tactful knowledge of informing thepatient when the medical care will be given andperforming the promised care accordingly.C. Assurance, signifies how the healthcare personnelestablish trust with patients which in turn earns thepatient's trust towards the institution. This can beachieved by providing healthcare personnel whomaintain courteous conduct at all time yet possessthe skill and ability to answer any arising questionsor handle any given situation.Furthermore, it involves the institutions ability tofoster a safe and secure environment for patients.D. Empathy, signifies the ability of healthcarepersonnels to establish effective communicationwhile delivering proper diagnosis. Caring is anessential component which can be implemented bygiving personal attention to each patient.E. Tangibles, correlates with the physicalenvironment of the establishment, which can comein the form of an attractive office design, availabilityof a full range medical equipment, and furthercomplemented by the pleasant appearance of thehealthcare personnels.Delivering optimum service is essential forinstitutions who thrive on gaining customersatisfaction and building towards patient loyalty inthe long run.

Abstract

Background: Indonesia as an agricultural country is still having nutritional problems (stunting and undernutrition) caused by mothers’ lack of knowledge, attitude and practice of eating behavior. Purpose: This research aims to investigate the impact of knowledge, attitude, and practice of eating behavior of mothers of children under five on stunting and undernutrition prevalence in the agricultural communities in Jember. Methods: This cross-sectional research used questionnaires as a method to collect data from respondents i.e. mothers who have toddlers about the knowledge, attitudes and practice of eating behavior. The research was conducted in 2018-2019 in Jember. A total of 414 respondents who met the inclusion criteria (those resided in the study area and were willing to participate in this study) were chosen through random sampling. The data were then analyzed by using Spearman rho’s test to search the effect of the knowledge and attitudes of the mothers of under fives toward stunting and undernutrition. While eating behavior was described based on the results of interviews with respondents on 2x24 hours of food recall and food frequency questionnaires.  Results: This study showed that knowledge and attitudes of mothers regarding eating behavior were related to the occurrence of stunting (p = 0.01; p = 0.04) and malnutrition (p = 0.04), except for the attitude variable towards malnutrition (p = 0.81). Data analysis results regarding eating behavior showed that there was still a lack of fulfillment of certain nutrients, both macronutrients (fiber) and micronutrients (Vitamin B1, Vitamin C, Vitamin E, Potassium, Sodium, Calcium and Folic Acid.) to the RDA reference which was recorded as severe deficiency in 60-97% of children under five. Conclusion: Adequate knowledge and attitudes of mothers of diet can be a factor that prevents nutritional problems (stunting and malnutrition). The consumption pattern assessed showed that there was still a lack of fulfillment of certain nutrients both macro (fiber) and micronutrients against RDA reference. Further research needs to be done to explore the right promotive and prevention approach based on the lack of nutrition found in each area.

Abstract

Klebsiella pneumoniae is an infectious bacteria in various parts of the body. The presence of proteins in pili that will bind to cell surface receptors (adhesin proteins) and cell membrane sugar molecules (hemagglutinin proteins) is a very influential factor in the ability of K. pneumoniae to enter the human body. The purpose of this study was to determine the role of pili 38.6 kDa K. pneumoniae protein as a hemagglutinin and adhesin protein which functions as a virulence factor. This type of research is pure experimental research using the electrophoresis method (SDS-PAGE) to obtain protein. The protein obtained was then tested for hemagglutination and adhesion test using erythrocyte cells and enterocytes of BALB / C mice to determine their role as hemagglutinin and adhesin proteins. Conclusion of this study pili protein 38.6 kDa Klebsiella pneumoniae acts as a hemagglutinin and adhesion protein which functions as a virulence factor. Abstract

One of the risk factor that can be changed in type 2 diabetes melitus is food consumption. Fat is one of the macronutrients that have significant influence in consumption. Unhealthy diet can be a trigger to macrovascular complication that is coronary heart disease (CHD) which started with atherosclerosis. Atherogenic index is a good predictor for CHD. This study aims to determine the correlation between fat consumption with atherogenic index in DMT 2 patients at RSD dr. Soebandi. This is an analitic observational study with cross sectional design, using 74 type 2 diabetes melitus patients from Internal Poly at RSD dr. Soebandi who met inclusion and exclusion criteria. Fat consumption were obtained with 24-hours food recall interview and atherogenic index were obtained from TG and HDL levels in medical record. The average of fat consumption is 34,01% and as much 33 subjects (44,6%) have high risk of CHD (AI>0,21). Spearman correlation test between fat consumption with atherogenic index is p=0,025 which mean there is significant correlation between fat consumption and atherogenic index in type 2 diabetes melitus patients at RSD dr. Soebandi Keywords: DM Type 2, fat consumption, carbohydrate consumption, atherogenic index

Abstract

Many farmers still use chemical pesticides. Chemical pesticides inside the body will form free radicals that will bind to unsaturated fatty acids (cell membrane structures) that cause increased levels of malondialdehyde (MDA). Organic pesticides can be used as an alternative to control pests. Organic pesticides leave no residues to plants and the environment. This study aims to determine the difference of plasma malondialdehid (MDA) levels in farmers who use chemical pesticides in Dawuhan Village, Tenggarang Subdistrict, Bondowoso Regency and farmers who use organic pesticides in Lombok Kulon Village, Wonosari Sub-district, Bondowoso District. This was an observational analytic research with cross sectional research design conducted on 30 farmers who use chemical pesticides and 30 farmers who use organic pesticides. Blood sampling was taken from Mediana cubiti vein. Measurements of plasma malondialdehyde (MDA) were performed using the TBARS method. Based on the result of Unpaired T-test, it was found that p < 0.001 which showed significant difference in plasma malondialdehyde (MDA) level in farmers using chemical pesticides and farmers using organic pesticides. The plasma malondialdehyde (MDA) level average were higher in farmers using chemical pesticides. Keywords: Chemical Pesticide, Organic Pesticide, Plasma Malondialdehyde
