Abstract

Background: DFU patients must be treated properly less than 30 days to prevent infection spread to the bone, causes smaller opportunity to recovery and higher risk for amputation. Ozone therpay has a clearer clinical effect, affects the wound healing process, it can be seen by decreasing the length of stay. Method: Case studies of 3 DFU patients who were given ORNi therapy. Wound evaluation using Bettes-Jenssen Assessment tool skoring system. Results: There were significant improvement in these cases after given a noninvasive regional ozone therapy (ORNi) as a combination in standard treatment for 5 – 7 times. There are no sign of infection, granulation and epithelization processes were running optimal and exudate production was controlled. Conclusions and recommendation: Using ORNi therapy as an adjunct to standard care has shown a significant improvement. The rate of granulation and epithelization during 15 days of treatment has an average growth rate of granulation and epithelization up to 22%. Researchers recommend to do more research with suficcient sample size related to the use of ozone therapy as an adjunct in wound care.

Abstract

AbstrakFistula merupakan saluran abnormal antara lumen organ berongga dengan organ berongga lainnya atau dengan kulit. Fistula gastrointestinal terjadi sebagai akibat dari komplikasi pascapembedahan abdomen dan manifestasi sekunder dari proses patologi intraabdomen. Fokus penatalaksanaannya pada upaya mempertahankan keseimbangan cairan dan elektrolit, mengistirahatkan intestin dan mendukung nutrisi, pengobatan medis, melindungi kulit sekitar luka, dan menampung cairan fistula. Strategi menampung cairan fistula merupakan komponen utama pada penatalaksanaan fistula non pembedahan. Studi kasus ini menggambarkan upaya\untuk menampung cairan fistula, memberikan kenyamanan, memfasilitasi penyembuhan luka, dan melindungi kulit sekitar luka seorang laki-laki berusia 66 tahun dengan luka dehiscence dan fistula di dalam luka. AbstractA fistula is an abnormal passage between the lumen of a hollow viscous organ and another hollow organ or the skin. Gastrointestinal fistula present as devastating complication following postoperative abdominal surgery and as secondary manifestation due to primary intra-abdominal pathologic processes. Management challenges focus on maintaining fluid and electrolyte balance, providing bowel rest and nutrition support, initiating medication treatment, ensuring skin protection, and containing the fistula effluent. Containing the fistula effluent is a key component of non-surgical fistula management. Following extensive abdominal surgery, the wound of a 66 years old man dehisced and a gastrointestinal fistula formed inside the wound. This case study describes efforts to contain fistula drainage, while providing comfort, facilitating wound healing, and protecting the periwound skin.

Abstract

Hipoglikemia merupakan salah satu efek samping pemberian terapi insulin pada pasien Diabetes Melitus tipe 2. Penelitian ini bertujuan untuk mengembangkan dan menguji keefektifan lembar pemantauan insulin terintegrasi dalam memantau kejadian hipoglikemia pada pasien DM tipe 2 yang mendapatkan terapi insulin analog. Hasil menunjukkan rerata kejadian hipoglikemia lebih tinggi saat makan pagi (t (18)= 2,9732; p= 0,0059) dan sore (t (18)= 3.0956; p= 0,0043) setelah pemberian rapid-acting insulinmelalui pemantauan lembar insulin terintegrasi Namun, hasil penelitian ini juga menunjukkan bahwa tidak ada hubungan signifikan antara pemantauan asupan kalori, dosis insulin, dan perubahan pola tidur dengan kejadian hipoglikemia. Sehingga lembar pemantauan insulin terintegrasi tidak efektif dalam mencegah kejadian hipoglikemia. Penelitian ini merekomendasikan perawat untuk lebih memahami pengkajian hipoglikemia dalam lembar pemantauan insulin terintegrasi. Kata Kunci: Kejadian hipoglikemia, lembar pemantauan insulin terintegrasi, pasien DM Tipe 2, terapi insulin Abstract The Effectiveness of the Integrated Insulin Monitoring Sheets to Decrease Hypoglycemia Incidence on the Diabetes Mellitus Type 2 Patients. Hypoglycemia is one of the side effects of insulin therapy administration for Type 2 Diabetes Mellitus patients. The study aimed to develop the integrated insulin monitoring sheets and examine its effectiveness in identifying and recording the incidence of hypoglycemia who received analog insulin therapies. The results showed that the integrated insulin monitoring sheet recorded the significantly higher incidence of hypoglycemia during the morning (t (18)= 2.9732; p= 0.0059) and evening (t (18)= 3.0956; p= 0.0043) meals after the administration of rapid-acting insulin). The record integrated insulin monitoring sheet, revealed no significant relationships between the incidence of hypoglycemia and the monitoring of calorie needs, insulin therapy administration, blood glucose, and level insomnia incidences. Abstract

COPD patients often experience shortness of breath as a result of inflammation in the lungs that develops in the long term. COPD is generally characterized by difficulty breathing, coughing with phlegm, and wheezing or wheezing. In this condition, the patient often finds a comfortable position to reduce shortness of breath. The aim of this study was to look at appropriate and effective interventions in reducing spasms in COPD patients. This literature study was created by analyzing scientific articles, at least in retrospective research from 2012 to 2020 and in English. Data obtained from databases include Pubmed, CINAHL, Scopus, Proquest and Google Scholar with the keywords position for breathing in COPD, position forward lean trunk for short breathing in COPD, and best position for COPD. The results of this literature obtained 5 articles that match the inclusion and exclusion criteria. The results have been found that the forward leaning position is effective in reducing shortness of breath in COPD patients because of the involvement of the respiratory accessory muscles so that the pulmonary ventilation process is good.Key words: Position for breathing, forward leaning position, COPD

Abstract

Kecemasan dapat menjadi komplikasi sebelum pembedahan dan dapat memengaruhi pemulihan pasca operasi sehingga menambah lamanya hari rawatan pasien di rumah sakit. Kecemasan sebelum operasi, mengubah status metabolisme pasien dan memengaruhi respon stres perioperatif. Tujuan evidence based nursing (EBN) untuk mengidentifikasi efektifitas mendengarkan Al-Qur’an surat Al Waqi’ah dan terjemahannya dalam mengurangi kecemasan pada pasien bedah digestif. EBN ini menggunakan quasi eksperimen yang melibatkan 8 partisipan di ruang perawatan bedah digestif dengan penilaian pre dan post intervensi, serta 30 menit sebelum pasien ke kamar operasi. Hasil analisis uji independent t-test, bahwa ada perbedaan signifikan rerata tingkat kecemasan berdasarkan STAI dengan p=0,03 (CI 95%). Kesimpulan: Mendengarkan Al-Qur’an surat Al Waqi’ah dan terjemahannya dapat dijadikan intervensi keperawatan manajemen menurunkan tingkat kecemasan pada pasien sebelum menjalani bedah digestif

Abstract

Cataract surgery is the most common surgical procedure performed to restore visual function due to opacification of the lens. One of the side effects of cataract surgery is the appearance of dry eye symptoms that can occur one to three weeks after surgery. Dry eye symptoms were reduced visual acuity can affect daily work, so that it can interfere with work productivity and quality of life. This paper is a literature review to identify dry eye management in patients after cataract surgery. The search was carried out on Web of Science, EBSCO, PubMed, and Google Scholar. The results showed that the management of dry eyes can be done with drugs, healthy living behaviors especially maintaining eye health and hygiene and also avoiding various risk factors for dry eyes. Assessment of early signs and symptoms of the dry eye needs to be done before surgery so that it can reduce the risk of dry eye events. Management of dry eye in post-cataract surgery patients needs to be done comprehensively so that it can improve the patient's visual ability and a better quality of life. Abstract

Abstract. Total Knee Arthroplasty (TKA) is an effective surgical treatment in patients with moderate to severe knee arthritis. This surgical intervention has the potential to cause blood loss of up to 1000cc, which can lead to a decrease in haemoglobin levels and postoperative functional abilities. There are many methods to reduce the amount of blood loss after TKA surgery, one of the most effective and efficient methods is to position the knee flexion 45° for 24 hours post-operative TKA. This study was the application of Evidence Based Nursing (EBN) with a sample of 10 that meet theInclusion criteria were patients with knee osteoarthritis stage III and IV and underwent TKA Total Knee Arthroplasty (TKA) is an effective surgical treatment in patients with moderate to severe knee arthritis. This case series aims to present the result of applying evidence-based leg positioning after TKA to reduce blood loss with a sample of 10 that meet the Inclusion criteria were patients with knee osteoarthritis stage III and IV and underwent TKA (Total Knee Arthroplasty) surgical procedures. The results show that all respondents are female, most of them are <60 years old (60%), and BMI in the overweight category is 70%, the amount of Hidden Blood Loss is in the range of 420cc-700cc, 30% of respondents have post-op Hb levels <10gr/dl, and 60% of respondents have a ROM value of 90° on day 5 Post Operation. The Implementation of EBN Knee flexion position for 24 hours post TKA effective to reduce the amount of blood loss and improve the patient's functional ability by facilitating the patient to start rehabilitation exercises early

Abstract

This study aims to identify and analyze the effect of acupuncture therapy on diabetic foot disorders. The design of this study was a systematic review by searching five scientific databases to obtain relevant sources related to specific problems related to acupuncture therapy in reducing disorders of the diabetic foot, namely Pubmed, Scientidirect, Springer Journals, Scopus, and Sage Journals. The research results were 3103 articles, and five articles were relevant and met the criteria. The use of acupuncture therapy affects disorders of the diabetic foot, namely a significant reduction in symptoms such as a prickling sensation or burning feet, numbness, to pain, and as an alternative therapy option in diabetic neuropathy sufferers. In conclusion, acupuncture effectively treats diabetic foot disorders and can be applied to nursing care as a non-pharmacological therapy. Keywords: Acupuncture, Diabetic Foot, Neuropathy

Abstract

Psychoreligious intervention of murottal Al-Qur'an is a therapy given to patients by listening to verses of the Qur'an for a few minutes to have a positive impact on the patient's physiological and psychological condition, thereby providing a relaxing effect to reduce pain and stress level. The purpose of this study was to determine the effectiveness of psychoreligious intervention of murrotal Al-Quran on pain and stress level of bone cancer patients in Semarang Regency. This was a quasi-experimental study with pre and post-test through a control group design. The population of this study were patients with bone cancer who were undergoing outpatient care in Semarang Regency Hospital, living in the regency of Semarang. The sampling technique used was purposive sampling with the number of respondents obtained 36 patients. The data were collected through Numeric Rating Scale aiming to measure the pain scale and DASS 42 questionnaire consisting of 14 statements to measure the level of stress. The therapeutic action procedure of psychoreligious intervention of murrotal Al-Quran used Ar-Rahman letter for 20 minutes which was in accordance with standard operating procedures. The data were analyzed using t-test. The results showed that there was a significant difference of pain and stress level before and after the intervention. Psychoreligious intervention of murottal Al-Qur'an was effective in reducing pain level in bone cancer patients (p-value=0.001<α=0.05). Psychoreligious intervention of murottal Al-Qur'an can effectively reduce stress level of bone cancer patients (p-value=0.001<α=0.05). Psychoreligious intervention of murrotal Al-Quran can be used as a therapeutic modality to reduce the pain and stress level of bone cancer patients during home treatment period which brings positive impact to improve the patient's physiological and psychological well-being. This intervention will help the process of recovery of the patients from their cancer.

Abstract

This article shows that an integrated and compatible independent nursing intervention model approach is important to be able to hope of providing a strong theoretical basis for nurses on improving the wellbeing of acute coronary syndrome patients through a conceptual model of integrated nursing interventions during treatment in the hospital. The purpose of this article is to present a simple theoretical construction using a 7-step approach which includes: origin, meaning, logical adequacy, usefulness, generaliability and parsimony as well as testability, about integrative nursing intervention theory in providing comfort to improve patient well being. This type of research is a development research model with a simple theory construction model approach. The problems faced by patients with acute coronary syndrome most often are chest pain, anxiety and hemodynamics which can interfere the patient's well-being. Chest pain that is occurred due to coronary blockage has an impact on the fear of death and can affect hemodynamic stability. Anxiety is the most prominent stress and greatly disturbs the patient's psychological condition. The integrative nursing intervention model becomes a model approach that can be applied by nurses in nursing care services for patients with acute coronary syndrome. Therefore, the role of nurses as service providers should be able to make the best contribution through the application of an integrative nursing intervention model. This intervention model approach is expected to be able to improve patient well being during hospitalization.
