Abstract

Kapitasi berbasis pemenuhan komitmen pelayanan (KBKP) merupakan salah satu sistem pembayaran dalam program jaminan kesehatan nasional pada puskesmas untuk meningkatkan pelayanan yang efektif dan efisien sehingga mutu layanan yang diberikan dapat terjaga. Kabupaten Jember sebagai salah satu kabupaten yang menjalankan kebijakan tersebut, namun diketahui terdapat kendala dalam pelaksanaan kebijakan tersebut. Penilaian terhadap puskesmas melalui KBKP dilihat berdasarkan pencapaian indikator yang meliputi angka contact rate, rasio rujukan rawat jalan non spesialistik, rasio peserta prolanis dan 1 indikator tambahan yaitu kunjungan rumah. Namun pelaksanaan kebijakan ini terdapat beberapa permasalahan yang dapat menghambat pencapaian target. Penelitian ini menggunakan metode Riset Implementasi. Riset ini membahas berbagai masalah implementasi dalam konteks yang beragam dimana pengambilan datanya dilakukan secara kualitatif (indepth interview) dan kuantitatif (analisis data sekunder). Evaluasi pelaksanaan KBKP tahun 2016 menunjukkan masih banyaknya tantangan dan hambatan sehingga BPJS Kesehatan mengeluarkan petunjuk teknis terkait KBKP. Tujuan penyusunan petunjuk teknis tersebut adalah memberikan panduan bersama pelaksanaan pembayaran kapitasi berbasis pemenuhan komitmen pelayanan di FKTP. Pemerintah daerah diketahui kurang terlibat dalam kebijakan KBKP tersebut, tidak terdapat kebijakan yang mendukung kebijakan KBKP di tingkat kabupaten. Pencapaian target indikator yang telah ditetapkan dalam regulasi KBKP masih sulit dicapai oleh puskesmas khususnya pada indikator contact rate, pencapaian target indicator contact rate hanya 15 puskesmas (30%). Pencapaian indikator rujukan non spesialistik yang telah ditetapkan dalam regulasi KBKP selalu dapat dicapai oleh puskesmas (100%), namun indikator prolanis target pencapaiannya hanya 38 puskesmas dari 50 puskesmas yang dapat mencapai (76%). Sulitnya pencapaian target indikator contact rate karena petugas kesehatan di puskesmas tidak sempat meng-entry data kontak sehat dan kontak sakit pada aplikasi P Care. Diketahui KBKP dapat meningkatkan kepuasan peserta karena memaksa puskesmas untuk meningkatkan contact rate dengan peserta JKN dan merasa di ‘spesial’kan dengan program prolanis. Kebijakan KBKP juga dapat meningkatkan mutu pelayanan puskesmas karena puskesmas harus meningkatkan sarana prasarana agar 145 diagnosis tersebut dapat diselesaikan.

Abstract

The child caring constitutes the deeds conducted by their family (mother, father, grand mother or other person) in giving the food, taking care of the health, giving stimulate and emotional support required for growth and development. Arjasa is one of the sub districts having high percentage of poor families and a nutrition vulnerable area. However, we can still find children under five years of age having good nutrition status. This is the reason why the writer is interested in studying further about the case of how among the aforesaid poor families are still able to prioritize the nutrition for their under five year children (positive deviance). The first step taken in this research was selected the subject of research by using the purposive method. It was intended to get the subject with the criteria of mothers from poor families, having the children of 6 to 59 months of age with good nutrition status and in possession of Kartu Menuju Sehat/KMS (Growth Monitoring Chart). The second step was carried out exploration. It was intended to explore and in this research three approaches had been applied, namely by combining the three methods of in depth interview, documentation study, and observation to cross check the data as well as to keep validity of the data. The third step taken in this research was applied the qualitative analysis. It was intended to analyze the practice of nutrition caring pattern by the family upon good nutrition status on children of 6 to 59 months of age at the poor families (positive deviance) at Sub-district of Arjasa, Regency of Jember. The result of this research showed that the factors that influence positive deviance at Sub-district Arjasa is the role of health officer, in this case is midwife in giving motivation to others, whether through certain approach at Posyandu or through individual approach. Abstract

Problem high-risk sexual behavior that teens when dating is the responbility of everyone, especially parents.. The parental presence of effective communication and appropriate parenting pattern is expected to cope with the increasing high-risk sexual behavior in adolescents. This study attempts to analyze the parental presence by sexual behavior risky on student dating in SMPN Sukorambi Jember Regency. The research is the study analytic by design cross-sectional. 101 students of SMP Sukorambi who claimed to have/was in the dating. The results showed that characteristics of respondents there are two variables related to sexual behavior during dating ie sex and age variable. The variable of parental presence both has a significant relationship with the sexual behavior of parent’s communication and parenting. Teen’s sexual behavior, when dating is classified as high-risk behavior, is mutually attached to the genitals with limited clothing. From this study, it was concluded that the parental presence through effective communication and discussing about sexuality and educating with autoritative parenting can prevent adolescents from high-risk sexual behavior when dating.

Abstract

Indonesia has experienced an increasing in contraceptive use. However, it is not in line with a decreased unmet need for family planning. The decentralization policy for family planning may lead to lower unmet need among poor women.  This research aims to analyze impacts on decentralization policy for family planning in meeting contraceptive needs for poor households. This study was a survey panel using the Indonesia Demographic Health Survey (IDHS) of 2007. The subjects were all married women aged 15-49 years old. Data analysis used a multilevel modeling technique to assess the effect of individual and community factors on the unmet need for family planning. The unmet need in Indonesia was 9.2%, 8.6% and 9.1% for the year of 1997, 2002-2003 and 2007, respectively. Poorest women had higher odds of unmet need than richest (odds ratio, 1.4). The odds were also higher among women who never attending school, lack of contraceptive knowledge, unexposed with family planning information and problems in the access. In community level, the odds of unmet need was smaller when a woman lived in a community with higher mean of family planning knowledge. The implementation of decentralization policy on family planning was reduced the odds of unmet need for poor households in which the policy is then followed by the improvement of family planning knowledge for individual and community. Abstract

Pregnant woman has a risk of mortality; therefore, it is necessary to prevent the maternal mortality by pregnancy test. PKH (Program Keluarga Harapan) is a program that supporta pregnancy test. This study aims to determine the different of antenatal care visits between pregnant women PKH participants and non PKH participants in Kalisat, Jember. This study was analytic with cross sectional approach. The data was analyzed by using chi-square test. The sample of this study were 36 pregnant women in each group randomized in 2016.The results showed that there was a significant difference between occupation status and accessibility to the antenatal care visit (completeness) between pregnant women PKH participants and non PKH participants where pregnant woman PKH participant who unemployed and have easy access are visit antenatal completely compared to pregnant woman non PKH participants. There is a difference between occupation status, husband support and accessibility to antenatal visit (accuracy) between pregnant women PKH participants and non PKH participants where pregnant woman PKH participants who unemployed have more visit antenatal care completely, and pregnant woman non-PKH participants with high husband support and easy accsess will be more appropriate visit compared with pregnant woman PKH participants. There were no differences in age, parity, education, family income, and medical history with antenatal care visit between pregnant women of PKH participants and non PKH participants.

Abstract

Globally, the HIV and AIDS epidemic shows stable conditions, but statistically there is still an increase in new infections in some areas including Jember District. The main barriers to HIV prevention are stigma and discrimination against people living with HIV. This study aims to analyze the predictors of stigma and discrimination attitude toward PLHIV in Jember District. This was an observational study used cross sectional design. Research subjects were people aged 15-24 years amounted to 247 respondents. Determination of the sample using a multistage sampling technique, ie the determination of simple random cluster of villages and then determine the respondents of each cluster randomly systematic. Data were collected using structured interview technique with questionnaire then analyzed using chi-square test and logistic regression with 5% significance level. The results showed the majority of respondents (81.4%) stated not willing to buy food if the seller knew HIV +. Fifty percent more they disagree if female teachers who are HIV + keep teaching. Similarly, children with HIV + cannot be admitted to school with HIV negative children. The distribution of stigma and discrimination attitudes toward people living with HIV significantly differed by age, sex, education level, marital status and occupational status. The level of knowledge and residence does not differ significantly. A significant factor to be a predictor of stigma and discrimination against people living with HIV is age and gender. Younger age (15-19 years) and women have a greater likelihood of being stigmatized and discriminating against people living with HIV. Dissemination of information about HIV and AIDS in adolescents aged 15-19 years is still needed to increase knowledge and accelerate the acceptance of PLHIV so that stigma and discrimination attitude toward PLWHA can be reduced. Abstract

Badan Layanan Umum Daerah yang selanjutnya disingkat BLUD adalah Satuan Kerja Perangkat Daerah di lingkungan pemerintah daerah yang dibentuk untuk memberikan pelayanan kepada masyarakat berupa barang dan/atau jasa yang dijual tanpa mengutamakan mencari keuntungan, dan dalam melaksanakannya kegiatan didasarkan pada prinsip-prinsip efisiensi dan produktivitas. Peraturan Menteri Dalam Negeri Nomor 79 Tahun 2018 suatu Unit Pelaksana Teknis Daerah dapat menerapkan BLUD apabila meyelenggarakan pelayanan umum baik sebagai penyediaan barang maupun jasa layanan umum kepada masyarakat terutama untuk penyedia pelayanan kesehatan. Unit – unit di bidang kesehatan yang dapat menerapkan BLUD diantaranya adalah puskesmas, rumah sakit, dan Laboratorium Kesehatan Daerah (Labkesda). Penyelenggaraan BLUD di Puskesmas bertujuan untuk memberikan keleluasaan bagi pemberi pelayanan kesehatan dalam memberikan dan meningkatkan pelayanan kesehatan sesuai dengan kebutuhan masyarakat di wilayah puskesmas. Akan tetapi untuk dapat menerapkan kebijakan BLUD tersebut puskesmas harus memenuhi persyaratan yang telah ditentukan baik itu persyaratan substantif, teknis, maupun administratif serta kesiapan struktur birokrasi, sumber daya organisasi, dan budaya organisasi. Analisis dilakukan terhadap 10 puskesmas terpilih di wilayah Kabupaten Jember yaitu Puskesmas Kaliwates, Ajung, Klatakan, Patrang, Sumberbaru, Lojejer, Kalisat, Silo 2, Sukowono, dan Sumberjambe. Hal ini dilakukan mengingat belum adanya puskesmas berstatus BLUD di wilayah tersebut. Mayoritas puskesmas sudah mendapatkan sosialisasi dan siap meningkatkan status mejadi BLUD namun perlu adanya dasar regulasi yang kuat, sumber pembiayaan dan melengkapi fasilitas dan sumber daya manusia terutama tenaga akuntan yang dibutuhkan untuk mendukung terbentuknya puskesmas BLUD. Puskesmas juga diharapkan melakukan survey kepuasan pelanggan sebagai salah satu indikator persyaratan yang harus dipenuhi untuk mengajukan diri sebagai puskesmas BLUD
