Abstract

Background: Pharmacists play a role as a part of health services both at pharmacies, hospitals and community health centers. Pharmaceutical care was regulated in the Regulation of the Minister of Health of the Republic of Indonesia. In the internship, students of the Pharmacists program Faculty of Mathematics and Natural Sciences, Udayana University can see clearly how the implementation of the role of Pharmacists in the Pharmacy, Hospital and Community Health Center. Objective: This study aims to assess the perceptions (responses) of Pharmacist students on the role of Pharmacists in various pharmacy services. Methods: This study used a one-group posttest only design methods. The population of the study used all of the Pharmacist students who were internship at the Pharmacy, Community Health Center and Hospital. The research instrument used a closed-ended questionnaire about the perceptions of Pharmacist students on the role of Pharmacists in internship locations. Results: The results showed that the role of pharmacists in pharmacies and community health centers was greater in the field of clinical pharmacy services. While in hospitals, pharmacists have a greater role in the field of drug management. Conclusion: Students have a positive perception of the role of pharmacists. There were no significant differences in perceptions of Pharmacist students on the role of Pharmacists in the field of drug management and clinical pharmacy services (p> 0.05). Abstract

Abstrak   KWT BaliCocos merupakan kelompok wanita tani di Desa Tengkudak yang memiliki potensi lahan kelapa yang luas. Namun, tingkat perekonomiannya masih belum mengalami peningkatan dari hasil kelapanya. Hal ini karena kelapa yang dihasilkan belum diolah menjadi produk bernilai ekonomis, sehingga perlu dilakukan penelitian dan pembinaan produksi VCO. Penelitian ini bertujuan untuk menganalisis hasil produksi VCO kelompok wanita tani dan melakukan pembinaan dalam mengembangkan diversifikasi produknya, sehingga memiliki daya saing dalam pemasaran. Penelitian dilakukan melalui dua tahap yaitu (1) produksi dan analisis VCO, (2) pengembangan diversifikasi produk dan strategi pemasaran. Teknik pembuatan VCO dilakukan dengan cara basah. Hasil penelitian menunjukkan VCO yang dihasilkan memiliki bau khas kelapa segar, tidak tengik, rasa normal khas minyak kelapa, tidak berwarna, mengandung asam kaproat 0,287%; asam kaprilat 6,115%; asam kaprat 5,832%; asam laurat 48,73%; asam miristat 18,505%; asam palmitat 8,754%; asam stearat 2,606%; asam oleat 10,476%; asam linoleat 0,060%, asam linolenat 0,062%, serta tidak mengandung cemaran mikroba. Hasil ini menunjukkan kesesuaian dengan SNI 7381:2008 sehingga sangat berpotensi dikembangkan produksi dan pemasarannya. Pengembangan produk yang telah dilakukan yaitu produksi VCO original dan aromaterapi. Pemasaran dilakukan melalui toko, apotik, spa, pameran, reseller, dan jejaring media sosial. Produk VCO sangat bermanfaat untuk pangan, kesehatan/pengobatan, dan kosmetik sehingga memiliki potensi pasar yang sangat tinggi.

Abstract

Diabetes Mellitus (DM) is a metabolic disease with characteristics of hyperglycemia that occurs due to insulin secretion abnormalities, insulin work or both. DM can cause macrovascular complications such as hypertension. Administration of therapy may lead to hypoglycemic and hypokalemia that may increase the risk of impaired brain and heart. Objective: The purpose of this study to analyze the incidence of hypoglycemia and hypokalemia side effects in patients with diabetic hypertension using antidiabetes and antihypertensives. Method: The study was conducted retrospectively with retrospective patient medical records from January to December 2016. Blood and potassium blood glucose levels were measured as long as the patient was treated. Random and potassium blood glucose levels prior to therapy were compared with after therapy using paired t-test. Result : From the results of the study found 27 patients suffering from diabetic hypertension and get therapy. The value of changes in the patient's random blood glucose level averaged 280.89 mg/dl to 189.15 mg/dl, which met the target of antidiabetic drug therapy, and no patients had hypoglycemia. From 27 patients, as many as 7 people did not get antihypertensive therapy. Three patients had hyperkalemia (serum potassium level > 5 mEq/L). The value of changes in blood potassium levels of patients averaged 4.74 mEq/L to 4.46 mEq/L, where no patients had hypokalemia. Conclusion : the occurrence of hypoglycemia and hyperkalemia were suspected due to side effect of insulin and antihypertensive drugs.

Abstract

Ulkus Kaki Diabetik (UKD) merupakan salah satu komplikasi dari Diabetes Melitus (DM), dimana penderita DM memiliki risiko 15% terjadinya UKD selama hidupnya. Risiko UKD akibat DM dapat dikurangi dengan melakukan penatalaksanaan terapi DM yang tepat, salah satunya dengan menggunakan obat antidiabetes baik dengan Obat Hipoglikemi Oral (OHO), Insulin ataupun kombinasi keduanya. Penelitian ini bertujuan untuk mengetahui jenis obat antidiabetes yang digunakan oleh pasien DM tipe 2 dengan UKD serta mengetahui bagaimana luaran terapi dari penggunaan obat. antidiabetes tersebut. Penelitian ini merupakan penelitian retrospektif observasional. Data yang digunakan adalah data rekam medis pasien DM tipe 2 dengan UKD antara bulan Januari hingga Desember 2018 yang diperoleh dari Instalasi Rekam Medis RSUP Sanglah Denpasar. Analisis data dilakukan secara deskriptif dan statistik (Paired T-Test). Hasil penelitian diperoleh 43 sampel yang memenuhi kriteria inklusi dan eksklusi. Seluruh pasien menerima terapi kombinasi insulin, yang terdiri dari kombinasi insulin Glargine + Aspart (n=32) dan Glargine + Glulisine (n=11). Penggunaan kombinasi Glargine + Aspart dan Glargine + Glulisine keduanya mampu memberikan penurunan yang signifikan pada kadar glukosa darah puasa dan kadar glukosa darah 2 jam post prandial dengan penurunan rata-rata dan nilai signifikansi berturut-turut yaitu 85,00±77,718 mg/dL (p=0,000); 85,656±70,276 mg/dL (p=0,000); 92,545±66,656 mg/dL (p=0,001) dan 86,818±59,189 mg/dL (p=0,001).

Abstract

Insulin merupakan salah satu terapi obat untuk pasien diabetes mellitus (DM) tipe 2. Terdapat 4 jenis insulin berdasarkan onset kerjanya. Tiap jenis insulin memiliki besaran biaya dan effektivitasnya masing - masing. Telah banyak penelitian dilakukan untuk menilai effektivitas penggunaan insulin, di lain pihak penelitian yang membandingkan biaya total tiap jenis insulin masih sangat sedikit. Tujuan penelitian ini adalah untuk mengetahui total biaya yang dibutuhkan untuk terapi insulin pada pasien DM tipe 2 rawat jalan di rumah sakit kotamadya Denpasar. Penelitian ini merupakan penelitian deskriptif yang dilakukan pada bulan Maret sampai dengan bulan Juni 2016 di rumah sakit pemerintah di Kotamadya Denpasar. Subyek penelitian adalah pasien DM tipe 2 rawat jalan. Data regimen terapi insulin dan biayanya diperoleh dari rekam medis pasien, data resep, lembar administrasi, laporan terapi pasien pada instalasi farmasi dan juga riwayat sosial pasien. Total biaya dihitung berdasarkan biaya medis langsung (jasa dokter, tes laboratorium, biaya obat dan biaya penyiapan obat serta alat kesehatan), biaya non medis langsung (biaya administrasi dan transportasi) serta biaya non medis tidak langsung (hilangnya gaji/upah karena tidak masuk kerja). Terdapat 8 nama dagang insulin yang diresepkan untuk pasien DM tipe 2, dengan masing – masing persentase penggunaannya adalah sebagai berikut : Apidra Solostar (12,50%), Humalog Mix (4,35%), Humalog Quickpen (3,80%), Humulin N (2,17%), Lantus Solostar (28,26%), Levemir (11,96%), Novomix (1,09%) and Novorapid (35,87%). Total biaya terendah regimen terapi insulin adalah penggunaan Lantus Solostar (Rp 528.480,-) dan total biaya tertinggi adalah Humalog Mix (Rp. 685.066,-). Rata – rata total biaya terapi insulin adalah sebesar Rp. 565.474,-

Abstract

Abstract Hypertension is a condition when blood pressure increases chronically. Referring to the results of Riset Kesehatan Dasar (Riskesdas), Bali has a proportion of hypertension of 19.9%. Several epidemiological studies suggest that the risk of damage to various vital organs directly correlates with an increase in blood pressure. Therefore, it takes regularity in controlling and also taking antihypertensive drugs. The most common antihypertensive drugs currently used are angiotensin converting enzyme inhibitors (ACEI) and angiotensin receptor blockers (ARBs). However, previous findings suggest that ACEI and ARB have not been fully effective in lowering blood pressure. To overcome these weaknesses, found a new direct renin inhibitor that is aliskiren. Aliskiren can block the renin-angiotensin-aldosterone system (RAAS) at the highest level, so the ability of aliskiren in lowering blood pressure can not be doubted. Aliskiren able to inhibit the conversion of angiotensinogen to angiotensin I, so it can lower blood pressure in a sustainable manner.

Abstract

ABSTRACT Type 2 diabetes mellitus (DMT2) is a medical condition characterized by an increase in blood glucose levels exceeding normal limits. World Health Organization (WHO) states that the number of DMT2 patients in Indonesia has increased from 8.4 million patients in 2000 to 21.3 million patients in 2030, which is 90% of them are patients with DMT2. Pathophysiology of DMT2 is a complex process and involves many factors. The ominous octet concept proposed by Ralph DeFronzo plays an important role to explain the pathophysiology of DMT2. The kidneys are organs that have a significant role to control blood glucose to stay within normal limits, thus being considered to be targets of new drug therapies. Sodium glucose co-transporters (SGLTs) such as SGLT-2 facilitate the reabsorption of glucose into plasma. Pharmacokinetics of the SGLT-2 inhibitors generally show good bioavailability when administered orally. DMT2 is a risk factor for chronic kidney disease (CKD) and cardiovascular disease (CVD). The presence of an inhibitory mechanism in SGLT-2 will benefit the reno-cardiovascular system through decrease in blood glucose, weight, and blood pressure. SGLT-2 inhibitors have some favorable adjunctive effects for metabolic syndrome, such as weight loss, decreased blood pressure (especially systolic), as well as decreased serum uric acid

Abstract

Introduction: Globally, drug-resistant tuberculosis (TB) still has a high number of cases. Pregnant women are one of the high-risk populations for TB infection, especially multidrug resistance (MDR)/Rifampicin resistance (RR) TB. Physiological differences in pregnant women and the safety of the fetus make drug resistance TB treatment challenging. Case: A 20-year-old woman was 22 weeks pregnant while undergoing the third month of the late phase for short-regimen. Thus, Kanamycin was one of her early phase treatment combinations during the first trimester of pregnancy. After we consulted with other departments, especially the obstetric department, we decided not to change her regimen. Her chief complaint before treatment was cough with white sputum and fluctuated shortness of breath for two months. The patient only experienced minor side effects in the early treatment phase. The patient was cured of MDR TB on 8 October 2020 at 34 weeks of gestation. Her baby was born on 10 November 2020 at 37 weeks without congenital abnormality. Observation for her baby continued by the pediatric department. The last follow-up of her baby in the middle of 2022 found no abnormality in the child’s growth and development. Conclusion: Drug-resistant TB therapy in pregnant women requires the collaboration of a multidisciplinary team to consider the benefits and risks of the therapy based on gestational age and disease severity.

Abstract

An autoimmune disorder is an abnormality that causes a disease. It is caused by a weakened immune system. One of the autoimmune diseases is Sjogren’s syndrome, which affects the salivary and lacrimal glands and causes dry mouth, dry eyes, and dry skin. Sjogren’s syndrome influences humans of every age, with the symptoms occurring at the age of 45–55 years and rarely in children. One of the factors causing Sjogren’s syndrome is genetic disorders. To identify genes that can influence Sjogren’s syndrome in this study, we used several databases, including GWAS Catalog, HaploReg Version 4.1, GTEX portal, and Ensembl, particularly to identify the gene expression profiles of TNIP1, TNFAIP3, and IRF5 and the quantitative properties of locus’ expression. This research showed that the missense variants and splice donor rs2233290, rs2230926, and rs2004640 influenced the susceptibility of autoimmune diseases, especially Sjogren’s syndrome, in the fibroblast tissue, sigmoid tissue, sigmoid colon, skin, esophagus, and adrenal glands. The allele frequency of each variant was then assessed in African, American, European, and Asian populations. Our data showed that TNIP1, TNFAIP3, and IRF5 genes in African and American populations had higher frequencies than in the Asian population. This implies that the last of the aforementioned populations might be relatively susceptible to the autoimmune disease Sjogren’s syndrome.
