Abstract

Backgrounds: In recent years, circulating tumor DNA (ctDNA) has emerged as a specific and sensitive blood-based biomarker to detect EGFR mutations. This study aims to determine the diagnostic accuracy of ctDNA in detecting EGFR gene mutations in adenocarcinoma lung cancer. Methods: This study was a cross-sectional study with the subjects were adenocarcinoma lung cancer patients from histopathology or cytology examination and examined EGFR mutations from plasma tissue biopsy and ctDNA specimens from April 2018 to February 2019 in several hospitals in the Medan City. Results: There were 100 data have been collected, with male were 71 subjects and female were 29 subjects. Found 20 mutations, single mutations of tissue biopsy were 19 cases, del exon 19 were 12 cases, mutation in exon 21 (L858R) were 6 cases, mutation exon 21 (L861Q) was 1 case, del exon 19 and 21 (L861Q double mutations) was 1 case. From plasma ctDNA examination EGFR mutations were found 15 cases, del exon 19 were 12 cases and del exon 21 (L858R) were 3 cases. Conclusions: The highest proportion of EGFR mutations by sex were women from tissue biopsy or ctDNA, the most often frequency of EGFR mutations from tissue biopsy and ctDNA in single mutations and exons19. Abstract

Background: Tuberculous pleural effusion is a paucibacillary manifestation of tuberculosis, so isolation of Mycobacterium tuberculosis is difficult, biomarkers being an alternative for diagnosis. Adenosine deaminase has the potential to optimize the diagnostic approach of tuberculous pleural effusion. Methods: This study is a diagnostic test observational (cross-sectional), which included 35 inpatient samples that meet inclusion and exclusion criteria from H. Adam Malik Medan General Hospital. Research began on February 1st until July 31st 2016 to examine 10 cc of pleural fluid specimens for the levels of Adenosine deaminase. Results: There are significant differences in the levels of adenosine deaminase from tuberculous and non-tuberculous pleural effusion (P=0.001). In the cut-off point 36.55 IU/L, level of sensitivity 95.8%, specificity 90.99%, positive predictive value 95.8%, negative predictive value 90.99%, and accuracy 94.2% of pleural fluid adenosine deaminase level test in tuberculous pleural effusion. Conclusion: Adenosine deaminase pleural fluid can be a diagnostic modality that is easy, fast, relatively affordable and applicable in the diagnosis of tuberculous pleural effusion. Abstract

Background: Patients with COPD exhibit muscle atrophy and dysfunction which lead into decondition state. This will in turn decrease maximal oxygen uptake (VO2 max) in tissues. Considered as the best predictor of cardiorespiratory endurance and survival, VO2max can be attenuated by aerobic training. This study aimed to assess the effects of lower-limb endurance training on VO2 max in COPD patient. Methods: An experimental study was conducted on 20 COPD patients in group C and D in June–July 2017 at USU Hospital. Abstract

Backgrounds: There is a wide range of individual responses to mycobacterium infection. However, the reason why certain people suffers from a disease when they are infected with mycobacterium while others remain healthy is still unknown. Genetic susceptibility is thought to be one of the important explanatory factors for individuals with the risk of developing pulmonary tuberculosis (TB). FokI polymorphism was one of the genetic variations within the vitamin D receptor (VDR) gene associated with pulmonary TB. In the present study, we investigated the association between VDR gene FokI polymorphism and pulmonary TB. Conclusion: There was no significant association between the VDR gene FokI polymorphism and pulmonary TB.

Abstract

Kasus TB RO saat ini semakin meningkat baik di tingkat global maupun Indonesia. Indonesia merupakan negara dengan urutan ke delapan terbesar kasus TB RO di dunia, di Sumatera Utara Kota Medan merupakan penderita TB RO tertinggi. Telah  dilakukan berbagai upaya untuk menangani kasus TB RO namun kasus TB RO di Kota Medan masih tetap tinggi. Penelitian ini bertujuan untuk menggambarkan karakteristik penderita TB RO dan persepsi lingkungan sekitar terhadap penderita.Desain penelitian crossectional study, populasi penelitian adalah penderita TB RO tahun 2017 sampai April 2018. Sampel ditentukan secara purposive yaitu pasien yang memulai menjalani pengobatan bulan Juli 2017 sampai April 2018 dan bersedia dikunjungi ke rumah dan mendapat sms reminder untuk meningkatkan kepatuhan berobatnya, diperoleh sampel 24 orang dari 60 pasien yang menjalani pengobatan dari tahun 2017 sampai April 2018. Data dikumpulkan dengan wawancara kepada responden dan dianalisis secara deskriptif. Hasil penelitian menunjukkan bahwa ; laki-laki lebih banyak dari perempuan, umur pada usia produktif, tingkat pendidikan dominan SLTA, suku terbanyak suku Batak dan status perkawinan menikah. Umumnya responden mengalami efek samping obat dari yang ringan sampai berat.  Setengah dari responden mengalami gangguan penyerta lebih dari satu jenis seperti asam urat, hipertensi, DM maupun HIV. Persepsi lingkungan terhadap penderita hanya sebagian kecil 1 orang (4,2%) mengalami perubahan status perkawinan, sebanyak 12 orang (50%) mengalami perubahan pekerjaan, perubahan status ekonomi 10 orang (41,7%), perubahan sikap keluarga 5 orang (20,8%), perubahan sikap masyarakat 1 orang (4,2%) dan  mendapatkan bantuan 12 orang (50%).Disarankan agar penderita patuh minum obat sampai sembuh, berkomunikasi dengan petugas kesehatan jika mengalami efek samping obat yang berat agar dapat berkonsultasi dengan Tim Ahli Klinis, dapat mencegah penularannya pada keluarga dan lingkungan.  Diharapkan dukungan psikologis, sosial dan ekonomi  dari keluarga dan lingkungan sekitar agar penderita dapat menjalani pengobatannya sampai sembuh.
