Abstract

Background: Nasopharyngeal carcinoma is the most common malignant tumor among malignant ENT tumors in Indonesia, where nasopharyngeal carcinoma is included in the top five largest malignant tumors with the highest frequency, whereas in the head and neck areas occupy the first place. The purpose of this study is to know the 3D-CRT management in patients with nasopharynx carcinoma and to know the result of manual radiotherapy verification process using Image Computed Radiography Portal at Sub Department Radiotherapy Rumkital Dr. Ramelan.Methods: This research type is qualitative with case study approach. The study subjects were 1 person nasopharyngeal carcinoma patients. The subjects of the study were 2 person oncologist, 1 medical physicist, 3 radiotherapy radiograph (RTT). Data were collected by observation, interview and documentation. Data analysis is done descriptivelyResult: The results showed management of 3D conformal nasopharyngeal carcinoma in Rumkital Radiotherapy Sub Department was in accordance with the literature and manual radiotherapy verification with Image Computed Radiography (PICR) Portal in Sub Department of Radiotherapy. Ramelan result can be accepted as replacement of Electronic Image Image Device (EPID).Conclution: result of management 3D conformal nasopharyngeal carcinomal can be accepted as replacement of Electronic Image Image Device (EPID)

Abstract

Background: In Indonesia, nasopharyngeal cancer ranks 4th most in malignancies. As a method of treatment, the development of radiotherapy has made it possible to give high doses to tumors with little risk of healthy tissue, but still maintain accuracy by performing geometry verification procedures. The purpose of this study was to determine the geometry verification procedure of nasopharyngeal cancer with EPID on the Linac plane in RSUP Dr. Kariadi Semarang; the average geometric shift that occurs and why is only done before fractions 1 and 4 only.Methods: This type of research is qualitative with a case study approach. The data is taken from February 2019 to June 2019 by the method of observation, documentation and interviews. The data obtained were analyzed by interactive models, making transcripts of interviews then reduced and processed in the form of open coding, presented in the form of quotations and concluded.Results: The results showed that the geometry verification procedure was started by making a calendar treatment, adjusting the patient's setup at the origin point, switching to the iso center point. Take the image portal with EPID AP and Lateral projections. Match image portals with DRR images. Then the geometric shift data were obtained with a mean shift from the iso center in 5 patient samples: vertical axis 0.15 cm to superior; longitudinal -0.01 cm anteriorly and laterally 0.04 cm to the right. Tolerance limit of 0.3 cm. This verification is only done before fractions 1 and 4 because of the high service load.Conclusion: The geometry verification procedure has been going well with the results of the shift is still below the tolerance limit. Verification information before the 1st and 4th fractions was not enough to assess the accuracy of the irradiation carried out properly maintained.

Abstract

Background: Research on the design of fixation devices as well as cassette holders for projection of abdominal radiography in LLD (left lateral decubitus) in non-cooperative patients is motivated by difficulties in fixing patients and placing tapes by radiographers during LLD (left lateral decubitus) radiographs with non-patient conditions cooperative. The purpose of making a design is to replace the patient's family role in holding patients and tapes during the examination.Methods: This type of research is applied research. The method used in data collection is designing tools, using tools, testing the performance of tools, testing the function of tools. The performance test and the function test of the tool were carried out directly on the patients at the RSK Radiology Installation Ngesti Waluyo Parakan. The results of the performance test and function test were analyzed based on the check list of respondents regarding the work value of the tool.Results: The design of this fixation tool consists of a supporting iron component, patient stand, cassette holder, strap strap. The performance test of the tool is in accordance with the specifications of the tool made by the author. Function tests are divided into two, namely function tests based on the technique of using tools and based on the radiograph produced. Function tests based on the use of tools techniques obtained a total work value of 3.1 can be concluded the function of a tool based on the technique of good use.Conclusions: Function tests based on the resulting radiographs obtained a total work value of 3.33 can be concluded that the function of a tool based on the resulting radiograph is good

Abstract

The rate of exposure to X-ray radiation on the radiation shielding wall at the Laboratory 3 of Radiology Study Program Purwokerto Diploma Three Program has been analyzed, to find out the difference in the effectiveness of the radiation shielding wall between a 10 cm thick Lead (2 mm) coated partition and a 28 cm thick stucco brick wall. Measurements were made using a radiation source, namely a mobile unit X-ray machine with a Fluke survey meter radiation measuring instrument. Measurement of the rate of exposure to X-ray radiation is carried out by adjusting the distance of the radiation source with the radiation shield wall from 100 cm, 150 cm and 200 cm and taken from 5 measurement points that represent the radiation shield wall. The measurement results show that measurement point C produces the highest radiation exposure rate and measurement point A produces the lowest radiation exposure rate for Do and D. There is a radiation exposure rate of more than 1 μSv/Hr after passing through a 10 cm thick Lead-coated partition wall (2 mm) at a distance of 100 cm to 150 cm. Radiation shielding walls of walls covered with stucco bricks with a thickness of 28 cm were more effective than partitions covered with lead (2 mm) with a thickness of 10 cm. It is necessary to pay attention to aspects of radiation protection, the use of a mobile unit X-ray machine in placing the X-ray tube in the direction of the X-ray tube and, the radiation source distance of at least 2 meters from the lead-coated partition radiation wall.

Abstract

Bitewing radiography is a valuable tool in detecting and monitoring dental decay and other oral health problems and is typically performed as part of a routine dental examination. It has certain limitations that should be considered. Some of these limitations may cause discomfort or pain to the patient if not positioned correctly. Some patients may find biting down on the film holder difficult or have a strong gag reflex, making the procedure uncomfortable or even impossible. Bitewing radiography can be costly, especially if it needs to be performed regularly, which may limit access to this diagnostic tool for some patients. This study aimed to evaluate the effect of a dental X-ray positioner with a silicone layer on the quality of radiographic images in posterior bitewing dental examinations. Using this positioner was expected to improve the precision of radiographic interpretation and subsequent patient treatment. The study used a multivariate general linear model to analyze the data obtained from radiographic images using the X-ray positioner with the silicone layer and the conventional X-ray positioner. The results showed no significant difference in image quality between the two positioners, indicating that adding the silicone layer did not significantly improve image quality. However, using any X-ray positioner is still beneficial in ensuring accurate radiographic interpretation and subsequent patient treatment.
