Abstract

Informations concerning imunological activity in subclinical leprosy is still lacking. Therefore, the objective of this study was to find out the prevalence of subclinical leprosy and interferon- (IFN-) production at several stages of leprosy. This observational study using a cross sectional approach was conducted in Semarang, Central Java, Indonesia. Twenty-six leprosy patients and 62 relatives who lived together with the leprosy patients were included in the study. The venous blood samples were taken from all (88) of the subjects. All of the venous blood samples were measured for IFN- levels by Elisa method. Sixty two blood samples from the relatives who lived together with the leprosy patients were measured for IgM anti PGL-1 to determine the subclinical stage of leprosy. Thus, there were three groups in this study: leprosy patients (LP), subclinical leprosy (SL) and sero-negative subjects (SN). Thirty three of the 62 relatives (53.2%) were in subclinical stage of leprosy, while 29 others were not (sero-negative). There was a significant difference (P= 0.001) in IFN- production capacity among the three groups (LP,SL and SN). Among the three groups, the LP group had the lowest, while SL group had the highest IFN- production capacity. Abstract

BackgroundIn 2013, the prevalence of stunting in Central Sulawesi province was 41%, which was higher than the national prevalence of 37.2%. Complementary feeding practice, infectious disease, history of exclusive breastfeeding, birth weight and birth length were assumed to contribute to the prevalence of stunting. The objective of the present study was to identify the risk factors for stunting among children aged 12 to 24 months.MethodsThis study was conducted using a case control design. Subjects were children aged 12-24 months, consisting of 58 cases and 58 controls. Data were obtained with a body length infantometer. Data on complementary feeding practices, history of exclusive breastfeeding, and history of infectious diseases were obtained using questionnaires. Nutrient intakes were measured by semi quantitative food frequency questionnaires. Data on birth weight and birth length were obtained from the buku KIA (maternal and child health record). Data were analyzed by chi-square and logistic regression tests. ResultsThere was no difference in sex, BMI, and parental education between cases and controls (p>0.05). After controlling for history of exclusive breastfeeding and birth weight, the risk factors for stunting were inappropriate complementary feeding practice in terms of quantity and quality (OR=8.26; 95% CI: 2.69-25.44), history of diarrhea (OR=4.73; 95% CI: 1.08-20.69), birth length (OR=5.11; 95% CI: 1.69-15.46) and respiratory tract infection (OR=5.30; 95% CI: 1.03-27.23). Inappropriate complementary feeding practice was the most dominant factor for stunting.ConclusionInappropriate complementary feeding practice increased the risk of stunting in 12-24 months old children by 8.26. This study confirms the need to scale up interventions during the first 2 years of life, including appropriate infant feeding practices.

Abstract

BackgroundPre-eclampsia is the largest direct cause (42%) of maternal mortality death in Brebes district in 2013. The cause of pre-eclampsia is not known with certainty, oxidative stress being one of its causes. The aim of this study was to determine macro- and micronutrient intakes that do not correspond to the needs, as pre-eclampsia risk factors in post-partum women. MethodsThe research was an observational study of case control design. Study subjects were 20-35 year-old post partum women consisting of 55 cases (pre-eclampsia) and 55 controls (non-pre-eclampsia). Nutrient intakes were measured using semi-quantitative food frequency questionnaires and data on levels of pesticide exposure, body mass index (BMI), mid-upper arm circumference (MUAC) and education were collected from structured interviews. Data was analyzed by chi-square tests and multiple logistic regression. ResultsThere was no difference in age and MUAC between the cases and controls. Low vitamin C intake (OR: 20.93; 95% CI: 2.72 – 161.36), high polyunsaturated fatty acid intake (OR: 10.50; 95% CI: 2.47 – 44.57), and low vitamin E intake (OR: 8.57; 95% CI: 2.07 – 35.46) were pre-eclampsia risk factors after controled for pesticide exposure, BMI and education. Low vitamin C intake played the most important role in the incidence of preeclampsia.ConclusionLow vitamin C intake played the most important role toward pre-eclampsia. Greater caution is needed when issuing recommendations regarding the consumption of vitamin supplements in pregnancy, as high doses of some vitamins may be deleterious.

Abstract

Background: The prevalence of underweight and severe malnutrition on under-five children in Southeast Sulawesi based on the latest Riskesdas was 23.9%, which higher than the national prevalence (19.6%). This study aimed to analyze risk factors of growth faltering on infants aged 6-12 months in Tongkuno. Methods: This was a cohort prospective study. The numbers of subjects were 79 infants: 32 infants who were given early complementary feeding and 47 infants who were not. Consecutive sampling technique was used in this study. The adequacy levels of energy and protein intake were collected using 2x24 hours food recall. The incidence of upper respiratory tract infection was recorded through interviews using structured questionnaires. Data were analyzed by logistic regression method. Results: There were 46.9% of the infants who had growth faltering among the early complementary feeding group. Multivariate analysis showed that the early complementary feeding, low levels of energy adequacy, and frequent URTI were the risk factors for growth faltering. The frequent URTI was the most influential factor for growth faltering (OR= 5.52; 95%CI: 1.63-18.72). Conclusion: The frequent URTI was the most important factor for growth faltering on infants aged 6-12 months in Tongkuno. Increased exclusive breastfeeding promotion and URTI prevention are recommended. Keywords: Risk factors, Growth faltering, Infants

Abstract

Background: Most of estimation height study was calculated with one predictor. This research tried to arrange regression model by combining two predictors. Methods: The type of study was observational with cross sectional design. The study population is aged 60-69 years old in Wonogiri District. The sample size of 136 included 65 men and 71 women with purposive sampling technique. Data collection technique is observation on anthropometry measurement. Data analyzed with Pearson Product Moment, Unpaired T-test and Linear Regression. Results: Arm span is positively correlated with value of p<0.001; r=0.886 in men and p<0.001; r=0.992 in women. Knee height is positively correlated with p value <0.001; r=0.989 in men and p<0.001; r=0.986 in women. The height estimated from the arm span–knee height formulas: Women height (cm) = 40.915+[0.457 x AS (cm)]+[0.818 x KH (cm)]. Men height (cm) = 34.426+[0.513 x AS (cm)]+[0.813 x KH (cm)]. The highest overestimation happen at Chumlea’s formula in men at women group (6.01 cm) and highest underestimation happen at Fatmah’s formula in women group (-0.72 cm). Conclusions: Arm span is more accurately used in women, whereas knee height is more accurate in men. Predictor combinations can be used to predict height more accurately.

Abstract

Total-Cholesterol (TC) to HDL-Cholesterol (HDL-C) ratio in athletes is well-known to be better than sedentary people. However, information about comparison of TC/HDL-C in different groups of cyclists based on cycling touring characteristics and anthropometry parameters is lacking. This study aimed to compare TC/HDL-C ratio between groups based on the type of tour in cyclists population. Eighty-eight participants were recruited in this cross-sectional study. Mean differences of parameters between groups was analyzed by One-Way Anova and independent t-test, whereas multivariate analyses was conducted by binary logistic-regression. P value <0.05 was considered statistically significant. TC/HDL-C ratio in all groups were less than 4.5. There is no differences of TC-levels (NC240K: 216.6±55.04, TdB140K: 208.1±27.13, TdB100K: 203.1±31.95; p=0.427). But there is significantly different level of HDL-C (NC240K: 68.9±19.09, TdB140K: 52.1±13.9, TdB100K: 53.6±12.45; p=0.0001) and TC/HDL-C ratio (NC240K: 3.3±1.12, TdB140K: 4.2±1.07, TdB100K: 4.0±1.06; p=0.007). Between TC/HDL ratio groups (≥4 or <4), there were differences of weight, BMI, waist-circumference, hip-circumference and type of tour (p<0.05). Finally, BMI and type of tour were the most influential factor. Long-distance cyclists have a synergistic effect of lipid profile and anthropometry measurements, and heavier cycling tour participant, that represent cycling training habits, tend to have lower TC/HDL ratio(< 4).

Abstract

Coverage of K4 in Semarang district in 2009 had not reached the target (90.1%). Preliminary study indicated that the average village midwives compliance to the implementation of antenatal service based on the standard of 7T was only 70%. The objective of this study was to know psychological and organizational factors that influenced midwives compliance to the operational standard of 7T in antenatal services in Semarang district.This was an observational study with analytical characteristic and using cross sectional approach. Data were collected by doing observation and interview using structured questionnaire. Dependent variable in this study was midwives compliance and independent variables were attitude, motivation, supervision, leadership, communication and facilities. The study subjects were 71 village midwives that were selected randomly and proportionally. Bivariate analysis was done using Rank Spearman test and multivariate analysis was done by applying logistic regression. Results of the study showed that most of respondents were between 25 and 46 years old with average of working period of 13.3 years. The average of village midwives compliance was 76%. Psychological factors, attitude and motivation of respondents, were mostly good. Organizational factors: perception towards supervision of coordinator midwives, perception on the head of puskesmas leaderships, perception on the midwives coordinator communication and facilities, were all good.Results of correlation test showed that attitudes, motivation, leaderships, communication and facilities variables were associated with compliance to the operational standard of 7T. On the other hand, perception on supervision was not related to the operational standard of 7T. Result of logistic regression test indicated that motivation, facilities and communication perception variables jointly influenced village midwives compliance.Village midwives compliance to the operational standard of 7T, especially hemoglobin and protein in the urine examinations, needed to be improved. Motivation of village midwives needed to be increased by conducting refreshing related to antenatal services. Sanction for midwives who did not comply the standard of 7T had to be applied. The number of facilities especially hemoglobin, protein in the urine examination tools and counseling media, had to be added gradually. Abstract

ABSTRAK   Rumah Sakit Islam Sunan Kudus terus mengalami penurunan jumlah kunjungan pasien. Selama beberapa tahun terakhir kunjungan pasien di poliklinik kebidanan tidak pernah mencapai target. Penelitian ini bertujuan untuk menganalisis hubungan persepsi pasien terhadap dokter, biaya pelayanan, dan akses tempat pelayanan antenatal di Poliklinik Kebidanan dengan minat kunjungan ulang pasien pelayanan antenatal. Jenis penelitian observasional analitik dengan desain  crossectional. Sebanyak 50 pasien pelayanan antenatal dipilih secara consecutive sampling. Pengumpulan data dilakukan dengan wawancara menggunakan kuesioner terstruktur. Variabel terikat adalah minat kunjungan ulang sedangkan variabel bebas adalah persepsi pasien terhadap dokter, biaya, dan akses tempat pelayanan antenatal. Analisis  data bivariat dilakukan dengan uji chi square. Hasil penelitian menunjukkan sebagian besar responden berusia 20-29 th (50%), usia kehamilan trimester 3 (50%), berpendidikan tinggi (36%), dan bekerja (72%). Persepsi pasien terhadap dokter hampir seluruhnya dalam kategori baik (94%), persepsi terhadap biaya kategori tidak terjangkau (56%), persepsi terhadap akses tempat pelayanan sebagian besar kategori mudah (86%). Sebagian besar responden (54%) tidak berminat melakukan kunjungan ulang ke poliklinik kebidanan. Hasil uji chi square menunjukkan bahwa persepsi pasien terhadap biaya berhubungan dengan minat kunjungan ulang. (p=0,0001). Persepsi pasien terhadap dokter dan akses tempat pelayanan tidak berhubungan dengan minat kunjungan ulang pasien pelayanan antenatal (p=0,054 dan 0,32). Disarankan bagi d RSI Sunan Kudus untuk menyesuaikan biaya pemeriksaan dengan kemampuan pasien. Selain itu letak poliklinik kebidanan dipindahkan ke tempat yang lebih strategis.   Kata kunci : Persepsi, pelayanan antenatal, Minat Kunjungan Ulang

Abstract

ABSTRACT Background : Adolescence is a period of growth spurt which result in the increase of nutritional needs. Physical and emotional changes often lead to body image dissatisfaction among adolescent girls.This study aimed to identify factors associated to body image and behaviors to reduce body weight among female students Methods : This survey study used a cross sectional design, with quantitative approach. This study was conducted on female students at SMAN 3 Semarang. The selection of 107 subjects was performed by stratified random sampling technique. Data on age, family characteristics, peer group influence, body image, motivation and behaviors to achieve ideal body shape collected by interviews using structured questionnaire. Weight, height and percent body fat were measured by Bio Impedance Analyzer (BIA) and microtoise. Data were analyzed by person Product Moment, Rank Spearman correlation tests and multiple liniar regression method. Results: Subjects aged 14-17 years old. One poin nine percents of subjects were obese, and 5.6% were overweight. Forty six point seven percents of the subjects were dissatisfied of their body image, eventhough 38 subjects of them were classified as normal and 7 subjects were underweight. Total of 68.2% of the subjects desired tall and slim body shape. Total of 50.4% subjects had made efforts to reach the ideal body shape in many ways, such as excessive diet (22.2%), consumption of drugs/ slimming tea (9.3%), excessive diet and exercise (37%). Percent body fat and peer group influence were the most affected factors on body image dissatisfaction. There was a possitive correlation between body image, motivation and the inappropriate behaviors to reach the ideal body shape among adolescence. Conclusions: Percent body fat and peers group influence affected body image dissatisfaction, which in turn increasing motivation in doing inappropriate behavior to attain the ideal body shape among female students. Perubahan fisik dan emosi yang tidak stabil seringkali menimbulkan ketidakpuasan citra tubuh di kalangan remaja putri. Penelitian ini bertujuan mengidentifikasi faktor yang berhubungan dengan citra tubuh dan perilaku dalam menurunkan berat badan pada remaja putri Metode : Penelitian survei ini dilakukan dengan desain cross sectional melalui pendekatan kuantitatif. Populasi adalah siswi SMAN 3 Semarang. Pemilihan 107 subjek dilakukan dengan rancangan stratifikasi sederhana. Data usia, karakteristik keluarga, pengaruh teman sebaya, citra tubuh, motivasi dan perilaku pencapaian bentuk tubuh ideal diperoleh melalui wawancara dengan kuesioner. Berat badan, tinggi badan dan persen lemak tubuh diukur menggunakan Bio Impedance Analyzer dan microtoise. Data dianalisis dengan uji korelasi Product Moment dari Pearson, Rank Spearman dan regresi linier berganda. Hasil : Subjek berusia 14-17 tahun. Sebesar 1,9 % subjek mengalami obesitas, dan 5,6% kelebihan berat badan. Sebanyak 50 subjek (46,7%) mengalami ketidakpuasan citra tubuh, padahal 38 subjek diantaranya berstatus gizi normal dan 7 subjek tergolong kurang, 68,2% menginginkan bentuk tubuh tinggi langsing, dan 50,4% pernah melakukan upaya pencapaian bentuk tubuh ideal secara tidak tepat, antara lain diet yang salah (22,2%), konsumsi obat/teh pelangsing (9,3%), diet dan olahraga berlebih (37%). Persen lemak tubuh dan pengaruh teman sebaya merupakan faktor yang paling berpengaruh terhadap ketidakpuasan citra tubuh. Ada hubungan positif antara ketidakpuasan citra tubuh dan motivasi dengan perilaku tidak tepat dalam pencapaian bentuk tubuh idea remaja. Simpulan : Persen lemak tubuh dan pengaruh teman sebaya berpengaruh terhadap ketidakpuasan citra tubuh. Selanjutnya ketidakpuasan citra tubuh meningkatkan motivasi remaja putri untuk melakukan upaya pencapaian bentuk tubuh ideal secara tidak tepat.

Abstract

Makanan jajanan berkontribusi untuk kecukupan gizi karena mengandung energi dan protein, namun seringkali ditambah sakarin dan siklamat yang dapat berdampak buruk pada kesehatan jika berlebihan. Tujuan penelitian ini adalah menganalisis kandungan energi, protein, sakarin, siklamat dan frekuensi konsumsi makanan jajanan oleh siswa. Jenis penelitian adalah deskriptif dengan pendekatan cross sectional. Sampel adalah 50 siswa dan 14 jenis makanan jajanan yang dipilih secara purposive. Hasil penelitian menunjukkan kandungan energi tertinggi pada es kelapa muda (323 kkal) dan protein tertinggi pada es cendol (3,1 g). Tujuh sampel mengandung sakarin dan 8 sampel mengandung siklamat. Hampir setiap hari kuantitas konsumsi rata-rata energi, protein, sakarin, dan siklamat yaitu 559 kkal; 4,7 g; 5,4 mg/hari; dan 8,3 mg/hari. Rata-rata makanan jajanan menyumbang 26% AKE (Angka Kecukupan Energi), 8% AKP (Angka Kecukupan Protein), dan semua responden mengkonsumsi sakarin dan siklamat kurang dari ADI (Acceptable Daily Intake). The puposes of this research was to analyze energy, protein, saccharine, cyclamate contents, and frequency consumption of street foods by students. This research was a descriptive study with a cross sectional design. The sample was 50 students and 14 kinds of street food elected by a purposive method. The results showed that the highest energy was found on the young coconut ice drink (323 kkal) and the highest protein was found on the es cendol (3,1 g). Seven samples contained saccharine and 8 samples contained cyclamate. Average consumption quantity of energy, protein, saccharine, and cyclamate from street foodsvwere 559 kcals; 4,7 g, 5,388 mg/day; and 8,317 mg/day respectively. Average street foods have 26% of Adequacy Rate of Energy, dan 8% of Adequacy Rate of Protein and all of the respondents consumed saccharine and cyclamate less than ADI (Acceptable Daily Intake).
