Abstract

Penelitian ini bertujuan mengidentifikasi insiden dan pola kejadian kanker tiroid di RS Dr. Kariadi periode Januari 2006 – Juni 2010. Penelitian deskriptif ini dilakukan terhadap 318 pasien yang dirawat dengan diagnosis Malignant Neoplasma of Thyroid (ICD-10 C73). Pengamatan dilakukan terhadap usia, jenis kelamin, histopatologi, ukuran tumor (T), metastasis KGB (N), metastasis jauh (M), tempat tinggal, dan komorbid. Sebanyak 29 pasien dieksklusi karena data tidak lengkap. Didapatkan 221 pasien perempuan (76,5%); usia tersering 20-50 tahun (56,1%); T3 sebanyak 31,8%; tidak terdapat metastasis KGB dan metastasis jauh 54,7% dan 86,5%; adenokarsinoma papiler 61,2%. Sebanyak 86,9% tinggal di ketinggian <50 m di atas permukaan laut. Hanya 39 penderita yang mengalami gangguan sesak napas. Kesimpulan, kanker tiroid masih menjadi masalah dalam penanganan kasus keganasan, karena jumlah kasus yang relatif masih tinggi dan pada umumnya penderita datang dalam kondisi penyakit yang lebih lanjut. Penanganan yang cepat dan tepat sejak diagnosis, baik klinis maupun patologis, hingga tahap terapi harus dapat dilakukan oleh ahli bedah dengan baik, sehingga dapat diperoleh hasil yang memuaskan.Kata Kunci: karsinoma tiroid, insidensi, faktor risiko

Abstract

Latar Belakang: Penanganan pada pasien yang menderita kanker salah satunya adalah kemoterapi karena terbukti dapat memperpanjang kelangsungan hidup dan meningkatkan kualitas hidup. Namun selain manfaat kemoterapi juga dapat menimbulkan efek negatif bagi penggunanya. Salah satunya adalah hand foot syndrome. Hand foot syndrome adalah efek samping kemoterapi yang sering ditemukan dan perlu penanganan yang tepat. Selain urea cream, minyak kelapa murni juga dapat melindungi kulit karena bekerja sebagai pelembab dan anti inflamasi.Tujuan: Membuktikan adanya efektivitas virgin coconut oil  dalam penanganan hand foot syndrome pada pasien kanker payudara yang mendapat kemoterapi capecitabine per oral.Metode: Penelitian ini merupakan penelitian eksperimental dengan pretest-posttest control group design . Penelitian ini dilakukan di RSUP Dr. Kariadi, Semarang. Sampel penelitian dibagi menjadi dua kelompok, yaitu kelompok perlakuan dan kelompok kontrol (n=31) yang diberikan pretest, lalu pemberian krim minyak kelapa murni, kemudian diberikan  posttest. Skor hand foot syndrome setelah diberikan krim minyak kelapa murni dianalisis dengan uji Kendall’s tau b dan Mann Whitney, skor kualitas hidup di analisis dengan Paired T test, dan uji korelasi antara grade hand foot syndrome dengan kualitas hidup pasien kanker payudara menggunakan uji Spearman. Hasil: Diantara kedua kelompok, skor hand foot syndrome pada kelompok perlakuan mengalami perbaikan (p=0,011)Kesimpulan: Terdapat perbaikan skor hand foot syndrome dan skor kualitas hidup pada pasien kanker payudara yang mendapat kemoterapi capecitabine per oral yang diberikan krim Virgin Coconut OilKata Kunci:  kanker payudara, kemoterapi, kualitas hidup, hand foot syndrome¸virgin coconut oil, urea cream

Abstract

COVID-19 crisis has posed an enormous challenge for all healthcare services. Therefore, this study aimed to describe the cancer patients’ knowledge and perceptions on oncology service quality during the COVID-19 pandemic. Cancer outpatient knowledge and perceptions toward the changes in chemotherapy and radiotherapy service in response to the pandemic was evaluated using a questionnaire. Majority of the 230 participants were knowledgeable about the pandemic and more than 95% of them claimed to sufficiently practice preventive measures. Television and the internet were their main source of information, and despite the fear of being infected by the COVID-19 virus, less than 15% of the respondents thought to stop or delay hospital visits. Generally, the changes in oncology service were well-received by the patients and they trust the health care workers in maintaining their safety. Also, oncology services delivery to outpatients remains consistent despite several alterations in hospital policies. Personal protective equipment utilization by health care workers and patients proved to be an important factor in reducing anxiety during hospital visits. Policy makers need to fully utilize the internet and associated mobile applications as an education tool.

Abstract

Background The 1st, 2nd and small 3rd degree hemorrhoids are non-operatively treated by using anti-inflammatory and plebotonic medicines, high fiber diet and changing lifestyle. Graptophyllum pictum (L.) Griff (GP) is potential to be a hemorrhoid medicine (anti-inflammatory and plebotonic medicine) since its anti-inflammatory effect may be related to antioxidant property. ObjectiveTo elaborate the anti-inflammatory and anti-oxidant effect of GP on experimental hemorrhoid in male Wistar rats. MethodsA Randomized Controlled Trial Post-test only design in hemorrhoid-like rats using croton oil induced into the anus for 30 seconds each day for 3 consecutive days. 14 Wistar rats were randomly allocated into 2 groups. Group 1 got normal saline solution, while group 2 treated with GPE 100mg/Kg bw for 5 consecutive days. On the 9th experimental day, the blood was drawn for Elisa examination of COX-2 and SOD levels. As COX-2 and SOD normally distributed, pool t-test and product moment correlation was used for statistical analysis. Results All of Wistar rats were still alive. COX-2 levels (ng/ml) in the GPE group was 266.28±158.47, was significantly lower compared to control (595.21±68.92) (p=0.000). Serum SOD levels (ng/ml) in the GPE was 77.00±11.0, and was significantly higher than the control 28.00±4.00 (p=0.02). Correlation between COX-2 and SOD serum level was significantly strong negative correlation (p=0,000 and r=-0.842) Conclusions GPE at a dose of 100 mg/kg bw has antioxidant and anti-inflammatory effects on artificial hemorrhoids through decreased COX-2 levels, and elevated SOD levels. Abstract

The healing process of a full‐thickness wound involves a complex cascade of cellular responses to reverse skin integrity formation. These processes require growth factors, particularly platelet‐derived growth factor (PDGF). Conversely, hypoxic mesenchymal stem‐cell‐conditioned medium (HMSC‐CM)‐contained growth factors notably contribute to acceleration of wound healing. This study aims to investigate the role of HMSC‐CM in controlling the serial levels of PDGF associated with accelerated wound closure in full‐thickness wounds. Twenty male Wistar rats with full‐thickness wounds were developed as animal models. The animals were randomly assigned to four groups, comprising two treatment groups (treated using HMSC‐CM at a high dose as P1 and at a low dose as P2), a control group (administration of base gel), and sham group (healthy group). PDGF levels were examined using an enzyme‐linked immunosorbent assay. Using ImageJ software, wound closure percentages were determined photographically. The study showed that there was a significant increase in PDGF levels on days 3 and 6 after HMSC‐CM treatment, followed by a decrease in PDGF levels on day 9. In line with these findings, wound closure percentage also increased significantly on days 6 and 9. In the rat model, HMSC‐CM administration may promote acceleration of wound closure by increasing serial PDGF levels in the full‐thickness wound.

Abstract

Background: Radiotherapy exposure is known to have a lethal effect on blood cells because hematopoietic cells are very sensitive to radiation exposure even at low doses. Cervical cancer with radiotherapy can have the effect of reducing blood cells. Blood cells that may experience the effects of radiotherapy include haemoglobin, leukocytes and platelets.Objective: Knowing the effects of radiotherapy towards blood components of cervical cancer patients.Methods: This study was an observational study with the type of study before and after design. Data collected from medical records as secondary data in the Radiotherapy Unit of Dr. Kariadi Semarang in 2019-2020 with a total of 128 cervical cancer patients undergoing 3D radiotherapy. The statistical test used the Saphiro Wilk data normality test and the result data analysis used the Paired T Test and the Wilcoxon test.Results: Hypothesis testing for Hb levels using the Paired T Test showed an insignificant value of p=0.749 and p=0.141. Hypothesis test for the number of leukocytes and platelets using the Wilcoxon test. Leukocytes received a significant value of p=0.000 and insignificant p=0.957. Platelets received a significant value of p=0.000 and p=0.000. Hb value decreased by -0.44, leukocytes by -3.5, and platelets by -84.34.Conclusion: There was an insignificant decrease in haemoglobin levels yet significant decrease in leukocyte and platelet counts in cervical cancer patients undergoing radiotherapy where the platelets experienced the most decrease compared to haemoglobin and leukocytes.

Abstract

Background : The 1st and 2nd degree hemorrhoids is managed non-operatively with the anti-inflammatory and plebothropic drugs. Graptophyllum pictum extract (GPE) has already been used widely in Indonesia to treat hemorrhoid with good result, however, the mechanism is not supported by the molecular research.Objective : This study is intended to study the phlebothropic effects of GPE by measuring the degree of edema and extravassal leucocytes of experimental wistar hemorrhoid.Methods : Experimental study with Randomized Controlled Trial Post-test only design in male wistar rats, weight around 200 gr, induced for the development of a disease-like condition of hemorrhoids by 6% croton oil on the anus for 3 days. Random allocation was performed to divide the 14 wistar rats in 2 groups. Group I as control got normal saline solution, while group II was treated with GPE 100mg/kgbw, started on day 4th for 5 consecutive days. On 9th day blood was extracted from retroorbital fossa and anus was resected up to 2 cm from anal verge and weighted. The degree of anal edema was measured by rectoanal coefficient, that is rasio between anal weight (miligrams) and body weight (grams).Results : Until the end of study, all wistar rat were still alive. The mean (±SD) of body weight   of control group was 173.84 ­(±13.37) and the treatment group was 171.70  (±13.10), and there was no significant differences (p = 0.833). The mean (±SD) of rectoanal coefficient in the treatment group was 2.46 (±0.41) and it was significantly lower than control group  (3.13 ± 0.85) (p = 0.029). The mean (±SD) of extravassal leukocytes in the treatment group was 900.14(±48.09) and it was significantly lower than the control (1003.28 ± 99.30) (p = 0.042).Conclusions : Graptophyllum pictum extract at doses of 100 mg/Kgbw have phlebotonic effect  in decreased edema and extravassal leukocytes.

Abstract

Background: According to the 2018 Global Cancer Observatory from the World Health Organization (WHO), breast cancer is the most common cancer in Indonesia. 3-dimensional conformal radiotherapy (3D-CRT) is one of the breast cancer treatment planning. Effect of radiation on the lungs can cause radiation pneumonitis. The incidence rate of pneumonitis in breast cancer patients who received radiotherapy using 3D-CRT techniques from several researchers is still varied and there is no accurate data yet at RSUP Dr. Kariadi.Objective: To determine the incidence of pneumonitis in breast cancer patients undergoing breast cancer 3D-CRT.Methods: The research design used in this study was cross sectional. The subject is chosen by consecutive sampling methods. The independent variables of this study was 3D breast cancer radiation therapy, while the dependent variable was radiation pneumonitis. This hypothesis test on this study is analyzed with chi-square test.Results: Forty one subjects were included in this study, two subjects were found with a picture of pneumonitis on chest radiographs. The incidence of radiation pneumonitis in breast cancer patients who received radiotherapy using 3-dimensional techniques was 4.9%. There is no significant relationship between breast cancer radiation therapy 3-dimensional techniques on the incidence of pneumonitis. And there was no significant difference in the proportion of pneumonitis in patients with breast cancer locations on the right and left sides.Conclusion: The incidence of radiation pneumonitis in breast cancer patients who received radiotherapy using a 3-dimensional technique is 4.9%.   Keywords: 3 Dimensional Technique Radiotherapy, Breast Cancer, Radiation Pneumonitis

Abstract

Background: Colon cancer, a colorectal cancer, is the third most common epithelial malignancy in the world. Family history, bloody stool, palpable mass, anemia, and abdominal MSCT are symptoms and signs of colon carcinoma.Objective: To determine the relationship between the 5 variables and the incidence of colon carcinoma at Dr. Kariadi Hospital, Semarang in 2016.Methods: a Cross-sectional observational analytical study using medical record (RM) and complementary primary data. The inclusion criteria werethe complete medical record, and clinical diagnosis of suspected colon carcinoma. Data obtained from the department of Anatomy Pathology/PA (11,794PA results) were traced to the medical record section (46 patients with suspected colon carcinoma). The incomplete data were confirmed by: contacting the patient/family, obtaining the archive in the laboratory and radiology resulting in 27 patients meeting the inclusion criteria. Analysis was done using chi-square test, Spearman-Kendall bivariate correlation, and logistic regression.Results: Abdominal MSCT was moderately associated with colon carcinoma (p = 0.003; r = 0.488), while family history, bloody stool, palpable mass, and anemia were not associated with colon cancer. Analysis between predictors of outcome: Bloody stool was moderately associated with anemia (p = 0.006; r = 0.411), and anemia was weakly associated MSCT (p = 0.035; r = 0.351). Abdominal MSCT was the predictive factor for colon carcinoma (p = 0.021).Conclusion: Abdominal MSCT was found to be associated with the incidence of colon carcinoma. Bloody stool was associated with anemia, and anemia was associated with abdominal MSCT. MSCT was the predictive factor for colon cancer.
