Abstract

Latar Belakang: Fakultas kedokteran dituntut untuk menghasilkan lulusan dokter berkualitas yang memenuhi standar kompetensi dokter. Lingkungan belajar dalam pembelajaran tahap profesi, yang diketahui berperan penting dalam proses pencapaian kompetensi, memiliki karakter khusus dan berbeda dengan tahap akademik. Tujuan: untuk mengetahui hubungan antara persepsi mahasiswa terhadap lingkungan belajar, jum- lah kasus yang dijumpai selama pendidikan klinik, umpan balik yang diterima dengan kompetensi mahasiswa. Metode: metode kuantitatif dengan pengambilan data secara potong lintang dilakukan untuk memeriksa variabel persepsi terhadap lingkungan belajar, kasus, umpan balik dan kompetensi mahasiswa. Sebanyak 120 mahasiswa tahap profesi FK UNRAM melengkapi kuesioner PHEEM dan kuesioner umpan balik, 330 Mini-CEX mahasiswa dan 67 logbook dari bagian Penyakit Dalam, Ilmu Kesehatan Anak, Obstetri dan Ginekologi serta Bedah. Analisis dengan PLS SEM dilakukan untuk menilai hubungan antar variabel. Hasil: dari seluruh data, hanya 24 subjek yang memiliki data yang lengkap untuk seluruh variabel. Dari analisis jalur didapatkan bahwa persepsi mahasiswa terhadap lingkungan belajar tidak berasosiasi dengan kompetensi (b 0,00, P 0,49), sedangkan ketersediaan kasus dan umpan balik berasosiasi positif dengan kompetensi (berturut-turut b 0,50, P < 0,01; b 0,46, P < 0,01). Dalam model ini, kompetensi mahasiswa dapat dijelaskan sebesar 46% (R2). Kesimpulan: ketersediaan kasus dan umpan balik sangat penting dalam mendukung pencapaian mahasiswa. Meskipun persepsi mahasiswa terhadap lingkungan belajar klinik belum terbukti berpengaruh langsung terhadap kompetensi, namun sangat dimungkinkan berpengaruh tidak langsung dengan dimediasi oleh variabel partisipasi mahasiswa. Abstract

Latar Belakang: Latar belakang: Pembelajaran di klinik menuntut mahasiswa untuk dapat aktif melakukan partisipasi. Partisipasi bukan hanya pembelajaran untuk mencapai kompetensi, namun juga mengembangkan identitas profesi. Penelitian ini bertujuan untuk mengidentifikasi bentuk partisipasi, dorongan dan hambatan dalam pendidikan di bagian klinik. Metode: penelitian ini menggunakan metode penelitian kuantitatif dan kualitatif dengan pengambilan data melalui survei. Partisipan survei adalah 102 mahasiswa dari total 129 mahasiswa pendidikan klinik. Hasil survei dianalisis secara deskriptif. Hasil jawaban pertanyaan terbuka diberikan kode dan dikelompokkan. Hasil: Keterampilan klinik paling banyak yang dilakukan mahasiswa adalah anamnesis (25 %), pemeriksaan fisik (23,1%), dan membuat diagnosis (21%) dengan setting terbanyak di poliklinik (41,3%), bangsal (28,8%), UGD (17,3%). Peran yang paling banyak adalah menjadi pelaku dalamkinerja. Hal yang mendorong maupun hambatan untuk melakukan partisipasi lebih banyak dari dalam diri sendiri. Kesimpulan: Partisipasi mahasiswa dalam pendidikan klinik memerlukan inisiasi dari mahasiswa yang didukung oleh superivisor klinik, perawat, dan tenaga kesehatan lain dalam lingkungan pendidikan klinik.

Abstract

Latar belakang: Dengue merupakan salah satu penyakit yang masih menjadi masalah utama dalam kesehatan masyarakat, khususnya di Indonesia. Infeksi ini sering dihubungkan dengan keterlibatan berbagai organ. Hati adalah organ yang paling sering terganggu. Manifestasi klinis dari dengue yang mengindikasikan keterlibatan dari organ hati yaitu hepatomegali serta peningkatan enzim hati (SGOT dan SGPT). Hingga saat ini data mengenai keterlibatan hati dalam infeksi dengue di NTB masih kurang. Tujuan: Untuk mengetahui prevalensi hepatitis dengue di Rumah Sakit Umum Daerah Provinsi NTB dan Rumah Sakit Harapan Keluarga Mataram dan untuk mengetahui perbedaan rerata kadar SGOT dan SGPT pada pasien hepatitis dengue perempuan dan laki-laki. Metode: Rancangan penelitian ini bersifat deskriptif dengan melakukan pengambilan data secara retrospektif yaitu pada data rekam medis pasien dengue yang menjalani tes fungsi hati. Data kemudian dikelompokkan berdasarkan peningkatan enzim SGOT dan SGPT. Selanjutnya data dianalisis untuk mengetahui distribusi dari aspek-aspek yang diteliti. Jumlah populasi pada penelitian ini adalah 110 orang. Dari 110 orang tersebut, jumlah pasien yang menjalani tes fungsi hati adalah 50 orang. Hasil: Persentase hepatitis dengue pada penelitian ini adalah 82%. Tidak didapatkan adanya perbedaan rerata SGOT dan SGPT pada pasien hepatitis dengue perempuan dan laki-laki. Kesimpulan: Persentase hepatitis dengue di Rumah Sakit Umum Daerah Provinsi NTB dan Rumah Sakit Harapan Keluarga Mataram tahun 2016 adalah 82%.

Abstract

Latar belakang: Pada studi-studi empirik, telah dibuktikan bahwa pencapaian kompetensi sebagai tujuan dari pembelajaran klinik dipengaruhi oleh lingkungan belajar. Selama sepuluh tahun melaksanakan pendidikan tahap klinik, Fakultas Kedokteran Universitas Mataram telah melakukan beberapa perubahan dan peningkatan pada komponen-komponen lingkungan belajar kliniknya. Dengan berbagai perubahan yang terjadi pada lingkungan belajar klinik Fakultas Kedokteran Universitas Mataram, maka perlu adanya studi mengenai persepsi mahasiswa tahap klinik terkait lingkungan belajar klinik. Metode: Penelitian ini menggunakan metode kuantitatif dengan metode pengambilan data secara cross-sectional menggunakan kuesioner Postgraduate Hospital Educational Environment Measure (PHEEM). Sebanyak 120 mahasiswa pendidikan tahap klinik berpartisipasi, yang terdiri atas 45 lakilaki (37,5%) dan 75 perempuan (62,5%). Data digunakan untuk melihat persepsi mahasiswa tahap klinik terhadap lingkungan belajar klinik berdasarkan jenis kelamin, penggunaan logbook baru, dan tahun angkatan. Analisis data dilakukan dengan uji statistik komparatif non parametrik Mann-whitney. Hasil: Rerata nilai total PHEEM untuk seluruh responden adalah 97,55. Nilai total PHEEM antara kelompok mahasiswa laki-laki dan perempuan memiliki perbedaaan yang bermakna (p=0,016), perbedaan lama koas maupun penggunaan jenis logbook tidak menunjukkan perbedaan yang signifikan (p=0,593;p=0,491). Kesimpulan: Persepsi mahasiswa tahap klinik terhadap lingkungan belajar dalam pendidikan klinik di Fakultas Kedokteran Universitas Mataram memiliki lebih banyak hal positif dibanding hal negatif. Mahasiswa wanita memiliki persepsi yang lebih baik dibanding mahasiswa laki-laki.

Abstract

Background: Clinical rotation guidance (logbook) in clinical education is very important to provide the structure and focus of teaching and learning activities for both students and clinical supervisors. Implementation of clinical education itself faces many challenges, however the problems and challenges in each faculty of medicine are different. This paper aims to identify the problems and challenges encountered during the revision process and the implementation of the logbook. Method: Case studies are carried out by taking notes during the curriculum revision and implementation process. The analysis was carried out by reflecting on the ongoing revision process. Results: Before the revision process, 10 of the 13 sections of the clinic had guidelines. The main problem faced is the lack of uniformity of perception about clinical rotation logbook and their contents. The revision process begins with socialization to build mutual commitment. The leadership of FK UNRAM has made an important contribution to building commitment and providing support for the revision process led by the Medical Education Unit (MEU). Through internal consolidation MEU developed the logbook format to be used. Prior to implementation, socialization to clinical supervisors and students was conducted to ensure smooth implementation. Conclusion: The revision of the clinical rotation logbook is basically changing the clinical education system and curriculum. Joint commitment of the clinical department, involvement and support of the leadership, active involvement of the clinician in decision making, as well as socialization of guidelines to all parties involved will support the success of this process.

Abstract

Background: conducting research is beneficial for the future of a medical doctor, but the medical students in Mataram University have unsatisfactory results in the course of research methodology. Research is also a determinant of the quality of a university. In a medical study, students are required to review or conducting relevant research as the proponent of their professionalism as long life learner. Regarding this study, it aims to measure the relationship of attitude and barriers towards research experiences of students in the medical faculty of Mataram University. Method: this study belonged to a quantitative study with a cross-sectional survey design. The subjects in this study were 129 medical students from a total of 140 of medical students in Mataram University in their 3-year and 4-year study. They have taken a research methodology course as the prerequisite to fill Revised Attitude Towards Research (R-ATR) questionnaire to measure attitude toward research, the questionnaire for both barriers and research experiences. Data from this study were analyzed using Spearman correlation. Results: Attitude toward research and research experience have small and positive correlation (r=0,322;p <0,001), while research barrier is small and negatively correlated towards research experience (r=-0,209;p <0,05). Conclusion: attitudes and barriers can be the factors that affected the frequency of student’s experiences in research.

Abstract

The concept of participation as a learning process is essential to foster professional identity development. Faculties are expected to provide a curriculum that supports students' participation in the profession's context. Curriculum evaluation is needed to assess the extent to which curriculum implementation supports participation. In this regard, this study aims to develop instruments that measure students' perceptions of the medical education curriculum. The blueprint for the instrument's development was based on the concept of participation in communities of practice theory. Qualitative research, which involved 17 pre-clinical and clinical medical students as participants, was conducted to explore medical students' perception about formal learning activities that encourage participation. The results were used to generate the items. A series of review processes, item reduction, revisions, and analysis generated 20 items in four factors, namely: engagement support, imagination support, convergence, and feedback. This shows that the instrument is multidimensional. The instrument also has good discriminant validity and composite reliability.

Abstract

Background: Due to higher attention on professionalism of doctor, greater concern in medical education should be placed on the development of the qualities of professional behavior. Assessment of professional behavior for medical students becomes very essential. However, there is still confusion in selecting and implementing assessment methods. This study aimed to provide an overview of professional behavior assessment and considerations in implementing assessment method especially P-MEX.Method: Relevant literatures are selected as basis of recommendations in P-MEX and professional behavior assessment.Results: P-MEX was discussed using the framework of 7 criteria for good assessment, related to the validity, consistency, equivalence, feasibility, educational effect, catalytic effect and acceptability. Professionalism is multidimensional and one method of assessment cannot give reliable representation of students’ behavior, for that reasons combination of assessment tools is required. The assessment must valid and reliable also. Many methods for assessing professional behavior have been developed, one of them is the Professionalism Mini-Evaluation Exercise (P-MEX), which format is based on Mini-Clinical Encounter Exercise (mini-CEX). This article will describe the outline about professional behavior definition, methods of assessment especially P-MEX, whether P-MEX is a good alternative for assessing professional behavior.Conclusion: P-MEX can be considered as one of assessment method to be applied in assessing student’s professional behavior. However, adapting the instrument to be ready to use here requires a process that can maintain the validity and reliability would be better if the instrument is tailored to Indonesian context.

Abstract

Background: In the process of becoming a doctor, a medical student experiencing the development of professional identity. In this process, students need a broad picture of  medical profession. They need to orient themselves in the medical profession to determine the future professional identity as a doctor. Source of the picture of medical profession needs to be explored further. This study aims to explore the ways students get picture of the medical profession.Method: Qualitative research by conducting in-depth interviews to 17 preclinical and clinical medical students. Determinants and emergent coding performed to analyze the results of the interview.Results: Source of imagination can be derived from the curriculum and outside the curriculum. Students get two important things, first was overview of the work environment and how the physician work and second was physician professional values. Characters of role models identified primarily from student interaction with faculties. The characters include faculty’s role as teacher, as doctor, their personal atributes and how they interact with students.Conclusion: Formal curriculum plays important roles in providing and facilitating students to get overview about the work environment and how the doctors work in the health service. Although the transfer of professional value to students is determined by the hidden curriculum, but the formal curriculum can facilitate by providing activities that allows the interaction between generations or conduct review on fiction and non-fiction medical stories and histories that is obtained from media.

Abstract

Background: Transition to clinical clerkships involves significant changes for students in terms of learning environment and approach to learning. As clinical supervisors, doctors are one of the essential learning resources who also provide access for participation in a clinical environment. This study explored undergraduate medical students’ experiences during the transition to clinical clerkships to understand how these experiences affect their learning.This paper aims to report factors affecting students’ interaction with supervisors during the transition period.Method: This was a qualitative phenomenological study. Participants were selected purposely to represent gender and group of student rotations. Eight 4th year medical students (five female, three male) submitted audio diaries during their first 12 weeks of clinical clerkships. Forty-six of the 73 audio diaries collected in the study contained interactions with supervisors, and these interactions were captured in 76 excerpts.Results: Six themes emerged regarding factors affecting the interaction: (1) characters of the supervisors including the willingness to teach, showing concerns, asking questions to test comprehension, inviting explicitly; (2) supervisors’ workload; (3) students’ clinical knowledge and skills; (4) students’ initiatives: actively participating, asking questions; (5) curriculum and organization: the chance to do mutual interaction (i.e.,mini-CEX), clarity of students’ roles and educational goals; (6) senior peers.Conclusions: Interactions between students and supervisors during the transition to clinical clerkships were influenced by factors internal to both parts and external factors like curriculum and organization, workload,and the presence of senior peers. However, during this phase, where students had not developed a situational understanding of the new environment, supervisors’ characters strongly influenced the interactions through the provision of affective support.
