Abstract

Objective: To identify relationship between progesterone receptor gene polymorphism promoter region +331G/A with the risk of endometriosis. Method: An observational case-control study. Population are women with endometriosis and/or adenomyosis who have been performed laparotomy/laparoscopy at Obstetrics and Gynecology Department Dr. Mohammad Hoesin General Hospital Palembang, January-November 2013. Subjects fulfilled inclusion criteria, given informed consent and performed blood sampling continued by PCRRFLP. Results were divided into A/A genotype (homozygote mutant), G/A (heterozygote mutant), and G/G (homozygote wild type). Data were analyzed by SPSS 21.0 version. Result: PCR-RFLP results for+331G/A genotype were 26 (54.1%) in case group and 14 (26.4%) in control. +331A/A genotype was not found in both groups. Abstract

Introduction: Panretinal laser photocoagulation (PRP) is a standard treatment for severe nonproliferative and proliferative diabetic retinopathy. Twenty-milisecond duration PRP show same effectiveness with 100-ms standard PRP in inhibit neovascularization progression. This shorter pulse tend to minimize retinal neuronal defect and visual field defect. This study aim to analyze the difference of visual field defect in diabetic retinopathy (DR) patients treated with 20-ms PRP compared with 100-ms PRP in Moh. Hoesin Hospital Palembang. Methods: A clinical trial with single blinding on severe-very severe NPDR and early PDR eyes treated with PRP between June and August 2016. Forty eyes (25 patients) were randomized into two groups. Twenty eyes were treated with 20-ms PRP, and other 20 eyes treated with 100-ms PRP. Visual field defect was evaluated using Humphrey Field Analyzer 30-2 SITA Standard at baseline and 2 weeks follow-up. Result: Unpaired t-test showed significant difference in mean deviation (MD) after laser on NPDR eyes (p=0.042, p<0.05), meanwhile there was no significant difference in early PDR eyes (p=0.17, p>0.05). In NPDR eyes, more MD improvement was found in 20-ms PRP group (0.79±0.93 dB) than in 100-ms group (-0.04±0.61 dB). In early PDR eyes, MD improvement was bigger (1.0±0.88 dB) in 20-ms PRP group than in 100-ms group (0.10±1.47 dB). There was no significant difference in pattern standard deviation (PSD) on both group at any DR grade (p=0.208; p=0.201; p>0.05). Conclusion: After 2 weeks, 20-ms PRP caused more improvement and lesser visual field defect (p=0.042, p<0.05) on NPDR eyes. There was no significant difference in PSD on both groups.

Abstract

Karsinoma ovarium adalah keganasan yang berasal dari sel-sel epitelial di ovarium, terbanyak keenam pada wanita di dunia. Di RS Umum Dr. Mohammad Hoesin Palembang, insiden karsinoma ovarium menempati urutan ketiga keganasan terbanyak pada wanita setelah karsinoma payudara dan leher rahim. Gen TP53 mensintesis protein p53 yang bila inaktif dapat mempengaruhi derajat histopatologi dan stadium klinik. Penelitian ini bertujuan mengetahui korelasi antara overekspresi p53 dengan derajat histopatologi dan stadium klinik karsinoma ovarium.  Penelitian dilakukan secara analitik observasional, pendekatan potong lintang di sentra diagnostik laboratorium Patologi Anatomi RS Umum Dr. Mohammad Hoesin Palembang. Sampel berjumlah 32 kasus, diambil dari arsip rekam medik periode 1 Januari 2012 sampai 30 Juni 2015, dipulas dengan antibodi monoklonal p53. Hasil penelitian menemukan frekuensi karsinoma ovarium derajat rendah sebanyak 65,6% dan derajat tinggi sebanyak 34,4%. Tipe histopatologi terbanyak adalah karsinoma serosa derajat tinggi (25%) dan karsinoma musinus (25%). Frekuensi karsinoma ovarium stadium lanjut (53,1%) lebih banyak dari stadium awal (46,9%).  Frekuensi overekspresi p53 pada karsinoma ovarium 43,8%. Tidak ada korelasi bermakna antara overekspresi p53 dengan derajat histopatologi (p=0,1) dan stadium klinis karsinoma ovarium (p=0,6). Terdapat korelasi bermakna antara derajat histopatologi dan stadium klinik (p=0,00, r=0,46)

Abstract

Objective: To know the correlation of the expression of transforming growth factor beta (TGF-β1) and tropoelastin in uterine prolapse. Method: A cross-sectional study of 30 subjects suffered from uterine prolapse in the Department of Obstetrics and Gynecology Dr. Mohammad Hoesin hospital Palembang. The study was conducted since December 1st, 2014 until July 31st, 2015. The sample was from the sacrouterine ligament and immunohistochemical examination was conducted to see the expression of TGF-β1 and tropoelastin. Result: Of the 30 subjects obtained, the expression of TGF-β1 was on 30 subjects consisting of 18 (60%) for weak expression and 12 (40%) for strong expression. Meanwhile, the strong tropoelastin expression was on 18 subjects (60%) and weak tropoelastin expression on 12 subjects (40%). There was a positive correlation between TGF-β1 and tropoelastin expression with moderate correlation (p=0.014; r=0.44). Conclusion: There is a positive correlation between the TGF-β1 and tropoelastin expression of sacrouterine ligament in uterine prolapse with moderate correlation. Abstract

Objective: To investigate the association of bladder wall thickness (BWT) with severity of symptoms in overactive bladder patients in Obstetrics and Gynecology Department Dr. Mohammad Hoesin general hospital Palembang. Methods: An analytical observational study was conducted at Gynecology clinic Dr. Mohammad Hoesin General Hospital Palembang from November 2015 to August 2016. Data were analyzed with SPSS 16.0 for Windows. Bivariate analysis with the Chi square and association Rank-Spearman test was used to assess the association between BWT and visual analog scale (VAS). Results: Fourty subjects were included in the study. The mean BWT in the overactive bladder group was thicker compared to those without overactive bladder (5.8522  0.5783 vs 5.2176  0.67937). There was significant association between BWT and overactive bladder complaints. Abnormal group (5mm) had 12 times risk of overactive bladder compared to normal sample (5mm) (p = 0.029, RR = 12). Conclusion: Thus, the thickness of the urinary bladder wall measured with ultrasound examination (USG) can be used to assess the status and degree of urinary disorders in women with complaints of painful urinate and urinary disorders.  Keywords: bladder wall thickness, detrusor overactivity, overactive bladder, visual analog scale

Objective: To analyze the relationship between levels of serum vascular endothelial growth factor (VEGF) and bleeding occurrence in depo medroxyprogesterone acetate (DMPA) acceptors. Method: We employed a cross‐sectional study on 70 DMPA acceptors with DMPA use of 3 to 6 months who presented for midwifery service in Palembang. Blood samples were obtained in order to assess levels of serum VEGF using ELISA (enzyme‐linked immunoabsorbent assay) method. Laboratory assessments were carried out in PRODIA laboratory in Jakarta. Result: We recruited 70 subjects into our study. After 3 to 6 months of using DMPA, as much as 26 subjects (37.1%) reported complaints of bleeding and 44 subjects (62.9%) reported no bleeding. The mean level of serum VEGF in DMPA acceptors with bleeding was 355 K 170 pg/ml, and 323 K 202 pg/ml in acceptors with no bleeding. We identified a significant association between duration of use and bleeding occurrence (p0.05). Conclusion: In our sample, we found an association between duration of DMPA use and presence of bleeding but VEGF levels was not found to be different in women experiencing abnormal uterine bleeding and those who did not. Abstract

Background: Over the past decade, antimalarial drug resistance has rapidly become a major public health problem in South East Asia region including South Sumatra. This study aimed to determine the extent of gene polymorphisms associated with chloroquine resistance (CQR) in P. falciparum isolates from Lahat, Sekayu, Baturaja and Palembang district.Methods: A molecular study was conducted to identify the mutant alleles of the genes associated with the resistance to chloroquine among the isolates of Plasmodium falciparum from South Sumatera. Blood from 25 patients was collected, DNA was isolated, and the sequences of two different genes (Plasmodium falciparum chloroquine resistance transporter/pfcrt and Plasmodium falciparum multidrug resistance/pfmdr1) were analyzed using polymerase chain reaction (PCR) and restriction fragment length polymorphism (RFLP).Results: This study identified polymorphism in the pfcrt 76-Thr in all isolates and pfmdr1 86-Tyr. These findings may reflect the failure of treatment with the standard dose of chloroquine within the last few years in South Sumatera. Conclusion: PCR-RFLP technique provide a simple and rapid method of detecting polymorphisms in genes that may predict chloroquine resistance (CQR). Although the identification of the polymorphism in the pfcrt and pfmdr1 genes provides a significant indicator of CQR, further studies are needed to determine the role of these polymorphisms in the in vivo and in vitro responses to drug treatment.Keywords: chloroquine, Plasmodium falciparum, pfmdr1, pfcrt
