Abstract

Kehamilan tidak diinginkan merupakan masalah global, dan menyumbang 700.000 kematian ibu setiap tahunnya.  Penelitian ini bertujuan untuk mengetahui faktor-faktor yang berhubungan dengan kehamilan tidak diinginkan di  Indonesia. Desain studi adalah potong lintang, dengan responden sebanyak 13.955 perempuan berusia 15-49 tahun, yang melahirkan anak terakhir dalam 5 tahun sebelum survei. Penelitian ini menggunakan data sekunder, dari Survei Demografi Kesehatan Indonesia (SDKI) tahun 2012.  Uji analisis yang digunakan adalah uji kai kuadrat (bivariat) dan regresi logistik ganda (multivariat).  Hasil studi menunjukkan prevalensi ibu yang mengalami kehamilan tidak diinginkan di Indonesia sebanyak 8%, dengan prevalensi tertinggi terdapat di provinsi Sulawesi Tengah (11,9%) dan terendah di  Papua (2.9%).  Pada analisis bivariat didapatkan hubungan yang bermakna antara tingkat ekonomi, tingkat pendidikan, wilayah tempat tinggal, status pernikahan, paritas, jarak kelahiran, usia dengan kehamilan tidak diinginkan dengan nilai p< 0,25.  Pada analisis multivariat, variabel status pernikahan berhubungan signifikan dengan kehamilan tidak diinginkan setelah dikontrol dengan variabel tingkat ekonomi, tingkat pendidikan, wilayah tempat tinggal, paritas, jarak kelahiran dan usia.  Disimpulkan faktor status pernikahan merupakan faktor paling dominan yang berhubungan dengan kehamilan tidak diinginkan. Penemuan ini diharapkan dapat menjadi masukan bagi pemerintah untuk meningkatkan pengetahuan dan partisipasi masyarakat tentang program Keluarga Berencana.

Abstract

Background: Exclusive breastfeeding practice during six-months could reduce morbidity and mortality of mother and infant. This study aims to examine the sociodemographic characteristics as the the factors associated with knowledge among six-months exclusively breastfeeding mothers in Cipayung Sub-District, Depok City, Indonesia. Methods: This was a cross-sectional collected from 122 mothers with six-months exclusively breastfeeding. The participants selected through purposive sampling in Cipayung Sub-District in year 2017. Results: Only 80% mothers exclusively breastfeeding for six months. More than half mothers (59%) had high educational level, advance and tertiary. Most of mothers (91%) were unemployed. Approximately 22.1% mothers had high status of social economic. Eighty-two percent mothers who have children more than one. Six-months exclusively breastfeeding mothers who had high educational level were more likely has high knowledge than those with low educational level (p=0.010; OR=2.85; CI95%=1.34-6.06). Mothers who had low educational level were 2.85 times higher risk to have low maternal knowledge of exclusive breastfeeding compared with high educational level. Conclusions: Knowledge of exclusive breastfeeding among mothers was significantly associated to educational level. Interventions emphasizing practical education should therefore be targeted at addressing factors that associated knowledge of exclusive breastfeeding.

Abstract

Background: Exclusive breastfeeding is breast milk given to infant since their birth to the age of 6 (six) months, without adding and/or replacing with other foods or beverages (except medicines, vitamins, and minerals). One of the factors that affect the exclusive breastfeeding was the perceptions of insufficient milk. Milk insufficiency is considered to happen when the mother stops breastfeeding exclusively with some reasons such as no breast milk, low milk supply, or insufficient milk. This research was to examine more deeply the perception of insufficient milk and factor influencing it. Methods: This was a qualitative study that used a primary data of study "Intervention of Breastfeeding Enhancement and Weaning Food in the Implementation of the First-1000-Days-of-Life Program". Informants selected through who has been intervened research by PKGK. Triangulation of data sources was a midwife, informants’ husbands, documents of mother and child health books (MCH) and secondary data from PKGK research. Results: Based on the result from the interview: a) there was found a tendency of knowledge increase on mothers receiving treatment. b) Mostly, the lactation guidance was given without supporting device. c) Mother perception about their will gained weight if giving the sufficient milk has not been in line with Body Mass Index (BMI) before pregnancy. d). Family support towards mothers breastfeeding has not been provided optimally. e) Early breast-feeding (EBF) was not complete in accordance with the standard. Conclusions: Mothers who had not received treatment were having lack of knowledge on exclusive breastfeeding. Implementation of EBF was not in line with the procedure. The role of a delivery helper on EBF is vital in order to make the implementation of the EBF carried out according to the procedure.

Abstract

Behavioral patterns and eating habits as adults are formed in childhood. Knowledge and eating habits will lead to changes in a more regular and balanced nutritional attitude. The purpose of this study is to look at the effect of nutrition program education in changes in the average attitude score before and after of Gerakan Nusantara program in 2014-2018. 7605 people are elementary school students grade III, IV, and V in several areas in the City of Jakarta, Bandung, Surabaya, Medan, Makassar, Yogyakarta, Pontianak, NTT, Pekanbaru, Padang, Jambi, and Sorong. The data used are secondary data from Gerakan Nusantara in 2014-2018. Data processing using SPSS with t-test dependent & t-test independent, correlation and One-way Anova. The analysis showed that there was a statistically significant change (p<0.05) in each year with a difference in 2014 related to an increase in attitude by 4%, in 2015 by 6%, in 2016 by 6%, in 2017 by 10%. in 2018 at 36%. Meanwhile, the relationship of knowledge to attitudes obtained statistically significant results (p<0.05), in 2014 (r = 0.47, p = 0,000), 2015 (r = 0.26, p = 0,000), year 2016 (r = 0.35, p = 0,000), 2017 (r = 0.26, p = 0,000) and 2018 (r = 0.41, p = 0,000). Nutrition education programs bring significant improvements in attitude changes that are also influenced by knowledge in primary school children each year.

Abstract

The exclusive breastfeeding in some parts of Indonesia is still relatively low, whereas exclusive breastfeeding is very important for the growth and development of babies. Many factors influence the success of exclusive breastfeeding. Health service factor is the most important factor. One of the efforts to improve these health services is through improving the quality of Antenatal Care (ANC). Quality ANC services include 10T standards, including an assessment of the mother's nutritional status and talks/counseling about exclusive breastfeeding. ANC coverage in Depok City has reached the target, but exclusive breastfeeding coverage is still low. The purpose of this study was to determine the factors associated with the success of exclusive breastfeeding, and also to overview the quality of ANC in the working area of ​​the Cipayung Public Health Center, Depok City. This cross-sectional study used secondary data conducted on 169 breastfeeding mothers. Bivariate analysis conducted by using chi-square test, and multivariate analysis using multiple logistic regressions. The results of the bivariate analysis showed that the quality of ANC, occupation, knowledge, and energy supplementation for mothers were related to the success of exclusive breastfeeding (p-value <0.05). The results of multivariate analysis showed that the dominant factor for the success of exclusive breastfeeding was energy supplementation for mothers (OR=5.460; 95% CI=1.63-18.18). The description of the quality of ANC related to exclusive breastfeeding is shown by measurements of BB, TB, LILA and counseling related to exclusive breastfeeding which have not been fully (100%) carried out. It can be concluded that the factors related to the success of exclusive breastfeeding are the quality of ANC, occupation, knowledge, and energy supplementation as the dominant factor. It is recommended that health stakeholders increase support for energy supplementation (milk) program for mothers, as well as improve the quality of ANC through counseling services about exclusive breastfeeding during ANC visits. Abstrak Pemberian Air Susu Ibu (ASI) eksklusif di beberapa wilayah Indonesia masih terbilang rendah. Padahal ASI eksklusif sangat penting untuk pertumbuhan dan perkembangan bayi. Banyak faktor yang memengaruhi keberhasilan ASI eksklusif. Faktor pelayanan kesehatan merupakan faktor yang paling berperan. Salah satu upaya meningkatkan pelayanan kesehatan tersebut adalah melalui peningkatan kualitas Antenatal Care (ANC). Pelayanan ANC berkualitas meliputi standar 10T, diantaranya adalah penilaian status gizi ibu dan temu wicara/konseling seputar ASI eksklusif. Cakupan ANC di Kota Depok sudah mencapai target, tetapi cakupan ASI eksklusif masih rendah. Tujuan penelitian ini adalah mengetahui faktor-faktor yang berhubungan dengan keberhasilan ASI eksklusif, serta gambaran kualitas ANC di wilayah kerja Puskesmas Cipayung Kota Depok. Studi cross-sectional ini menggunakan data sekunder yang dilakukan terhadap 169 ibu menyusui. Analisis bivariat menggunakan uji chi-square, dan analisis multivariat dengan uji regresi logistik ganda. Hasil analisis bivariat menunjukkan bahwa kualitas ANC, pekerjaan, pengetahuan, dan suplementasi energi pada ibu berhubungan dengan keberhasilan ASI eksklusif (p-value <0,05). Hasil analisis multivariat menunjukkan bahwa faktor dominan keberhasilan ASI eksklusif adalah suplementasi energi pada ibu (OR=5,460; CI 95%=1,63-18,18). Gambaran kualitas ANC yang berhubungan dengan ASI eksklusif ditunjukkan oleh pengukuran BB, TB, LILA serta konseling terkait ASI eksklusif yang belum sepenuhnya (100%) dilakukan. Dapat disimpulkan bahwa faktor yang berhubungan dengan keberhasilan ASI eksklusif adalah kualitas ANC, pekerjaan, pengetahuan, dan suplementasi energi sebagai faktor dominan. Disarankan agar para stakeholderkesehatan meningkatkan dukungan program pemberian suplementasi energi (susu) pada ibu, serta meningkatkan kualitas ANC melalui pelayanan konseling seputar ASI eksklusif pada saat kunjungan ANC.

Abstract

Adolescent is a vulnerable group in reproductive health area. This period is marked by strong desire to try and explore new things. Pornography is a medium which can influence adolescent toward high risk sexual behaviour. The exposure to pornographic materials is a serious problem among adolescent since it could have negative impacts such as unwanted pregnancy, unsafe abortion, sexually transmitted diseases and HIV-AIDS. This cross-sectional study was conducted to understand various types of pornographic exposures, effects of pornographic exposure, and factors influence the effect. Study was conducted at five state junior high schools in Pontianak District in 2008 with 395 respondents from December 2007 to January 2008. The result shows that 83.3 % adolescence has exposed to pornography and 79.5% of them had experienced the effects of pornographic exposure. 19.8% respondent who experienced the effects of pornography was in the addiction stage. 69.2% respondent of those in addiction stage was in escalation stage. 61.1% respondent of escalation stage was in desensitization stage, and 31.8% respondent of desensitization was in act-out stage. Multivariate analysis shows that there were four variables that have significant relationship to the effect of pornographic exposure, that is gender (male), grade at school (third), length of exposure (recent) and frequency of exposure (often). The analysis also shown that the frequency of pornography (often) is the most dominant factor related to the effect of pornographic exposure among adolescence with Odds Ratio of 5.02 (95% CI: 1.39-18.09). It is suggested to provide information in a professional way and targeted to the right group; to improve preventive efforts through collaboration within school, and to study further as to align positively those in the act-out stage.
