Abstract

Background: Currently, there are numerous analgesic advertisements which have been published in various media and have also attracted attention of the society. The aim of this study is to find out effects of analgesic advertisements on awareness and attention towards these advertisements on the community in Hegarmanah Village, Jatinangor.Methods: The study used the descriptive method with participants consisting of community members in Hegarmanah Village who have seen, watched or heard about the analgesic advertisements and who were aged 18 years and above. The sample for this study consisted of 100 respondents. This study was conducted in September 2012–December 2012.Results: The results showed that 82% of the respondents have seen the ads in at least the last 3 months and mostly watched them on television. About 52% of respondents agreed that many of the ads did not provide sufficient information. In addition, 50% only read a little bit of the ads rather than the whole advertisement. Fifty three percents of the respondents had the intention to try the medication after seeing the ads. More than 80% were aware about how to use the medication, medication’s side effects, warnings and contraindications and 65% agreed that, they could make a better decision on their health condition after seeing the ads.Conclusions: The analgesic advertisements indeed affected the community by making them aware about the ads and attracted them to buy as well as try the product itself. Further studies on factors which influence intake of over-the-counter analgesic drugs and also about the self-medication are required. Penelitian ini bertujuan untuk mengetahui pengaruh iklan analgesik terhadap kesadaran dan pemahaman mengenai obat analgesik pada masyarakat di Desa Hegarmanah, Jatinangor.Metode: Penelitian ini menggunakan metode deskriptif dengan melibatkan partisipan yang terdiri dari anggota masyarakat di Desa Hegarmanah yang telah melihat, menyaksikan atau mendengar tentang iklan analgesik dan berusia 18 tahun ke atas. Sampel penelitian ini terdiri dari 100 responden. Penelitian ini dilakukan pada bulan September 2012-Desember 2012.Hasil: Hasil penelitian menunjukkan bahwa 82% dari responden telah melihat iklan di setidaknya 3 bulan terakhir dan sebagian besar menonton di televisi. Sekitar 52% dari responden setuju bahwa banyak dari iklan tidak memberikan informasi yang cukup. Selain itu, 50% hanya membaca sedikit iklan daripada seluruh iklan. Lima puluh tiga persen dari responden memiliki niat untuk mencoba obat setelah melihat iklan. Lebih dari 80% yang sadar tentang penggunakan, efek samping, peringatan dan kontraindikasi obat  dan 65% setuju bahwa, mereka bisa membuat keputusan yang lebih baik tentang kondisi kesehatan mereka setelah melihat iklan.Simpulan: Iklan analgesik memang memengaruhi masyarakat dengan membuat mereka paham tentang obat yang diiklankan dan menarik mereka untuk membeli serta mencoba produk tersebut. Penelitian lebih lanjut tentang faktor-faktor yang memengaruhi penggunaan obat analgesik yang dijual bebas dan juga tentang pengobatan sendiri masih diperlukan. Kata kunci: iklan analgesik, efek, masyarakat DOI: 10.15850/amj.v1n2.346

Abstract

Background: Bladder cancer, particularly urothelial carcinoma, is prevalent in male and ≥55 years old population. Grade of the bladder tumor affects clinical characteristics, management, and prognosis of the patient. This study aimed to analyze the association between age and histopathological grade of bladder urothelial carcinoma. Methods: This cross-sectional analytical study was approved by the Ethical Clearance Committee of Dr. Hasan Sadikin General Hospital, involving 241 medical records of bladder urothelial carcinoma patients in the Department of Anatomical Pathology at Dr. Hasan Sadikin General Hospital during 2009–2013. Chi-square test was applied to 45 patients with complete record of histopathological grade.Results: Most of the subjects were 65 years old. Male patients were the most frequent. The proportion of low-grade carcinoma in <65 years old age group was only 39.1%, while in ≥65 years old age group was 72.7%. The proportion of high-grade carcinoma in <65 years old age group reached 60.9%, which was 27.3% higher than in ≥65 years old age group. Chi-square test result showed a statistically significant difference between histopathological grade of urothelial carcinoma in <65 years and ≥65 years age groups (p=0.023).Conclusions: Proportion of high-grade and low-grade urothelial carcinoma between <65 years and ≥65 years age groups are statistically different; therefore, an association between age and histopathological grade of bladder urothelial carcinoma is shown.

Abstract

Objective: To describe our experience about laparoscopic partial nephrectomy to treat localized renal tumour patients in Urology Department Hasan Sadikin General Hospital Bandung. Material & method: In 2014, there were 6 female patients that underwent laparoscopic partial nephrectomy in Hasan Sadikin General Hospital Bandung. In one case, there were tumours on both kidneys. Five patients were underwent transperitoneal approach and 1 patient were undergo retroperitoneal approach. All patients were examined with ultrasonography and Abdominal CT scan with contrast. We also calculate RENAL nephrometry score. Results: There were 6 female patients with left flank pain that undergo laparoscopic partial nephrectomy. In one case, there are tumours on both kidneys. In RENAL nephrometry score calculation, there were 3 patients with 10x score, 1 patient with 11x score, 1 patient with 7x score, and 1 patient with 6p score. From all of them, 4 patients were successfully performed laparoscopic partial nephrectomy, 1 patients was undergo laparoscopic nephrectomyand 1 patient is converted to open partial nephrectomy. Five patients were undergo transperitoneal approach and 1 patient was undergo retroperitoneal approach. Surgery ranged from 180-240 minutes. Intra operative bleeding was ranged from 50-200 cc. Tramadol intravenous was used for post operative pain control. With VAS score in first post operative day was 6, and 2 at the time of discharge. Hospitalized time was ranged from 4-6 days. Conclusion: Laparoscopic partial nephrectomy is an alternative treatment that safe for localized renal tumour. This procedure is depend on the technique and approach from each Urologist. Our limitations are we didn’t have laparoscopic ultrasonography and the tumour close to pelvocalyces system.

Abstract

Objective: It has been reported that normal value of penile length were varied among countries. Difference in race, ethnicity, genetics and nutrition plays a factor in penile length. Micropenis is an abnormality in which penile length were considered smaller in comparison of  a ‘normal penile length’. There are published article in Asia and Europe in terms of local penile length figures unfortunately none in West Java. This study was aim to establish local data as a reference in order to define micropenis. Material & method: All subjects in pediatric outpatient clinics were collected with cross sectional method. Exclusion criteria includes: congenital penile disease (hypospadias, burried penis, chordee, fistulas, webbed, etc), history of hormonal therapy, malnourished, and congenital syndromes. Subjects will undergone examination of stretched penile length. Results: A total of 276 children were included in the study with 23 subjects in each Tanner groups. The average penile length in Hasan Sadikin Hospital were inferior in all groups in contrast with the Tanner age groups as cited in table 1. Conclusion: Normative local data of penile length is necessary. Not using the local reference may result in underestimation and overdiagnosis.

Abstract

Objective: To share our experience of retroperitoneal laparoscopic dismembered pyeloplasty as the treatment of Ureteropelvic Junction Obstruction (UPJO) in Department of Urology, Hasan Sadikin Hospital Bandung. Material & Methods: Boy 8 years old came for a chance to relieve mass at both flank region especially at left flank region. From the ultrasonography there were bilateral hydronephrosis. We decided to urethrocystoscopy, bilateral ureteral catether insertion, bilateral retrograd pyelography guided with c-arm and left retroperitoneal laparoscopic dismembered pyeloplasty. Results: Operative time was 240 minutes and blood loss was about 200cc. Double J Stent was used for ureteral stenting. Parenteral ketorolac was given as needed to control the pain. The Visual Analogue Scale (VAS) post operative day 1 until 4 were6, 4,, 2, 2 respectively. Urethral catheter was removed at post operative day 2 and drain was removed at post operative day 3. Operating wound was good. Patient was hospitalization until post operative day 4. Left nephrostomy was removed at postoperative day 7 after there was no leakage, confirmed with left antegrad pyelography. From post operative ultrasonography we found that hydronephrosis at left kidney became almost normal. Conclusion: In our opinion that the retroperitoneal laparoscopic dismembered pyeloplasty is an alternative treatment for UPJO. This procedure depends on experience and pristine surgical tehnique from the surgeon

Abstract

Objective: To determine the effect of topical gentamycin application at the urethral stent to the stent’s colony-forming-unit (CFU) in hypospadias patients. Material & methods: This study is a double blind prospective randomized controlled study, conducted from November 2016 - August 2017 in Urology Department Hasan Sadikin Hospital Bandung. We include patients with distal until penoscrotal hypospadias who underwent urethroplasty for the first time. The subjects were divided into 2 groups: Group A were patients performed urethroplasty with nasogastric tube (NGT) urethral stents only. Group B were patients performed urethroplasty with NGT urethral stents that were coated with topical gentamycin. Both group were operated by a single surgeon using either Tubularized Incised Plate (TIP) or Onlay Preputial Flap technique. Both group used supra pubic urinary diversion. Both group were given the same preoperative and postoperative intravenous antibiotics and given same dressing. At post operative day 7, the stents were removed and swabbed for bacterial culture and resistance test and colony-forming-unit (CFU) counts. Demographic data is described and, among others were analyzed statistically. Other adverse events and complications were also documented. Results: There were 25 hypospadias patients who were performed urethroplasty (12 patients in Group A and 13 patients in Group B). The median age in group A was 7 ± 2.995 (years old) and the mean age in group B was 6 ± 3.178  (years old). In group A, 1 patient (8.3%) were distal shaft type, 1 patient (8.3%) were middle shaft type, 8 patients (66.7%) were penoscrotal type, 1 patient (8.3%) were proximal shaft type, and 1 patient (8.3%) were subcoronal type. In group B, 3 patients (23.1.%) were distal shaft type, 3 patients (23.1%) were middle shaft type, 5 patients (38.5%) were penoscrotal type, and 2 patients (15.7%) were proximal shaft type. In group A, 6 patients (50%) using TIP, 6 patients (50%) using onlay preputial flap. In group B, 5 patients (38.5%) using TIP, 8 patients (61.5%) using onlay preputial flap. In Group A, Staphylococus haemoliticus was the most common bacterial found in urethral stent swabbed culture (50%), followed by Pseudomonas aeroginosa (16.7%), Acinetobacter baumannii (8.3%), Enterococcus faecalis (8.3%), Morganella morgagnii (8.3%) and sterile cultures (8.3%). In Group B, 38.5% were sterile urethral stents culture, followed by Staphylococus haemoliticus (30.8%), Enterobacter cloacae (7.7%), Staphylococus warneri (7.7%), Staphylococus epidermidis (7.7%), and Moraxella catarrhalis (7.7%). The most sensitive antibiotics in both group were Amikacin, Gentamycin, Meropenem, Cotrimoxazole, and Cefepime. In group A, 11 patients (91.7%) had >100.000 CFU counts, and 1 patient (8.3%) with no colony found.  In Group B, 2 patients (15.4 %) had >100.000 CFU counts, 6 patients (46.2%) had <100.000 CFU counts, and 5 patient (38.5%) with no colony found. In group A, there were 3 patients (25%) with urethra-cutaneous fistula, 4 patients (33.3%) with hematoma, and 5 patients (38.5%) without any complications. In group B, there were 2 patients (15.4%) with urethra-cutaneous fistula, 3 patients (23.1%) with hematoma and 8 patients (61.5%) without any complication. Using Mann-Whitney U-test, we found a significant difference of CFU counts formation between the two groups (p=0.001). Statistically, the lesser CFU counts found, the lesser complication were developed (p=0.001). Conclusion: Topical gentamycin application at the urethral stents in hypospadias patients performed urethroplasty significantly reduced CFU counts found at the urethral stents and thus reduced the urethra-cutaneous fistula and hematoma formation.

Abstract

Objective: Conjoined twins represent one of the rarest congenital anomalies occurring with a varying incidence of about approximately 1 : 50.000 - 1 : 200.000 births. Ischiopagus conjoined  twinning (Fuse at the hip region) is even rare representing only 6% of all conjoined  twins. Here we report our experience in lower urinary tract reconstruction during separation of a conjoined twin ischiopagus tripus in Hasan Sadikin Hospital, Bandung. Case(s) Presentation: Two years old female conjoined twins, weighing in total of 8 kilograms, were brought to our hospital. The twins were fused  at  the lower  halves of  their bodies with two separate lower limbs at the right side and a fused left lower limb. External genitalia were absent. From CT Angiography we found four kidneys, two in each child with two separate bladders, with right ureter of baby A and left ureter of  baby B enter the  right bladder, while left ureter of  baby A and  right ureter of  baby B enter the left bladder. There was one cloaca that drains urine and feces. During the separation surgery, we performed ureteroneocystostomy and temporary ureteral stenting, followed by cystostomy. The pediatric surgeon performed bowel separation by stapling technique, pelvic wall separation, ileostomy, and pelvic floor reconstruction. The orthopedic surgeon performed femur separation, pelvic floor reconstruction, reconstruction of the pelvic ring, and femur reconstruction. The patients underwent defect closure, which performed by plastic surgeon. The operation was successful, without any complication to the kidney and the urinary tract of both babies. Discussion: Ischiopagus conjoined twins are the most complex conjoined twins and occur in only 6% of all conjoined twin cases. Ischiopagus is usually located along the axis with the head located on the opposite side. This case usually has one umbilicus and the two bodies are fused below this level, the union occurs in the lower abdomen and the pelvis. The internal organs that usually coalesce in cases of ischiopagus conjoined twins are liver, lower gastrointestinal, and genitourinary. The fused gastrointestinal part is the ileo-colic part of terminal ileum (Meckel's diverticulum). Conclusion: The separation of conjoined twin is a unique challenge due to its complex anatomy and physiology. Although advancement in imaging and monitoring has improved the survival rate separation can be successfully achieved only with meticulous planning and team work.

Abstract

Objective: To determine the average size of external urethral meatus in children in West Java, and to find out whether there was a correlation between the size of external urethral meatus and age of boys in West Java. Material & Methods: This research was an analytical with prospective study design. Our research subjects consist of children hospitalized between November 2017-November 2018 at Hasan Sadikin General Hospital, Bandung. Measurement is done by measuring the diameter of the external urethra using meatal calibrator, recorded as meatus size on a French scale. Inclusion criteria in this study were all pediatric patients aged 0-12 years who consented to be involved in the study. The exclusion criteria were patients who had urethral diseases such as hypospadias, epispadias, strictures, and history of instrumentation and interventional procedures on the urethra. After data collection, data were analyzed using SPSS 18. Results: We included 187 patients in our study, with the youngest being 1 month old and the oldest being 12 years. We documented the mean external urethra meatal measurement and grouped the subjects into 4 age groups. The average size of the external urethral meatus in the infant age group (0-12 months) was 5.82 Fr, the toddler age group (1-3 years) it was 6.34 Fr, in the preschool age group (3-6 years) it was 8.51 Fr, and in the primary school age group (6-12 years) it was 12.90 Fr. Based on the results of measurements, there is a correlation between age and external urethral meatal size (p=0,000). The older the child, the greater the size of the external urethral meatus obtained. Conclusion: The size of the external urethral meatus maximum stretch in boys in West Java increases with age. The results of this study are expected to be used as a reference of normal anatomical size, particularly the external urethral meatal size in boys in West Java.
