Abstract

Objective: The aim of the study is to determain the relation between blood glucose level and cognitive function score in stroke ischemic patient at RSUD Dr. Moewardi. Background: . An ischemic stroke is death of an area of brain tissue (cerebral infarction) resulting from an inadequate supply of blood and oxygen to the brain due to blockage of an artery. Cognitive impairments that may be experienced after the occurrence of strokeinclude difficulty concentrating for the amount of time, organization, forgetfulnessand being easily confused. The state of hyperglycemia is common in the acute phase of stroke caused by stress and an increase in serum catecholamines. This study aimed to proof that their blood glucose levels rise will increase infarct size, decreased cognitive function and clinical output effects are worse, especially in non-hemorrhagic stroke Methods: This analytic study was analytic observational with cross-sectional approach. A sample of 50 study subjects was selected by random sampling from outpatients with stroke ischemic visiting the Neurology Clinics, RSUD Dr. Moewardi Surakarta. The data were collected by interview using a set of questionnaire. The data was analyszed using multiple linear regression model on SPSS version 17 for Windows. Results: There is a negative significant association between blood glucose level and Patients stroke ischemic with blood glucose level ≥145mg/dL has MMSE score 5,39 point lower than blood glucose level <145mg/dL ( b= -5,39 ; CI 95% -6,98 sampai -3,81; p<0,001). This estimate has controlled for the effects of confounding variables such as level of education and age. Conclusion: There is a statistically significant association between blood glucose level and cognitive function score in stroke ischemic patient. This conclusion is drawn after controlling for the effects of confounding factors such as level of education and age.   Keywords: Stroke ischemic,blood glucose level, cognitive functional score.

Abstract

Background: There is no definite epidemiological data about the number of patients with epilepsy in Indonesia. It is estimated that there are 900.000–1.800.000 epilepsy patients. The onset can be at any age, in which 30–32.9% patients experience the first convulsion less than 4 years old, 50-51.5% in less than 10 years old, 75–83.5% in less than 20 years old, 15% in more than 25 years old  and 2% in more than 50 years old. Several risk factors cause epilepsy in children e.g. hereditary factor, asphyxia, premature, complex febrile seizure and head trauma. Methods: This was a case control study. Subject were selected consecutively from epilepsy children in Dr. Moewardi Hospital and matched with pediatric patients in Outpatlient Pediatric Clinics. The data was collected by structured interview and medical record. The data analyzed by Chi-Square test and Logistic Regression. All statistic analysis was measured by Statistical Product and Service Solution (SPSS) 16.00 for Windows. Results: There are 84 subjects eligible for the criteria. Asphyxia (p= 0.121) and premature (p= 0.393) are not risk factor for epilepsy. Hereditary factor (p= 0.001), complex febrile seizure (p= 0.000), and head trauma history (p= 0.001) are significant risk factor for epilepsy. Logistic regression result shows that the risk factors affecting epilepsy in children at Dr. Moewardi Hospital were complex febrile seizure (OR= 18.267 ; Cl 95% 5.393 s.d. 61.873 ; p= 0.000) and head trauma history (OR= 16.341 ; Cl 95% 3.346 s.d. 79.801 ;  p= 0.001). Conclusions: Risk factor of epilepsy in children are complex febrile seizure and head trauma history. Background    : Stroke is a disease caused high morbidity and mortality rate in low-middle income countries. Low physical activity is one of the risk factors for ischemic stroke. The impact of low physical activity is high level of NADPH, atherosclerosis, and vascular elasticity disorder. Global Physical Activity Questionnaire version 2 (GPAQv2) is a questionnaire released by WHO which has been especially designed to measure the physical activity of people in developing countries. The aim of this study is to determine the correlation between man physical activities and ischemic stroke in Dr Moewardi Hospital Surakarta. Methods: This was an analytical observational study with case control approach. This study was conducted during May - July 2013 at the Neurology Department of Dr. Moewardi Hospital Surakarta. Subjects were acute ischemic stroke patients in neurology ward of Dr. Moewardi Hospital Surakarta which were taken by using fixed disease sampling. Data were collected by interviewed patients based on GPAQv2 questionnaire. Data analysis used multivariate with double logistic regression and have been processed by Statistical Product and Service Solution (SPSS) 16.00 for Windows. Results: This study showed statistically significant (p=0,036) between man physical activities and acute ischemic stroke in Neurology Department Dr. Moewardi Hospital Surakarta. Age and smoking as comfounding factors have been controlled. Male patients with low physical activity had 13.95 times (OR = 13.95; CI 95% 1.19 – 163.58) to develop ischemic stroke than ones with adequate physical activity.
