Abstract

Background: The emerging of sex selection technology in Indonesia is sperm sorting, meanwhile sex selection with Preimplantation Genetic Diagnosis (PGD) methods is not widely used. The use of sperm sorting has bigger chance to fail than PGD, thus potentially cause ethical and legal problems that is selective abortion during pregnancy. The potency for selective abortion is enlarged by Indonesian law that permitting sex selection without distinction of medical and non-medical reasons. There is no special policy to regulate the selective abortion because of sex selection failure.  Aims: This study aims to find out the legal concept of selective abortion after preimplantation sex selection that appropriate to be applied in Indonesia.Methods: This research is normative research that use analytics method with legal approach and conceptual approach. The research material consists of primary legal material (legislation about sex selection and abortion in Indonesia), secondary legal materials (legal journals, law books, and legal proceedings), and also non-legal materials (journals, books, and health proceedings about sex selection and abortion).Results: In Indonesia meanwhile in general, abortion is permitted for pregnancy with medical indication and rape victim. Through a statue approach in Indonesia, selective abortion after preimplantation sex selection can be implemented for strong medical reasons. While the failure for non-medical reasons can’t be aborted. This regulation contrary with ethics, morals and religion. Selective abortion should not be done because of preimplantation sex selection failure either medical or non-medical reasons. Conclusion: Selective abortion after preimplantation sex selection both medical and non medical reason contrary with moral, ethical, and religion. Indonesia needs to regulate further policy about selective abortion if there is a failure of preimplantation sex selection.

Abstract

The infant mortality rate is the important things to measure health status and achievement of Sustained Development Goals (SDGs). It can be used to audit the cause of perinatal death and used to planning program to prevent the death. In addition, data on perinatal deaths must be recorded in terms of population administration. The problem is underreported perinatal death so that data not valid. This is because many government stakeholder such as Health Office, Administrative Village, Family Empowerment and Welfare (PKK), and the Information and Communication Department has reported and recorded perinatal death themself but not integrated. This study aims to develop a concept of integrated perinatal death report and record. This research is qualitative research with following steps advocation, initial assessment, and development models through interview, observation, and Focus Group Discussion methods. The result of this study is existing condition and concept model that can integrate of  perinatal death record and reported. The concept model in Semarang City, Indonesia can be done in two ways. First, data comes from the Health Office (SIM-KIA). Second, data comes from Administrative Village and Family Empowerment and Welfare (PKK). All of the data forwarded to the Integrated Data Integration System (SIDADU) in Information and Communication Service in Semarang City. To support the validity of data, the Health Office should identify all public and private health facilities related to perinatal care. The local government should release Circular Letter concerning the notification of services regarding the recording and reporting of perinatal deaths in Administrative Village.

Abstract

Akreditasi merupakan penilaian fasilitas kesehatan oleh Komisi Akreditasi agar fasilitas kesehatan dapat memberikan pelayanan yang berkualitas sesuai standar. Belum dilakukanya akreditasi berdampak pada mutu kinerja Klinik Pratama, kepuasan pasien, kualitas SDM, keamanan SDM dan keberlanjutan kerjasama antara Klinik Pratama dengan BPJS. Penelitian ini bertujuan untuk menganalisis kesiapan Klinik Pratama dalam menghadapi akreditasi di Kapubaten Cirebon dan faktor–faktor yang mempengaruhinya menggunakan metode kualitatif – deskriptif. Pengambilan data dilakukan melalui wawancara mendalam kepada subjek yang dipilih menggunakan teknik purposive sampling. Berdasarkan hasil penelitian, Klinik Pratama di Kabupaten Cirebon belum membentuk tim akreditasi, dikarenakan pengetahuan SDM terkait persiapan akreditasi masih minim dan belum memiliki pedoman akreditasi. Akibatnya, perencanaan administratif manajemen akreditasi belum dilakukan. Selain itu, anggaran akreditasi masih dialokasikan untuk pemenuhan fasilitas yang menyebabkan alokasi anggaran pada akreditasi tidak maksimal. Berdasarkan hal tersebut, Klinik Pratama di Kabupaten Cirebon belum siap menghadapi akreditasi karena terkendala persiapan akreditasi pada faktor standar dan tujuan kebijakan, sumber daya, komunikasi, karakteristik lembaga pelaksana, dan dukungan dinas kesehatan yang belum optimal.

Abstract

ABSTRAKLatar belakang: Program SPGDT belum menunjukkan hasil maksimal, sehingga banyak dikeluhkan oleh masyarakat ketika mereka membutuhkan pelayanan kesehatan. Meskipun hampir di setiap kota terdapat Instalasi Gawat Darurat dari semua tipe rumah sakit baik pemerintah atau swasta, pelayanan ambulans dan berbagai fasilitas kesehatan lainnya, namun keterpaduan dalam melayani penderita gawat darurat belum sistematis, kurangnya komunikasi baik antar fasilitas kesehatan dan antar tenaga kesehatan dengan masyarakat. Tujuan dari literatur review ini adalah untuk menjelaskan gambaran pelakasanaan SPGDT dan faktor-faktor yang mempengaruhi keberjalanan SPGDT.Metode: Penelitian ini merupakan penelitian yang menggunakan literature review sederhana. Pencarian database yang digunakan termasuk ScienceDirect, Scopus, PubMed, Portal Garuda dan Google Cendikia dengan kata kunci serta kriteria inklusi dan eksklusi yang sudah ditetapkan. Hasil: Faktor yang mempengaruhi program SPGDT adalah standar operasional prosedur (SOP) dan sumber daya. Hasil penelitian menemukan bahwa SOP diberbagai daerah masih kurang baik dalam pembahasan SOP dan sosialisasi terhadap masyarakat. Sumber daya di puskesmas sudah baik dalam hal anggaran akan tetapi masih kurang pada bagian sumber daya manusian karena petugas pelaksana unit puskesmas belum pernah dilatih dan belum mengantongi sertifikat pelatihan kegawatdaruratan.Simpulan: Dalam keberjalanan program SPGDT diberbagai daerah masih belum dapat memenuhi standar pemerintah pusat baik dalam waktu tanggap, SOP, dan sumber daya.Kata kunci: SPGDT; gawat darurat; implementasi; kesiapan ABSTRACTTitle: Analysis of the Implementation of the SPGDT Program in IndonesiaSPGDT; gawat darurat; implementasi; kesiapanBackground: The SPGDT program has not shown maximum results, so that many people complain about it when they need health services. Although in almost every city there are Emergency Departments of all types of hospitals, both government and private, ambulance services and various other health facilities, but integration in serving emergency patients has not been systematic, lack of good communication between health facilities and between health workers with the community. The purpose of this literature review is to explain the description of the SPGDT implementation and the factors that influence the running of the SPGDT.Method: This research is a research that uses a simple literature review. The database searches used included ScienceDirect, Scopus, PubMed, Portal Garuda and Google Scholar with predefined keywords and inclusion and exclusion criteria.Result: The factors that influence the SPGDT program are standard operating procedures (SOPs) and resources. The results of the study found that SOPs in various regions were still not good at discussing SOPs and socialization to the community. The resources at the puskesmas are good in terms of budget but are still lacking in the human resources section because the implementing officers of the puskesmas unit have never been trained and have not yet obtained an emergency training certificate.Conclusion: In the course of the SPGDT program in various regions, it has not been able to meet central government standards in terms of response time, SOP, and resources.Keywords: SPGDT; emergency; implementation; readiness

Abstract

ABSTRAKLatar belakang: Terjadinya penyebaran Coronavirus Disease 2019 (COVID-19) di beberapa negara termasuk Indonesia di tahun 2020, dimana provinsi Jawa Tengah termasuk kedalam lima besar provinsi dengan kasus COVID-19 tertinggi. Pandemi COVID-19 ini dapat membuat kualitas hidup masyarakat menjadi lebih buruk dan menimbulkan kerentanan, dimana karakteristik suatu keluarga juga dapat mempengaruhi resiko kerentanan. Dengan adanya pandemi COVID-19 dapat mendorong adanya permasalahan ekonomi. Tujuan dari penelitian ini yaitu menganalisis hubungan karakteristik terhadap status kerentanan keluarga menghadapi pandemi COVID-19 di Provinsi Jawa Tengah.Metode: Jenis penelitian ini yaitu penelitian survei, pendekatan kuantitatif yang bersifat cross sectional. Terdiri dari 167 sampel dengan teknik pengambilan sampel menggunakan accidental sampling. Penelitian ini dilakukan di Provinsi Jawa Tengah pada bulan April hingga Desember 2020 yang dilakukan secara online. Variabel dalam penelitian ini yaitu karakteristik dan status kerentanan keluarga. Pengumpulan data menggunakan kuesioner berupa google form. Analisis univariat menggunakan distribusi frekuensi. Analisis bivariat menggunakan chi square, dan rank spearman.Hasil: Hasil uji hubungan menunjukkan bahwa variabel rata-rata penghasilan/bulan (p=0,006) berhubungan dengan status kerentanan keluarga. Sedangkan variabel umur(p=0,525), tingkat pendidikan (p=0,142), status pekerjaan (p=0,074), jumlah tanggungan keluarga (p=0,364), dan status keluarga (p=0,692) tidak berhubungan dengan status kerentanan keluarga.Simpulan: Karakteristik responden berdasarkan rata-rata penghasilan/bulan berhubungan dengan status kerentanan keluarga dalam menghadapi COVID-19 di Provinsi Jawa Tengah.Kata kunci: Karakteristik Individu, Kerentanan Keluarga, COVID-19 ABSTRACT Title: Family Vulnerability Status in terms of Characteristics of Respondents to Face the Covid-19 Pandemic in Central Java Province Background: The spread of Coronavirus Disease 2019 (COVID-19) in several countries including Indonesia in 2020, where the province of Central Java is included in the top five provinces with the highest COVID-19 cases. The COVID-19 pandemic can make people's quality of life worse and lead to vulnerability, where the characteristics of a family can also affect the risk of vulnerability. With the COVID-19 pandemic it can lead to economic problems. The purpose of this study is to analyze the relationship between characteristics and vulnerability status of families to face the COVID-19 pandemic in Central Java Province.Method: This is a survey research, quantitative approach is cross sectional. There are 167 samples, the sampling technique using accidental sampling. This research was conducted in Central Java Province from April to Desember 2020 by online. The variables are characteristics, and perceptions of physical distancing, and family vulnerability status. Data collection using a questionnaire using google form. Univariate analysis using frequency distribution. Bivariate analysis using chi square, and rank spearman.Result: The results of the correlaton test is the variable of average income / month (p= 0.006) were related to the vulnerability status of the family. Meanwhile, age (p = 0.525), education level (p = 0.142), type of work (p = 0.074), number of family dependents (p = 0.364), and family status (p = 0.692) were not related to family vulnerability status.Conclusion: Characteristics of respondents based on average income / month are related to the vulnerability status of families in facing COVID-19 in Central Java Province.Keywords: Individual Characteristics, Family Vulnerability, COVID-19

Abstract

Indonesia targeting that at least 70% of the population vaccinated and formed the herd immunity. However, there are several challenges in reaching that target, such as the anti-vaccine movements, disinformation or hoaxes, fear of the Adverse Event Following Immunization (AEFI), and fear of needles. Based on the preliminary study, the area of Bandarharjo Public Health Centre was in the lowest 3rd place related to the COVID-19 vaccination coverage in Semarang. This was due to lack of education to the community regarding the vaccination, the high resistance to vaccination due to concerns about AEFI, and so on. Health cadres are one of the main actors in the supporting the health programs in PHC, therefore the role of health cadres need to be optimized in education and management of AEFI related to COVID-19 vaccination. This community service program was carried out offline in 30 health cadres in the working area of Bandarharjo PHC. The activity began with a pre-test, continued with the presentation of the topic, discussion session, and ends with post-test. The results of the pre-test and post-test showed that there was an increase in the knowledge of cadres between before the presentation and after the presentation session. Health cadres also received booklets containing general information about COVID-18 vaccination and also related to AEFI. Booklets given to cadres not only to increase their knowledge but also can be used to educate the community. Hopefully, with the increase in knowledge and understanding of cadres, the community will be easily get the right information. The health cadres commitment to educate the community regarding COVID-19 vaccination also hoped to increase the community knowledge related to the vaccination and also the willingness of community to receive the COVID-19 vaccination, especially in the working area of Bandarharjo PHC, Semarang City, Central Java Province.

Abstract

ABSTRAK Tidak semua ibu peserta Kelas Ibu Hamil (KIH) melakukan praktik pencegahan komplikasi sesuai anjuran tenaga kesehatan. Ada kesenjangan persepsi dengan belum optimalnya praktik pencegahan komplikasi. Tujuan penelitian melihat pengaruh faktor dukungan dan persepsi terhadap perilaku pencegahan komplikasi peserta KIH. Merupakan studi kuantitatif berdesain cross-sectional. Populasi peserta KIH di Kota Semarang dengan sampel 120 orang. Pengumpulan data melalui wawancara, selanjutnya dianalisis secara parsial dan simultan dengan rank-spearman test dan regresi logistik berganda. Persepsi berhubungan dengan pengetahuan, sikap dan praktik. Dukungan fasilitator berkorelasi dengan pengetahuan dan sikap, namun tidak dengan praktik. Dukungan keluarga/masyarakat hanya berkorelasi dengan praktik. Sikap secara simultan dipengaruhi dukungan fasilitator dan pengetahuan (OR=1,444 dan OR=1,226), sedangkan praktik dipengaruhi dukungan keluarga/masyarakat (OR=2,239), persepsi (OR=1,540) dan dukungan fasilitator (OR=0,692). Sikap bukan pembentuk langsung praktik, karena lebih ditentukan persepsi yang terbentuk dari keikutsertaan KIH dan dukungan fasilitator. Persepsi dan dukungan mempengaruhi perilaku pencegahan komplikasi kehamilan. Dukungan keluarga/masyarakat adalah faktor pengaruh dominan sekaligus faktor penguat. Melalui sosialisasi, pendampingan dan konseling dari tenaga kesehatan, serta penguatan dari keluarga/masyarakat dapat meningkatkan perilaku pencegahan komplikasi kehamilan. ABSTRACT Not all mothers participating in Antenatal-Classes practiced the prevention of pregnancy complications according to the health worker’s advice. There was perception gap with the not yet optimal complication prevention practice. The study purpose was analysing the effect of supporting factors and perception to preventive behavior of complications. The quantitative study with cross-sectional design. Population was AC-participants in Semarang city with 120 samples. Data collected through interviews, analyzed partially and simultaneously with rank-spearman test and multiple logistic regression. Perception was related to knowledge, attitudes and practices. Facilitator support was correlated with knowledge and attitude. Family/community support only correlated with practices. Attitudes were simultaneously influenced by facilitator support and knowledge (OR=1.444 and OR=1.226), while practice was influenced by family/community support (OR=2.239), perception (OR=1.540) and facilitator support (OR=0.692). Practice was not directly caused by attitude, because it was determined more by perceptions that were formed from participation in AC and facilitator support. Perception and support influenced the complications preventive behavior. Family/community support were a dominant influencing and reinforcing factor. Through socialization, assistance and counseling from health workers and strengthening from family/community could improving the pregnancy complications preventive behavior. Abstract

Latar belakang: Puskesmas sebagai salah satu fasilitas pelayanan kesehatan berkewajiban dalam memberikan pelayanan kesehatan yang aman dan bermutu. Pelaksanaan program keselamatan pasien juga merupakan suatu hal yang wajib dilaksanakan oleh seluruh fasilitas pelayanan kesehatan dalam memberikan pelayanan kesehatan. Puskesmas X adalah salah satu Puskesmas yang ada di Kabupaten Temanggung dengan status akreditasi madya. Adapun tujuan dalam penelitian ini yaitu menganalisis pelaksanaan program keselamatan pasien di Puskesmas X Kabupaten Temanggung.Metode: Penelitian kualitatif menggunakan pendekatan studi kasus di Puskesmas X. Teknik pengumpulan data yaitu indepth interview dengan purposive sampling. Subyek penelitian ini yaitu Kepala Puskesmas, Penanggungjawab Keselamatan Pasien, Penanggungjawab Ruangan serta pasien.Hasil: Pelaksanaan program keselamatan pasien di Puskesmas X belum dilaksanakan secara optimal. Hal ini dikarenakan komitmen akan pelaksanaannya yang masih kurang terutama pada sisem pelaporan insiden. Adapun Puskesmas X juga belum menerapkan Tujuh Langkah Menuju Keselamatan Pasien sehingga dalam pelaksanaannya masih belum sesuai dengan Permenkes yang mengatur tentang keselamatan pasien. Selain itu juga terdapat beberapa faktor yang menyebabkan pelaksanaan keselamatan pasien belum optimal yaitu, jumlah SDM yang kurang dan kurangnya pelatihan terkait keselamatan pasien, tidak adanya anggaran dana untuk program keselamatan pasien, serta belum adaya kebijakan/SOP yang mengatur tentang penerapan Tujuh Langkah Menuju Keselamatan Pasien.Simpulan: Oleh karena itu, Puskesmas perlu membuat kebijakan/SOP yang mengatur tentang penerapan Tujuh Langkah Menuju Keselamatan Pasien sehingga dalam pelaksanaan program keselamatan pasien dapat lebih tersistem dan terukur agar dapat lebih optimal.Kata kunci: Keselamatan Pasien, Program, Tujuh Langkah Menuju Keselamatan Pasien ABSTRACTTitle: Analysis of the implementation of patient safety program at  X  Public Health Center, Temanggung RegencyBackground: Publis Health Center as a health service facility is obliged to provide safe and quality health services. The purpose of this study is to analyze the implementation of the patient safety program at the X Public Health Center, Temanggung Regency.Methods: Qualitative research using a case study  approach at X Public Health Center. The data collection technique was in-depth interview with purposive sampling. The subjects of this study were the head of the Public health center, the person in charge of patient safety, the person in charge of the room and the patient.Result: The implementation of the patient safety program at the X Public Health Center has not been implemented optimally. This is due to the lack of commitment to implementation, especially in the incident reporting system. The X Public Health Center also has not implemented the Seven Steps to Patient Safety so that its implementation is still not in accordance with the Permenkes which regulates patient safety. In addition, there are several factors that cause the implementation of patient safety is not optimal include, the lack of human resources and lack of training related to patient safety, the absence of a budget for patient safety programs, and no policies / SOPs that regulat about implementing the Seven Steps to Patient Safety.Conclusion: Therefore, the Puskesmas needs to make a policy / SOP that regulates the implementation of the Seven Steps to Patient Safety so that the implementation of patient safety programs can be more systematic and measured in order to be more optimal.Keywords: Patient Safety, Program, Seven Steps to Patient Safety

Abstract

The contact tracing policy strategy implemented during the COVID-19 pandemic requires data on COVID-19 patients to be accessible to the public so that people who have been in contact with the positive patient can do self-isolate. However, this is contrary to medical secrets and the confidentiality of COVID-19 patient data as a form of private law. This research is normative research with a statutory approach (statue approach) and conceptual (conceptual approach). Based on ethical and legal aspects, the confidentiality of COVID-19 patient data must be maintained. This data can be disclosed with the patient consent or disclosed to the Health Office to conduct epidemiological investigations. Suppose the contact investigation involves several parties: the neighborhood unit, the village head, Village Community Empowerment Service, police, and other parties. In that case, further regulation is needed to ensure the commitment of these parties in maintaining COVID-19 patient data. Disseminating patient data openly or publicly can make harm to the patient, especially in the not good environment with risk of stigmatization. There is also a need for harmony between existing regulations and the programs or policies implemented by the government.

Abstract

COVID-19 disease has the potency to increase maternal and child mortality rates. It is because pregnant women and children include the vulnerable groups. To increase maternal and child health resilience during the COVID-19 pandemic, community empowerment was needed. Many programs can be done through innovative planning and budgeting. The village fund can be an alternative solution in this condition. This research was qualitative research that used Focus Group Discussion with the participant contains village head, cadres, health professional, Department of Health agent, Village Community Empowerment Service, Regional Planning and Development Agency in the Cirebon Regency.  Based on the research, maternal and child health resilience could be improved through developing the innovation program related to maternal and child health such as support in COVID-19 vaccination to pregnant women, support to high-risk pregnant women, and so on. The collaborative model could be done between village funding, Corporate Social Responsibility (CSR), Family Welfare Empowerment’s funding, and non-governmental funding.  To identify the situation analysis it can be done through discussion at the village level (Musdes). It also can collaborate the maternal and child program with the national program, adaptation of new habits, and strategic local issues.Abstrak: Penyakit COVID-19 berpotensi meningkatkan angka kematian ibu dan anak. Sebab, ibu hamil dan anak-anak termasuk kelompok rentan. Untuk meningkatkan ketahanan kesehatan ibu dan anak di masa pandemi COVID-19, diperlukan pemberdayaan masyarakat. Banyak program yang dapat dilakukan melalui perencanaan dan penganggaran yang inovatif. Dana desa bisa menjadi alternatif solusi dalam kondisi ini. Penelitian ini merupakan penelitian kualitatif dengan metode Focus Group Discussion dengan peserta terdiri dari kepala desa, kader, tenaga kesehatan, Dinas Kesehatan, Dinas Pemberdayaan Masyarakat Desa, Pemberdayaan Kesejahteraan Keluarga (PKK), dan Badan Perencanaan dan Pembangunan Daerah di Kabupaten Cirebon. Berdasarkan penelitian, ketahanan kesehatan ibu dan anak dapat ditingkatkan melalui pengembangan program inovasi terkait kesehatan ibu dan anak seperti dukungan vaksinasi COVID-19 pada ibu hamil, dukungan pada ibu hamil risiko tinggi, dan sebagainya. Model kolaboratif dapat dilakukan dengan dukungan dana desa, Corporate Social Responsibility (CSR), pendanaan PKK, dan swadaya masyarakat. Untuk mengetahui kebutuhan serta analisis situasi, diskusi dapat dilakukan melalui musyawarah tingkat desa (Musdes). Kolaborasi program dan pendanaan juga dapat dilakukan dengan menginsersikan program kesehatan ibu dan anak dengan program strategis nasional, adaptasi kebiasaan baru, dan program strategis terkait isu lokal.
