Abstract

The purpose of this study were to prove that the administration of heated palm oil can cause hepatocytes damage and cholesterol serum level incerase in mouse. This research was an experimental research using 38 male Mus musculus, Swiss derived. They were divided into 3 groups; 9 were the first negative control by aquadest administration, 9 were the second negative control by fresh palm oil administration, and 20 were intervention by 27-times heated palm oil administration. The dose of administration was 1 ml/100 gram body weight, every day for 14 days. Parameter used in examining liver microscopically was the percentage of hepatocytes getting degeneration of each vision field by 100-times magnificence. The result of microscopic examination were normal hepatocytes for aquadest administration group, fatty degeneration for fresh palm oil administration group, fatty degeneration and central zone necrosis for 27-times heated palm oil group. The average value of cholesterol serum level in aquadest administration group was 101,44 ± 8,71 mg/dl, in fresh palm oil administration group was 104,66 ± 7,75 mg/dl, and in 27-times heated palm oil group was 130,15 ± 12,26 mg/dl. It could be concluded that administration of 27-times heated palm oil for 14 days can cause degeneration of hepatocytes (p<0,05) and cholesterol serum level increase (p<0,05). Abstract

Treatment of cancer such as surgery, radiotherapy and chemotherapy has many side effects. Chemopreventive agent is needed to reduce the side effect and increase the effectivity of therapy. The discovery of  cochemopreventive agent should consider on its selectivity to reduce side effects. The selective cochemopreventive agents work effectively in cancer cells and safe for normal cells. Buah Makassar (Brucea javanica) is a natural product that is empirically used for anti-inflammatory and antitumor. The purpose of this study is to determine the cytotoxic effect of ethanolic extract of buah Makassar against 4T1, MCF7, HeLa, and Vero cell lines. The cytotoxic test is performed by MTT assay. The parameter obtained from the cytotoxic test was IC50. Selectivity index is determined from IC50 ratio of cancer cells to normal cells. The results showed that ethanolic extract of buah Makassar has a cytotoxic activity on 4T1, MCF7, HeLa, and Vero cells with IC50 were 49,9±0,83 μg/mL; 107,6±8,14 μg/mL; 228,9±4,16 μg/mL and 395,5± 4,21 μg/mL respectively. It also has high selectivity on 4T1 metastatic breast cancer cell with selectivity index of 7,93. It can be concluded that the ethanolic extract of buah Makassar has potential to be delevoped as cochemopreventive agent especially on metastatic breast cancer. Abstract

Less optimized therapeutic effects constrain the use of doxorubicin as the main agent of chemotherapy for metastatic breast cancer, resistance and side effects. Therefore we need a combination of more than one chemopreventive agent which has different molecular targets to solve that problem. The aims of this study is to prove the inhibitory effect of ethanolic extract of rhizome of Curcuma xanthorrhiza (ECx), fruit of Brucea javanica (EBj), leave of Ficus septica (EFs) and doxorubicin (Dox) alone and its combination on migration and invasion of a highly metastatic 4T1 breast cancer cell line. Cytotoxic activity of single and combination treatment was evaluated by MTT assay, followed by an experiment of apoptosis induction by using flow cytometry. The inhibitory effect on migration was observed by the scratch wound-healing assay. Furthermore, the observation of the activity of matrix metalloproteinase-9 (MMP-9) was analyzed by gelatin zymography. The results showed that ECx, EBj, EFs, and Dox has cytotoxic activity on 4T1 cells with the value of IC50 respectively 49.7±1.53mg/mL, 59.9±1.79mg/mL, 15.2±2.12mg/mL and 1.2±0.23mM. Furthermore, combination of ECx-EBj-Dox and ECx-EBj-EFs revealed synergistic effect on 4T1 cells and decrease cell viability through the induction of apoptosis and necrosis. Based on wound healing assay, 24 hours incubation of this combination inhibited 4T1 cells migration compared to single treatment. Gelatin zymography analysis showed that this combination also inhibited the activity of MMP-9 greater than a single use. Curcuma xanthorrhiza, Brucea javanica, and Ficus septica may have potential to be developed as a combination with or without doxorubicin for metastatic breast cancer treatment.

Abstract

Scabies is a common skin disease caused by infection of Sarcoptes scabei, which is transmitted directly human-to-human contact especially in crowded community like pesantren. The characteristics of the students in pesantren are seemingly to be associated with Scabies. The purpose of this study was to determine the prevalence, characteristics of Scabies and its association with gender and education level of students in Pesantren Nurul Qarnain Jember. This cross sectional study was conducted to all students (1120 students) in 2016. Diagnosis was made based on anamnesis and dermatology examination. Data were analyzed using chi square test. The results showed that the prevalence of Scabies was 13.48% (male 24.89% and female 5.82%; education level tsanawiyah 15.14.1% and aliyah 10.85%). Most lesions were found in the interdigital space of the hands (40.40%). Chi square test showed significant difference between the prevalence of Scabies with gender (p=0.048) and education level (p=0.023) of the students. In conclusion, the prevalence of Scabies in Pesantren Nurul Qarnain Jember was 13.48% and there was association between the prevalence of Scabies with gender and education level of the students.  Keywords: Scabies, prevalence, students, gender, educational level

Abstract

Matrix metalloproteinase-9 (MMP-9) plays an important role in regulating the degradation and deposition extracellular matrix (ECM) that are important for wound reepithelization. However, excessive protease activity can lead to delayed wound healing. Kaemferol in spinach can inhibit the activity of MMP-9. Chitosan and collagen can help tissue growth and have an antimicrobial effect. Chitosan-collagen membranes have been widely used for wound healing but, the results are unsatisfactory. Therefore, the researcher wanted to know the activity of bakiko (Spinach-Chitosan-Collagen) membrane to MMP-9 expression on second degree burn with immunohistochemical method. This research was true-experimental laboratories design with post test only control group design. The experimental animals used were 36 white wistar rats (Rattus novergicus) male, divided into 4 groups ie control group, negative control, positive control (bioplacenton), and treatment (bakiko membrane). Data were obtained by calculating MMP-9 using ImageJ software. The analysis using anova test showed that p value less than 0.05 indicated a significant difference. This study showed that there was a decrease in MMP-9 activity by bakiko membrane in grade II burn with immunohistochemical method in wistar rats.

Abstract

Bromelain is a proteolytic enzyme which can be found in pineapple. The proteolytic activity of bromelain commonly used as an anti-inflammatory agent and antioedema, antithrombotic, and fibrinolytic agent. This study aimed at determining the combination effectivity of paracetamol and bromelain, pineapple crude extract’s protein, on pain response in mice and its interaction. This research used true experimental design with posttest only control group. Data analysis using Kruskal-Wallis test and application CompuSyn. The mice induced by acetic acid as the pain’s trigger and writhing reflex calculated every 5 minutes for 1 hour. The results of this study were expressed by pain protection persentage of paracetamol, crude extract of pineapple, a combination dose of paracetamol and crude extract of pineapple ½:½ and 1:1 was 52.23 ± 3.94%; 51.28 ± 6.71%; 77.88 ± 4.02%; and 71.24 ± 10.54%. The results of the CompuSyn dose combination analysis showed a synergistic effect that is seen from the CI value. In conclusion, analgesic effectivity combination of paracetamol and pineapple crude extract more effective than single dose usage and showed a synergistic effect. Abstract

Penelitian ini bertujuan untuk membuktikan hubungan karakteristik pesenam terhadap keluhan nyeri lutut. Penelitian ini merupakan penelitian analitik observasional dengan rancangan cross sectional. Populasi dalam penelitian ini adalah 60 orang, denagn rincian 30 untuk kelompok pesenam aerobik dan 30 untuk pesenam zumba. Kriteria inklusi yang dipakai adalah jenis kelamin wanita, frekuensi senam minimal satu kali dalam seminggu secara rutin selama minimal selama enam bulan, waktu senam dalam sekali latihan antara tiga puluh hingga enam puluh menit, dan telah mengisi informed consent sebelum pengisian kuesioner. Pengambilan data dilakukan dengan cara memberikan kuesioner mengunakan google form. Karakteristik pesenam yang dibandingkan pada penelitian ini antara lain frekuensi, usia, jenis senam, indeks massa tubuh (IMT), dan lama mengikuti senam dalam satu tahun. Skala nyeri lutut diukur menggunakan Number Rating Scale (NRS). Hasil analisis multivariat dengan uji logistik berganda menunjukkan hanya satu variabel yang berpengaruh signifikan, yaitu frekuensi senam dalam seminggu (p=0,044). Frekuensi senam berpengaruh terhadap nyeri lutut pesenam dengan Odds ratio (OR) sebesar 5,218. Karakteristik frekuensi senam berhubungan terhadap keluhan nyeri lutut (p=0,044) dengan nilai odds ratio 5,218. Usia tidak menunjukkan korelasi dengan keluhan nyeri lutut. Tidak ada hubungan IMT dengan nyeri lutut pesenam (p=0,178). Tidak ada hubungan jenis senam, baik zumba maupun aerobik dengan keluhan nyeri lutut. Lama mengikuti senam (dalam tahun) tidak berhubungan dengan keluhan nyeri lutut. Penelitian ini menyimpulkan frekuensi senam merupakan faktor risiko yang memengaruhi keluhan nyeri lutut pesenamThe correlation of gymnast characteristics with knee pain incidenceAbstracthis study aims to prove the relationship between the characteristics of the gymnast and complaints of knee pain. This research is an observational analytic study with a cross-sectional design. The population in this study was 60 people, with details of 30 for the aerobic gymnast group and 30 for the zumba gymnast. The inclusion criteria used were female gender, frequency of exercise at least once a week regularly for a minimum of six months, exercise time in one exercise between thirty to sixty minutes, and having filled out informed consent before filling out the questionnaire. Data collection was done by giving a questionnaire using a google form. The characteristics of the gymnasts compared in this study included frequency, age, type of exercise, body mass index (BMI), and length of time following the exercise in one year. The knee pain scale was measured using the Number Rating Scale (NRS). The results of multivariate analysis with multiple logistic tests showed that only one variable had a significant effect, namely the frequency of exercise in a week (p=0.044). The frequency of exercise has an effect on gymnast knee pain with an Odds ratio (OR) of 5.218. The characteristics of the frequency of exercise were associated with complaints of knee pain (p=0.044) with an odds ratio of 5.218. Age did not show a correlation with complaints of knee pain. There was no relationship between BMI and gymnast knee pain (p=0.178). There is no relationship between types of exercise, both Zumba and aerobics, with complaints of knee pain. Duration of exercise (in years) was not associated with complaints of knee pain. This study concludes that the frequency of exercise is a risk factor that affects the gymnast's knee pain complaints.

Abstract

Latar Belakang: Skabies masih menjadi salah satu penyakit kulit utama santri pondokpesantren Nurul Qarnain desa Baletbaru kecamatan Sukowono. Penyakit ini menggangguproses belajar santri & menyebabkan produktivitas berkurang. Faktor yang mempengaruhiperkembangan penyakit adalah lingkungan kurang bersih dan personal higiene yang jelek.Kebersihan lingkungan dan fasilitas ponpes sudah sangat memadai sehingga faktor yangberperan utama menyebabkan penyakit adalah personal higiene yang jelek akibattidakpahamnya santri terhadap penyakit ini serta pengobatan awal yang kurang cepat dantepat.Tujuan Penelitian: Salah satu bahan penting untuk pencegahan dan pengobatan yaitu sabunbelerang harganya mahal, sehingga santri tidak membeli sabun jenis ini. Sehingga diperlukanpenerapan teknologi tepat guna berupa pelatihan pembuatan sabun belerang antiskabies.Metode Penelitian: Metode kegiatan berupa pendataan jumlah santri penderita skabies,penyuluhan tentang penyakit skabies dan pencegahannya, pelatihan pembuatan sabunbelerang antiskabies, dan pengkaderan santri untuk penanganan awal penyakit skabies.Seluruh rangkaian kegiatan dilaksanakan pada bulan Agustus-Oktober 2016 di pondokpesantren Nurul Qarnain Kecamatan Sukowono. Hasil Penelitian: Hasil penelitian awal menunjukkan 151 santri menderita skabies(prevalensi 13,48%) dan setelah kegiatan berakhir jumlah santri yang menderita skabies jauhberkurang dan tersisa sebanyak 15 santri. Abstract

Breast cancer is one of the most commonmalignancy on woman in Indonesia (Kemenkes RI,2015). Breast cancer patients usually come todoctor in a late stage & the deaths are due to thecomplications of metastasis in vital organs (NationalCancer Institute, 2012). The death case mostprobably happened when cancer is aggresive &distant metastazed. One factor that causingaggresivity & metastasis is presence of HER2 protein.HER2 is found in 25% cases of Ca mammae. Her2status also influences preference of drug for cancertreatment. Besides of antibodi monoklonal &antireseptor of tirosine kinase, doxorubicin is stillused for treatment breast cancer with overexpressHER2 (Nielsen et al., 2009). Long term use ofdoxorubicin causes several side effects, resistanceand toxicity to normal tissues (Smith et al., 2010).Therefore, combining doxorubicin withchemopreventive agent is needed to increaseactivity of doxorubicin, to overcome the drugresistance and to reduce its side effect (Sarkar andLi, 2006). Chemopreventive agent used in thisresearch are temulawak (Curcuma xanthorrhiza) andawar-awar (Ficus septica). The leaves of Ficus septicahave been used to treat colds, fungal and bacterialdiseases, and various cancers. The ethanolic extractof Ficus septica showed a cytotoxic effect on breastcancer T47D cell lines with IC50 value of 13 μg/mL.The extract at 4.88 μg/mL also showed an optimumsynergistic effect in combination with doxorubicin(3.75 nmol) (Pratama et al., 2010). In addition, theextract induced apoptosis and down regulated theexpression of Bcl-2 protein in breast cancer cellsMCF-7 (Sekti et al., 2010). The major components ofC. xanthorrhiza are curcumin and xanthorrhizol.Curcumin suppresses a number of key elements incellular signal transduction pathways pertinent togrowth, differentiation, and malignanttransformation (Kunumakkara et al., 2008). Choi etal. (2005) observed that injection of 0.2-1.0 mg/kgbw xanthorrhizol had an antimetastatic effect in amouse lung metastasis model.It is a general assumption that combinationtreatment is sometimes more beneficial than singlecompounds. The previous study reported thatcombination of doxorubicin and chemopreventiveagent showed synergistic effect in cancer treatment(Lewandowska et al., 2014). The purpose of thisstudy is to prove antimetastasis effect ofcombination of ethanolic extract of rhizome ofCurcuma xanthorrhiza (ECx), ethanolic extract ofleave of Ficus septica (EFs), and doxorubicin solelyand its combination on MCF7-HER2 breast cancercell line through inhibition activity of MMP-9.Cytotoxic assay is used to determine the synergy ofcombinations. Antimetastasis effect was observedthrough inhibition of cancer cell invasion. Gelatinzymography signify the inhibitory activity of MMP-9,which plays an important role in cancer cell invasion.
