Abstract

Background: Competition between hospitals is increasing, for thathospital should maintain the quality of service and patient satisfaction. Hospitals should have innovation targets for cost effectiveness and productivity without compromising patient safety and quality of care. Operation delays are one barrier to optimal patient flow, increasing patient anxiety, affecting interprofessional teamwork across the medical discipline and ultimately putting patients at risk of safety. Methods: This research is descriptive research with a quantitative approach, data obtained from the surgery room register and interview which then grouped and analyzed. Results: There was an elective delay in surgery of 38%, with the highest cause being the staffing factor of 81%, the highest percentage of delays was plastic surgery 61% of total surgery performed, The highest number of delays occurred in Urology that is 213 cases. Conclusions: To reduce the delay required clear rules in the event of delays and rules of payment based on performance services, which is timeliness of attendance elective operation schedule is one of the assessment tools, and scheduling operations outside the time outpatient service.

Abstract

Medication error remain extremely common, and the health care system can do much more to prevent it. Electronic prescribing is increasingly being viewed by health care stakeholders as an important step toward improved medication safety, better management of medication costs, increased practice efficiency, and improved health care quality. However, the adoption of e-prescribing has been difficult to attain owing to numerous barriers throughout the industry. Even with all the benefits of e-prescribing, many providers and pharmacists remained hesitant about completely adopting an e-prescribing system. The main purpose of this study was to explore and to assess the barriers of electronic prescribing implementation. The methodology for this study followed the basic principles of a systematic review with PRISMA methode retrieved from online database PubMed using a keyword “barriers” OR “obstacles” AND “electronic prescribing” OR “electronic prescription” AND “implementation”. Finally 6 documents were selected by full text inclucion and assessed for eligibility. The result of the study are  inefficiency, the low uptake of the physicians, the cost of implementing the electronic prescription, system errors and the privacy and legacy. The conclusion from the studies should that electronic prescribing implementation barriers those we divided into 2 groups : the user factors and the system factors of the electronic prescription. Vendors or the consultants to facilitate more adequately the adoption of e-prescribing by giving the physicians the free trial and provide evaluation and improvement according to the physicians' needs for the features in the e-prescription.

Abstract

Background: Hospitals, as service providers, have a large role in meeting people's needs regarding their health. The increasing growth of hospitals adds to the menu of choices for people to utilize health services according to their needs. This competition has changed the mechanism by which customers are not only users of health services but as determinants of the future and the continuity of a hospital. Marketing activities can help hospitals improve and win the competition. Marketing managers must be able to combine the marketing mix variables appropriately so that all variables can increase consumer's decision and patient satisfaction.  Aims. The purpose of this study was to determine the effect of mix marketing on patient satisfaction at the hospital. Method: This study was conducted in a literature review by searching for articles and journals related to hospital mix marketing. Search journal articles are done through Google Scholar database with the search year 2010 until 2018. The keywords used are "marketing mix and "hospital." So that six articles will be reviewed.  Results: In the six studies reviewed there was one study that showed that all the mix marketing variables namely product, price, people, place, promotion, process, and the physical evidence had a significant influence on patient satisfaction and the hospital for medical treatment. The significant results are known from the results of statistical tests showing that p-value <0.05. In several other studies, it shows that there are significant variables and some that are not significant for consumer decision and patient satisfaction.   Conclusion: The Marketing mix has an important cost for the decision to choose a hospital. The mix marketing has varied significant influence on consumers? decision and satisfaction to choose the hospital for medical treatment. By knowing the mix marketing variable that affects customer satisfaction, hospital management, namely the marketing department, can optimize and develop marketing types that can increase public interest so that the target profit of the hospital is achieved.

Abstract

Background: Adverse drug events (ADE) are a leading cause of injury and death within health care systems around the world. Up to 67% of patients? prescription medication histories recorded on admission to hospital have one or more errors and 30 ? 80% of patients have a discrepancy between the medicines ordered in hospital and those they were taking at home. This study aims to systematically evaluate the available literature on the medication history records as a quality improvement in reducing medication discrepancies during the transition of care. Methods: This study was used systematic review which performed according to the PRISMA method. The search included articles were obtained through databases: MEDLINE (1946), EMBASE (1966), CINAHL (1937) and PubMed (1946). Some of the key words or Medical Subject Heading (MeSH) terms used in the search were: ?transition of care,? ?medication discrepancies,? ?medication errors,? ?patient safety,? ?medication history,? ?patient admission,? ?patient discharge,? ?patient transfer,? and ?hospital?. Only studies published in English were included. Exploring literature was focused on the articles published from 2009 to 2019. Results: Initially, a total of 162 potentially relevant articles were obtained. After screening tittle and reviewing abstracts, 14 full text were assessed for eligibility. Of the 10 articles met all inclusion criteria, 5 studies were randomized controlled trials, 2 quasi-experimental studies, 1 cohort study, and 2 qualitative studies with quantitative approaches. All studies found that involving best possible medication history in identifying medication discrepancies and communicating this information affected medication discrepancies in the medical record. Conclusions: The available literature such as lack of well-designed studies precluded us from concluding that no effect exists. Medication reconciliation supported by information technology was an important tool for minimizing the percentage of medications with unintentional discrepancies

Abstract

The quality of the hospital’s medical resume is one of hospital’s services quality indicator, which Parama Sidhi General Hospital still have figure of 17,1% for the incompleteness of medical record data (KLPCM). The goal of this research is to improve the attending physician medical resume’s completion through Sort, Straighten, Scrub, Systematize, and Sustain concept (5S) by acknowledged connection between 5S concept and medical resume completeness efforts based on continues improvement. This research is a mixed method analysis, by using primary and secondary data. The secondary data showed 26,80% among 1362 medical resume files were not completed. Univariate analysis of the medical resumes displayed 1,8% incompletion administrative data, whereas 19,2% final diagnosis and 26% working diagnosis uncomplete as the minor and one of major issues from the clinical data perspective. Univariate analysis of primary data used 5S implementation questionnaire instrument showed that Scrub (S3) 81% and Systematize (S4) 19% as the most common and the least implemented component of 5S, called improvements at re-evaluation of the Standard Operating Procedures (SOP) implementation and its socialization, forms formatting and its filing mechanism, and hospital’s service policy to overcome the physician’s time shortage.

Abstract

Hospital is complex and full of risk medical facility. Adverse events reporting is one part of the patient safety system that has an important roles to improve the hospital’s quality. Nurses have the longest contact's time with patients so they become the most important component in reporting the Adverse Event Reporting. This study aims to determine the factors that affects perception in adverse event reporting at Prima Medika Hospital Denpasar 2020. Prima MedikaHospitalis a type C private hospital.This studies quantitative study using a cross- sectional design with 140 nurses as samples and conducted in June 2020. The nurses’s perception who never report adverses event is 63.6%, assuming they never found any adverse event or maybe have found it but not have no courage to report it. The variable that significantly associated with perceptions on adverse event reporting by nurse are attitude (p value=0.002), education(p value=0.046), and workload ( heavy workload with p value=0.003 and light workload with p value=0.026). The most affecting variable is the nurses attitude (OR 4.33). It is proposed for the hospital to build the nurses positive attitude to report adverse events one through giving appreciation to those who report the adverse event, hospital needs to give regular patient safety training to maintain the nurse knowledge, and to revisit the 12-hour nightshift which is regarded as unusual long hours.

Abstract

This study aimed to evaluate the role of Jaminan Kesehatan Nasional (JKN) in improving the quality of cochlear implant services using Strength, Weakness, Opportunity, and Threat (SWOT) analysis. The study identified the internal factors of the cochlear implant center in Cipto Mangunkusumo Hospital as a consideration for developing hospital policies and the external factors to determine strategies for future development. The role of JKN was highlighted on the SWOT analysis, showing that the cochlear implant center was strategically located, thus increased the quality and the competitive factor compared to other hospitals. Strength-Opportunity strategy was recommended in improving service quality and identifying the possibility of JKN and other grants to cover the cost and improve the services, research, and education. The SWOT of the Cochlear Implant Center was identified as: 1. Good internal condition (average score of the strengths is superior to the weaknesses), 2. Good external condition (average score of the opportunities is higher than the threats), 3. Using strengths to maximize the opportunities and alternative strategy to be an aggressive strategy. 4. The role of JKN presented in the SWOT quadrants would give JKN a potency and important role in improving the treatment of severe hearing impairment in Indonesia.

Abstract

Introduction: The most common cause of worry for health in modern culture is non-communicable diseases (NCDs). Effective management of risk factors is essential when dealing with NCDs. Addressing NCDs requires a multifaceted approach, involving a range of solutions from different perspectives and at various levels, encompassing both individual and national dimensions.Method: Five databases were searched: JBI, Embase, Emcare, Medline, and Cochrane Systematic Reviews. The papers included reviews of social marketing in any form or empirical information about how social marketing affects the prevention of non-communicable illnesses.Result: Five papers were chosen from a pool of 36 titles and abstracts. Among the papers, 11 (45.8%) were observational studies conducted in various settings such as communities, hospitals, and clinics, while the remaining articles were from different healthcare facilities. The collected study designs encompassed systematic reviews, narrative reviews, brief communications, scientific reviews, non-randomized control trials, and randomized control trials. Five publications supported their strategies with empirical evidence.Conclusion: Social marketing approaches intended to prevent and handle NCDs need to be based on a deep comprehension of the intended audience and customized to fit the particular circumstances. Furthermore, they should promptly involve the public and establish connections with stakeholders. The elements of the intervention mix should work together harmoniously and complement each other. Furthermore, it's vital to include health education and capacity building and ensure cultural relevance.

Abstract

Covid19 has changed how we serve patients in all areas of health, and cardiology is no exception. Telemedicine is an option that hospitals can do during this pandemic. Telecardiology is part of telemedicine. By transmitting clinical data and electrocardiograms, telecardiology allows access to cardiology consultation services (telecardiology) without the need to travel to the practice area for both patients and cardiologists. These services can be in the form of teleconsultation, teleelectrocardiography, telerehabilitation, or teleechocardiography. Telecardiology can be applied in prehospital (teleconsultation, teleelectrocardiography,) if there is a referral for a heart attack patient or consult for chronic heart disease through online media; at the hospital in the form of discussions with peripheral hospitals and referral hospitals (teleechocardiography with other small hospitals); and posthospital treatment can be performed for cardiac telerehabilitation in patients with heart failure or teleconsultation of posttreatment patients.

Abstract

Patient safety culture in a hospital setting is the foundation for all health care activities,with the aim to provide safe and comfortable environment throughout the healing process of the patients. This systematic review aims to describe:1) the relationship between hospital as an organization and patient safety culture,2) the traditional and the modern concepts on patient safety, 3)the seven basic elements as founda tion of hospital patient safety culture and 4) the instrument to analyse hospital patient safety culture. The paradigm has changed from traditional way of punishing hospital staff when doing medical error to the just culture, with the philosophy that hospital system as an organization to some extent could contribute to the errors made by staff. The AHRQ survey further elaborates the seven basic elements of patient safety culture into 12 dimensions and this instrument has been used by hundreds of hospitals.. It is recommended for hospitals to conduct situation analysis on patient safety culture to identify the strengths and the weaknesses as evdence to improve patient safety.
