Abstract

Data mengenai KTD atau KNC di Indonesia masih sangat terbatas, belum ada penelitian terkait angka kejadian tersebut, tetapi laporan mengenai mal praktik semakin lama semakin meningkat. Hal tersebut membuktikan bahwa pelaksanaan keselamatan pasien di Indonesia belum maksimal. Keselamatan pasien diterapka demi tercapainya enam sasaran keselamatan pasien. Keenam komponen tersebut merupakan area kerja profesi keperawatan, sehingga dapat dikatakan perawat memiliki peran penting dalam menentukan keberhasilan program keselamatan pasien. Tujuan penelitian ini adalah mengetahui pengalaman perawat pelaksana dalam menerapkan 6 sasaran keselamatan pasien.Desain penelitian ini adalah kualitatif yang melibatkan lima partisipan sesuai kriteria inklusi. Pengumpulan data menggunakan wawancara semi structure, kemudian dianalisa dengan metode Collaizi. Hasil penelitian ini membentuk 6 tema yaitu identifikasi pasien, peningkatan komunikasi efektif, peningkatan keamanan terhadap obat yang perlu diwaspadai, memastikan tindakan bedah yang benar letak, benar prosedur, dan benar pasien, pencegahan risiko infeksi terkait pelayanan kesehatan, dan pencegahan risiko jatuh. Setiap tema terdiri atas beberapa kategori. Kesimpulan penelitian yaitu kelima partisipan telah menerapkan 6 sasaran keselamatan pasien. Partisipan menemui berbagai hambatan dalam menerapkan keselamatan pasien. Partisipan juga memiliki cara tersendiri untuk mengatasi setiap hambatan yang dialami. Saran yang diajukan kepada rumah sakit sebaiknya selalu memperbarui pengetahuan perawat pelaksananya terkait keselamatan pasien.

Abstract

Standard of hospital services is the authority given by government to the hospital to provide maximum services at patients. Service time to the patient affect the level of patient satisfaction. Purpose is to find out the patient service time at the outpatient unit. Survey research methods with quantitative approaches. Population consist of all outpatient both new and old at Hospital in Semarang. Sampling using non probability sampling, specifically purposive sampling. Respondents totaled 108 people. The results showed that the factors affecting the waiting time of service. The total service time of registration less than 10 minutes, as many as 34 (31.48%) and more than 10 minutes as many as 74 (68.52%). Total care clinic less than 30 minutes by 50 (46.29%) and more than 30 minutes were 58 (53.70%). Total pharmacy service time less than 30 minutes by 82 (75.92%) and more than 30 minutes by 26 (24.07%). The total financial service time is less than 5 minutes by 51 (47.22%) and more than 5 minutes total of 57 (52.77%). The results of factor analysis of hospital outpatient services Semarang still need to improve the quality of care by improving the performance of human resource system and better equipment.

Abstract

Introduction: Several nursing implementations do not match with nursing intervention classification. This situation requires further study to prove it. This study aims to describe nursing implementations on tuberculosis patients with airway clearance ineffective and imbalance nutrition: less than body requirement. Methods: This study was a quantitative observational descriptive study. Data were collected by observing nursing implementation documentations during two months. The research variable was nursing implementation in pulmonary tuberculosis patients. The sampling technique was purposive sampling.Results: Most of the implementation carried out by nurses in the diagnosis of ineffective airway clearance was providing oxygen (58.1%) and the second order was recommending sputum in the pot (10,5%). Most nursing implementation in diagnosis imbalance nutrition: less than body required was to educate the diet (85.7%), and the smallest is to recommend eating that is not spicy (1.2%). Conclusion: The most implementation that nurses do to overcome the diagnosis of ineffective airway clearance was giving oxygen to the patient. While the most implementation that nurses do in diagnosis imbalance nutrition: less than body required was educate the patient's diet. Interventions carried out by nurses are activities of the intervention, not nursing interventions. Further studies are required to improve nurses’ skills to implement according to standards.

Abstract

Introduction: Continuing Professional Development (CPD) implementation is not optimal. There is no evaluation regarding CPD implementation by nurses in RSUD R.A. Kartini, Jepara Regency. The purpose of this study was to describe the implementation of PKB by nurses in RSUD R.A. Kartini, Jepara Regency.Methods: This study used quantitative research design using descriptive studies. The sampling technique used simple random sampling with 169 respondents. Data collection used a questionnaire containing 55 question items from Indonesian National Nurses Association.Results: Description of CPD by nurses at R.A. Kartini Hospital in Jepara Regency are as follows: professional practice activities (100%), scientific activities (94.67%), scientific development activities (26.62%), and community service activities (71%).  Conclusion: Based on the research results, nurses should optimize themselves in various aspects of the form of CPD activities so that it can fulfill the achievement of 25 credits in five years while increasing the self-development of nurses.

Abstract

Angka penundaan operasi elektif di Rumah Sakit Pemerintah Di Semarang masih tinggi dan kurang sesuai dengan indikator mutu klinis rumah sakit. Penundaan operasi elektif disebabkan karena faktor medis, faktor pasien, faktor logistik dan administrasi dan faktor lain-lain. Tujuan penelitian ini untuk mengetahui gambaran penyebab penundaan operasi elektif di Rumah Sakit Pemerintah di Semarang. Penelitian ini merupakan jenis penelitian deskripsi kuantitatif dengan metode survei. Pengumpulan data dilakukan dengan lembar observasi dan check list. Jumlah responden sebanyak 54 orang dengan teknik sampling jenuh. Data yang diperoleh diolah dengan menggunakan analisa univariat untuk mengetahui distribusi dan persentase masing-masing variabel. Karakteristik responden  sebagian besar adalah laki-laki (53,7%), usia paling banyak 41-60 tahun (40,7%),  tingkat pendidikan paling banyak SMA (25,9%), pekerjaan lain-lain berjumlah (53,7%), paling banyak dirawat di kelas III  (66,7%) dengan fasilitas Jamkesmas (42,6%). Lama penundaan operasi elektif paling banyak >5 hari (38,9%). Penundaan operasi elektif di Rumah Sakit Pemerintah di Semarang disebabkan karena faktor medis (48,1%), faktor pasien (14,8%), faktor logistik dan administrasi (27,8%) dan faktor lain-lain  (9,3%). Penundaan operasi elektif paling banyak disebabkan karena faktor medis dan sebagian besar disebabkan karena perubahan akut fungsi kardiovaskuler dan pernapasan, nilai laboratorium tidak normal dan pasien menolak operasi. Perawat diharapkan melakukan monitoring lebih intensive terhadap fungsi kardiovaskuler dan pernapasan, nilai laboratorium  dan memberikan edukasi serta motivasi pasien pre-operasi.

Abstract

Background. Writing nursing goals and outcome criteria found in nursing care documentation was very varied and not appropriately, it needs to be explored more deeply about the phenomenon. The Study objective was to determine the writing of nursing goals and the outcome criteria for nursing diagnoses. Methods. Research design was qualitative research with a direct observation approach. Data was taken in the MDR TB ward, samples were 100 documentation of pulmonary TB patients. Sampling technique was nonprobability sampling. Result. The goal of nursing diagnosis of ineffective airway clearance was airway clearance effectively again, the problem was resolved, the patient's airway returned to normal and coughing the patient returns to normal, etc. The goal of nursing diagnosis of ineffective breathing patterns was effective breathing patterns, ineffective breathing patterns resolved, etc. The outcome criteria of nursing diagnosis ineffective airway clearence was negative cough, normal respiratory rate, normal vital signs, negative sputum, shortness of breath, coughing resolved, comfortable patients, patients can demonstrate coughing effectively, sputum can come out, breath was relieved, sputum can come out, etc. The outcome criteria for nursing diagnosis ineffective breathing patterns are normal respiratory rate, normal vital signs, respiratory rate was 20 x/minute, it was not weakness, It was not nausea, etc. Recomendation. Nurses are advised to be given trainings, sosialisation, or workshops related to the goals and outcome criteria of nursing diagnoses, and are expected to use NANDA and NIC-NOC references.

Abstract

Background: The logbook by the Nursing Committee is one of the methods used to ensure that nurses are competent to carry out the clinical authority given to them. The use of the nurse logbook is proven useful as a monitoring tool and evaluation of the clinical competencies implementation and nurses' accountability tools for the clinical assignments given to them. Objective: To identify the completeness of nurses' competency logbook documents at the Hospital. Method: This study used mixed methods, which is a method that combines quantitative and qualitative approaches with a sequential explanatory model, with 136 respondents in the first stage by observing 136 nurses log book, and in the second stage conducting in-depth interviews involving 9 main participants and 6 triangulation participants to find out the inhibiting factors for implementing the nurses' competency logbook. Results: From 136 log book of administrators nurses obtain 13 complete logbook (9.5%), 41 (30.1%) logbook completeness is sufficient, and 82 (60.3%) logbook completeness is less. The results of the In-depth Interview obtain 8 themes, namely the function of the logbook, the completeness of the logbook documentation, the timing of filling, the filling factor, the supporting / driving factors of the filling, and the impact of not carrying out the logbook filling on time. Conclusion: The need for re-socialization to all nurses in order to improve understanding of the correct logbook method, the need to increase nurse motivation through gradual and continuous supervision and the need to establish clear rewards related to the implementation of filling out the nurse logbook.

Abstract

Tuberculosis is one of the biggest health problems in the world. This disease is also the highest cause of death and morbidity in developing countries, such as Indonesia. Most nurses do not carry out treatment plans for pulmonary tuberculosis patients because there is no standard structure even though there are guidelines for nursing management of pulmonary TB patients. The purpose of this study was to determine nursing diagnoses that are often enforced by nurses in pulmonary tuberculosis patients. The study was conducted in the inpatient hospital. This study used descriptive observational and descriptive explorative approaches. The samples were the documentation of the TB patients by using nonprobability sampling, namely purposive sampling saturation. Data analysis used univariate analysis with frequency distribution. The results of the study show that the formulation of nursing diagnosis is ineffective airway clearance(52%), the writing of 1 (one) diagnosis of ineffective airway clearance into 13 different diagnostic names. The nurse wrote the diagnosis of ineffective breathing patterns into 7 forms of diagnosis. Nurses also wrote diagnosis of acute pain into 5 forms of nursing diagnosis, and hyperthermy diagnosis into 2 forms of nursing diagnosis. The research recommendation is that hospital management is expected to improve nurses' skills in writing the latest nursing diagnoses, monitoring and evaluating both qualitatively and quantitatively  in the writing of nursing diagnoses, applying the latest standards of nursing diagnoses. Nurses are expected to apply the latest nursing diagnoses in accordance with the use of NANDA.

Abstract

The implementation of supervising the head of a room in one hospital in Semarang has not been optimal due to the absence of a schedule, assessment instruments, guidance, documentation of supervision results, and standard operating procedures (SOP) supervision. Supervision activities are incidental in accordance with needs and have not been implemented in a structured and well-documented manner. The writing of this article aims to find out the description of the implementation of head supervision in a hospital in Semarang. This study uses descriptive research design. The subjects in this study were all heads of inpatient rooms. The object of this research is the implementation of the supervision of the head of the room according to the SOP and the results of documentation of the implementation of supervision. The instrument in this study used a draft sheet for evaluation of the superficial room leader evaluation. Shows that there is a change in the implementation of supervision based on the SOP before and after the dissemination of supervision is carried out to the head of the room. Documented supervision results can help the head of the room to see the extent of the ability of staff and can jointly improve capabilities, correct errors in improving the quality of nursing care services. The implementation of supervising the head of a room in one of the Semarang hospitals needs to be improved, especially in terms of post-supervision documentation, development of thematic supervision themes, and structured supervision scheduling.
