Abstract

Background: Hemodialysis is a kidney replacement therapy, but it has many complications identified, one of them is pain. Thus, we need non-pharmacological interventions that can be done by nurses, among them is progressive muscle relaxation (PMR), which is economically-efficient and no side effects.Objective: To determine the effect of progressive muscle relaxation on pain in hemodialysis patients.Methods: This is a quasi experimental design of time series design. The number of samples was 100 patients taken by simple random sampling. Progressive muscle relaxation (PMR) was given every day for 4 weeks. Research instrument  applied was Brief Pain Inventory. The data analysis employed independent t-test.Results: There was a decrease in pain level from week to week after the intervention, from the scale of 6 (moderate pain) to scale 3 (mild pain). There was a difference in the level of pain between the intervention group and the control group p <0.001.Conclusion: The therapy of non-pharmacological using a progressive muscle relaxation is proven to reduce pain in hemodialysis patients.

Abstract

Strok adalah gangguan fungsional otak fokal maupun global, bersifat akut, terjadi lebih dari 24 jam, berasal dari gangguan aliran darah otak dan bukan disebabkan oleh gangguan peredaran darah otak. Salah satu dampak dari strok adalah kelemahan anggota gerak baik hemiplegia atau hemiparesis sehingga mengganggu kemandirian pasien dalam aktivitas kehidupan sehari-hari (AKS). Tujuan penelitian ini mengetahui perkembangan tingkat kemandirian pasien strok berdasar atas skor Modifikasi Indeks Barthel serta faktor-faktor yang memengaruhi tingkat kemandirian pasien. Penelitian dilakukan di rumah sakit PKU Muhammadiyah Yogyakarta dan Rumah Sakit PKU Muhammadiyah Gamping pada bulan Mei hingga Juni 2016. Penelitian dilakukan dengan desain prospective longitudinal. Sejumlah 42 responden dipilih melalui accidental sampling. Instrumen yang digunakan adalah kuesioner Modifikasi Indeks Barthel.  Data dianalisis dengan regresi logistik berganda. Hasil penelitian menunjukkan untuk pasien strok perdarahan, pada saat masuk rumah sakit mayoritas dengan kondisi tingkat ketergantungan total (44%) dan pada saat keluar sakit sebagian besar menjadi tingkat ketergantungan parah (44%). Untuk pasien dengan strok iskemik, saat masuk rumah sakit mayoritas juga berada pada tingkat ketergantungan total (49%) dan terjadi perbaikan kondisi menjadi tingkat ketergantungan parah (42%) pada saat keluar rumah sakit. Variabel lama rawat inap berhubungan secara signifikan terhadap skor Modifikasi Indeks Barthel (p<0,05). Tidak ada faktor yang paling dominan berpengaruh terhadap skor Modifikasi Indeks Barthel pasien strok. Kata kunci: Aktivitas kehidupan sehari-hari, modifikasi indeks barthel, strok Development and Factors Affecting the Level of Independence of Stroke Patients During Hospitalization in Yogyakarta based on Modified Barthel Index ScoreStroke is a focal or global brain functional disorder which occur more than 24 hours and originating from a disruption of cerebral blood flow which is not caused by cerebral circulatory disorders. Consequences of stroke are extremity disability, hemiplegia, or hemiparesis. Furthermore, stroke makes it difficult for patients to do activities of daily life (ADL). The aim of this study was to determine the DLA independency level of stroke patient based on the Modified of Barthel Index score and to analyze the determinants that affect patient’s dependency level. The study was conducted at PKU Muhammadiyah Yogyakarta Hospital and PKU Muhammadiyah Gamping Hospital in May to June 2016. This was a prospective longitudinal study on forty two respondents selected through accidental sampling. Barthel Index Modification instrument was used to measure the ADL independency level. Data were analyzed by multiple logistic regression. Majority of hemorrhagic stroke patients had total dependency when they were admitted to hospital (44.4%) which improved to moderate dependency when they were discharged (44.4%). Meanwhile, ischemic stroke patients were in total dependency the first time they came to hospital (44.4%) and they also improved to severe dependency at discharge (42.4%). Length of hospital stay has a significant correlation with Barthel Index score (p<0.05). None of the variables affects the Modified of Barthel Index score dominantly. Abstract

Keadaan umum lemah, badan kurus, tekanan darah tinggi, anemia, gatal di kulit, warna kulit menjadi lebih gelap, mual adalah tanda tidak adekuatnya hemodialisis secara klinis. Berdasarkan telaah literatur didapatkan sembilan faktor yang berpengaruh langsung terhadap adekuasi dialisis yaitu body mass index (BMI), jenis akses vaskular, lama hemodialisis, frekuensi hemodialisis, kecepatan aliran darah, ultrafiltrasi rata-rata, luas permukaan dializer, jenis heparinisasi, dan hematokrit. Penelitian bertujuan untuk mengetahui prediktor dominan terhadap adekuasi dialisis pada pasien hemodialisis di Rumah Sakit PKU Muhammadiyah Yogyakarta berlangsung dari bulan Mei–Juni 2013. Studi potong lintang dilakukan pada 90 responden secara purposive sampling. Adekuasi dialisis dihitung dengan rumus Kt/V. Semua data diambil pada sesi kedua hemodialisis. Data dianalisis menggunakan uji regresi linier ganda. Diperoleh adekuasi dialisis rata-rata 1,36±0,377. Terdapat hubungan yang bermakna adekuasi dialisis dengan BMI dan jenis heparinisasi. Tidak terdapat hubungan yang bermakna jenis akses vaskular, kecepatan aliran darah, ultrafiltrasi rata-rata, hematokrit, lama hemodialisis, frekuensi hemodialisis, dan luas permukaan dializer dengan adekuasi dialisis. Simpulan, faktor dominan yang memengaruhi adekuasi dialisis pada pasien hemodialisis adalah BMI dan jenis heparinisasi. Based on literature reviews, there are 9 factors that directly influence the adequacy of hemodialysis; body mass index (BMI), vascular access, length and frequency of hemodialysis, blood flow rate, ultrafiltration rate, dialyzer surface area, heparinization, and hematocrit. This study aimed to study the dominant predictor of dialysis adequacy in hemodialysis patients in PKU Muhammadiyah Hospital Yogyakarta. Cross sectional study was done involving 90 respondents through purposive sampling method. The hemodialysis adequacy was assessed using the Kt/V formula. All data were collected during the second session of hemodialysis. Data were examined using double linier regression. The ,ean dialysis adequacy was 1.36±0.377. The statistic test result revealed a significant correlation between dialysis adequacy and BMI and the type of heparinization. There was no significant correlation with the type of vascular access, blood flow rate, ultrafiltration rate, hematocrit, length and frequency of hemodialysis, and dialyzer surface area with dialysis adequacy. In conclusion, the dominant factors influencing dialysis adequacy on hemodialysis patients are  BMI and type of heparinization. Abstract

General appearance of weakness, skinny, high blood pressure, anemia, itches, darkness skin color and nausea showing inadequate hemodialysis clinically. Based on literature review there are some factors directly infl uencing on adequate hemodialysis, one of them is dialyzer surface area. The research aimed to study the effect of dialyzer surface area on dialysis adequacy on hemodialysis patient in PKU Muhammadiyah Hospital Yogyakarta from May until June 2013. Cross sectional study was done involving 90 respondents through purposive sampling method. The hemodialysis adequacy counted by Kt/V formula. Data was taken during the second session of hemodialysis. The data were examined with Kruskal Wallis. Mean dialysis adequacy was 1,36±0,377. The statistic test result revealed no signifi cant correlation on dialyzer surface area with dialysis adequacy (p=0.880).Key words: adequacy, dialyzer, hemodialysis

The research source came from two databases, namely ProQuest and EBSCO, with the keywords "Stroke" AND "Home service" AND Physical "OR" Psychological "OR" Spiritual. " From the ProQuest database, there were 66,665 journals found, and from EBSCO, there were 69,501 journals. Out of all the articles, only 8 articles met the inclusion criteria. The article must relate to the types of rehabilitation of stroke patients at home with the outcomes seen in the physical, psychological, and spiritual patients. Abstract

It is disconcerting that until today there are no agencies or research to recommend the appropriate indicators of home nursing care of stroke patients according to Indonesian conditions. Hence, this research aimed to verify the validity and reliability of quality indicators of home nursing care for stroke patient that resulted from Delphy process. Confirmatory factor analysis was carried out for validity and reliability testing with data collected from 350 respondents. Based on the value of loading factors, 62 indicators were declared valid and five indicators were invalid. This study successfully proved the validity of 62 indicators of quality home care for stroke patients. Further research is needed to pilot project this indicator in a larger area.

Abstract

ABSTRAKKrebet adalah sentra industri batik kayu yang mempunyai potensi kepariwisataan baik dari sisi budaya maupun alamnya. Pemberdayaan kelompok PKK di desa Krebet sangat bermanfaat untuk meningkatkan ketahanan keluarga baik dari segi ekonomi, sosial, budaya maupun agama. Berbagai keterampilan yang diperoleh, akan bisa membekali anggotanya  untuk bisa hidup lebih baik. Kondisi pandemi Covid sangat membutuhkan daya imun yang cukup kuat untuk bisa menangkal virus Covid-19. Hasil need assessment menunjukkan bahwa kelompok PKK memerlukan tambahan informasi tentang Toga. Toga termasuk tanaman yang murah dalam produksinya, tidak membutuhkan lahan yang luas, cocok ditanam baik di dataran rendah maupun tinggi, sudah dikenal masyarakat Indonesia serta cara pengolahan hasil produksi yang mudah untuk skala rumah tangga. Tujuan dari pengabdian masyarakat adalah mengenalkan tentang jenis Toga, manfaat Toga dan pengolahan Toga sederhana. Kegiatan pengabdian masyarakat diikuti oleh 14 peserta diawali pemberian pendidikan kesehatan tentang Toga dan manfaatnya, dan tata cara budidaya Toga. Kegiatan berikutnya adalah praktek mengolah jahe menjadi puding. Evaluasi berdasarkan kuesioner menunjukkan terjadi peningkatan pengetahuan peserta sebelum dan setelah pemberian pendidikan kesehatan. Kata kunci: toga; desa wisata krebet; covid-19; pemberdayaan masyarakat; ketahanan keluarga.ABSTRACTKrebet is the center of the wooden batik industry that has tourism potential both in terms of culture and nature. The main activities carried out by mothers are mostly housewives, and are involved in PKK social activities. The Covid-19 pandemic requires a strong immune system to be able to ward off the Covid-19 virus. The results of the assessment in the area, the agreed skill to be taught to PKK mothers is the cultivation of the Toga plant. Toga is a plant that is cheap to produce, does not require a large area of land, suitable for planting in both the lowlands and highlands, well known to the Indonesian people and  an easy way of processing production for household scale. Empowerment of the PKK group is very useful for increasing family resilience in terms of economic, social, cultural and religious aspects. Various skills obtained will be able to equip its members to be able to live better. This community service activity was attended by 14 participants, carried out in three stages, namely providing health education about Toga and its benefits, as well as procedures for cultivating Toga. The third stage is the practice of how to process the Toga. The evaluation was carried out by giving a questionnaire of knowledge about Toga and skills in practicing how to process Toga results before and after the activity. The results of data analysis showed an increase in participants' knowledge before and after the provision of health education. Keywords: toga; krebet tourism village; covid-19; community empowerment; family resilience

Abstract

Background: One method to identify the level of performance is using quality of care indicators. Even though many quality indicators of health care have been announced, those which specify in home care services are limited in that none of them describe the stroke patients’ condition. The purposes of this study are to determine through systematic literature review what methods can be used to assess the quality of home care that patients received and to identify what components can be used as a determinant of the quality of home care services. Method: Google Scholar, EBSCO, ProQuest and PubMed database websites were searched for articles and information. Results: The method that was used is a qualitative study using a literature review with quantitative analysis of a previously accepted research instrument with a questionnaire that has been widely available and considered reliable. The researchers identified thematic differentiation in grouping the quality indicators used by those articles, however most of them are classified as outcome indicators, one as process indicator, and none as structure or input indicator. Overall quality indicators in home care that are used by the three articles are based on the Home Care Quality Indicators Instrument (HCQIs). Conclusions: Several studies discussed home care quality indicators but no articles specifically analyze home care provision for stroke patients. Further research is needed to clarify the components indicators for stroke patients and more importantly, these indicators should be valid, and reliable.

Abstract

This study aims to determine the effect of applying head elevation to improve cerebral tissue perfusion in stroke patients. The method used is a single data, namely a literature review. The results showed that 30° head elevation was recommended in improving cerebral tissue perfusion, although the difference was not too high. In conclusion, there is an effect of the SPO2 value after the head elevation is 30°, and 15° in stroke patients. Keywords: Head Elevation, Cerebral Perfusion, Stroke

Abstract

This study aimed to examine several studies on early screening and prevention of the risk of developing Deep Vein Thrombosis (DVT). The method used in this literature review consists of 4 stages: (1) identification of the search for journal articles; (2) identification of literary works based on title, abstract, and purpose; (3) inclusion criteria; (4) complete and specific texts are available according to the purpose of the review. The process of searching for journal articles using Google Scholar, ScienceDirect, DOAJ, Scopus, ProQuest, Ebsco and Pubmed. The results showed that most early DVT screenings used ultrasound as a non-pharmacological DVT detection and prevention tool, the most frequently used was compression stockings. In conclusion, compression stockings are more effective in preventing DVT. Keywords: Early Detection, DVT, Bed Rest
