Abstract

In the life of society, people will always be in touch with other community groups. Thus, inevitably every individual human will be in touch with many interests, needs and ideologies that exist in society. Such conditions cause human life will not be separated from political affairs. In other words, every side of human life will be in touch with political affairs. Political struggle is closely related to the affairs of state administration or government. In the case of the state or government, politics should be used to realize the good of the people. No different from the clerics entering the political vortex of his role in politics has given a positive color in the national political scene, especially in fighting the Islamic Shari?ah in it. But it can not be denied, too, that his involvement has led to a negative excess for Islam it self.

Abstract

Abstract

Introduction. Major depression criteria can be found in more than 20% of coronary heart patients and more than 30% acute coronary syndrome patients have significant depressive symptoms. Several studies have shown that reduction of hearth rate variability (HRV) and enhancement of neutrophil lymphocyte ratio (NLR) can cause enhancement of morbidity and mortality in myocardial infarction patients. On the other hand, depression can cause reduction of HRV, enhancement of NLR and reduction of quality of life. This study aimed to determine differences in HRV, NLR and quality of life between acute myocardial infarction (AMI) patients with depressive symptoms and AMI patients without depressive symptoms. Methods. A cross-sectional study was conducted among AMI patients who were treated during the period of March to July 2021 at the Intensive Cardiac Care Unit (ICCU) Cipto Mangunkusumo Hospital Jakarta. Samples were taken consecutively until the amount of sample is met. Data were analyzed using unp aired T test. Results. There was a total of 46 subjects AMI patients in this study, ten subjects with depressive symptoms and 36 subjects without depressive symptoms. From the analysis, it was found that AMI patients with depressive symptoms had lower standard deviation normal to normal (SDNN) average (p=0.447), lower low frequency/high frequency (LF/HF) (p=0.323), and higher NLR average (p=0.438) than AMI patients without depressive symptom, although it was not statistically significant. Analysis showed that the mean value of SF36 total scores (p<0.001), physical function domain scores (p<0.001), energy domain scores (p<0.001), mental health domain scores (p<0.001), social function domain scores (p=0.028), bodily pain domain scores (p=0.002), and general health domain scores (p=0.002) were lower in AMI patients with depressive symptoms. Conclusion. AMI patients with depressive symptoms had lower SDNN, lower LF/HF, and higher NLR than AMI patients without depressive symptoms, although it was not statistically significant. AMI patients with depressive symptoms significantly have lower quality of life than AMI patients without depressive symptoms.

Abstract

Introduction. One of the causes of death in acute coronary syndrome is the occurrence of complication known as major adverse cardiac events (MACE). There are several predictors of the occurance of MACE in patients with ACS, including depression and anxiety. Thi study aimed to determine the association between depression and anxiety with major adverse cardiac events within 7 days in patients with acute coronary syndrome. Methods. Study with prospective cohort design to examine the association between depression and anxiety with MACE within 7 days of ACS patients, using HADS questionnaires on ACS patients undergoing treatment at ICCU, Hospitalization RSCM in January - May 2018. Bivariate analysis was performed to calculate the risk ratio (RR) of MACE occurrence within 7 days in the depression and anxiety group using SPSS. Results. Obtained number of subjects who meet the inclusion criteria of 114 people. Depression was obtained in 7% of subjects, Anxiety was obtained in 28,95% of subjects, and MACE was obtained in 9.6% of subjects. In the Depression group, MACE 7 days occurred in 12.5% of subjects. In the Anxiety group, MACE 7 days occurred in 21,2% of subjects. In bivariate analysis, Anxiety increased the risk of MACE within 7 days in patients with ACS, with relative risk (RR) of 4,2 (IK 1,34 – 13,70). Conclusions. Anxiety in patients with ACS is an independent predictor of MACE within 7 days and increases the risk of a 7 day MACE. There was no correlation between depression and MACE within 7 days in patients with ACS.

Abstract

Introduction. Major adverse cardiac events (MACE) is a complicating myocard infarct which consist of acute heart failure, cardiogenic shock, and fatal arrhytmia. An accurate, easy and cost-effective biomarker is needed to predict MACE and mortality in patients with myocard infarct. Hypoxic liver injury (HLI) is a potential biomarker using serum glutamic-oxaloacetic transaminase (SGOT) level as the parameter. This study is aimed to discover HLI’s role in predicting MACE in non ST elevation myocard infarct (NSTEMI). Methods. This study was designed as cross sectional to predict MACE and prospective cohort for survival analysis. Study population was all NSTEMI patients admitted to ICCU of Cipto Mangunkusumo Hospital and study sample were NSTEMI patients admitted to ICCU of Cipto Mangunkusumo Hospital that meets all criteria during 2006-2016. Cut-off level of SGOT for HLI to predict MACE and mortality was analyzed using ROC curve and AUC. Survival analysis was done using Kaplan Meier curve and the difference was tested with log-rank. Results. A total of 277 subjects were included in this study. Incidence of MACE in this study was 51.3% (acute heart failure 48.4%, fatal arrhytmia 6.5%, and cardiogenic shock 7.2%). The mortality rate was 6.13%. The median of SGOT level on all subject was 35 U/L, 40 (range 8-2062) U/L in subjects with MACE and 31 (range 6-1642) U/L in subjects without MACE (p = 0.003). Cut-off level for SGOT used to predict MACE was 101 U/L (sensitivity 21.8%; specificity 89.6%; POR 2.727 (CI 95% 1.306- 5.696), p = 0.006). In multivariate analysis, HLI was insignificantly related to MACE. Cut-off level for SGOT used to predict survival was 99 U/L (sensitivity 23.5%; specificity 83.8%; likelihood ratio +1.46). There were no significant difference of survival between groups with HLI level below and above the cut-off SGOT level. Conclusion. Hypoxic liver injury (HLI) cannot be used to predict MACE in NSTEMI patients unless combined with other variables. There is no significant difference of survival between subjects with or without HLI.

Abstract

Introduction. Various studies have concluded the association between various markers of inflammation with the clinical outcome in STEMI patients. However, most of these markers are quite expensive and not available in healthcare facilities. Hence, we aim to study the correlation between haemoglobin platelet ratio (HPR) and leucocyte platelet ratio (RPL) and the clinical outcomes of STEMI patients. Methods. This is a cross-sectional study performed in adult patients with STEMI who were hospitalized in Intensive Coronary Care Unit Cipto Mangunkusumo Hospital between January 2016 and October 2020. The data was analyzed by testing the correlation between HPR and LPR with SYNTAX-2 score. Results. There were 114 subjects in this study, with a mean age was 53.87 years and dominated by male (86.8%). We found no correlation between HPR and LPR with SYNTAX-2 Score. From the sub-group analysis, there was a moderate positive correlation between HPR and SYNTAX-2 score (r = 0.587; p= 0.005) and a strong positive correlation between LPR and SYNTAX-2 score (r = 0.606; p = 0.004) in subjects with normal body mass index (BMI). Additional results obtained a strong negative correlation between neutrophil lymphocyte ratio (NLR) with SYNTAX-2 score (r = -0.738; p= 0.0037) and a very strong negative correlation between platelet lymphocyte ratio (PLR) with SYNTAX-2 score (r= -0,857; p= 0,007) in subjects with low BMI. Conclusion. There is no correlation between HPR and LPR with SYNTAX-2 Score
