Abstract

Hipertensi adalah salah satu penyakit yang banyak menyebabkan kematian dan komplikasi yang berkaitan dengan penyakit kardiovaskuler. Prevalensi hipertensi di Indonesia sebesar 25,8%. Salah satu faktor yang memengaruhi kejadian hipertensi adalah tingginya kadar kolesterol dalam darah (hiperkolesterolemia). Penelitian ini bertujuan untuk mengetahui hubungan antara kadar kolesterol total dengan tekanan darah pada masyarakat Jatinangor. Penelitian ini merupakan observasional analitik korelasional dengan rancangan potong lintang. Populasi penelitian adalah masyarakat di kecamatan Jatinangor dari tiga desa yaitu Cilayung, Cipacing dan Hegarmanah. Data diperoleh dari penelitian “Epidemiologi Hipertensi dan Albuminuria di Jatinangor pada tahun 2014”. Penelitian dilaksanakan pada bulan Agustus hingga November 2016. Subjek yang terlibat dalam penelitian ini adalah 107 responden. Terdiri dari 75 perempuan (70,1 %) dan 32 laki-laki (29,9%), dengan rentang usia 23-82 tahun. Nilai tengah tekanan darah sistolik dan diastolik adalah 144 mmHg (rentang= 98-224 mmHg) dan 85 mmHg (rentang= 58-139 mmHg), nilai tengah kadar kolesterol total adalah 189 mg/dL (rentang=110-310 mg/dL). Hasil analisis korelasi rank-Spearman diperoleh hubungan antara kadar kolesterol total dengan tekanan darah sistolik meskipun menunjukkan korelasi lemah (r=0,297; p=0,004); dan dengan tekanan darah diastolik (r=0,145; p=0,136). Secara statistik diperoleh hubungan yang bermakna dengan nilai korelasi lemah antara kadar kolesterol total dengan tekanan darah sistolik.Kata Kunci: Kadar Kolesterol Total, Tekanan Darah Diastolik, Tekanan Darah Sistolik.

Abstract

Background : Empirically, breadfruit (Artocarpus altilis) leaf has been used as traditional medicine for several diseases, such as acute renal failure. This research is aimed at finding the effects of breadfruit leaf infusion on blood creatinine levels in rats to model acute renal failure.Methods : This research is an experimental study, using 30 male wistar rats, weighing 150–250 g, divided into 5 groups. The first group is a negative control and the second group is a positive control. Groups 2–5 were induced with Gentamicine and peroxicam for 7 days, and then groups 3–5 given infusion of breadfruit leaf in different doses for 7 days, and then blood creatinine levels were tested at the 15th day. This data was analyzed using a multiple comparison Dunnett T3 test because this data is not homogenous. This research was done in pharmacology laboratorium universitas padjadjaran from Oktober–November 2012.Result : Bases on average creatinine levels of 0.62, 0.87, 0.98, and 0.87 mg/dL observed for the diffetent groups, it was statistically show that significantly different creatinine levels, differing by 0.038 (p < 0.05) were observed for the control group and the group which was given an infusion of breadfruit leaf.Conclusion : It can be concluded that infusions of breadfruit leaf do have an effects on blood creatinine levels in acute renal failure.Key words : acute renal failure, breadfruit leaf, creatinine  DOI: 10.15850/amj.v2n4.527

Abstract

Background: Hypertension is one of the most prominent global diseases. Despite the availability of effective therapies, hypertension remains poorly controlled in Indonesia. In many cases, patient’s noncompliance may be attributable to the low patients’ knowledge, attitude, and life-style practices such as polypharmacy. Polypharmacy is defined as the administration of many drugs at in one prescription. Polypharmacy increases expenses, possible adverse reaction to a single agent, incidence of drug interactions, and decreases patient’s compliance This study aimed to identify the practice of polypharmacy in hypertension treatment in primary health centers.Methods: A cross-sectional descriptive observational study was carried out on 60 patients from two primary health centers in Jatinangor, West Java, Indonesia in October 2013. Sociodemographic profile, degree of hypertension, types of antihypertensive drugs, concomitant drugs given together with antihypertensive drugs, and treatment compliance data were collected and presented in tables and figures.Results: The incidence of hypertension was more common among male patients compared to female patients. Thirty-three patients (55%) have low compliance to their medication. Twenty-nine patients (48%) received single drug and 31 patients (52%) received more than one drugs.Conclusions: The percentage of polypharmacy practice in treating hypertension in primary health centers is 52%. The most frequently prescribed anti-hypertensive are angiotensin-converting enzyme (ACE) inhibitors and calcium-channel blockers (CCB). Most of hypertensive patients have low compliance to therapy. Abstract

Objective: Adherence to medication is crucial for hypertension patients in order to control and prevent complications. This study aimed to describe the adherence to antihypertensive drugs among patients with hypertension in Bandung District, West Java, Indonesia.Methods: This was a cross-sectional observational study on patients with hypertension in the work area of Cileunyi Public Health Center (PHC), Bandung District, West Java, Indonesia. The inclusion criteria used were patients with hypertension who were prescribed an antihypertensive drug by a physician, above 18 years old, and had been using an antihypertensive medication for at least two months before the study. Data were obtained through interviews during the month of July to August 2018 using a modified MMAS-8 questionnaire of the Morisky Medication Adherence Scale (MMAS-8), which includes Likert-type scale questions. Data collected were then analyzed using the Rasch Model, and the mean logit person was used as a cut off between adherence and non-adherence categories.Results: A total of 75 patients with hypertension were recruited. The prevalence of adherence to medication was 59%. Most patients who adhered to the medication were > 55 years old (31%),  female (59%), had been diagnosed with hypertension for < 5 years (31%), and received support from family/social environment (40%).Conclusion: Many factors support the adherence to antihypertensive medication, including age, gender, time of hypertension diagnosis, and support from family or social environment. The adherence to antihypertensive medication among patients with hypertension visited Cileunyi Primary Health Care, Bandung District, West Java, Indonesia is good.

Abstract

Objective: To determine the relative change in endothelin-1 (ET-1) during hemodialysis procedure in Stage Five Dialysis-Dependent Chronic Kidney Disease (CKD-5D)patients with and without intradialytic hypertension (IDH).Methods: This was a cross-sectional, observational study involving 40 CKD-5D patients who received two hemodialysis per week for at least three months at the dialysis unit of Dr. Hasan Sadikin Hospital General Bandung, Indonesia during September 2016. Subjects were divided two groups: with IDH (n=20) and without IDH (n=20). Plasma ET-1 level was examined before and after hemodialysis.Results: There was a significant elevation of ET-1 level (mean±SD pg/mL) between pre- and post-dialysis state in patients with IDH (3.33±1.28 vs. 3.84±1.75; relative changes: 15.32%, p=0.013). No change was observed in patients without IDH (3.99±2.30 vs. 4.38±1.81; relative changes: 9.77% p=0.083). The post-dialysis absolute ET-1 level was significantly lower in CKD-5D patients with IDH (3.84±1.75 vs. 4.38±1.81; p=0.024).Conclusion: There was a significant elevation of ET-1 level in CKD-5D patients with IDH during hemodialysis procedure at the dialysis unit of Dr. Hasan Sadikin General Hospital Bandung.

Abstract

Objective: To investigate the association of malnutrition with latent tuberculosis (TB) among chronic kidney disease (CKD) patients on hemodialysis (HD).Methods: This was a cross-sectional study conducted at the Hemodiaylisis Unit of Dr. Hasan Sadikin General Hospital, Bandung, Indonesia. Subjects were patients aged >18 years who had undergone HD twice a week for at least three months. Patients suspected of active tuberculosis (TB), malignancy, or immunocompromised were excluded. Latent TB was diagnosed using the interferon-gamma release assays (IGRA). Malnutrition was defined by a malnutrition inflammation score (MIS) of less than 5. All data including age, sex, CKD etiologies, and laboratory findings were obtained and recorded in a case report form.Results: A total of 120 subjects were involved in this study. Subjects with positive, negative, and indeterminate IGRA results were 39.2%, 56.7%, and 4.2%, respectively. There was no significant differences in subjects characteristics between positive and negative IGRA subjects.  The MIS>5 was shown to have no statistically significant association with positive IGRA subjects (OR=3.47, 95%CI 0.93–12.93).Conclusion: Malnutrition based on an MIS score of less than 5 is not statically associated, but clinically associated, with latent TB. Further causal inference study to investigate these associations is needed.

Abstract

The prevalence of chronic kidney disease on dialysis or CKD5D is increasing with a significant impact on disease burden in many countries. Patients are usually listed in the national renal registries, which report demographic data, incidence, prevalence, and outcome. The survival rate is an important outcome measure to characterize the impact of treatment in the CKD5 patient population in the national and international renal registries. Indonesian Society of Nephrology (InaSN) has the Indonesian Renal Registry program to collect data that was endorsed to monitor dialysis treatment quality in Indonesia.  IRR releases an annual report, but there is no survival analysis yet.   This study aimed to discover the five-year survival rate of CKD5D patients in West Java between 2007–2018 and its factor based on the IRR database. A retrospective cohort study was performed by gaining all patients' data from the IRR database, then data on all of the patients from West Java province who completed a 5-year follow-up on December 31, 2018.  Kaplan-Meier analysis and Cox proportional hazard's model were used to analyze the risk factors. There were 3,199 data included in this study. In total, the 1, 2, 3, 4, and 5 year survival rates are 82%, 70%, 62%, 58%, and 55 %, respectively.  Patients whose age is above 55 years and with unknown underlying kidney disease have a worse survival rate with a hazard ratio of 1.28 and 1.50, respectively. Further exploration of IRR data will provide better information on dialysis treatment in Indonesia. Ketahanan Hidup Pasien Penyakit Ginjal Kronis dengan Hemodialisis di Jawa Barat Indonesia tahun 2007-2018Prevalensi penyakit ginjal kronis pada dialisis atau PGK5D meningkat dan memberi beban penanganan penyakit di banyak negara. Pasien biasanya terdaftar dalam register ginjal nasional yang melaporkan data demografis, insiden, prevalensi dan luaran klisis. Tingkat ketahanan hidup merupakan parameter penting dalam registrasi ginjal untuk menggambarkan kualitas terapi pada  populasi pasien PGK stadium 5. Perhimpunan Nefrologi Indonesia (Pernefri) memiliki program Registri Ginjal Indonesia yang disebut Indonesian Renal Registry (IRR) untuk mengumpulkan data pasien PGK5D untuk  memantau kualitas pengobatan dialisis di Indonesia. IRR merilis laporan tahunan tetapi belum dilengkapi dengan  analisis ketahanan hidup. Tujuan penelitian ini adalah mengetahui angka ketahanan hidup lima tahun pasien PGK5D di Jawa Barat antara tahun 2007-2018 dan faktor risikonya berdasarkan basis data dari IRR. Studi kohort retrospektif dengan mengambil semua data pasien dari basis data IRR kemudian ditentukan data pasien Jawa Barat yang lengkap dan di follow up selama 5 tahun pada 31 Desember 2018.   Analisis Kaplan-Meier dan model proporsional hazard Cox digunakan untuk menganalisis faktor risiko. Subjek berjumlah 3199 data yang dimasukkan dalam penelitian ini. Kesimpulannya, angka harapan hidup satu, 2, 3, 4, dan 5 tahun berturut-turut adalah 82%, 70%, 62%, 58%, dan 55%. Usia lebih dari 55 tahun dan penyakit ginjal yang mendasari memiliki kelangsungan hidup yang lebih buruk dengan hazard ratio 1,28 dan 1,50. Eksplorasi lebih lanjut dari data IRR akan memberikan informasi yang lebih baik tentang perawatan dialisis di Indonesia.
