Abstract

Swamedikasi merupakan upaya seseorang dalam mengobati gejala sakit atau penyakit tanpa berkonsultasi dengan dokter terlebih dahulu. Menurut hasil Riskesdas (2013) sebanyak 35,2% rumah tangga menyimpan obat untuk swamedikasi, terdapat obat keras, obat bebas, antibiotika, obat tradisional dan obat-obat yang tidak teridentifikasi. Apotik dan toko obat/warung merupakan sumber utama mendapatkan obat rumah tangga dengan proporsi masing-masing 41% dan 37,2% serta 23,4% rumah tangga memperoleh obat langsung dari tenaga kesehatan. Tujuan dilakukannya penelitian ini adalah untuk mengetahui persepsi pasien terhadap pelayanan swamedikasi oleh Apoteker di apotik. Penelitian ini menggunakan teknik consecutive sampling, sampel yang digunakan sebanyak 100 pasien yang melakukan swamedikasi, diperoleh dari sepuluh apotik di wilayah Sidoarjo yang Apotekernya memberikan pelayanan swamedikasi secara langsung. Berdasarkan hasil penelitian diperoleh bahwa persepsi mengenai pelayanan konsultasi swamedikasi oleh apoteker dikategorikan kurang baik (<60%), sedangkan persepsi pasien mengenai saran dan peran apoteker dalam pelayanan swamedikasi dikategorikan cukup baik (antara 60-80%).

Abstract

Polimorfisme genetik CYP1A2 berkaitan dengan metabolisme teofilin sehingga dapat memengaruhi kadar obat dalam darah serta berpengaruh terhadap kejadian adverse drug reaction (ADR) dan outcome klinis terapi asma. Frekuensi polimorfisme CYP1A2 diketahui bervariasi antar etnis. Diduga populasi Indonesia memiliki frekuensi varian gen CYP1A2*1F yang tinggi. Penelitian ini bertujuan untuk mengetahui profil polimorfisme gen CYP1A2*1F pada sampel nonasma dan asma di Indonesia dengan populasi lain berdasarkan literatur. Pengambilan data dilakukan pada Januari–Juni 2014. Sampel darah diperoleh dari 29 orang nonasma dan 16 pasien asma. Setelah dilakukan ekstraksi DNA genomik kemudian ditentukan polimorfisme gen CYP1A2*1F dengan metode PCR-RFLP. Hasil penelitian ini menunjukkan bahwa polimorfisme gen CYP1A2*1F pada sampel nonasma adalah 10,35% (3/29) untuk C/C, 37,93% (11/29) untuk C/A dan 51,72% (15/29) untuk A/A. Pada penderita asma frekuensi distribusi genotip C/A sebesar 81,25% (13/16) dan A/A sebesar 18,75% (3/16). Tidak terdapat perbedaan signifikan (p=0,276) frekuensi alel antara sampel nonasma dan pasien asma. Frekuensi gen CYP1A2*1F pada populasi  Indonesia lebih besar dibandingkan dengan populasi Mesir, Jepang, dan Inggris akan tetapi lebih rendah dibandingkan dengan Malaysia. Oleh karena itu, dapat disimpulkan tidak terdapat perbedaan frekuensi.Kata kunci: Asma, CYP1A2*1F, polimorfisme genetikPilot Study on Genetic Polymorphisms CYP1A2*1F on Asthma Patients and Nonasthma in IndonesiaGenetic polymorphisms of CYP1A2 is related to the theophylline metabolism that may affect drug levels in the blood, which can also affect incidence of adverse drug reaction (ADR) and clinical outcomes of asthma therapy. The frequency of CYP1A2 polymorphism is known to vary among ethnic. Allegedly the Indonesian population has high frequency of gene variants of CYP1A2*1F. This study aims to determine the profile of CYP1A2*1F gene polymorphism in a sample of nonasthma and asthma in Indonesia with other populations based on the literature. Data were taken on January–June 2014. Blood samples were obtained from 29 nonasthma samples and 16 patients with asthma. After extraction of genomic DNA, CYP1A2*1F gene polymorphisms determined by PCR-RFLP. The results of this study indicate that the CYP1A2*1F gene polymorphism in nonasthma samples was 10.35% (3/29) for C/C, 37.93% (11/29) for the C/A, and 51.72% (15/29) for A/A. The asthmatics genotype have a frequency distribution of C/A genotype of 81.25% (13/16) and A/A of 18.75% (3/16). There was no significant difference (p=0.276) allele frequencies between samples of nonasthma and asthma patients. The frequency of CYP1A2*1F gene in Indonesian population is higher than the population of Egypt, Japan, and UK, but lower compared to Malaysia. Abstract

Faktor risiko kardiovaskular memegang peranan penting dalam perjalanan penyakit jantung koroner. Faktor risiko yang dapat ditangani dengan baik dapat menurunkan risiko seseorang mengalami penyakit jantung koroner. Apoteker memegang peranan penting di apotek dalam memberikan pharmaceutical care khususnya pada pasien dengan faktor risiko kardiovaskular. Tujuan penelitian ini untuk melihat pengaruh pharmaceutical care pada pasien dengan faktor risiko kardiovaskular di komunitas, khususnya di apotek. Penelitian ini merupakan penelitian eksperimental dengan desain before after study. Pharmaceutical care dilakukan selama 3 bulan pada tahun 2013 dan meliputi: review pengobatan, pemberian edukasi, dan monitoring pengobatan. Perhitungan faktor risiko menggunakan Framingham score. Faktor risiko kardiovaskular pasien diukur sebelum dan sesudah pemberian pharmaceutical care dan diuji secara statistik menggunakan t-test. Terdapat perbedaan yang signifikan pada parameternilai kolesterol, HDL-C, tekanan darah sistol, dan Framingham score pada nilai sebelum dan sesudah mendapatkan pharmaceutical care. Dengan demikian dapat disimpulkan bahwa Pharmaceutical care memberikan pengaruh pada pasien dengan faktor risiko kardiovaskular. Pharmaceutical care memberikan nilai tambah dalam hal pengobatan pasien. Pasien tidak hanya mendapatkan terapi obat tetapi juga mendapatkan edukasi serta perhatian khusus dari apoteker terkait pengobatan yang diterimanya.Kata kunci: Apotek, apoteker, faktor risiko kardiovaskular, pharmaceutical care Effect of Pharmaceutical Care in Patients with Cardiovascular Risk in CommunityCardiovascular risk plays a central role in pathogenesis of cardiovascular disease. Pharmacist have strategic position in community with pharmaceutical care, especially in patients with cardiovascular risk. Objective of this study is to analyse the effect of pharmaceutical care in patients with cardiovascular risk in community. Experimental design with before after study was used. Pharmaceutical care were held in 3 months in 2013, include drug therapy review, patient education, and monitoring outcome of drug therapy. Cardiovascular risk before and after study was analysed using t-test. A significance difference was seen in cholesterol total, HDL-C, systolic blood pressure, and Framingham score in before and after study. Pharmaceutical care give an impact on patients with cardiovascular risk. Pharmaceutical care give an additional benefit in patient’s drug therapy. Patients not only get drug therapy but also education andmonitoring for their clinical condition.Key words: Cardiovascular risk factor, pharmaceutical care, pharmacist, pharmacy

Abstract

Measurement of lung fungction  is a way to determine the effectiveness of a given therapy. A monitoring asthma symptoms is to see the value of peak expiratory flow (PEF) with a peak flow meter. The measurement is objective and can be used to predict the prognosis of asthma through the monitoring of conditions of the lung. This study aims to find out information on the use of a peak flow meter explanations given to the patient’s pharmacist in a pharmacy in a town in East Java. This research method is a non-experimental with cross sectional study. This study uses observation techniques in the form of a checklist, using the concept of a mysterious patient. Sampling by the proportional method, and then the data will be processed descriptively. The research sample of 80 pharmaceutical personnel consisting of 33 pharmacists and 47 non-pharmacists. In the group of non-pharmacists and pharmacists, the points most overlooked is the explanation of the recording of the results (number 6) and compared with the best value (number 8). Pharmacists still need to improve in understanding the basics and techniques in order to use a peak flow meter can help asthma patients to monitor their asthma medication

Abstract

ABSTRACT Treatment of respiratory disorders using a metered-dose inhaler (MDI) with spacer becomes one of the important points. Pharmacists at pharmacies are the final key in therapy to ascertain how the use of inhalation tools is correct. This study aims to determine the completeness of each step of information on how to use MDI with spacer given by pharmacists at pharmacies in East Surabaya. The research design used is cross-sectional study with the subject of research is pharmacist in pharmacy. The research variable is the explanation of how to use MDI inhaler with spacer. An assessment method of 8 steps on how to use MDI with spacer and each number gets a value. Research subjects were 22 pharmacists. Most pharmacists describe each step orally by using the tool directly, ie at step: 1a (77.27%), 1b (77.27%), 2 (81.81%), 3 (31.81%) , 4a (59.09%), 4b (13.63%), 5a (100%), 5b (63.63%), 6a (90.90%), 6b (40.90%), 8a (9 , 09%), and 8b (9.09%). Most of the pharmacists explain verbally with a demonstration tool but only one pharmacist can explain all the steps completely. Abstract

Asthma is a chronic disease that can be controlled by treatment with a controller containing a corticosteroid combination with beta-2 agonists, which are available in the form Diskus® and Turbuhaler® which is DPI (dry powder inhaler) in Indonesia. Techniques improper use can give less than optimal results, therefore, the role of pharmacists to provide education in the form of how to explain the inhaler. This study aims to find out information on how to use Diskus® and Turbuhaler® by pharmacists to patients with asthma in the pharmacy area of the city of Surabaya. This research is non-experimental research with cross sectional method, and using observation techniques in the form of a checklist. Sampling by non-random sampling method by using purposive. The data will be processed with descriptive statistical analysis. Based on the results of research on the use of information studies preparation Diskus® explanation is given, it can be concluded there is no pharmacist in the category optimal technique that is able to mention all the steps properly. Most pharmacists categorized adequate technique for the pharmacist is able to name all of the critical step. While on how to use Turbuhaler® information provided by pharmacists in explaining Turbuhaler to patients in pharmacies is still lacking for by an explanation of each step are not all pharmacists can explain all the steps correctly and based on the critical step yet pharmacists who achieve adequate and optimal techniques.
