Abstract

Introduction: Coronavirus Disease-19 (COVID-19) was a disease caused by SARS-CoV-2. The positivity rate for COVID-19 at the Pauh Health Center until August 2021 reached 34.95%. Most of the people lived in Pauh who have been confirmed COVID-19 were self-isolating. Method: This study used a political and logical approach with descriptive and cross-sectional design. The research was conducted from August – September 2021 in Cupak Tangah, South Limau Manis, and Koto Luar. The research data was obtained by filling out the respondent's questionnaire and guided interview. Result: The problem found using the Hanlon method was the high positivity rate of COVID-19 at the Pauh Health Center. This was due to several factors, such as the lack of public knowledge about COVID-19 and self-isolation, lack of awareness in implementing health protocols, lack of concern and negative stigma towards people who are confirmed to have COVID-19, lack of strict supervision of health protocols by cross-sectors, and obstacles in purchasing. Standard Personal Protective Equipment (PPE). Discussion: Health promotion with modules was chosen based on the demographic and geographical conditions of the Pauh with the aim of providing an understanding of self-isolation. Other activities carried out are socialization and evaluation in the form of pre and post tests targeting the community, health workers, and officials in the Pauh. Conclusion: The solution to the high COVID-19 positivity rate in the Pauh Health Center work area was to form an independent isolation module using the Plan, Do, Check, and Action (PDCA) flow, making this module can be an outcome in helping to improve people's behavior and educating the public to behave well according to the rules. Abstract

Introduction: Generalized Anxiety Disorder (GAD) was a psychiatric disorder characterized by anxiety, motor tension, and autonomic overactivity. Piper methysticum was a plant that had efficacy in various therapies including GAD because of its chemical substance cavaine. Cavaine was a biological ingredient in Piper methysticum which had potent anxiolytic activity. Piper methysticum extract can be used as a preventive therapy for GAD due to its anxiolytic effect on the symptoms of GAD. Discussion: Piper methysticum in its efficacy was able to potentiate GABAARs without subtype selectivity, besides that the concentration of GABA responds to the performance of 4?2? GABAARs significantly, so that the performance of cavaine works effectively and sensitively in the target system in addition to the 1?2?2L receptor. Cavaine also showed a modest increase in GABA concentrations below EC45, but it did not affect EC50 and maximal efficacy of GABA. In addition, the chalcone synthase chain which is broken due to neurodegeneration by the RpBASi enzyme which can extend a single polypeptide chain can be repaired by cavaine. Another effect of cavaine was that it can modulate volt-gated calcium and sodium channels and antagonize Ca2+ channels. Another important activity of cavaine was being able to bind to GABA receptors in the central nervous system besides functioning as an anxiolytic in the serotonergic and glutamanergic pathways. Conclusion: The use of Piper methysticum as an effort to prevent GAD has proven to be superior because it was cheap and easy to obtain, had no side effects, and had high effectiveness in reducing GAD in accordance with its effects and clinical features. Therefore, further research on clinical trials of the use of Piper methysticum continues to be developed so that it can be implemented ethically to prevent GAD. Abstract

Introduction: Breast cancer ranks first in the most cases of malignancy in Indonesia. Phytochemical studies of acetogenin as the main substance of soursop have the potential as a cytotoxic agent and trigger apoptosis of breast cancer cells. Method: This research is a true experimental type using in-vitro studies using continuous cell T47D, the treatment group was given a solution of soursop leaf ethanol extract. The results of this study are seen from the RT-PCR cycle quantification which will be analyzed of the Mann-Whitney Test, and determination of gene expression using the Livak formula. Result: The results showed that the treated T47D cells expressed 1.36 times higher than the control group cells to express the anti-apoptotic protein BCL-2. It’s a significant difference between the treatment and control groups for the expression of the BCL-2 T47D gene, p=0.002 (p<0.005). Discussion: Acetogenin is a strong inhibitor of NADH dehydrogenase which can decrease the production of ATP which can cause death in cancer cells. This inhibition can trigger the activation of the apoptotic pathway and the activation of the BCL-2 gene which can stop the cell cycle which will prevent uncontrolled proliferation. Conclusion: There was a significant difference value of the cycle quantification RT-PCR of the BCL-2 gene in group treatment and control. In addition, soursop leaf extract containing acetogenin was not effective in reducing the expression levels of the anti-apoptotic gene BCL-2 in continuous T47D cells of breast cancer. Keywords: Acetogenin, BCL-2 gene, Continuous cell T47D, Soursop leaf extract, RT-PCR.
