Abstract

Nosocomial infection is an infection by viruses , bacterias, and fungal pathogens that attack a patient undergoing treatment at a hospital. The source of nosocomial infections can be derived from ones body own endogenous flora and cross-infection through the hands of health care workers. The aim of this study was to identify the types of contaminants bacterias on the hands of the nurses of Disease Ward at RSUD Ulin Banjarmasin from June to August 2014. This is a descriptive study with cross sectional approach. A total of 26 samples were selected using total sampling method. The results of the nurses hand swab which had been cultured in an isolation media were viewed using a microscope. Descriptive analysis showed that there were four types of bacteria on the hands of nurses hand swab results: Staphylococcus aureus (53,85%), Staphylococcus epidermidis (34,62%), Escherichia coli (7,69%), and Bacillus sp.(3,84%). Therefore it can be concluded that bacterial contaminants found on the hands of nurses were Staphylococcus aureus, Staphylococcus epidermidis, Escherichia coli, and Bacillus sp. Keywords: nosocomial infection, contaminant bacterias, nurses hand

Abstract

Abstract: Lactate dehydrogenase (LDH) is tumor prognostic for Non Hodgkin Lymphoma (NHL) and it levels would be elevated. The several papers has reported there are an association of total serum LDH levels and staging group of NHL. The study aimed to determine the differences of total serum LDH levels before and after the combination chemotherapy CHOP (Cyclophosphamide, Doxorubicin, Vincristine, and Prednisone) for NHL’s patients at RSUD Ulin Banjarmasin during 3 years. It was an observational analyticstudy design and cross-sectional approach, data collected with design method of totally sampling. Of the 51 patient medical records who included, 54,9% were staging group I-II and 45,1% were staging group III-IV. The first statistic analytic used dependent T test with p=0,303 (p>0,05)showed no differencesof total serum LDH levels before and after the combination chemotherapy CHOP in NHL’s patients with staging group I-II. The second statistic analytic used alternative wilcoxon test with p=0,013 (p<0,05) showed differencesof total serum LDH levels before and after the combination chemotherapy CHOP in NHL’s patients with staging groupIII-IV. The third statistic analytic used alternative mann whitney test with p=0,161 (p>0,05) showed no differencesof total serum LDH levels after the combination chemotherapy CHOP in NHL’s patients between staging group I-II and III-IV.Keywords: total serum LDH levels, NHL, combination chemotherapy CHOP Abstrak: Laktat dehidrogenase (LDH) adalah tumor prognostic pada kanker Limfoma Non Hodgkin (LNH) dan kadarnya selalu menunjukan peningkatan. Penelitian sebelumnya melaporkan bahwa terdapat hubungan antara kadar LDH serum total dengan kelompok stadium kanker LNH. Abstract

Abstract: Urinary Tract Infection (UTI) is a general term that indicates the presences or propagation of microorganism in urine. The urinary tract infection is a very common condition that occurs in both women and men of all ages. Diabetes Mellitus (DM) can lead to the development of bacteriuria. Bacterial resistance to antibiotics prolongs the UTI recover. The aim of this research was to find out bacterial sensitivity urinary tract infection in patients diabetes mellitus to selected antibiotic during July-September 2015. It was a descriptional study with cross-sectional. The subject of this research is all patients diabetes mellitus with urinary tract infection in internal medicine wards Ulin General Hospital Banjarmasin. The sampling technique in this research was a total sampling. The sensitivity test to antibiotic test in the research used Kirby Bauer method and analyzed according to CLSI 2013 standard.This type of bacteria found in this research are Escherichia coli, Pseudomonas aerugenosa, Staphylococcus epidermidis, and Staphylococcus aureus. The sensitivity test result the percentage type of bacteria are sensitive to ceftriaxone 73%, levofloxaxin 84,6%, and gentamicin 73%. The conclution is that most sensitive antibiotic is levofloxaxin (84,6%) Keywords: diabetes mellitus (DM), urinary tract infection (UTI), ceftriaxone, levofloxaxin, gentamicin. Abstrak: Infeksi Saluran Kemih (ISK) didefinisikan sebagai adanya infeksi bakteri dalam urin yang ditandai dengan bakteriuria bermakna. Infeksi saluran kemih merupakan kondisi yang sangat umum terjadi baik pada wanita maupun pria pada semua usia. Diabetes Melitus (DM) dapat menyebabkan perkembangan bakteriuria. Resistensi bakteri terhadap antibiotik menyebabkan lamanya penyembuhan dari ISK. Penelitian ini bertujuan mengetahui sensitivitas bakteri penyebab ISK pada pasien DM terhadap antibiotik terpilih periode Juli-September 2015. Penelitian ini merupakan penelitian deskriptif dengan pendekatan Cross Sectional. Subjek penelitian ini adalah seluruh pasien DM dengan ISK Rawat Inap di Bangsal Penyakit Dalam RSUD Ulin Banjarmasin. Teknik pengambilan sampel pada penelitian ini adalah Total Sampling. Uji sensitivitas bakteriuria terhadap antibiotik uji pada penelitian ini menggunakan metode Kirby-Bauer dan dianalisis secara deskriptif sesuai dengan standar CLSI 2013. Jenis bakteri yang ditemukan pada penelitian ini adalah Escherichia coli, Pseudomonas aerugenosa, Staphylococcus epidermidis, Staphylococcus aureus. Hasil uji sensitivitas menunjukkan bahwa presentase jenis bakteri yang sensitif terhadap Seftriakson 73%, Levofloksasin 84,6%, dan Gentamisin 73%. Persentase isolat bakteri yang resisten terhadap Seftriakson 15,3%, Levofloksasin 3,8%, dan Gentamisin 3,8%. Kesimpulan antibiotik yang paling sensitif terhadap bakteri penyebab ISK pada penilitian ini adalah Levofloksasin (84,6%). Kata-Kata kunci: diabetes melitus (DM), infeksi saluran kemih (ISK), seftriaxon, levofloksasin, gentamisin

Abstract

Abstract: Breast cancer is the most common malignancies among woman in Indonesian. One of breast cancer treatment is chemotherapy which resist cancer’s cells to grow. But chemotherapy also cause DNA damage that interfere lymphocyte proliferation. Depletion on lymphocyte counts will increased patient’s susceptibility to opportunistic infection. The aim of this research was to knowing the difference of lymphocyte counts because of chemotherapy on breast cancer patients in general hospital Ulin Banjarmasin. This research was an observational analytic with cross sectional approach. The sampling technique was non-probability sampling followed by consecutive sampling. Total samples were 61 patients that match with inclusion criteria. Paired T-test was used to analyze the hypothesis with the result of pvalue=0.000 (ρ<0.05), that means there’s a significant decrease in the number of lymphocyte counts after receiving chemotherapy in general hospital Ulin Banjarmasin, January 2014 to December 2015 period. Keywords: chemotherapy, lymphocyte, breast cancer

Abstract

Eritropoetin (Epo) adalah hormon glikoprotein 30,4 kDa yang merupakan regulator utama produksi eritrosit sebagai respons terhadap penurunan oksigenasi jaringan. Produksi Epo terutama terjadi di ginjal dan sebagian kecil di hati, juga oleh sel-sel jaringan dan tumor. Efek Epo pada sel diperantarai oleh reseptor eritropoetin (EpoR). Aktivasi EpoR dimulai saat Epo berikatan dengan EpoR dan membentuk dimerisasi EpoR yang kemudian mengaktivasi jalur pensinyalan JAK2. Aktivasi JAK2 menyebabkan aktivasi jalur-jalur pensinyalan intraseluler, seperti STAT, Ras/MAPK, dan PI3-K. Pada sel-sel kanker, aktivasi jalur-jalur pensinyalan tersebut diduga mengakibatkan peningkatan proliferasi sel dan penurunan apoptosis. Ekspresi EpoR pada sel-sel kanker masih merupakan kontroversi. Penelitian dengan Epo-binding assay tidak dapat mengkonfirmasi keberadaan EpoR di membran sel kanker dan aktivasinya. Hubungan antara ekspresi EpoR dan progresivitas kanker diduga terjadi melalui efek parakrin ke sel-sel lain dalam lingkungan mikrotumor, seperti endotel dan trombosit. Efek Epo pada EpoR di sel-sel endotel dan trombosit tersebut dapat memicu angiogenesis dan mikrometastasis, sehingga menyebabkan perburukan kanker. Peran Epo dan EpoR pada progresivitas sel-sel kanker masih kontroversial dan memerlukan pembuktian lebih lanjut. Sejauh ini pedoman penatalaksanaan anemia terkait kanker yang dikeluarkan oleh NCCN dan ASCO tetap merekomendasikan penggunaan eritropoetin untuk pasien kanker dengan pembatasan-pembatasan tertentu.Erythropoetin (Epo) is a glycoprotein hormone of 30.4 kDa, is the main regulator of erythrocyte production in response to decreased tissue oxygenation. Epo production occurs mainly in the kidney and to a much lesser degree in the liver, it is also produced by other tissues and tumor cells. Activation of EpoR starts when Epo binds to EpoR resulting dimerization which then activates the JAK-2 signaling pathway. JAK2 activation triggers the activation of other intracellular signaling pathways, such as STAT, Ras/MAPK, and PI3-K. In cancer cells, activation of those signaling pathways could lead to increased cell proliferation and reduced cell apoptosis. The expression of EpoR on cancer cells is still an open debate. Studies using Epo-binding assay could not confirm the existence of EpoR on the cell membrane nor its activation. The association between EpoR expression and cancer progression is thought to occur through the paracrine effect on other cells within the tumor microenvironment, such as endothelial cells and platelets. The effect of Epo on the EpoR in endothelial cells and platelets could induce angiogenesis and micrometastasis, which then could lead to cancer progression. Role of Epo and EpoR in cancer progression has been controversial and further research is needed. Cancer-related anemia guidelines from NCCN and ASCO still recommend the use of Epo in cancer patients with certain restrictions.

Abstract

Abstract: One of the most widely used anticoagulants for a complete blood count is ethylenediaminetetraacetic acid (EDTA).  Pseudothrombocytopenia (PTCP) may be caused by EDTA, this condition may lead to inappropriate diagnosis and treatment. We report a 25-year-old female with unspecific headache and joint pain with very low platelet count since 1 month before hospital admission. She was diagnosed with Dengue fever infection and got some platelet transfusion from the previous secondary hospital. She was carried out for a blood test with another anticoagulant (sodium citrate) and bone marrow aspiration. The results showed that she had normal platelet count and bone marrow cellularity. When a patient was identified with thrombocytopenia without any bleeding manifestation, hematology disease, and family history, PTCP should be taken into consideration to prevent unnecessary intervention. Keywords: platelet, pseudothrombocytopenia, ethylenediaminetetraacetic acid, Dengue fever

Abstract

Abstract: Breast cancer is the most common malignancies among woman in Indonesian. One of breast cancer treatment is chemotherapy which resist cancer’s cells to grow. But chemotherapy also cause DNA damage that interfere lymphocyte proliferation. Depletion on lymphocyte counts will increased patient’s susceptibility to opportunistic infection. The aim of this research was to knowing the difference of lymphocyte counts because of chemotherapy on breast cancer patients in general hospital Ulin Banjarmasin. This research was an observational analytic with cross sectional approach. The sampling technique was non-probability sampling followed by consecutive sampling. Total samples were 61 patients that match with inclusion criteria. Paired T-test was used to analyze the hypothesis with the result of pvalue=0.000 (ρ<0.05), that means there’s a significant decrease in the number of lymphocyte counts after receiving chemotherapy in general hospital Ulin Banjarmasin, January 2014 to December 2015 period. Keywords: chemotherapy, lymphocyte, breast cancer
