Abstract

Abstract Physical freshness is a possibility and capability to do adaptation towards physical burden ( from daily activity ) without emerge plentiful fatigue. A nurse, as one of healthy force also should have good physical freshness condition also, especially for them that doing shift work. Based on healthy survey and safety work, about 50% the caution of safety and healthy work is disturbed by an effect from the Nurses physical descent condition, especially for them that tread shift work. The goal of this research is to know the influence night shift towards physical freshness the nurse of night shift at Cempaka room Bapelkes RSD Jombang. This research use an analytic description research project, with cohort approach, that is time approach according to longitudinal, this research also named prospective research. The influence of night shift towards a physical freshness was examined by statistic with significant degree 0.05. The result from statistic test Two Way Anova  got significant value 0.08 which means there is no an influence between night shifts with physical freshness for Nurse at Cempaka room Bapelkes RSD Jombang. The descent of physical freshness in Cempaka room influenced by big burden work and the nurse tread it for a long time. The other important factor is the nurse in Cempaka room never does sport after night shift. While the function of sports it self are increasing the power resistance of Cardio Vascular and breathing system, the amount of erythrocyte and blood volume increase so that vitality and body power resistance will be good. And the nurse will easy to get back their physical freshness to do their daily activity. Keywords: Nurse of night shift, physical freshness.

Abstract

Thalasemia adalah penyakit kronis yang mengarah pada beberapa masalah, yang melibatkan aspek biologis, psikologis, sosial dan spiritual. Pengasuh memainkan peran penting untuk mengadvokasi keluarga. Sementara itu, keluarga bertindak sebagai pengambil keputusan untuk menjaga kesehatan anggota keluarga. Selain itu, mereka harus mempertahankan anggota mereka, dalam hal perawatan diri. Selanjutnya mengenai peran keluarga dalam menjaga kesehatan para anggota. Penelitian ini menggunakan desain pre- eksperimental dan desain one group pretest-posttest. Populasinya adalah keluarga dengan anak thalas- semic di RSUD Daerah Jombang. Jumlah total sampling adalah 14 keluarga. Intervensi keluarga psiko- pendidikan diberikan dalam lima sesi. Sesi pertama adalah pertemuan dengan keluarga yang anaknya menderita thalassemia. Sementara itu, sesi keempat dan kelima digunakan untuk mengunjungi keluarga. Selanjutnya, intervensi diberikan dalam satu atau dua minggu setelah kunjungan. Hasil penelitian menunjukkan pengaruh signifikan perawatan diri keluarga yang diukur oleh keluarga APGAR. Oleh karena itu, dapat direkomendasikan bahwa intervensi keluarga Psiko-pendidikan adalah salah satu intervensi yang digunakan untuk mencari anak-anak dengan thalasemia kronis. Kata kunci: Intervensi keluarga psiko-pendidikan, perawatan diri keluarga

Abstract

Abstract  Thalassemia is a chronic descent disease, where clients with thalassemia will experience a continuous decrease in Hb. This condition causes thalassemia clients to undergo regular hospitalization to obtain blood transfusions. Currently, tansfusion is the best treatment of thalassemia.This research uses qualitative method with case study approach. The researcher chose the research sample with the sampling quota. Researchers perform nursing care on 2 clients thalasemia who are undergoing inpatient at RSUD Jombang.The results show that the priority problem in thalassemia clients is peripheral tissue perfusion ineffectiveness. This problem occurs because the decrease in client Hb levels that cause oxygen supply to the entire body tissue is disrupted. The examination results on both clients showed low Hb levels. Implementations performed for both clients are: observation of vital signs, including capillary refill observation (CRT), blood transfusion administration, nutrition and activity settings. The results of evaluation on clients 1 and 2 found that in addition to transfusion of nutrition and activity management is also very necessary in clients with thalassemia, so it is important that health education given to his family Keywords: nursing care, thalassemia, peripheral tissue perfusion effectiveness

Abstract

Abstract Thalassemia is a disease of chronic that occur in children. Thalassemia is a disease abnormality of blood which is caused by disruption of production hemoglobin the body is reduced . The purpose of the literature study was to determine the nursing process in 4 respondents with thalassemia. The research design uses a literature study with case studies of nursing care in thalassemia patients in Seruni RSUD Jombang in the period 2014-2019. The results    of    the    study    literature     shows     that     problem     of     priority     patients     with thalassemia is perfused peripheral     not effective with a     decrease     in     concentration of hemoglobin. Literature study shows the results of the similarity of the causative factors in 4 respondents who experienced changes in tissue perfusion decreased hemoglobin concentration resulting in decreased O2 binding by erythrocytes so that blood flow to the peripheral tissues decreases and ineffective peripheral perfusion occurs which is characterized by patients feeling weak, conjunctival pallor , CRT> 2 seconds . Peripheral perfusion is not effective as a priority diagnosis because thalassemia patients experience abnormal red blood cell formation, in which blood cells rupture or lysis before 100 days, causing severe anemia that causes decreased  oxygen transport in cells. Keyword : Childcare Nursing, Thalassemia.
