Abstract

Pendahuluan: Pasien tidak bergigi lengkap yang sudah terlalu lama, umumnya mengalami perubahan anatomi dalam rongga mulut seiring bertambahnya usia, termasuk perubahan pada relasi maksilomandibula, sehingga keadaan ini akan menambah tingkat kesulitan dalam perawatan. American College of Prosthodontic telah mengembangkan suatu sistem klasifikasi untuk membantu dalam menyusun rencana perawatan gigi tiruan lengkap yang terdiri dari beberapa kriteria diagnostik utama salah satunya adalah relasi maksilomandibula. Tujuan penelitian adalah untuk mengidentifikasi klasifikasi relasi maksilomandibula pada rahang tidak bergigi lengkap pada pasien di Klinik Prostodonsia RSGM FKG Universitas Padjadjaran (Unpad). Metode: Jenis penelitian ini merupakan penelitian deskriptif dengan teknik survei. Pengambilan sampel menggunakan teknik purposive sampling. Sampel penelitian didapat dari 20 artikulator yang sudah dipasang model kerja rahang tidak bergigi lengkap atas dan bawah. Relasi maksilomandibula didapat dari pemeriksaan artikulator gigi tiruan lengkap secara langsung yang sedang dikerjakan oleh ko-ass atau residen. Hasil: Klasifikasi relasi maksilomandibula kelas I ditemukan sebanyak 12 responden (60%), kelas II sebanyak 2 responden (10%), dan kelas III sebanyak 6 responden (30%). Simpulan: Klasifikasi relasi maksilomandibula kelas I sebanyak 12 responden (60%) merupakan relasi yang paling banyak ditemukan pada pasien rahang tidak bergigi lengkap di RSGM FKG Unpad. ABSTRACTIntroduction: Patients with long period edentulous jaw, generally experience anatomic changes in the oral cavity as we get older, including changes in the maxillomandibular relation, so this condition will increase the level of difficulty in treatment. The American College of Prosthodontic has developed a classification system to assist in developing a complete denture treatment plan consisting of several main diagnostic criteria, one of which is maxillomandibular relations. The aim of the study was to identify the classification of maxillomandibular relations in edentulous patients at the RSGM FKG Prosthodontics Clinic, Padjadjaran University (Unpad). Methods: This type of research is a descriptive study with survey techniques. Sampling using purposive sampling technique. The research sample was obtained from 20 articulators that had been installed with a complete upper and lower toothless jaw working model. Maxillomandibular relation was obtained from direct complete denture articulator examination that was being done by co-ass or resident. Result: Classification of class I maxillomandibular relations was found as many as 12 respondents (60%), class II as many as 2 respondents (10%), and class III as many as 6 respondents (30%). Conclusion: Classification of class I maxillomandibular relationships as many as 12 respondents (60%) is the most common relation in incomplete toothless jaw patients at FKG Unpad RSGM.Keywords: Edentulous jaw, maxillomandibular relation classification.

Abstract

Patients with removable partial denture need abutment tooth as retainer for the clasp. In some cases, this tooth requires endodontic treatment or even jacket crown so it can be used as a clasp retainer. For patient with removable partial denture, post core and crown fabrication in abutment teeth require adjustments to the crown to fit with denture clasp. First case, a 23-year-old male patient with fractured lower second premolar tooth because of previous metal restoration failure and had been treated with root canal treatment. The tooth was used as an abutment for removable partial denture that was still in good condition. The tooth was later treated with cast post and core with PFM (Porcelain fused to metal) crown. Second case, a 60-year-old female patient came to the clinic with a complaint of fracture in upper first premolar. This tooth was also used as abutment for maxillary framework partial denture. A template made of pattern resin was made to reproduce the dentures for both cases. Acrylic resin has good accuracy and stability to reproduce parts of the denture. The advantage of this technique is that patients can use their denture during fabrication of the crown.

Abstract

Introduction: The Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) has become the most widely used diagnostic protocol for research in temporomandibular disorders (TMD). The invalidity of RDC/ TMD in clinical application causes the revision of RDC/TMD to be the Diagnostic Criteria for Temporomandibular Disorder (DC/TMD). The purpose of this study was to determine the differences in diagnosis of TMD between RDC/ TMD examination and DC/TMD Axis I on students of Faculty of Dentistry, Padjadjaran University. Method: The type of this research was comparative survey approach using clinical examinations and questionnaires. The sample was collected from 48 people using random sampling techniques. The diagnosis of TMD was obtained by filling in the symptom questionnaire and clinical examination based on RDC/TMD Axis I and DC/TMD Axis I, which is then entered into the RDC/TMD diagnosis algorithm and DC/TMD decision tree. Results: The results showed that from 48 samples there were 36 (75%) people with the same diagnosis of RDC/TMD and DC/TMD, and 12 (25%) people with different diagnoses between RDC/TMD and DC/TMD. Conclusions: Based on the results of the study, the diagnosis of TMD based on RDC/TMD were still categorized the same as the diagnosis based on DC/TMD.

Abstract

Pembersihan kelebihan semen yang tidak adekuat dapat menginisiasi terjadinya proses inflamasi sekitar implan. Pemilihan jenis koneksi dan teknik sementasi implan dan restorasi adalah dua aspek yang harus diperhatikan untuk menunjang keberhasilan implan. Teknik sementasi restorasi yang dilakukan ekstraoral dapat digunakan sebagai salah satu cara untuk mencegah terjadinya kelebihan semen di sekitar implan. Studi kasus ini bertujuan untuk menunjukkan metode dalam mengurangi risiko kelebihan semen yang terakumulasi di sekitar implan. Pasien laki-laki, usia 50 tahun datang dengan keluhan kehilangan gigi regio 35 dan 36, ingin dibuatkan protesa implan untuk mengganti gigi 35 dan 36. Hasil radiograf CBCT tidak ada kontraindikasi untuk implan dan pasien tidak ada riwayat penyakit sistemik. Bedah implan dilakukan di regio 35 dan 36. Fase pemasangan restorasi mahkota pada implan dilakukan dengan teknik sementasi ekstraoral pada gigi 35 dan 36. Manfaat utama dari sementasi ekstra oral adalah untuk menghilangkan secara langsung kelebihan semen di sekitar margin. Teknik ini mengurangi risiko terjadinya inflamasi pada jaringan lunak dan kehilangan tulang di sekitar implan. Teknik sementasi ekstraoral dapat menjadi salah satu alternatif untuk mencegah terjadinya kegagalan implan yang disebabkan karena kelebihan semen yang tertinggal di sekitar implan.

Abstract

Background: The use of fiber post has become commonplace among dental practitioners due to its several advantages. In accordance with the intended use of post which provides retention for coronal restoration, a cement is used that can provide high quality adhesion. Conventional resin cement has long been adopted as a cementation material for consumer fiber post. However, allowing attachment fiber post failure due to errors in the cementing procedure leads to complications. Purpose: This study aimed to compare the adhesion strength of zinc phosphate cement and self-adhesive resin cement as fiber post cementation material. Both consumer cements were easy to use and cheap. Methods: The samples used numbered up to 20 and were divided into two groups. group 1 used zinc phosphate cement, while group 2 used self-adhesive resin cement. Results: The value of the average adhesion strength of group 1 (zinc phosphate) was 82.65 N, whereas that of group 2 (self-adhesive resin) was 402.81 N. Conclusion: This study concluded that the adhesive strength of self-adhesive resin cement as fiber post cementation material was higher than that of zinc phosphate cement.

Abstract

Introduction: The improvement of ceramic has increased due to highly demand for aesthetic restoration. Ceramic at its nature is a brittle material that highly susceptible to be cracked, that leads to chipping and fracture of the restoration. Composite resin frequently used as a replacement for ceramic repair as a simple and fast solution. The success relies on the bond strength between ceramic and composite. The purpose of this study was to evaluate the shear bond strength of two ceramic repair system to low-glass ceramic lithium disilicate. Methods: This study was experimental laboratory, ten specimens of lithium disilicate ceramic was fabricated. Specimen was divided into two groups, first group include: grinding and treated with ceramic primer and second group: acid etching with hydrofluoric acid and treated with silane. Each specimen was subjected to a shear load at the parallel speed of 0,5mm/minutes until fracture using universal testing machine. Results: The shear bond strength achieved in group two (acid etching and silane) was higher than the group one (grinding and ceramic primer). The t-test one tailed analysis yielded p value 0,0057. The different was significant (p value < 0,05). Conclusion: In this study it is concluded that when treated with acid etching hydrofluoric acid and silane, lithium disilicate yielded higher bond strength with resin composite than when treated with grinding and ceramic primer.

Abstract

Dowel crown restoration is a restoration which is often performed by dentists in daily practice. Dental crowns requirethe dowel crown restoration due to several factors including the crown damage cause has been very severe. Dowelcrown restoration usually consists of two parts, namely dowel with core and crown restoration on it. The core must behigh enough in cervico-occlusal direction to obtain sufficient retention for the attachment of the crown. In some casesthis condition cannot be fulfilled because the cervico-occlusal distances are too short resulting from extrusion of thetooth. This paper is reporting a case of management tooth maxillary second premolar with limited cervico-occlusaldimension with dowel crown restoration in one unit. The technique was a combination of direct and indirect method.The result was an accurate and retentive dowel crown, and has a good aesthetic.

Abstract

In making removable denture, some aspects must be considered: retention, stabilization, support, comfort, andaesthetic. Retention plays an important role in the success of a removable denture. Retention can be obtained from theundercut area, or friction of the denture’s components to the abutment or abutment. Clasp as a retainer, both of metalcasting and wire have been frequently used in the removable partial denture. The clasp has some shortcomings in itsusing, namely color of the metal can sometimes interfere with the aesthetic of the wearer and cannot be placed on teeththat have experienced extensive caries. Telescopic crown as retainer has several advantages, which can be used oncrown of abutment that has experienced extensive caries or tooth that will be an abutment but have endodontic-treated.On telescopic crowns, retention is obtained from the friction between the primary crowns against secondary crown thatserves as a retainer. This case report describes two cases of a removable denture with a metal framework using atelescopic crown on one tooth that serves as a retainer in the case of one-side free-end edentulous.

Abstract

Objective: To determine differences in the height of the mandibular cortical bone in patients with bruxism and those who were not.Material and Methods: This study used an analytic method with a cross-sectional approach. Samples taken were divided into two groups, each with 30 digital panoramic radiograph samples of people with bruxism and those not with bruxism. The study was conducted by measuring the height of the mandibular cortical bone on digital panoramic radiograph based Gonion Index (GI) and Antegonial Index (AI) using Image J application.Results: Bruxism patients aged 21-30 years were 27 people and aged 31-40 years were 3 people, with 12 male samples, and 18 female samples. Samples of non-bruxism patients aged 21-30 years were 28 people and aged 31-40 years were 2 people, with 15 male samples and 15 female samples.Conclusion: There was no difference in the height of the mandibular cortical bone in patients with bruxism and non-bruxism patients based on GI and AI.
