Abstract

Dentist As A Health Worker Claimed To Be Responsible Widely In A Variety Of Existing Public Health Efforts, Especially In The Dental Clinic. Infection Control Is An Effort To Control And Prevent The Spread Of Infectious Diseases In Practice Setting. This Study Aimed To Describe The Implementation Of Infection Control In Dental Practices In The Yogyakarta City In The Availability And Utilization Of Facilities And Infrastructure. A CrossSectional Study Was Conducted In Yogyakarta City With Subjects Of 30 Dentist Who Has Independent Private Practice Chosen By Simple Random Sampling. Observational Techniques Using Check List Was Applied To Assess The Implementation Of Infection Control. Results Showed Existing Deficiencies Of Control Infection In The Components Of Equipment, Dentist, And Room. It Can Be Concluded That The Implementation Of Infection Control In The Yogyakarta City Has Not Gone Well, It Is Source Of Concern As The Possibility Of Infectious Disease Transmission In Dental Clinic Occured. This Because The Level Of Awareness Is Still Lacking And The Dentist Thinks Will Impact The Length Of The Culture Is Still Neglected

Abstract

Permasalahan dan kendala utama dari pelaksanaan rekam medis pada pelayanan kesehatan adalah dokter dan dokter gigi tidak menyadari sepenuhnya manfaat dan kegunaan rekam medis pada sarana pelayanan kesehatan. Akibatnya, rekam medis seringkali dibuat tidak lengkap, tidak jelas dan tidak tepat waktu. Desain penelitian ini adalah cross-sectional dengan bentuk survey. Cara pengambilan sampel dilakukan secara acak dengan limitasi sampel yaitu dokter gigi praktek swasta mandiri di wilayah Kota Yogyakarta, dengan masa pengalaman buka praktek dokter gigi 0 sampai dengan 10 tahun. Cara penelitan sampel menggunakan teknik observasional dengan instrumen checklist yang diisi oleh surveyor. Data yang diperoleh dianalisis secara deskriptif. Subyek penelitian adalah 40 dokter gigi praktek sore di kota Yogyakarta. Seorang dokter gigi tidak menggunakan rekam medik pada saat praktek swasta mandiri. Terdapat 12 dokter gigi (30%) yang menggunakan media buku sebagai rekam medik dan 19 dokter gigi (47,5%) yang menggunakan rekam medis sederhana, yaitu hanya berisi data pasien dan terapi. Standar rekam medis nasional yang mengharuskan menulis odontogram (gambar skema gigi dengan penomoran khusus sesuai FDI World Dental Federation) hanya dilakukan oleh 2 dokter gigi (5%). Hasil penelitian ini menunjukkan masih banyak rekam medis dokter gigi yang harus diperbaiki sesuai dengan standar yang berlaku. Hal ini menggambarkan rentannya profesi kedokteran gigi di bidang regulasi hukum di Indonesia.The main problem and obstacle of the implementation of medical/ dental record in the health care services is that medical doctors and dentists do not have sufficient understanding about the importance and functions of medical/ dental records in health care facilities. Consequently, health care providers often write incomplete, unclear and untimely medical/ dental records. This research was a cross sectional study done in a survey. The sampling method used was a random sampling with sample limitation i.e. independent private-practicing dentists in Yogyakarta with practice length of 0-10 years. Research instrument used in this research was a checklist completed by a surveyor. Data collected was analyzed descriptively. Research subjects were 40 independent private-practicing dentists in Yogyakarta. One dentist did not use dental record in his private practice. There were 12 dentists (30%) who used books as media for dental record and 19 dentists (47.5%) us/ed simple dental record consisting of patient data and treatment. National standard of medical record, which obliges writing odontogram (teeth diagram with special numbering in accordance to FDI World Dental Federation) was only done by 2 dentists (5%). The findings revealed that the writing of dental record needed to be improved in accordance with the prevailing standard. This shows the vulnerability of dental profession in the field of law regulation in Indonesia.

Abstract

Tahun 2018, 57,6% penduduk Indonesia memiliki masalah kesehatan gigi dan mulut. Fakta ini menunjukkan perlunya upaya preventif dan promotif untuk menurunkan angka tersebut. Penelitian ini bertujuan membuat formularium tata laksana preventif dan promotif di Rumah Sakit Gigi dan Mulut Universitas Muhammadiyah Yogyakarta (RSGM UMY), menilai komunikasi terapeutik mahasiswa profesi, dan mengetahui efektivitas formularium tata laksana preventif dan promotif. Jenis penelitian ini adalah mixed method, jenis quasi eksperimental dengan rancangan pre-test post-test with control group dengan 140 responden yang dipilih dengan metode consecutive. Pengukuran skor plak dengan metode Loe dan Silness. Analisis data menggunakan paired sample t-test. Hasil penelitian menunjukkan terbentuknya standar formularium edukasi tata laksana preventif dan promotif, keterampilan komunikasi mahasiswa profesi terhadap pasien adalah baik (74%), serta terdapat perbedaan indeks plak dengan nilai signifikansi 0,000. Kesimpulan penelitian menunjukkan adanya standar formularium edukasi tata laksana preventif dan promotif, keterampilan komunikasi mahasiswa profesi Dokter Gigi terhadap pasien di RSGM UMY adalah baik dan terdapat perbedaan indeks plak sebelum dan sesudah diberikan formularium edukasi tata laksana preventif dan promotif pada pasien perawatan scaling.

Abstract

Pelaksanaan jaminan kesehatan nasional (JKN) di Indonesia yang di implementasikan pada tahun 2014 merupakan tantangan untuk dapat melakukan perubahan pelayanan yang lebih terstruktur. Konsep pelayanan sistem jaminan kesehatan nasional di Indonesia membagi pelayanan menjadi 3 struktur layanan yaitu pelayanan primer, pelayanansekunder dan pelayanan tersier. Pola pembiayaan yang digunakan untuk pelayanan primer adalah sistem kapitasi, sedangkan untuk pelayanan sekunder dan tersier menggunakan sistem DRG (Diagnosis Related Group) yang di Indonesia digunakan istilah Indonesia Case-Based Group (INA CBG`s). Kondisi kesadaran masyarakat Indonesia untuk kesehatangigi dan mulut masih belum baik, sehingga memerlukan perbaikan proses, aksesibilitas, dan konsep pelayanan yang lebih baik. Perbaikan tersebut dapat dilaksanakan dalam bentuk pelayanan primer kedokteran gigi, dengan konsep kendali mutu dan kendali biaya. Tujuan dari penulisan telaah ilmiah ini adalah memberikan kajian pada penetapan pelayanan di bidang kedokteran gigi menjadi pelayanan primer dalam sistem JKN, sesuai dengan kaedah, kondisi dan peraturan yang berlaku. Dokter gigi layanan primer sebagai first professional degree yang peran dan fungsinya adalah di pelayanan tingkat primer (primary health services) berfungsi sebagai gate-keeper pada pemberi pelayanan kesehatan gigi dan mulut yang diharapkan dapat menertibkan sistem rujukan dalam Sistem Kesehatan Nasional. Dokter gigi layanan primerdiharapkan dapat menyelesaikan keluhan masyarakat akan kesehatan gigi yang termasuk dalam batas kompetensi dan kewenangannya, sehingga masyarakat tidak perlu mengorbankan sumber daya yang lebih besar untuk mendapatkan perawatan tingkat spesialis yang sesungguhnya tidak mereka perlukan. Peningkatan kualitas dan kuantitas pelayanan kesehatan gigi dan mulut kepada masyarakat merupakan hal yang mutlak diperlukan oleh masyarakat Indonesia di masa yang akan datang. Determination Of Primary Dental Services In Indonesia. The implementation of Indonesia National Health Security (JKN) is a challange from developing a better structure of healthcare. The structure of health services is divided into three tiered system, which is primary care, secondary care, and tertiary care. Model of financing used for the primary care is capitation system, while for secondary and tertiary care system is claim system based on Indonesia Case-Based Group (CBG INA `s). Improvement of oral health services by general dentist can be carried out in the form of primary care dentistry by quality and cost control.The aim of this study is to provide a scientific study on the determination of dental services offered by general dentist as primary care in National Health Security system, according to the rules, conditions and regulations. Primary care dentists have function at the main level of care (primary health services) as a gate-keeper. They are expected to enforce referral system in the National Health Security System. Primary care dentist issupposed to resolve oral health problem within their competence and authority, so people do not need to go to specialist care that is not needed. Thus, improving the quality of oral health services provided for the public is a must.

Abstract

Antioxidant compounds have an essential role in inhibiting the process of cell proliferation and have a chemopreventive effect. This study aims to trace the presence of antioxidant compounds allegedly contained in tea leaves (Camelia Sinensis L.) and soursop leaves (Anonna muricata L.) and investigate their potency as chemopreventive agents. Research steps include (1) identify the active compounds using thin-layer chromatography (TLC); (2) find out the potential compounds against cancer cells by molecular docking using Autodock Vina; (3) conduct a potential antioxidant test using free radicals DPPH (1,1-diphenyl-2-pikrihidrazil); and (4) identify the cytotoxic effect on WiDr colon cancer cells test using MTT Assay method. The results showed that the ethanolic extract of Camellia sinensis-Annona muricata leaf combination was suspected of containing flavonoid compounds with Rf values of 0.66 and 0.68. Besides, the DPPH antioxidant test showed an IC50 value of 26.9 μg/mL. Cytotoxic potential against WiDr cells resulted in an IC50 value of 41 μg/mL. Furthermore, the molecular docking test of epigallocatechin gallate (EGCG) and Acetogenin compounds against bcl-xl target proteins showed the docking score of -8.1 kcal/mol and -6.7 kcal/mol, respectively. It concluded that the extract combination of Camelia Sinensis-Annona muricata leaf had strong potency as an antioxidant and chemopreventive agent against the WiDr colon cancer cells line.

Abstract

Abstrak: Perioritas utama Indonesia di bidang kesehatan adalah penguatan sistem kesehatan melalui Klinik Pratama yang terstandar. Jumlah klinik pratama yang telah terakreditasi hingga bulan April 2021 didapatkan kurang dari 2%. Aplikasi klik@akreditasi klinik telah dikembangkan untuk membantu klinik dalam mengelola dokumen sesuai standar akreditasi. Namun aplikasi ini bermanfaat apabila klinik telah menyiapkan dokumen akreditasi. Pengabdian masyarakat ini bertujuan untuk mengukur kesiapan Klinik Muhammadiyah-‘Aisyiyah di Indonesia dalam penerapan aplikasi klik@akreditasi klinik dan melakukan pendampingan untuk menyiapkan dokumen akreditasi klinik. Jenis kegiatan pengabdian ini action research. Sebelum menerapkan aplikasi ini, tim membagikan kuesioner kesiapan klinik dalam bentuk Google Form kepada peserta webinar “Peningkatan Mutu Klinik Muhammadiyah-‘Aisyiyah” yang telah diselenggarakan pada Februari 2021. Sasaran responden adalah kepala klinik pratama Muhammadiyah-‘Aisyiyah yang berasal dari seluruh Indonesia dengan respond rate peserta sebanyak 92% atau 84 klinik dari 91 klinik. Hasil pengukuran menunjukkan hanya 27,79% klinik yang siap menerapkan aplikasi klik@akreditasi klinik. Kegiatan pendampingan dilaksanakan secara terstruktur dan rutin sebanyak 15 kali untuk meningkatkan kesiapan.Abstract: Indonesia's main priority in the health sector is strengthening the health system through standardized Primary Clinics. The number of Pratama clinics that have been accredited until April 2021 is less than 2%. The klik@akreditasi klinik application has been developed to assist clinics in managing documents according to accreditation standards. However, this application is useful if the clinic has prepared accreditation documents. This community service aims to measure the readiness of the Muhammadiyah-'Aisyiyah Clinic in Indonesia in implementing the klik@akreditasi klinik application and providing assistance to prepare clinical accreditation documents. This type of service activity is action research. Before implementing this application, the team distributed a clinical readiness questionnaire in the form of a Google Form to the participants of the "Muhammadiyah-'Aisyiyah Clinic Quality Improvement," held in February 2021. The respondent's target is the head of the Muhammadiyah-'Aisyiyah primary clinic, from Indonesia with a participant response rate of 92% or 84 clinics from 91 clinics. The measurement results show that only 27.79% of clinics are ready to apply the klik@akreditasi klinik application. Mentoring activities are carried out in a structured and routine manner 15 times to increase readiness.

Abstract

The implementation of the procurement of medical devices and the layout of the dental polyclinic unit is an important factor in supporting health service efforts. Procurement of medical equipment and spatial planning requires additional budget, this is due to adjustments that must be carried out by the dental polyclinic unit of Batang Hospital. The purpose of this study was to determine the process of implementing procurement of goods and adjusting the layout of the dental polyclinic unit of the Batang Regional General Hospital in the Covid 19 pandemic era. Method: The study used a qualitative method (case study). The research subjects were employees of RSUD Batang totaling 7 respondents and 2 respondents triangulation with total sampling technique. Data analysis using a qualitative interactive model Research Results: The implementation of the proposal, planning, budgeting, procurement and spatial adjustment of the dental polyclinic unit has been going well and in accordance with the procedure, although it is not perfect as for the need for proposing medical equipment, it is not fully in accordance with the needs of the dental clinic. Barriers to procurement of goods and adjustments to the layout of the dental polyclinic unit are caused by limited budgets or not in accordance with realization, so that the proposed needs are considered according to a priority scale. The risk of procurement of goods and adjustments to the layout of the dental polyclinic unit that has not been fulfilled, the management is responsible in this case to reduce the risk by providing level 3 PPE, vaccines and other forms of protection with assistance. Conclusion: The process of implementing the procurement of goods and adjusting the layout of the dental polyclinic unit of the Batang Regional General Hospital in the era of the Covid 19 pandemic has been going well and not fully in accordance with the needs of the user/dentist.

Abstract

Introduction: Institution of Dentist Education (IPDG) which requires professional students to meet a number of requirements tend to get constraints, such as delay in graduation due to students unable to meet all the requirements. This constraint relates to important factors in professional education, namely lecturer factor, patient factor, and student factor. Objectives: This study aimed to understand the factors that influence the fulfillment of the requirements of clinical dental students of Muhammadiyah University of Yogyakarta (UMY). Methods: This research was an observational research with cross sectional design. Research subjects amounted to 100, taken by purposive sampling technique. Research data consisted of primary and secondary data. Primary data were obtained from answers to questionnaires and secondary data were obtained from reports management information system in dental and mouth hospital of UMY. Research data were analyzed by Pearson correlation and multiple linear regression. Results: There was a difference between the questionnaire answers and the reports of management information system regarding the fulfillment of the requirements. Pearson correlation analysis results obtained significance value of the three factors of 0.000 (p<0.05). The result of multiple linear regression analysis showed that lecturer factor had significance value 0.003 (p<0.05), patient factor had significance value equal to 0.013 (p<0.05), and student factor had significance value equal to 0.001 (p<0.05). Simultaneously factor of lecturer, patient factor, and student factor significantly influence to fulfill requirement of clinical dental student with F count equal to 24.836 and significance number 0.000. The contribution of these three factors had an influence of 41.9%, the remaining 58.1% was determined by other variables that were not in this study. Conclusion: Lecturer factor, patient factor, and student factor have relevance and influence the fulfillment of the requirements of clinical dental students of UMY.

Abstract

Introduction: In 2014, the government of Indonesia launched the National Health Insurance (NHI) system. The system faced challenges in terms of its implementation because it changed the funding scheme within the pre-established health services structure. Under the new NHI system, the funding scheme for primary care providers is a capitation system; in the field of dental care, this means primary dentists received payment based on the amount number of participants enrolled within their geographical area. Objectives: The aim of this study is to describe primary dentists’ perceived constraints toward the implementation of the NHI system and their level of knowledge about managed care in Yogyakarta, Indonesia. Methods: To determine dentists’ perceptions about the implementation of NHI and their knowledge about managed care, we surveyed dentists in a descriptive study. Two types of questionnaire were administered to 91 dentists who work at community health centers (CHCs) in Yogyakarta, using the total population sampling technique. Result: Through the surveys, we determined that 78 dentists (71%) perceived constraints related to capitation, 65 dentists (72%) perceived constraints related to benefits packages, 59 dentists (65%) perceived constraints related to workload, and 23 dentists (25%) perceived constraints related to dental health facilities. In terms of their knowledge of managed care, 46 respondents (50.5%) demonstrated a good knowledge of managed care, 43 respondents (47.3%) had moderate knowledge, and 2 respondents (2.2%) had poor knowledge. Conclusion: The constraints to managed care, as perceived by primary dentists, should be taken into account when implementing the NHI system in Yogyakarta, as these perceptions could affect the system’s success.
