Abstract

Background: Exposure to ultraviolet A & B (UVA-UVB) plays a role in the survival of human life, but it may cause negative effects, such as immunosuppression and skin cancer. The effect of solar UVA-UVB exposure on apoptosis (Bax/Bcl-2 ratio) of peripheral blood cutaneous lymphocyte antigen (CLA)+ T-lymphocyte; the immune competent cells in the skin, has not been investigated. Apoptosis of peripheral blood CLA+ Tlymphocyte affects its function; which serves as the skin's resistance and is involved in infectious diseases, skin inflammation, and malignancies. This study observed the effect of solar UVA-UVB to apoptosis (Bax/Bcl2 ratio) of peripheral blood CLA+ T-lymphocytes. Methods: An observational cohort study of 37 male outdoor workers (caddies on the golf course) and 33 indoor workers in Surabaya, aged 20-45 years, with skin phototype IV/V. Measurement of solar UVA-UVB doses received by the subjects was conducted for 2, 4, and 8 weeks. Examination of Bax and Bcl-2 of peripheral blood CLA+ T-lymphocyte was conducted at the beginning of the study, after 4 and 8 weeks. Results: The average dose of solar UVA-UVB for 8 weeks received by outdoor workers was 12450.51±3948.81 (J/m2 ) and that by indoor workers was 1793.97±1518.46 (J/m2 ). Exposure to solar UVAUVB leads to the increase of apoptosis (Bax/Bcl-2 ratio) of peripheral blood CLA+ T-lymphocytes (p=0.003). Conclusion: Cumulative exposure to solar UVA-UVB radiation in high-dose or received within 8 weeks resulted in the increase in apoptosis of peripheral blood CLA+ T-lymphocytes. Abstract

Abstrak. Latar Belakang:  Penyakit Ulkus Genital meningkatkan risiko tertularnya HIV. Di Indonesia, belum ada data pasti prevalensi pasien Ulkus Genital, juga di RSUD Dr. Soetomo Surabaya. Tujuan: Mengevaluasi profil pasien Ulkus Genital di Unit Rawat Kulit dan Kelamin RSUD Dr. Soetomo Surabaya.Metode: Penelitian ini merupakan jenis penelitian deskriptif retrospektif dengan data sekunder. Populasi penelitian adalah semua rekam medik pasien dengan Ulkus Genital yang dirawat di Unit Rawat Jalan Kulit dan Kelamin RSUD Dr. Soetomo Surabaya tahun 2016, 2017, dan 2018. Sampel diambil menggunakan purposive sampling tanpa eksklusi. Hasil : Ditemukan 36 sampel pasien dengan Ulkus Genital selama tiga tahun dengan distribusi usia antara 13 sampai 64 tahun. Usia yang paling mendominasi adalah kelompok usia 20 – 24 tahun sebanyak 30,56%. Sebagian besar sampel berjenis kelamin perempuan, sudah kawin, dan hanya berhubungan dengan lawan jenis. Diagnosis utama pasien Ulkus Genital paling banyak adalah Herpesviral infection of genitalia and urogenital tract. Tipe Ulkus Genital terbanyak adalah Ulkus Herpes dan pemeriksaan penunjang terbanyak yang dilakukan adalah Pemeriksaan Gram. Outcome/Hasil Terapi kebanyakan menunjukkan perkembangan yang baik kearah kesembuhan. Sebagian besar pasien Ulkus Genital datang tanpa kelainan penyerta. Kesimpulan: Mayoritas pasien sebanyak 53,8% dengan Ulkus Genital mengidap Herpes Simplek Primer. Kata Kunci: Ulkus Genital, Herpes Simplek PrimerAbstract. Background:  Genital ulcer disease increases the risk of contracting HIV. In Indonesia, there is no exact data on the prevalence of Genital Ulcer patients, also in Dr. Soetomo Surabaya.Purpose: Evaluating the profile of genital ulcer patients in outpatient care unit Department of Dermatology and Venereology Dr. Soetomo General Academic Hospital.Methods: This is a retrospective descriptive reseach with secondary data. The study population was all medical records of patients with genital ulcers who were treated in the Department of Dermatology and Venereology Dr. Soetomo General Academic Hospital in 2016, 2017 and 2018. Samples were taken using purposive sampling without exclusion.Results: There were 36 samples of patients with genital ulcers for three years with age distribution between 13 to 64 years. The most dominant age group is in the age group of 20-24 years by 30.56%. Most of the samples were female, married, and only related to the opposite sex. The main diagnosis of most genital ulcer patients is Herpesviral infection of genitalia and urogenital tract. The most common type of genital ulcer is herpes ulcer and the most investigation is carried out is Gram examination. Outcome results mostly show good progress toward healing. Most patients with genital ulcer come without comorbidities.Conclusion: The majority of patients were 53.8% with genital ulcer suffering from Herpesviral infection of genitalia and urogenital tract.Keywords: Genital Ulcer, Herpesviral infection

Abstract

Background: Clinical manifestations of leprosy depends on host’s immunity. Regulatory T (Treg) cells are known as CD4+CD25+FOXP3+ have roles in immune respons to maintain homeostasis. Transforming growth factor (TGF)-β are one of cytokine that is secreted by Treg cells. Treg cells are thought as unresponsiveness T cells (anergy) to Mycobacterium leprae and caused disease’s progression to lepromatosa. Purpose: To evaluate expression of TGF-β on Treg cells from patient leprosy’s skin lession. Methods: Design of study was cross-sectional, descriptive observation involving 18 leprosy patients who visited the Leprosy Division, Dermatology and Venereology Outpatient Clinic, Dr. Soetomo General Hospital, Surabaya. Biopsy from skin lession were performed then continued with histopathological examination. The type of leprosy was determined by clinical and histopathological examinations. Expression of TGF-β on Treg cell were performed by double staining immunohistochemistry in Biomoleculer and Pathology and Anatomy Laboratory at Brawijaya University, Malang. Result: Numbers of mid-borderline type (BB) leprosy’s patients were 2 (11,1%),  9 (50%) patients with borderline lepromatosa (BL) type, and 7 (38,5%) patients with Lepromatosa (LL) type. Mean expression of TGF-β on Treg cells from skin lession’s leprosy on BB type was 12 ± 2,12 cells/field of view, on BL type was 16 ± 3,64 cells/fields of view and on LL type was  22± 3,69 cells/field of view. Conclusion: Expression of TGF-β on Treg cells are increasing on skin lession’s leprosy patients might as prognostic indicator of severity of leprosy (lepromatosa type).                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                          Key words: immunohistochemistry, leprosy, Morbus Hansen, regulatory T cell, TGF-β.

Abstract

Background: Acne vulgaris (AV) is a skin disease that is commonly found in adolescence. In addition to causing physical disorders, acne vulgaris affects the quality of life of patients that cause social disorder, psychological, and emotional. Based on the observations of the experts, AV is rarely found in non-westernized populations, indicates an underlying environmental factors including diet. Excessive sebum production has an important role in the pathogenesis of AV. One of the factors which allegedly associated with the production of sebum and diet is IGF-1 in serum. Purpose: To evaluate the levels of IGF-1 serum in patients with AV with varying degrees of severity. Methods: This study is an observational cross-sectional study in 38 AV patients aged 15-24 years at the Outpatient Clinic of Dermatovenereology Department of Dr. Soetomo Hospital Surabaya as many as 38 people. The severity of AV are grouped according to the method of Combined Acne Severity Classification (CASC). Anamnesis of diet based on the table of load and glycemic index foods. Subjects has been collected through consecutive sampling. Results: Serum IGF-1 levels in the mild AV group was 155,000 (SD±23,381) µg/ml, in the moderate AV group was 211,052 (SD±85,498) µg/ml and in severe AV group was 150,000 (SD±45,826) µg/ml. Serum IGF-1 levels in male group was higher than female, was 199,230 (SD±85,582) µg/ml and in female group was 174,000 (SD±58,238) µg/ml. Serum IGF-1 levels in the aged group of 15-20 years was 192,270 (SD±80,766) µg/ml, higher than the aged group of 21-24 years, was 169,380 (SD±46,829) µg/ml. Conclusion: The mean levels of IGF-1 in patients with moderate degrees of AV was higher than mild or severe. IGF-1 levels can vary influenced by many factors. Key words: acne vulgaris, serum IGF-1, severity degrees of acne vulgaris.

Abstract

Introduction: Xeroderma pigmentosum (XP) is a rare autosomal recessive disease caused by a gene defect in nucleotide excision pathway named nucleotide excision repair (NER),  characterized by photosensitivity of the skin. Case: A 5 year old girl, 15 kgs, came with  brown and black spots on the entire body since she was 7 month old. These complaints were accompanied with itchiness and burning sensation. Since two years ago, a small bump appeared on the nose and below the left eye, increasingly enlarged, also easily  bled. She complained red and watery eyes since two weeks ago. Patient often felt glare. Physical examination all over the body found obtained multiple hypopigmented and hyperpigmented macules unsharply marginated with varying  size from pinpoint to few cm and also dry skin. Right and left opthalmic region showed red, watery eye, and photophobia.  Nasal region showed nodules about 1 cm in size, accompanied by erosion and crusting. Histopathology examination result of left infraoculi  region was malignant squamous cell carcinoma and nasal region was malignant basal cell carcinoma. Patient were diagnosed with XP with infraocular squamous cell carcinoma and nasal basal cell carcinoma. Patient was treated with natrium fusidic 2% cream, sunscreen with sun protecting factor (SPF) 30, moisturizer, chlorpheniramine maleate, chemotherapy, and also educated to avoid sun exposure. Discussion: XP causes a variety of clinical manifestations, often with a skin malignancies. Skin biopsy is mandatory to establish the diagnosis

Abstract

Background: The prevalence of atopic dermatitis (AD) has been increased for several decades. T-helper cell (Th)1 and Th2 disregulation influence the level of immunoglobulin (Ig)E and interfere Scoring Atopic Dermatitis (SCORAD) index. Management approachment only relieved the symptoms, therapy with probiotic are potentially  improve this disregulation. Lactobacillus plantarum (LP) has been fullfilled as probiotic criteria and it’s safely used in human. Methods: In a randomized, double-blind, placebo-controlled trial, twenty (22) patients were randomized to take LP or placebo for 12 weeks with 2 weeks wash-out period in Pediatric Division, Dermatology and Venereology Outpatient Clinic. Total IgE serum level  and SCORING indeks were evaluted before and after intervention. Results: Twelve (12) patients in probiotic group and 10 patient in placebo group completed the study. Our findings revealed that there is no significant different in total IgE serum between LP (504.533 IU/ml ± 415.686 IU/ml) and placebo (909.580 ± 885.051 IU/ml) respectively (p=0.117). Mean decreased difference of SCORAD index in LP group (36.784 ± 8.705) significant greater than placebo (26.860 ± 5.021) respectively (p= 0.000). Conclusion: Total IgE serum in both group not significantly different. However, SCORAD indeks in LP group showed a significantly greater reduction.  LP is beneficial in the treatment of children with AD.

Abstract

Background: Condylomata acuminata are mostly found in adult patients through sexual contact. Non-sexual transmission should be considered in children with condylomata acuminata. There are no Food Drug Administration (FDA) guidelines approved for condyloma acuminata in children less than 12 years old. Purpose: To report two rare cases of condylomata acuminata in children. Cases: Two girls, aged 1 and 2 years old came at different times to Dermatology and Venereology Outpatient Clinic Dr. Soetomo General Hospital with perianal tumors. Both were born by vaginal delivery. There was no history of the same disease on the genital or the skin of both parents. Physical examinations on the perianal area were multiple flesh-colored papules with ‘cauliflower’ appearance. There was no sign or symptom of sexual abuse. Histopathological examinations in both patient show epidermis with hyperkeratosis, acanthosis, papilomatosis, some epithelial cell show koilocytosis. On the dermis layer there were proliferation from capillary blood vessel, infiltration mononuclear, intact membrane basale and no sign of malignancy. Human papillomavirus (HPV) subtypes examination results were type 11. Discussion: Trichloroacetic acid (TCA) was giving different results on both cases. The patient who did not show improvement with TCA was consulted to pediatric surgery to get electrodessication therapy by hefrycauter. Conclusion: Accurate history and physical examination isneeded to determine the mode transmissions of condylomata acuminata in children. Human papillomavirus (HPV) subtypes examination is not routinely performed and has limitation to determine the mode of transmission, especially in children. Perinatal transmission should be considered in children up to 2 years old. Multiple modalities are available for the treatment of warts in children.

Abstract

Background: Malassezia folliculitis (MF) is a pilosebaceous follicle infection caused by  Malassezia sp., in the form of papules, solid, diameter <1cm, follicular pustules, moderate itchy, in the upper body, and rarely on the face. Microscopic examination with potassium hydroxide (KOH) found spores. May Grunwald Giemsa (MGG) cytologic staining to diagnose MF spores may demonstrate a more detailed and sharper. Objective: Identification of MF with MGG compared KOH in Dermatology outpatient clinic Dr. Soetomo General Hospital. Methods: The study was observational analytic to identify MF using MGG compared KOH. Involving 52 new patients fulfield the criteria of the study from March to May 2016. Results:  An examination of 52 patients with MF, 7 patients (13.5%) were negative with KOH examination, while the examination MGG negative in 2 patients (3.8%), but not statistically significant (p ≥ 0.5 ). PPV=90% ; NPV value=100% ; sensitivity=95,7% ; spesificity=28,5% Conclusion: KOH still a good investigation for diagnose of MF, but if clinically result positive MF whereas negative KOH,  MGG examination should be performed as a diagnostic confirmation of MF

Abstract

Latar Belakang: Steven Johnson Syndrome (SJS) dan Toxic Epidermal Necrolysis (TEN) merupakan suatu penyakit yang tergolong langka namun dapat mengancam nyawa. Ketidakjelasan etiologi, serta komplikasi yang mungkin timbul pada pasien anak dapat berdampak buruk terhadap prognosis penyakit. Tujuan: Menjelaskan gambaran klinis SJS, SJS – TEN overlap dan TEN pada pasien usia 0 – 18 tahun. Metode: Penelitian deskriptif retrospektif dengan menggunakan total sampling data rekam medis pasien anak usia 0 – 18 tahun di Instalasi Rawat Inap RSUD Dr. Soetomo Surabaya periode 2013 – 2016. Hasil: Terdapat 19 data rekam medis pasien yang memenuhi kriteria inklusi dengan rincian, 16 pasien dengan SJS, 1 pasien dengan SJS – TEN overlap, dan 2 pasien dengan TEN. Kesimpulan: Dugaan etiologi terbanyak adalah reaksi alergi yang diinduksi oleh obat jenis acetaminophen (24%), manifestasi klinis terbanyak adalah makula (100%), baik eritematosa dan hiperpigmentasi, penyakit penyerta terbanyak adalah malnutrisi (31%) dan konjungtivis (31%), komplikasi terbanyak adalah konjungtivitis (67%). Lama perawatan terbanyak adalah selama 1 hingga 7 hari (53%) dengan rata – rata lama perawatan selama 11,6 hari, konsultasi ke dokter lain terbanyak yaitu ke dokter spesialis mata (80%), dan tatalaksana terapi terbanyak adalah melakukan penghentian obat penginduksi timbulnya penyakit.
