<![CDATA[AbstractThe inaccuracy of disease code determination and procedure is caused by several factors according to the condition of each health service institution. The author tries to synthesize the previous studies by looking at the 5M elements (Man, Money, Material, method, Machine) cause the result of coding is not accurate. The purpose of this study is to identify and review the literature that learn about the accuracy of coding and the factors that influence it is on the 5M element. The research method used is with Systematic review. What has not been explained in previous research, and identified in this research is from the side of Method and Machine, that there is still no policy or SPO that regulate the encoding of disease, SPO coding that still not specific, the unavailable coding support books, and SIMRS that is not yet user friendly. It is recommended that there is one working guideline that regulates the coding of disease event and more specific actions as a reference of the coder in health care. Penulis mencoba melakukan sintesis terhadap penelitian-penelitian sebelumya dengan melihat pada unsur 5M (Man, Money, Material, method, Machine) penyebab hasil koding tidak akurat. Tujuan penelitian ini adalah untuk mengidentifikasi dan mereview literature yang mempelajari tentang akurasi koding dan faktor – faktor yang mempengaruhinya yaitu pada unsur 5M.. Metode penelitian yang digunakan adalah dengan Systematic review. Yang belum dijelaskan pada penelitian sebelumnya, dan teridentifikasi dalam penelitian ini adalah dari sisi Method dan Machine, yaitu masih belum adanya kebijakan maupun SPO yang mengatur tentang pengkodean penyakit, SPO pengkodean yang masih belum spesifik, ketidak tersediaannya buku-buku penunjang koding, dan penggunaan SIM RS yang belum user friendly.
