<![CDATA[Abstract : In 2016, at the RSUD dr. Soediran Mangun Sumarso Wonogiri. This research uses descriptive study. The metod used is the method of observation to use the tools of the check ist. Based on observation methods and approaches used are retrospective. In the 98 medical record documents in surgical patients who have been analyzed entirely incomplete document. The highest incomplete in the reporting report of 45 forms or 45.91%. The lowest incomplete in the identification review of 4 forms or 4.08%. Consent form actions medicine (informed consent) seen from the legal aspect of the clarity of the physician in conveying information to the patientâs family and record in the form in clear and accompained by the physicianâs physician, doctorâs name, witnessâs name, witnessâs witness place and date of writting number. There needs to be coordination through official forums between medical records officers and other units especially doctors and nurses in completing the consent form actions medicine (informed consent). Preferably after giving medical treatment to the patient, the medical personnel especially doctors and nurses immediately complete informed consent so there is no void on item item in consent form actions medicine (informed consent).Keyword : Identification Review, Reporting Review, Autentifikasi Review,    Registration Review, Legal Adpects Of Consent Form Medical Actions. Abstrak : Pada tahun 2016 di RSUD dr. Soediran Mangun Sumarso Wonogiri tercatat dengan total tindakan operasi sebesar 4458. Berdasarkan survey pendahuluan sampel 10 DRM rawat inap terdapat ketidaklengkapan dalam pengisian formulir persetujuan tindakan kedokteran lengkap sebanyak 30%. Sedangkan formulir persetujuan tindakan kedokteran tidak lengkap sebanyak 70%. Pengisian formulir persetujuan tindakan kedokteran masih banyak yang tidak lengkap sehingga dapat berpengaruh terhadap proses pelayanan. Jenis penelitiannya menggunakan penelitian deskriptif. Metode yang digunakan adalah metode observasi dengan memakai alat bantu check list. Berdasarkan metode observasi dan pendekatan yang digunakan adalah retrospektif. Dari 98 dokumen rekam medis pada pasien operasi yang telah dianalisis seluruhnya belum lengkap. Ketidaklengkapan tertinggi pada review pelaporan sebanyak 45 formulir atau 45.91%. Ketidaklengkapan terendah pada review identifikasi sebanyak 4 formulir atau 4.08%. Formulir Persetujuan Tindakan Kedokteran (informed consent) dilihat dari aspek hukum yaitu adanya kejelasan dokter dalam menyampaikan informasi kepada pasien atau keluarga pasien serta mencatat didalam formulir tersebut dalam keadaan jelas dan disertai paraf dokter, nama dokter, nama saksi, paraf saksi, dibubuhkan tempat dan tanggal penulisan formulir. Perlu adanya koordinasi melalui forum resmi antara petugas rekam medis dengan unit lain khususnya dokter dan perawat dalam melengkapi formulir persetujuan tindakan kedokteran (informed consent).Sebaiknya setelah memberikan tindakan medis pada pasien, petugas medis khususnya dokter dan perawat segera melengkapi informed consent agar tidak terdapat kekosongan pada item-item di formulir persetujuan tindakan kedokteran (informed consent).Kata Kunci : Review Identifikasi, Review Pelaporan, Review Autentifikasi, Review Pencatatan, Aspek Hukum Dari Formulir PersetujuanTindakan Kedokteran]]>
