Abstract

The discovery of a decline in the cellular immune system in endometrial tissue of women with endometriosis triggers the idea that immunologic factors may play a role in the process of endometriosis. This study was conducted to determine differences in blood plasma P-selectin levels in women with endometriosis and non endometriosis. This cross-sectional study was conducted at the Department of Obstetrics and Gynecology of the Faculty of Medicine, University of North Sumatra (FK USU) from April to August 2014. P-selectin levels were measured through blood plasma of women with endometriosis (cases) and non-endometriosis (control) ELISA. Diagnosis of endometriosis is confirmed by histopathologic examination with hematoxylin eosin staining. Statistical tests were performed with t-test and anova. Each of 28 cases of endometriosis and control with age 30-39 years (endometriosis 42.9% and control 57.1%). The endometriosis group is entirely with 0 (100%) parity, while the control with parity is ≥3 (92.9%). Most cases of endometriosis in stage 4 (46.4%). P-selectin levels in endometriosis were clinically higher when compared with non endometriosis (30.89 ± 8.27 vs. 28.36 ± 7.78 ng / ml), but no statistically significant difference was found (p> 0.05). There was an average difference of P-selectin levels based on endometriosis stage but statistically no significant difference was found (p> 0.05). P-selectin levels in endometriosis were clinically higher when compared with non-endometriosis but there was no statistically significant difference.

Abstract

This study aims to prove the effect of ginger extract on rat liver glycogen levels in hyperglycemic. The design of this study is the post-test only control group design with independent variables (free) include a high-fat diet, injections of streptozotocin, the ethanol extract of ginger doses of 300, 400, 500 mg / kg and the dependent variable (dependent) include blood glucose levels ( GTTO II), glycogen levels. Based on this study, the results of different test GTTO end shows significant differences between negative control group and a positive control, a positive control and P1. Besides, also, there is a significant difference between P1 to P2. This suggests that, dosing higher ginger extract can increase the amount of glycogen levels to near normal. Abstract

Carpal Tunnel Syndrome  adalah salah satu kelainan pada bidang Orthopaedi, dimana terjadi penekanan  pada  Nervus Medianus oleh ligamentum transverase carpal. Nervus ini memberikan persyarafan pada jari jempol, jari telunjuk, jari tengah dan sebagian jari manis. Ketika tendon membengkak atau ligamen menebal maka volume didalam terowongan tersebut akan mengecil dan menyebabkan Nervus Medianus  akan terjepit. Artikel ini akan menjelaskan tentang gejala yang dialami, cara mendiagnosa, terapi yang dilakukan dan terapi lanjutannya serta  apa saja komplikasinya. (QM 2017;01:60-64)

Abstract

Background: Maternal mortality rate is one indicator used to assess overall women health status. According to the WHO, 99% of maternal deaths are delivery related and dominantly occur in developing countries. Objective: To determine the causes of maternal deaths at H. Adam Malik General Hospital Medan based on the following factors: remote determinants, the intermediate determinants, and outcome determinants as a risk factor of maternal deaths. Methods: This retrospective analytical study reviewed medical records of women who underwent delivery at H. Adam Malik General Hospital, Medan from January 1st 2010 until December 31th 2012. Data were statiscally analysed using a descriptive and inferential approach. Univariate, bivariate, and multivariate logistic regression analysis were used. Results: This study obtained as much as 681 complete medical records for 3 years, with 37 maternal deaths. Pre-eclampsia / eclampsia is the most common cause of death by 20 cases (54.1%). Maternal education degree, referral systems, facilities for delivery, history of systemic diseases, pregnancy and postpartum related complications were significantly correlated to the outcome of labor with OR of 1.97; 10.11; 41.36; 5.818; 3.49; and 685.7, respectively and p<0.05. Based on the multiple logistic regression test, maternal history of systemic diseases, pregnancy and postpartum related complications were the three factors that most significantly correlated to maternal mortality with p <0.05. Conclusion: Delivery outcomes were significantly associated with the followingfacrors: formal education, referral status, facilities for delivery, history of systemic diseases, mode of delivery, pregnancy and postpartum related complications. Maternal history of systemic diseases, pregnancy and postpartum related complicationswere the three factors that most significantly correlated to maternal mortality.

Abstract

Secara klinis berbagai kelainan vaskuler memiliki kemiripan, flat atau noduler dengan variasi bentuk, variasi warna darikebiruan, pinkish sampai kemerahan. Pemeriksaan yang teliti dapat menjadi kunci utama untuk membedakan anomali vaskuler, hemangioma atau malformasi.Terapi anomali vaskuler memiliki banyak modalitas, mulai dari hanya observasi / konservatif,  farmakoterapi, kemoterapi,injeksi corticosteroid, embolisasi,  eksisi sampai dengan terapi kombinasi antara embolisasi dan eksisi. Kami melaporkan perbandingan terapi dari beberapa kasushemangiomadan arteriovenous malformations (AVM), yang diterapi dengan injeksi corticosteroid, embolisasi, pada kasus yang lain dilakukan eksisi, namun ada pula yang dilakukan kombinasi keduanya serta satu kasus yang hanya dilakukan observasi. Pengambilan keputusan untuk dilakukan terapi tersebut  berdasarkan hasil diskusi beberapa departemen terkait.Semuanya memiliki hasil akhir yang berbeda dan  tetap diperlukan follow up terhadap pasien-pasien tersebut. Abstract

Diabetes during pregnancy can be divided into pregestational diabetes and gestational diabetes. The risk of fetal Diabetes Mellitus pregestational (DMpG) patients can be either macrosomia or low birth weight. The purpose of this study was to investigate the differences in the administration effect of insulin and Zingiber officinale extract on birth weight of Rattus norvegicus pregestational diabetes mellitus model. This research is an experimental research using post test only control group design. Samples contained 30 pregnant Rattus norvegicus and divided into 5 groups, where K0 (negative control group) and K1 (positive control group) given aquadest 1cc, K2 given insulin 1IU, K3 given ginger extract 500mg/ kg BW, K4 given insulin 1IU and ginger extract 500mg/ kg BW. Treatment was administered during the first 16 days of pregnancy. On the 17th day, the rats were terminated and then the birth weight was measured with the scale. Data analysis using One-way ANOVA test followed by Tamhane test. The results showed that there was a difference of birth weight in the insulin treatment group and ginger extract treatment group (p-value = 0,037 < α =0,05). The conclusion of this study was Zingiber officinale can be used as a single therapy or combination with insulin of Rattus norvegicus pregestational diabetes mellitus model.

Abstract

Stroke is the leading cause of death in Indonesia with the significant increase in prevalence each year. The dominant stroke is ischemic stroke which is often caused by abnormal blood lipid profile which is the increase of total cholesterol to High-Density Lipoprotein (HDL) ratio that is an increase of total cholesterol and decrease of HDL. The risk of ischemic stroke will increase if there is an increase of total cholesterol to HDL ratio because it can stimulate the formation of atherosclerosis. The increase of total cholesterol and the low of HDL level after stroke attack will inhibit the recovery, both of them can influence outcomes and increase mortality in ischemic stroke. This study aim is to determine the correlation between total cholesterol to HDL ratio with outcomes prediction of acute ischemic stroke.This study is an analytic study using cross-sectional study design. Sampling studied are all of the acute ischemic stroke patients at General Hospital of Cut Meutia North Aceh District from May to August 2017 used total sampling that met inclusion and exclusion criteria. Analysis of the correlation between total cholesterol to HDL ratio with outcomes prediction of acute ischemic stroke uses Spearman correlation test with a significant degree (α) of 0,01. The result of this study showed that the mean of total cholesterol to HDL ratio was 3,7 (moderate risk) and outcomes prediction of acute ischemic stroke was good. There is a correlation between total cholesterol to HDL ratio with outcomes prediction of acute ischemic stroke (r=- 0,623;p=0,000). Lower total cholesterol to HDL ratio was correlated with better outcomes of acute ischemic stroke. Total cholesterol to HDL ratio can be used as a predictor of ischemic stroke outcomes.

Abstract

Diabetes mellitus (DM) is a metabolic disease with characteristics of hyperglycemia that has an effect on increasing morbidity and mortality in the mother. One of the complications of pregestational DM (pGDM) is preeclampsia (PE). The perinatal mortality rate increases 20x in pGDM women with PE complication. Exposure to hyperglycemia causes an increase in oxidative stress that increases apoptosis of trophoblast cells which in turn causes the decrease of proliferation and the number of trophoblastic cells and leads to endothelial dysfunction and hypoxia. The purpose of this study was to distinguish the effect of insulin administration and ginger extract on the number of trophoblast cells of R. norvegicus. This research was an experimental research using post-test only control group design. The population used were pregnant R. norvegicus. The sampling technique was simple random sampling with independent variables are insulin and Z. officinale extract and the dependent variable is the number of trophoblast cells. Data analysis of the number of trophoblast cells used wereKruskal-Wallis and Mann-Whitney test. The result showed that there was a significant difference in the trophoblast cell number with p-value <0,05 (p = 0,001) on positive control with insulin treatment group. The conclusion of this study was the presence of ginger extract can be used as a single therapy or combination with insulin for pregestational diabetes.

Abstract

The Global Burden Disease /GBD menjadi suatu standar dari WHO sejak tahun 1990 untuk melaporkan informasi kesehatan global yang terkait dengan penyakit-penyakit lingkungan termasuk yang disebabkan oleh gangguan nutrisi dan komunikasi. Salah satu penyakit yang prevalensiya terus meningkat dan perlu mendapatkan perhatin serius dalam GBD adalah Acquired Immune Deficiency Syndrome (AIDS). Tujuan penulisan artikel ini adalah untuk memperluas wawasan tentang apa itu HIV-AIDS, penyebabnya, faktor resiko, epidemiologi dan prevalensinya, bagaimana mekanisme penurunan imunitas pada infeksi HIV, manifestasi klinis yang mungkin ditimbulkan, apa saja infeksi oportunistik penyebab kematian terbesar pada pasien yang terinfeksi virus HIV, dan jenis-jenis pemeriksaan untuk  mendeteksi HIV dan infeksi oportunistik. Abstract

HISTORY: The girl 15 years old, had main complaint, there was a mass in her pedis since she got 2 years old, look red, no pain although in walk, size equal with the skin. The mass got bigger and pain in walk. The mass get bleedy after had crushed by stone, after that bleedy always happen when there was a contact, although soft touch. Patient had history contact with tuberculosis patient.INTERESTING POINTS FOR DISCUSSION: Tuberculoma is a mass that happened because infected bacil mycobacterium tuberculosis, hemangioma is a soft tissue mass that formed by vascular. hemangioma commonly happened in children about 4-10% in children’s tumor. It happened in a girl 3-5 more times than in a boy. Both of them are rare happen in the same time, especially in pedis. From this case, we  can found both type in the same patient.SOLUTION AND RATIONALE: For this case of soft tissue tumor,we use open biopsy. After we found the result , the best treatment for hemangioma and tuberculoma of the pedis for this case is below knee amputation.FINAL OUTCOME: For this case, the best treatment is amputation, and after amputation, the patient never complain about pain anymore, she feel satisfied and overall her quality of life is improving.
