Abstract

Background: Mycoplasma hominis and Ureaplasma urealyticum are commonly involved in pathogenesis of bacterial vaginosis and non-specific genital infection, while doing vaginal douching also already known as predisposing factor. Methods: To evaluate the prevalence of Mycoplasma hominis and Ureaplasma urealyticum infection in female sex workers and its association with vaginal douching, we conducted a study through 180 female sex workers in East Jakarta, Indonesia. Vaginal fluid samples were collected from these women. It was revealed that most of the women used betel soap, baby soap, tooth paste, or herbal medicine as vaginal douching materials. The association of vaginal douching and prevalence of Mycoplasma hominis and Ureaplasma urealyticum was analyzed using cross sectional statistical methods. Results: There is a high prevalence of Mycoplasma hominis and Ureaplasma urealyticum infection (72%), but there are no association between vaginal douching practice and infection of Mycoplasma hominis and Ureaplasma urealyticum. Conclusions: The high prevalence of Mycoplasma hominis and Ureaplasma urealyticum infection in female sex workers in East Jakarta warrants a routine screening of these infections. Unusual1 materials used for vaginal douching in this study might cause the negative association between high prevalence of these bacteria with vaginal douching. Abstract

Background: Abnormalities of pigmentation is a cosmetic problem which affects the overall appearance of an individual. Pigmentation disorder are caused by the increase (hypermelanosis) or the decrease (hypomelanosis) amount of melanin. Objective: To determine the pattern of pigmentation disorder in Cosmetic Dermatology Clinic H. Adam Malik General Hospital Medan in 2012-2015. Methods: A retrospective study using subject's medical records from the Cosmetic Dermatology Clinic H. Adam Malik General Hospital Medan in 2012 - 2015, presented descriptively. Results: In 2012 - 2015 the number of patients who visited the Cosmetic Dermatology Clinic were respectively 210, 228, 211, 142. Hypermelanosis were found respectively in 24.76%, 16.20%, 12.79%, 5.63%. Whereas hypomelanosis were found 18.10%, 9.21%, 8.53%, 8.45%. During 2012 - 2015 melasma was the most common disorder found among hypermelanosis group, followed by post inflammatory hyperpigmentation. Meanwhile vitiligo was the only diagnosis in hypomelanosis group. Each year most of the pigmentation disorder patients came from the age group 38-48 years old except in 2015. Women were more affected than men. Conclusion: In 2012-2014, the largest proportion of pigmentation disorder were hypermelanosis, mostly melasma. In 2015, it was hypomelanosis with vitiligo as the diagnosis. Keywords: pigmentation disorder, cosmetic division, retrospective

Abstract

Aims: This study investigated the seroprevalence and sexual risk behavior factors associated with syphilis and herpes simplex virus type 2 (HSV-2) among men who have sex with men (MSM), in Manado, Indonesia. Methods: Serum samples and questionnaires regarding their socio-demographic and sexual behaviour were collected from 60 MSM in Manado. Samples were screened for anti-HSV-2 IgG for herpes, the Venereal Disease Research Laboratory (VDRL) and the Treponema Pallidum Haemagglutination (TPHA) test for syphilis. A cross sectional method was used in this study. Results: Of 60 MSM (median age was 20 years), 35% had homosexual orientation and 65% had bisexual orientation. First sexual intercourse of most participants (48.3%) within 16 to 18 years old. In the last 12 months, most participants (55%) had only one male sex partner. There were 63.3% and 16.7% participants performed oral and anal intercourse. About 23.3% participants used condom consistently, but only 35.7% used compatible lubricant. Seropositivity of VDRL and TPHA was detected in 5% participants and anti-HSV-2 IgG in 23.3% participants. The positive serology test for syphilis was associated with sexual intercourse mode (p= 0.001). Significant association was found between anti-HSV-2 IgG seropositivity and condom-use behaviour (p=0,028). Conclusions: The study demonstrated 5% and 23.3% positivity of serology test for syphilis and anti-HSV-2 IgG respectively. Seropositivity of syphilis was correlated with oral and anal intercourse while HSV-2 with no condom-use behaviour among MSM in Manado, Indonesia. Abstract

Background: Cajuputi/Cajeput/Melalueca leucadendron oil is a type of essential oil, widely known and used by Indonesian for infants, children, and elderly. It is also included in telon oil compositions (&plusmn;42%). Excessive application of cajeput oil and telon oil may lead to skin irritation.Methods: This randomized single-blind control trial was conducted with 10 subjects who met the inclusion criteria. Four areas with diameter of 10 mm each were made on the flexor surface of the upper arm. Area 1 was designated as negative control. Area 2 to 4 were pre-irritated with tape stripping for 40 &plusmn; 10 times using Nachitape&reg;, followed by repeated open application test (ROAT) with cajeput oil, telon oil and alcohol (positive control). The skin irritation was examined in 15 minutes after each cycle by measuring transepidermal water loss (TEWL) using Tewameter&reg; TM300 (Courage-Khazaka, Germany) and erythema using Mexameter&reg; MDD4. Data were analyzed using One-way ANOVA with p<0.05 considered significance.Results: The average TEWL and erythema of cajeput oil (15.59; 345.42) were the highest compared to telon oil (12.63; 316.60), alcohol (13.87; 319.06) and negative control (7,48; 296.68). All treatment groups had significant differences in TEWL (p<0.000) and erythema (p<0.002) when compared to the negative control. However, cajeput oil showed the largest difference.Conclusion: Cajeput oil caused the most irritation compared to alcohol and telon oil. Therefore, it should be used with caution. Abstract

Background: Demodicosis is a disease, caused by parasitisation of the opportunistic parasites from the acariasis group &ndash; Demodex mites. This article presents a comparative study of two methods (light microscopy of skin scrapings and confocal laser scanning in vivo microscopy) for identification of Demodex mites on the facial skin in acne and rosacea patients. The use of confocal laser scanning in vivo microscopy in dermatology today is considered as one of the most promising methods. Methods: A total of 90 subjects were included in the study, comprising 30 patients with acne and rosacea complicated by demodicosis, 30 patients with acne and rosacea not complicated by demodicosis, and 30 healthy volunteers. All patients were examined by scraping of the skin, eyebrow and/or eyelash epilation and confocal laser scanning in vivo microscopy. Results: The specificity of light microscopy of skin scrapings was 65.5%, while the specificity of confocal laser scanning in vivo microscopy for the diagnosis of demodicosis was 68.8%. Conclusion: The study showed advantages of confocal laser scanning in vivo microscopy compared to the traditional method of investigation. Abstract

Background: Vitiligo is a skin pigmentation disorder, affecting approximately 1-2% population worldwide. Currently, there are wide varieties of treatment to achieve re-pigmentation, and phototherapy is one of the effective treatments. Narrowband ultraviolet B (NB-UVB) is administered twice a week according to the phototherapy protocol for vitiligo. Some vitiligo patients have difficulty following this schedule, leading to decrease of patient&rsquo;s compliance. Therefore, we performed study comparing the efficacy of phototherapy frequency, once a week and twice a week NB-UVB in vitiligo. Methods: This preliminary study divided 10 patients into 2 groups of 5 patients each with randomized, double-blind method. Group A was scheduled for NB-UVB phototherapy twice a week while Group B was scheduled for once a week. All patients received topical desoxymetasone cream once daily. Re-pigmentation were compared using Vitiligo Area Scoring Index (VASI). VASI was graded as good (&ge;25%), moderate (10-24%), and poor (<10 %) responses. Evaluation for melanin index for each lesion was assessed by Mexameter&reg; and cumulative doses of phototherapy were compared. Results: Thirty-seven lesions were treated on face and neck, trunk and extremities. Good responses were obtained in 14.29% group A patients and 13.04% group B patients. The mean of cumulative doses phototherapy in group A was 850 mJ/cm2, whereas group B was 800 mJ/cm2. There was no significant differences (p>0.05) between both groups in terms of re-pigmentation, cumulative doses of phototherapy, and melanin index. Conclusion: In vitiligo, once a week NB-UVB phototherapy appears to be as efficacious as twice a week treatment schedule. Abstract

Introduction: Systemic contact dermatitis (SCD) is a systemic reactivation of a previous allergic contact dermatitis. The initial exposure may usually be topical, followed by oral, intravenous or inhalation exposure leading to a systemic hypersensitivity reaction. A case of a 27 year-old male with SCD due to nickel is reportedCase Report: A 27 year-old male presented with recurrent pruritic eruption consist of deep seated vesicles on both palmar and left plantar since 6 months before admission. This complaint began after patient consumed excessive amounts of chocolate, canned food, and beans. The patient worked as a technician in a food factory. History of allergy due to nickel was acknowledged since childhood. The clinical presentation was diffuse deep seated vesicles, and multiple erythematous macules to plaques, with collarette scale. Patch test using the European standard showed a +3 result to nickel. The patient was diagnosed as systemic contact dermatitis due to nickel. The treatments were topical corticosteroid and patient education of avoidance of both contact and systemic exposure to nickel. The patient showed clinical improvement after 2 weeks.Discussion: SCD was diagnosed due to the history of massive consumption of food containing nickel in a patient who had initial sensitization to nickel, with clinical features and the patch test result. Advice to be aware of nickel and its avoidance is important in SCD management. Abstract

Background: Reticulate acropigmentations of Kitamura (RAPK) is an autosomal dominant inherited disorder characterized by pigmented, angulated, irregular freckle-like lesion with atrophy on the surface, arranged in a reticulate pattern on the dorsa of the hands and feet. It was first described by Kitamura and Akamatsu in Japan in 1943. The usual age of onset is the first and second decades of life. Palms and soles reveal pits and breaks in the epidermal ridge pattern. The histopathological examination show epidermal atrophy, digitate and filiform elongated rete ridges with clumps of heavy melanin pigmentation at their tips.Case: A 59-year-old male presented with asymptomatic and progressive brownish-black discoloration in a reticulate pattern on the dorsal aspect of his hands and feet. The lesions initially appeared when the patient was 45 years old. It was not preceded by any erythema or inflammation. There was no similar case in the family. Laboratory findings were within normal limits.Discussion: Skin biopsy taken from the dorsal of the hand and foot revealed hyperkeratosis, thinning of epithelium, filiform elongation of the rete ridges, increased melanocyte numbers in the basal layer, and lymphocyte infiltration in the dermis. Based on the clinical and histological findings he was diagnosed as RAPK. From some reports, sporadic cases without the involvement of other family members may occur, like our patient. Palms and soles involvement in RAPK is still debated, some considered it as a characteristic sign of this disorder while others refuted it. Abstract

Background: Atypical mycobacterium infection occurs under certain skin conditions, namely the disruption of skin integrity and mucous membranes accompanied by the reduction of cellular immunity. However, atypical mycobacterial infection in elderly patients is rarely reported. Case: A 64 years old male patient, complained of red lumps on the upper-backfor a month, accompanied by mild fever and minimal pain. Three months before, the patient had accupuncture on the neck and upper back. Physical examination showed multiple miliar to lenticular sized papules and pustules on an erythematous-violaceus base with hard and immobile palpable nodes and infiltrate. After clinical and laboratory workup, the patient was diagnosed withcarbuncle with Candida spp colonization. The treatment consisted of systemicantibiotics and topical antifungals. There was no clinical improvement after 3 weeks. Histopathology and laboratory results suggested atypical mycobacterium infection. Discussion: Atypical mycobacterium infection should be considered in elderly patients with skin and soft tissue infections that show no clinical improvement tostandard therapy. Abstract

Disability is one of problems in leprosy or Morbus Hansen (MH), which can cause the patient loose his autonomy and may affect his social relationship with family and community. Disability occurs due to neurological inflammation that can manifest as silent neuritis (which develops without any pain). Silent neuritis can be recognized early with a routine prevention of disability (POD) assessment. A 19-year-old male patient was referred from a District General Hospital with a history of numbness and stiffness of his 4th and 5th fingers of his left hand since 1 month before admittance. The patient was refered by Community Health Center (CHC or PUSKESMAS) after a one year treatment and RFT. During his treatment at the CHC, no assessment of peripheral nerve or POD had ever been performed. The POD assessment at our hospital demonstrated sensory deficit at some points of assessment on both palms and reduced muscle strength of the first and 5th fingers in both hands. Nerve conduction velocity (NCV) performed at the outpatient of Neurology Department, showed multiple mononeuropathy MH with irreversible damage. Nerve damage is still considered reversible when it occurs less than 6 months. In this case, the silent neuritis was not detected early and there was delayed treatment; as showed by NCV which revealed a manifestation of irreversible nerve damage. Routine POD assessment may detect the condition and appropriate treatment may overcome the nerve damage. Keywords: Morbus Hansen, silent neuritis, prevention of disability assessment
