Focus and Scope Jurnal Keperawatan Indonesia (JKI, or Nursing Journal of Indonesia) contributes to the dissemination of information related to nursing research and evidence-based study on urban nursing issues in low-middle income countries. The scope of this journal is broadly multi-perspective in nursing areas such as Nursing Education, Clinical Practice, Community Health Care, Management and Health System, Health Informatics, and Transcultural Nursing, with a focus on urban nursing issues in low-middle income countries. JKI is committed to communicating and being open to the discussion of ideas, facts, and issues related to health across a wide range of disciplines. The journal accepts original research articles, synthesized literature, and best practice reports or case reports that use the quantitative, qualitative, or mixed-method approach. JKI adheres to journalistic standards that require transparency of real and potential conflicts of interest that authors and editors may have. It follows publishing standards set by the International Committee of Medical Journal Editors (ICMJE), the World Association of Medical Editors (WAME), and the Committee on Publication Ethics (COPE). Letters and commentaries about our published articles are welcome. All submitted contributions will undergo a blind peer-review process according to appropriate criteria.

Abstract

Tuntutan pelayanan keperawatan membuat sebagian rumah sakit menerapkan pendokumentasian asuhan keperawatan dengan menggunakan komputerisasi. Pasien di rumah sakit sering merasa tidak puas dengan pelayanan karena perawat terlalu lama mencari data-data riwayat penyakit pasien. Penelitian ini bertujuan untuk mengetahui tentang kepuasan perawat dengan kualitas pendokumentasian asuhan keperawatan berbasis komputer. Desain penelitian menggunakan rancangan deskriftif analitik. Sampel penelitian menggunakan total sampling yaitu seluruh perawat rawat inap berjumlah 81 responden. Hasil penelitian didapatkan 58% responden menyatakan tidak puas, dan sebanyak 96,3% responden melakukan pendokumentasian asuhan keperawatan berbasis komputerisasi dengan kualitas baik. Persepsi perawat tentang kepuasan bahwa perawat merasa tidak puas terutama pada aspek dokumentasi yang dapat melindungi perawat dari hukum tetapi persepsi perawat tentang kualitas pendokumentasian yang mereka lakukan dalam penggunaan komputerisasi cenderung menilai baik kualitas pendokumentasian. Pihak manajemen rumah sakit perlu membuat kebijakan baku tentang perlindungan perawat dari pendokumentasian asuhan keperawatan karena menyangkut tentang kepuasan perawat sebagai pengguna. Kata Kunci: kepuasan perawat, kualitas dokumentasi, manajemen, sistem informasi Abstract Nursing satisfaction to quality of nursing of computer-based nursing.Nursing service demands make some hospitals apply documentation of nursing care using computerization. Patients is often unsatisfactory service so that the patient's time is wasted because the nurse is too long looking for patient history history data. This study aims to find out about nurse satisfaction with the quality of documenting computer-based nursing care. The design uses analytical descriptive research design. The study sample used total sampling, namely all nurses inpatients amounted to 81 respondents. The results showed that more than half of nurses (58%) expressed dissatisfaction, more than half of nurses (96.3%) documented good quality computerized nursing care. Nurses 'perceptions of satisfaction that nurses feel dissatisfied, especially in the documentation aspect that can protect nurses from the law, but nurses' perceptions about the quality of documentation they do in computerized use tend to judge both the quality of documentation. The hospital management needs to make a standard policy regarding the protection of nurses from documenting nursing care because it concerns the satisfaction of nurses as users. Keywords: nurse's satisfaction, quality of documentation, management, information systems

Abstract

Penggunaan pembalut yang kurang tepat dapat berimplikasi pada terjadinya berbagai masalah kesehatan reproduksi. Perilaku penggunaan pembalut diinisiasi oleh domain pengetahuan dan sikap. Remaja putri merupakan kelompok paling rentan dimana terdapat kecenderungan mengembangkan stigma negatif terhadap menstruasi sehingga berimbas pada kekurangterpaparan terhadap informasi higienitas menstruasi. Penelitian ini dikembangkan untuk mengonfirmasi pengetahuan dan sikap dalam menginisiasi pembentukan perilaku penggunaan pembalut pada remaja putri di daerah pedesaan. Jenis penelitian yang digunakan adalah observasional analitik dengan rancangan cross-sectional. Pemilihan lokasi dan sampel penelitian ditentukan secara purposive dengan memasukkan seluruh siswi Sekolah Menengah Per-tama Negeri X Tondong Tallasa yang telah mengalami menstruasi. Hasil penelitian menunjukkan remaja putri memiliki pengetahuan, sikap, dan perilaku penggunaan pembalut yang masih kurang dengan persentase berturut-turut 57,45%, 57,45%, dan 70,21%. Temuan lain dalam penelitian ini mengindikasikan adanya perbedaan perilaku penggunaan pem-balut pada remaja putri menurut pengetahuan dan sikap. Meskipun demikian, tidak ditemukan adanya signifikansi pengetahuan dan sikap dalam perilaku penggunaan pembalut pada responden (p> 0,05). Kesimpulan dalam penelitian ini adalah pengetahuan dan sikap tidak menunjukkan asosiasi dengan perilaku penggunaan pembalut pada remaja putri di daerah pedesaan. Pada penelitian berikutnya diharapkan menyertakan variabel kontributor yang lain dengan me-libatkan jumlah sampel yang lebih besar. Kata Kunci: menstruasi, pembalut, pengetahuan, perilaku, remaja putri, sikap Abstract Are Knowledge and Attitude Being Main Contributors in Shaping Behavior of Pad Use among Female Adolescents in Rural Pangkep District?Inappropriate use of pads may lead to various reproductive health problems. Behavior of pad use is initiated by knowledge and attitude domain. Female adolescents are the most vulnerable group in which there is a tendency to develop negative stigma against menstruation so that impact on lack of exposure to the menstrual hygiene information. This study aims to determine association between knowledge and attitude in shaping behavior of pad use among female adolescents in rural. The type of research used was analytic observational with cross-sectional design. Location and sample of the study were determined purposively by taking all students of SMPN X Tondong Tallasa who have experienced menstruation. This study found that female adolescents still had poor knowledge, attitudes, and behaviors in pad use with a percentage of 57,45%, 57,45%, and 70,21% respectively. Other findings in this study showed there was difference in behavior of pad use in female adolescents according to knowledge and attitude. Nevertheless, there was no significance of knowledge and attitude in the behavior of pad use on respondents (p> 0,05). Conclusions in this study found that knowledge and attitudes do not show association with behavior of pad use in female adolescents in rural area. Abstract

Integrated Health Post for Child Health (Posyandu) has been endorsed since 1984 as a community-based health program in Indonesia. Despite its potential in improving child health outcomes, evidence related to its current implementation is lacking. This study aimed to explore the current implementation of posyandu in the country. Data of 638 posyandu surveyed in the 5th wave of Indonesia Family Life Survey (IFLS-5) located in urban and rural areas were analyzed, applying chi-square and independent t-test method. Posyandu surveyed run a number of activities (mean type of activities=8.1 in rural vs. 7.4 in urban areas, p<0.001), focusing on weight monitoring, supplementary food provision, and vitamin A supplementation. Approximately 38% of Community Health Workers (CHWs) has never been trained in posyandu’s management nor child health care. Financial barriers, medicine and equipment supplies, and location to conduct posyandu’s activities remained as challenges in the implementation. While posyandu has been implemented for decades, the study found that its implementation has not been optimal. Actions should be taken to improve posyandu’s implementation. These include strengthening collaboration to address the problems and improving CHWs activities.  Keywords: posyandu, child health, community-based health program

Abstract

Studi literatur menyebutkan bahwa kebutuhan keluarga saat mendampingi anak dirawat di PICU sangatlah kompleks dan bersifat subjektif sehingga pengkajian kuesioner dirasa belum mampu mewakili gambaran kebutuhan keluarga yang sebenarnya. Tujuan dari penelitian ini adalah untuk membandingkan persepsi perawat PICU dengan keluarga pasien PICU mengenai prioritas kebutuhan keluarga dari anak yang dirawat di ruang rawat intensif. Penelitian ini meng-gunakan mixed method approach dengan pendekatan kuantitatif menggunakan desain cross sectional di tahap pertama pengumpulan data, dilanjutkan dengan pendekatan kualitatif yang dilakukan dengan cara wawancara mendalam di tahap ke dua. Data kuantitatif diperoleh dengan menggunakan Critical Care Family Need Inventory yang telah di-modifikasi. Hasil analisis data menunjukkan tidak adanya perbedaan yang signifikan antara persepsi perawat dan keluarga dalam menilai kebutuhan keluarga pasien PICU. Hasil ini menunjukkan bahwa secara umum perawat PICU di dua rumah sakit yang menjadi sampel dalam penelitian ini lebih mampu memahami kebutuhan keluarga pasien dibandingkan dengan perawat dalam penelitian-penelitian sebelumnya. Kata Kunci: kebutuhan keluarga pasien PICU, mixed method, persepsi perawat Abstract  Mutual understanding between nurses and patients’ family on family needs during their child’s stay in PICU. The literature review indicated that factors affecting parents’ needs in a PICU are interrelated and not easily, or appropriately, assessed by using an inventory. Therefore, the objective of this study was to explore the needs of parents whose child has been an in-patient in a PICU, not only from the parents, but also from the perspective of nurses. A mixed method approach has been employed in this study. Quantitative data has been collected by using modified version of Critical Care Family Need Inventory, while the qualitative data was gained through in-depth interview.  The result of data analysis showed that there were no significant differences between the perception of the nurses and parents regarding the needs. The finding suggests that in general, PICU nurses in these two hospitals have better understanding about the parents' needs in comparation with most of nurses in the previous studies.  Keywords:  families’ needs in PICU, mixed method, nurses’ perception

Abstract

Perawat kepala ruang melakukan pendelegasian keperawatan sebagai salah satu kegiatan dalam melaksanakan fungsi manajemen keperawatan. Tujuan penelitian adalah menggali pengalaman perawat kepala ruang tentang pelaksanaan Model Delegasi Keperawatan ’Relactor’ (MDK’R’), serta kendala-kendala yang dihadapi dalam pelaksanaan MDK’R’ di Rumah Sakit (RS) Roemani Semarang. Desain penelitian kualitatif dengan pendekatan fenomenologi deskriptif. Pengumpulan data dilakukan dengan wawancara dan menggunakan pedoman wawancara sebagai instrumen penelitian dan peneliti sendiri. Partisipan dalam penelitian adalah 3 orang perawat kepala ruang. Penelitian dilakukan di RS Roemani Semarang, di ruang rawat anak, ruang rawat penyakit dalam, dan Intensive Care Unit (ICU). Penelitian ini mendapatkan hasil bahwa dalam pelaksanaan MDK’R’ terdapat pengisian formulir yang belum optimal, penundaan pengisian formulir, dan ketidakpahaman cara pengisian formulir akibat belum adanya standardisasi dan kurangnya kontrol dari atasan. Kendala dalam pelaksanaan MDK’R’ pada penelitian ini akibat banyaknya pekerjaan perawat kepala ruang, kurangnya kedisiplinan, kesadaran diri rendah, dan kurangnya manajemen waktu dan prioritas pekerjaan. Kata Kunci: Model Delegasi Keperawatan Relactor, pelaksanaan MDK’R’, pengalaman, perawat kepala ruang Abstract The experience of head nurses in the implementation of the relactor nursing delegation model at roemani hospital. The head of the nursing room delegates nursing as one of the activities in carrying out the nursing management function. The purpose of this study was to explore the experiences of the head nurses about the implementation of Relactor Nursing Delegation Model (RNDM) and the constraints faced in implementing RNDM at Roemani Hospital Semarang. In this study, the qualitative research design was conducted with a descriptive phenomenological approach. Data collection was done by interviewing and using interview guidelines as an instrument of research and the researcher itself. Participants in this study were three head nurses. The study was conducted in Roemani Hospital Semarang in the children ward, internal medicine ward, and ICU. The results of this study showed that in the implementation of RNDM there is not optimal and delay in filling out forms, lack of understanding about filling out forms because there is no standardization and lack of control from superiors. The constraints in the implementation of RNDM in this study were affected by many worloads of the head nurses, lack of discipline, low self-awareness, and lack of time management and work priorities. Keyword: Relactor Nursing Delegation Model, MDK’R’ Implementation, experience, head of the nursing room.
Hand Hygiene Among Hospital Staff: A Survey of Knowledge, Attitude, and Practice in A General Hospital in Syria Cornelis Jonker; Maha Othman

Abstract

Poor hand-hygiene adherence endangers the safety of both healthcare workers and patients. A cross-sectional Knowledge Attitudes Practicing (KAP) survey (n= 268) and an observation of hand-hygiene compliance (n= 36) were conducted among staff in a general hospital in Syria. The aim of this study was to investigate the degree of compliance with hand hygiene protocols by general health care workers including their knowledge, attitude and practice. The mean overall observed hand-hygiene compliance rate was 45.7% (95% CI 37.1 – 54.3). Nurses were observed to have better compliance than physicians. Staff were observed to be more concerned in performing hand-hygiene after than before patient contact (before mean= 32.0%, after mean= 51.2%, p< 0.05). The questionnaire showed that there was a significant correlation between knowledge, attitude, and facilities on the one hand and poor self-reported adherence on the other. Multivariate analysis showed that poor adherence was statistically significantly associated with males (63.5%), untrained staff (58.5%) and unavailability of washing basins (60.4%).  Poor adherence was high in ICU, among younger and unaware participants. Keywords: compliance, hand hygiene, hospital acquired infection, healthcare workers, Knowledge-Attituded-Practice (KAP), patient safetyAbstrak Kepatuhan pada kebersihan tangan yang buruk membahayakan keselamatan tenaga kesehatan dan pasien. Survei potong lintang Pengetahuan Sikap Praktek (n= 268) dan observasi kepatuhan kebersihan tangan (n= 36) dilakukan di antara staf di rumah sakit umum di Suriah. Tujuan dari penelitian ini adalah untuk mengidentifikasi tingkat kepatuhan dengan protokol kebersihan tangan oleh tenaga kesehatan termasuk pengetahuan, sikap dan praktik mereka. Rerata tingkat kepatuhan kebersihan tangan yang diamati secara keseluruhan adalah 45,7% (95% CI 37,1-54,3). Perawat diamati memiliki kepatuhan yang lebih baik daripada dokter. Staf yang diamati tampak lebih peduli melakukan kebersihan tangan setelah kontak dengan pasien dibanding sebelum kontak (rerata sebelum= 32,0%, rerata setelah= 51,2%, p< 0,05). Hasil kuesioner menunjukkan bahwa ada korelasi yang signifikan antara pengetahuan, sikap, dan fasilitas, namun disisi lain kepatuhan dilaporkan buruk. Analisis multivariat menunjukkan bahwa kepatuhan yang buruk secara statistik terkait secara signifikan dengan jenis kelamin laki-laki (63,5%), staf tidak terlatih (58,5%) dan tidak tersedianya tempat cuci tangan (60,4%). Abstract

Model kepemimpinan kepala ruangan dapat meningkatkan kinerja perawat pelaksana dalam memberikan asuhan keperawatan. Tujuan penelitian ini adalah untuk mengidentifikasi hubungan antara model kepemimpinan kepala ruangan dengan kinerja perawat pelaksana. Metode penelitian menggunakan cross sectional, dengan melibatkan 141 responden di sebuah RS. Instrumen yang digunakan adalah instrumen model kepemimpinan yang pernah digunakan sebelumnya. Model kepemimpinan yang berhubungan dengan kinerja adalah servant, visioner, dan transaksional. Model kepemimpinan yang paling dominan berhubungan dengan kinerja adalah servant (p= 0,0001; α= 0,05; CI: 2,733–11,853; Odd Ratio: 5,691). Kepala ruangan yang dipersepsikan oleh perawat pelaksana menggunakan model kepemimpinan servant berpeluang meningkatkan kinerja perawat pelaksana sebesar 5 kali lebih tinggi dibandingkan dengan yang tidak dipersepsikan menggunakan model kepemimpinan servant. Hasil penelitian ini merekomendasikan kepada pimpinan Rumah sakit khususnya untuk kepala ruangan dapat menggunakan model kepemimpinan servant dalam peran dan fungsinya untuk meningkatkan kinerja perawat pelaksana. Kata kunci: Kinerja perawat pelaksana, Model kepemimpinan servant.  Abstract Head Nurse Leadership Model Can Improve The Performance of Nurses in Providing Nursing Care. The purpose of this study was to identify the correlation between the head nurse leadership model and the nurses’ performance. This study applied a cross-sectional method and involved 141 nurses at a Hospital. This study used the instruments was developed in the other studies. The results showed that the leadership model related to the performance was a servant, visionary, and transactional. The most dominant leadership model related to the performance was servant (p= 0.0001; α= 0.05, CI: 2.733 to 11.853; odds ratio: 5.691). Head nurses that were perceived by nurses as using the servant leadership model had an opportunity to improve the nurses’ performance five times higher than those who were not perceived as using the servant leadership model. It is recommended that the head of Hospitals, especially the head nurses, to apply the servant leadership model in their roles and functions to improve the nurses’ performance.  Keywords: Nurses’ performance, Servant Leadership model
