Abstract

Pituitary adenoma contributes to 15% of all intracranial neoplasm. It is usually following benign course and some of them are silent (asymptomatic clinically, but hormone-secreting). Silent adenoma usually found incidentally or when the patients show mass effect (neurological deficits). Many of histologically aggressive silent adenoma subtypes are associated with invasiveness, recurrence and progression to clinically functioning adenomas. Aggressive silent adenoma radiologically tends to invade in downward direction, invading bone, sinus cavernosus, parasellar region. The nature of aggressive silent adenoma subtypes is differing in nature compared to benign nature of pituitary adenoma and should be confirmed immunohistochemically to determine the prognosis and anticipate the risk of recurrence or progression. The case illustration show a real case of 46 years old female progressive headache and visual disturbance diagnosed with non-functional pituitary macroadenoma but positive for more than one immunochemistry biomarker (plurihormonal aggressive silent adenoma).

Abstract

Nasopharyngeal cancer (NPC) is endemic in Indonesia. The purpose of the review is to describe the risk factors, clinical symptoms, diagnostic procedures and staging, management and treatment options, and follow-up. Multi-disciplinary approach to the NPC not only for treatment, but also has been started from the diagnosis proccess. The multidisciplinary approach also plays role after treatment, especially for the follow-up and rehabilitation of post-treatment toxicity.Keywords: cancer, nasopharynx, radiotherapy.

Abstract

Third-degree atrioventricular (AV) block also termed complete heart block is present when there is complete absence of conduction between atria and ventricles. In adults the most common causes are acute myocardial infarction and age-related degeneration of conduction system. Incidence of conduction block is reported to be 25-30 % in the setting of acute myocardial infarction (AMI) and is 2 to 3 times as commonly associated with inferior than anterior infarction. Proper diagnosis involves noninvasive diagnostic tests (12 lead electrocardiogram (ECG), Holter ECG, or stress/exercise ECG).Keywords: third-degree AV block, complete av block, acute myocardial infarction

Abstract

Uterine cervical cancer is the most common cancer in Indonesia. The purpose of the review is to describe diagnosis and staging, the basic principles of radiotherapy, the role of radiotherapy in cervical cancer treatment, and radiotherapy side effects. The goal of radiotherapy in cervical cancer consists of definitive, adjuvant, and palliative. Effort to increase radiation response also develops by administered small dose of chemotherapy as a radiosensitizer.Keywords: cancer, cervix, radiotherapy

Abstract

Menyebarnya penyakit dengue di dunia, khususnya Indonesia adalah sebagai akibat dari pengaruh meningkatnya proporsi kasus dewasa. Infeksi virus dengue seringkali memberikan gejala klinis non-spesifik dan singkatnya waktu infeksi, karena itu uji diagnostik laboratorium cepat, dan identifikasi penanda dengue yang dapat segera dilakukan dan mudah diperoleh tetap menjadi prioritas. Deteksi dini pada pasien infeksi virus dengue secara substansial ditujukan untuk mengkonfirmasi dan mengendalikan manajemen klinis. WHO (Organisasi Kesehatan Dunia) dan beberapa penelitian menunjukkan bahwa deteksi antidengue NS-1 dengan/atau tanpa IgM sebagai faktor indicator untuk menyingkirkan infeksi virus lainnya. Risiko kejadian penyakit dengue yang parah dan dirawat di rumah sakit sering tidak  nampak pada pasien dewasa yang lebih tua, hal ini menunjukkan bahwa kelompok pasien ini harus mendapatkan manfaat dari deteksi dini. Pada pasien dewasa yang mempunyai gejala demam dan leukopenia harus dilakukan pemeriksaan terhadap virus dengue. Namun demikian, gejala klinis demam, trombositopenia, leukopenia, dan monositosis lebih konsisten ditemukan pada pasien terinfeksi virus dengue daripada kasus negatif.

Abstract

Astrositoma merupakan glioma tersering. Tumor ini bisa mengenai  dewasa dan anak-anak.World Health Organization (WHO) mengelompokkan astrositoma menjadi 4 grade berdasarkan karakteristik histologik. Astrositoma high grade terdiri atas astrositoma anaplastik (grade III) dan glioblastoma (grade IV).Data Departemen Patologi Anatomik Rumah Sakit Cipto Mangunkusumo (RSCM) tahun 2001-2010 melaporkan kejadian astrositoma sebanyak 179 kasus atau sekitar 20% dari seluruh tumor intrakranial, astrositoma anaplastik ditemukan sebanyak 12 kasus, dan  glioblastoma 42 kasus.Seperti pada tumor otak lain, astrositoma high grade mengakibatkan gejala dan tanda gangguan neurologik fokal dan umum. Pemeriksaan radiologik pilihan adalah dengan Magnetic Resonance Imaging (MRI). Astrositoma anaplastik memberikan gambaran hypointense pada T1 dan hyperintense pada T2 dengan efek massa yang bervariasi. Karakteristik glioblastoma pada MRI berupa lesi iregular menyangat kontras di sekeliling nekrosis sentral (ring enhancement) dan edema vasogenik luas di sekitar tumor.Astrositoma anaplastik secara histopatologik dicirikan dengan atipia inti, peningkatan selularitas, serta aktivitas proliferasi yang nyata. Glioblastoma secara histopatologik serupa dengan astrositoma anaplastik, disertai adanya proliferasi vaskular dan/atau nekrosis. Astrositoma anaplastik dan khususnya glioblastoma mempunyai variasi gambaran histologik yang beragam, antara lain varian small cell, granular cell, giant cell, dan gliosarcoma.

Abstract

Adiposity assessment is a part of the evaluation in coronary artery disease (CAD) prevention. Waist to height ratio (WHtR) is one of adiposity parameter in CAD management guidelines. This cross sectional study enrolled 30 subject with stable CAD undergone elective coronary angiography. Eightyfive subjects has abnormal WHtR (≥0,5). Median WHtR is 0,54 with 0,471-0,706 range, and Duke CAD prognostic index with median 37 (range 19-82). Analysis using spearman’s rank correlation show a significant moderate positive correlation (r=0,425; p=0,019) between WHtR and Duke CAD prognostic index. Waist to height ratio could be one of valuable indicator for coronary artery stenosis severity. We conclude that there is a relationship between WHtR and Duke CAD prognostic index.

Abstract

Disseminated Intravascular Coagulation (DIC) is an acquired pathological syndrome, featured by a hypercoagulable state, bleeding symptoms, and multiple organ failure. Based by these very distinct features, DIC is classified into four types namely Bleeding type, Organ failure type, Massive bleeding type, and Asymptomatic type. Diagnosing DIC is a challenge to the health practitioner, considering that DIC is a multifactorial syndrome, which always is a complication of some underlying diseases. To diagnose DIC, it is necessary to do a comprehensive evaluation of clinical symptoms and laboratory results. The necessary laboratory results include platelets count, fibrin degradation products (FDPs), fibrinogen, and PT-aPTT.

Abstract

Epidermis and dermis of the skin attach each other because of the role of a structure which is called dermal-epidermal junction. The dermal-epidermal junction can be divided into three distinct zones named hemidesmosome-keratin intermediate filament complex, basement membrane, and anchoring fibril. Each part of the distinct zone has significant components for the integrity of the skin. Bullous pemphigoid is an autoimmune disorder caused by autoantibody against a 180-kDa molecule now called Bullous Pemphigoid Antigen 2 (BPAG2) or type XVII collagen within basement membrane. To date, the patophysiology and the components of dermal-epidermal junction that induce the blister formation remain unclear. Epidermolysis bullosa acquisita is an autoimmune disease with sub-epidermal blistering associated with autoimmunity to the collagen within anchoring fibrils. The pathophysiology and etiology—again—remain unknown.

Abstract

Loss of consciousness (LOC) is a clinical presentation of patients with diabetes mellitus (DM) in emergency department which is caused by complications of the disease. Diabetic ketoacidosis, hyperosmolar hyperglycemic state, lactic acidosis, uremic encephalopathy, and hypoglycemia are the several main causes of LOC in patients with DM.  The comprehension of pathophysiology of LOC in patients with DM will be helpful in making accurate diagnosis. History taking, physical examination, and laboratory tests are essential in diagnosing that particular condition.
