Abstract

Background: Pulmonary TB is one of the most important health problems in Indonesia. The number of pulmonary TB patients ranked second after India. Vitamin D has a complex effect in the immune system. One of the effects of vitamin D is to stimulate the production of Cathelicidin which increases macrophage phagocytosis. This study aimed to analyze the effect of complementary vitamin D on sputum conversion in pulmonary tuberculosis patients.Subjects and Method: This study was a randomized controlled trial conducted at several health centers in Deli Serdang, North Sumatera, from November 2015 to June 2016. A total sample of 80 patients with positive acid-resistant bacilli (BTA) was selected for this study by consecutive sampling. The sample was allocated randomly into two groups: (1) TB drug group + 2.5 mg vitamin D; (2) TB drug group + placebo. The dependent variable was sputum conversion. The independent variable was complementary vitamin D. Pre and post vitamin D levels were examined by ELISA technique. Sputum conversion was examined by TB laboratory worker at the health center where patients received TB drug treatment. Difference in percent of sputum conversion between the two groups was tested by chi-square test. Difference in mean of sputum conversion time between the two groups was tested by t-test.Results: The mean serum vitamin D level in the intervention group (62.05±22.08) was higher than the control group (28.56±7.48) after the intervention, and it was statistically significant (p<0.001). After 56 days, all subjects in the vitamin D group experienced sputum conversion (100%) with a median sputum conversion time of 28 days, whereas 90% of subjects in the placebo group experienced sputum conversion with a median sputum conversion time of 48 days (p= 0.002).Conclusion: Adding vitamin D to anti-TB drug treatment effectively improves and accelerates sputum conversion of patients with tuberculosis acid-fast bacilli smear positive.Keywords: Vitamin D, sputum conversion, pulmonary tuberculosisCorrespondence: Nita Andriani Lubis. Abstract

Background: The incidence of HIV was found to be higher in uncircumcised men compared to circumcised men. Circumcision was known to protect and fight against the acquired HIV infection. HIV infection was classified into sexual transmitted infections since it is mostly transmitted sexually (95%). The risk was higher in unsafe sexual practices, such as having multiple sexual partners and not using protections like condoms.Subjects and methods: This was a descriptive study with cross-sectional design study. There were 340 patients who came to the special care center in General Hospital Center Haji Adam Malik Hospital and participated in this study. Data such as history taking and clinical examination were then collected.Result: The proportion of HIV patients with unsafe sexual practices was lower in circumcised men group, which was 141 patients (41.5%), compared to uncircumcised men group which was 199 patients (58.5%).Conclusion: The proportion of HIV infection in uncircumcised men with unsafe sexual practices was higher than in circumcised men.Keywords: HIV, Circumcision, Sexual practiceCorespondence: Juliyanti. Abstract

Background: Community-based case finding efforts of hyperthyroidism rather not to do, but constrained because accurate markers measurements using hormones.  TDQ as a screening in­strument which meets several criteria for diagnostic tests, provide more convenience for the patient, cheaper, easier and simpler. This study aimed to determine the precision and accuracy of the diagnostic test of hyperthyroidism screening in childbearing age women.Subjects and Method: The study was a diagnostic study with the cross-sectional design. It conducted in Prambanan, Sleman, Yogyakarta. The sample consisted of 112 randomly selected of childbearing age women. Clinical symptoms of hyperthyroidism were measured using the TDQ, thyroid status measured by thyroid stimulating hormone (TSH) and free thyroxin (FT4). TDQ diagnostic precision and accuracy were tested using the percent agreement (PA), Kappa coefficient, sensitivity (Se), specificity (Sp), LR (+), LR (-), Youden’s index, the area under the curve (AUC) in the receiver operating curve (ROC) and diagnostic odds ratio (DOR).Results: The best value in diagnostic precision is TDQ score with the cutoff point score of 40, with PA 81.2 % and Kappa coefficient 0.62. Similarly, the best accuracy value Se 90%, Sp 76%, LR(+) 3.75, LR(-) 0.13, Youden’s index 0.66, AUC 0.831 and DOR 28.8.Conclusion: The values and the optimal accuracy obtained from the best score of 40 and this cut off point of the score is sufficient to be used as a threshold determination of a diagnosis of hyperthyroidism by TDQ scores. Abstract

Background: One of the efforts to increase the usage of Long-term Contraceptive Methods is an effective counselling using decision-making tools. This counselling is very important to assist acceptors in making decisions and providing convenience to problem solving, behaviour changes or attitudes towards contraception. This study aimed to analyse the counselling effectiveness using decision-making tools in selecting methods of long-active reversible contraception.Subjects and Method: This was an analytic observational study with case-control design. This study was conducted at District of Kendal, Central Java, from May 18 to August 18, 2016. Population of this study was the entire contraceptive acceptors who were at district of Kendal. The samples were as much as 100 people, 35 contraceptive acceptors in case group and 65 in control group using quota sampling technique. The exogenous variables were counselling on Decision-Making Tools, self-efficacy, subjective norms, behaviour of women of reproductive age, ages of women of reproductive age, parity of women of reproductive age, levels of education of women of reproductive age. The endogenous variables were the usage of long-term contra­ceptive methods. The data were collected by a set of questionnaires and analyzed by path analysis.Results: Age≥35 years old (b=-2.19; CI 95%=-3.60 to -0.77; p=0.002), multiparity (b=-2.04; CI 95%=-3.99 to -0.09; p=0.040). Primary education level(b=-0.55; CI 95%=0.12 to 1.85; p= 0.359). There was a positive correlation between counselling and behaviour (b=0.98; CI 95%=0.12 to 1.85; p=0.025). There was a correlation between counselling and efficacy (b=1.56; CI 95%=0.62 to 2.50; p= 0.001). There was a correlation between behaviour and participation in the long-term contraceptive methods (b=4.02; CI 95%=1.71 to 6.34;p=0.001). There was a correlation between self-efficacy and participation in the long-term contraceptive methods (b=3.23; CI 95%=0.71 to 5.75; p=0.012). There was a correlation between subjective norms and participation in the long-term contraceptive methods (b=3.25; CI 95%=0.92 to 5.59; p=0.006).Conclusion: Counselling using decision-making tools influences behaviour and self-efficacy in using the long-term contraceptive methods indirectly. Abstract

Background: Type 2 Diabetes Mellitus (Type 2 DM) is one of the major causes of morbidity and mor­tality worldwide. Morbidity and mortality due to diabetes are associated with the development of various microvascular complications (retinopathy, nephropathy, and neuropathy) and macro­vascular complications (coronary heart disease, stroke, and peripheral vascular disease). This study aimed to examine the determinants of biopsychosocial macro and microvascular complications in pa­tients with type 2 DM at Dr. Moewardi Surakarta.Subjects and Method: This was an analytic observational with a case-control design. The study was conducted at Dr. Moewardi, Surakarta, Central Java, from January to March 2018. A sample of 75 Type 2 DM cases with complication and 75 Type 2 DM cases without complication was selected for this study by fixed disease sampling. The dependent variables were macro and microvascular com­plications. The independent variables were body mass index (BMI), blood pressure, quality of life, depression, duration of disease, physical activity, diet, medication adherence, health financing, and routine blood sample. Clinical data were taken from medical record. The data of other varia­bles were collected by a set of pre-tested questionnaire. Logistic regression was employed for data ana­lysis.Results: The risk of macro and microvascular complications in type 2 DM patients increased with high BMI (OR= 22.00; 95% CI= 7.50 to 64.52, p<0.001), poor quality of life (OR= 7.06; CI 95%= 3.51 to 14.16, p<0.001), depression (OR= 3.30; 95% CI= 1.39 to 7.79, p= 0.007), longer duration of disease (OR= 9.71; 95% CI= 2.79 to 33.71, p<0.001), lack of physical activity (OR= 8.16; 95% CI= 1.23 to 53.93, p=0.029), poor diet (OR= 3.58; 95% CI= 1.39 to 9.18, p=0.008), poor medication adherence (OR= 4.54; 95% CI= 1.71 to 12.05, p=0.002), and irregular blood sample control (OR= 5.80; 95% CI= 1.97 to 14.65, p=0.001).Conclusion: The risk of macro and microvascular complications in patients with type 2 DM is de­ter­mined by BMI, quality of life, depression, duration of disease, physical activity, diet, medication adhe­rence, and blood sample control.Keywords: biopsychosocial, macrovascular, microvascular, complications, type 2 DMCorrespondence: Handoko. Abstract

Background: Language proficiency should be mastered by children before school admision. The process of language development was affected by cognitive and neurologic maturity. Language proficiency determine communication foundation, social and academic interaction. Children with retardation in language proficiency may encounter problem in the social and academic relationship. This study investigated the association between parental education, occupation, income, language activity, and language proficiency in children.Subjects and Method: This was an analytic observational study with cross sectional design. This study was conducted  in Gondangrejo sub-district, Karanganyar, Central Java. A total sample of 102 children aged 4 to 5 years were selected for this study. The dependent variable was language proficiency. The independent variables were parental education, occupation, income, and language activity. The data were collected by a set of questionnaire and analyzed by multiple logistic regression.Results: Parental education ≥ senior high school (OR= 2.95; 95% CI= 1.03 to 8.39; p= 0.043), employed parents (OR= 3.45; 95% CI= 1.27 to 9.39; p= 0.015), family income ≥ regional minimum wage (OR= 6.35; 95% CI= 2.02 to 19.93; p= 0.002), and intense language activity (OR= 4.32; 95% CI= 1.61 to 11.51; p= 0.003) were associated with better language proficiency.Conclusion: Parental education ≥ senior high school, employed parents, family income ≥ regional minimum wage, and intense language activity are associated with better language proficiency.Keywords: parental education, occupation, language activity, language proficiency, childrenCorrespondence: Hafidz Triantoro Aji Pratomo. Abstract

Background: Heavy physical exercise can reduce blood flow and metabolism in kidney that eventually release free radicals. The free radicals can form oxydative stress and damage renal tissue. Exogenous antioxydant administration is usually recommended to minimize the renal tissue damage. This study aimed to examined the effect of purple sweet potato (Ipomoiea batatas L.) extract on renal tissue damage  in mice  (mus musculus L.) after heavy physical exercise.Subjects and Method: This was a randomized controlled trial with post test only control design. The study subjects included twenty four male white mice with DD Webster strain. These mice were divided into six groups. After undergoing excessive swimming exercises that lasted forteen days, purple sweet potato extract was given to the experimental group. There were three experimental groups receiving three different doses of purple sweet potato. The mice kidney was taken as sample for microscopic examination to determine the extent of tissue damage. Difference in renal tissue damage was tested by Kruskal-Wallis.Results: Microscopic examination showed statistically significant difference in tissue damage both in right (p=0.001) and left (p=0.036) kidneys, between study groups. Abstract

Background: Aloxan tetrahyd micee causes free radicals and higher reactive oxidative species (ROS) production, being able to lead to reduce insulin secretion from pancreatic β cells and receptor sensitivity in insulin-receptor cells resulting in hyperglycemia conditions. Bean extract ethanol can reduce oxidative stress. The purpose of this study was to determine bean’s ethanol extract effectiveness in lowering blood sugar levels and increasing the activity of superoxide dismutase (SOD) enzyme.Subjects and Methods: This was a randomized controlled trial (RCT) study. The samples were mice (Micetus norvegicus) of wistar strains randomized in five groups: P1-negative control, positive P2-control, P-3 ethanol extract group of 200mg / kgBW, P4-group 400mg / kgBW, P5-group 600 mg / kgBW. Dependent variables are blood sugar (mg/ dL) and superoxide dismutase enzymes. The independent variable is the bean extract of ethanol. Differences in blood sugar levels and superoxide enzyme levels of intergroup dismutase were tested by Anova test, followed by post hoc test, to determine the effectiveness of bean extract ethanol.Results: This study showed a statistically significant decrease in blood glucose levels after ethanol extract of green beans 600 mg / kg BW from day 14 to day 28 (p <0.05). This study did not show a decrease in blood sugar levels after ethanol extract of green beans 200 mg / kgBW and 400 mg / kgBW from day 7 until day 28 (p> 0.05). Giving ethanol extract of green beans did not affect the activity of superoxide dismutase enzyme (p = 0.830).Conclusion: Giving ethanol extract of green beans 600 mg / kgBW effectively decrease blood sugar in male wistar mice (Micetus norvegicus) from day 14 to day 28. Abstract

Background:  Hemodialysis is one of renal replacement therapy for patients with chronic renal failure (CRF). The quality of hemodialysis is largely determined by the recommended dose of hemodialysis. Biological and psychological changes are often faced by patients undergoing hemodialysis, which can affect their quality of life. Family support is an important factor that serves as a support system for the patients to face the health problems. This study aimed to determine the association between hemodialysis adequacy, family support, and quality of life in chronic renal failure patients.Subjects and Method: This was an analitic observational study with cross sectional design.  It was conducted at Kasih Ibu Hospital,  Surakarta, Central Java. A total sample of  102 patients with chronic renal failure who underwent hemodialysis were selected for this study using random sampling. The dependent variable was quality of life. The independent variables were hemodialysis adequacy and family support. The quality of life was assessed using the WHOQoL questionnaire. The hemodialysis adequacy  was measured by Ureum Reduction Rate (URR) formula.  Family support was measured by family support questionnaire. The data were analyzed using Chi Square test, Mann Whitney test, and logistic regressionResults: Patients with chronic renal failure had better quality of life if they underwent adequate hemodialysis (OR= 5.34 95% CI= 2.20 to 12.98 p= 0.001) and received strong family support (OR= 7.74; 95% CI= 3.13 to 19.13  p= 0.001).Conclusion: Quality of life of the patients with chronic renal failure is determined by hemodialysis adequacy and family support.Keywords: chronic renal failure, hemodialysis, adequacy, family support, quality of lifeCorespondence: Leo Chandra Wisnu Pandu Winata. Abstract

Background: Hormonal contraceptive contains estrogen and progesterone hormones. These hormones are hypothesized to affect blood glucose level. This study aimed to determine the association between hormonal contraception and blood glucose level among childbearing aged women.Subjects and Method: This was an analytic observational study with cross-sectional design. The study subjects were childbearing aged women who used and did not use hormonal contraceptive in Sangkrah community health center, Surakarta. A total sample of 47 people consisting of 32 hormonal contraceptive users and 15 non users were selected for study by fixed exposure sampling. The data were collected by questionnaire, interview, anthropometric measurement, and direct measurement of blood glucose level. The data were analyzed using a multiple linear regression with SPSS for Windows.Results: Women of childbearing age who used hormonal contraceptive had an average blood glucose 26 mg / dL higher than the non-users of hormonal contraceptive (b = 26.18; 95% CI 15.03 to 37.33; p <0.001). These estimates had controlled for the effect of confounding variables (i.e. Body Mass Index and diabetes mellitus risk factors).Conclusion: There is a positive and statistically significant association between the use of hormonal contraceptive and blood glucose level among women of childbearing age.Keywords: hormonal contraception, blood glucose level.Correspondence: Brillian Ferika Sabania.
