Abstract

PCR–electrospray ionization mass spectrometry (PCR-ESI/MS), suatu peralatan baru yang mampu mendeteksi hampir semua mikroorganisme patogen, namun belum pernah digunakan di Indonesia. Tujuan penelitian ini untuk memperoleh pengalaman dengan PCR/ESI-MS dan panel food borne pathogen, mengetahui kelebihan dan kelemahan teknologi ini. Rancangan penelitian potong lintang, menggunakan spesimen dari poliklinik anak, dilakukan dari bulan juli 2012 hingga desember 2012, dan akhirnya 24 spesimen diperiksa dengan PCR/ESI-MS. Karakteristik diare terdiri dari 12 dengan lendir, 3 diare berdarah dan 20 hanya diare cair saja. Pada sebagian besar kasus dijumpai lebih dari satu bakteri. Bakteri E. coli FDA29 ditemukan pada kasus disertai demam dan sakit perut, namun tidak ada pada kasus tanpa kelainan tersebut. Jenis bakteri dengan PCR/ESI-MS berbeda dengan kultur. Metoda ini dapat mendeteksi bakteri dalam waktu 24 jam dan sangat bermanfaat pada wabah. Berdasarkan kemampuan metoda ini untuk mendeteksi beberapa bakteri dari satu spesimen, tampaknya biayanya tidak lebih mahal dibanding kultur. Metoda ini sangat bermanfaat untuk surveilans dalam hal menemukan kesamaaan antara penyebab penyakit dan pejamu. PCR–electrospray ionization mass spectrometry (PCR-ESI/MS), a new equipment that can detect almost all human patogenic microorganism, but never used before in Indonesia.The aim of this study to get experience with PCR/ESI-MS and panel food borne pathogen, to know the profile of bacteria that caused diarhea by PCR/ESI-MS, and know the advantage and handicapped of this equipment.Study design was descriptive cross sectional, using specimen from Child ambulatory clinic, was done from July 2012 until December 2012 and finally 24 was examined with PCR/ESI-MS. Clinical characteristic consist of fever 9 (37,5%), vomiting 17 (70.8%)and abdominal pain 11 (45.8%). Mayority (75%) got diarhea from 3-5 daily. Characteristic of diarhea, 12 cases with mucus, 3 cases bloody diarhea and 20 cases only liquid faeces. In most cases, we found multiple bacteria . Bacteria E. coli FDA29 was detected on cases with fever and abdominal pain but not found in cases without those manifestation. Type of bacteria in this study was different with cultur result. This method can detect bacteria in 24 hours, and very useful in outbreak, and if we calculate the cost and the ability of this metod to detect several bacteria from single specimen, it seems nearly the same. It seems that this method will be very useful for surveilans ie to find similarity between agent and host.Keywords: diarrhea, PCR/ESI-MS

Abstract

Abstrak : Tuberkulosis (TB) dan stunting masih menjadi masalah kesehatan di Indonesia. Tujuan penelitian adalah mengetahui hubungan stunting dengan kejadian TB pada anak usia 1-59 bulan. Penelitian ini menggunakan desain kasus-kontrol. Kasus adalah anak usia 1-59 bulan yang berobat di puskesmas yang didiagnosis TB oleh dokter menggunakan sistem skoring. Kontrol adalah anak usia 1-59 bulan yang berkunjung ke puskesmas yang sama dengan kasus, didiagnosis bukan TB. Pemilihan kontrol menggunakan teknik sampling acak sederhana. Balita dengan TB dan bukan TB terdistribusi yang hampir sama menurut jenis kelamin dan ventilasi rumah. Jika dibandingkan dengan balita gizi normal, balita gizi stunting mempunyai risiko yang lebih tinggi sakit TB. Balita pendek dan sangat pendek mempunyai risiko masing-masing 3,5 kali dan 9 kali sakit TB [adjusted odds ratio (OR = 3.54; P = 0,004 and and OR = 9.06; P = 0.001) respectively. Ditinjau dari segi imunisasi BCG, balita yang tidak diimunisasi dibandingkan yang diimunisasi BCG mempunyai risiko 4 kali sakit TB. Pada kontak serumah dengan pasien TB, balita yang mempunyai kontak dibandingkan tidak mempunyai kontak serumah dengan pasien TB berisiko hampir 12 kali sakit TB (OR = 11.96; P = 0.000). Sedangkan jika ditinjau dari usia balita, balita usia < 24 bulan dibandingkan balita usia > 24 bulan mempunyai risiko 2,8 kali sakit TB OR = 2.84; P = 0.011). Balita stunting, yang tidak diimunisasi, dan yang mempunyai kontak TB serumah TB mempunyai risiko lebih besar sakit TB. Abstract : Tuberculosis (TB) and stunting remain a health problem in Indonesia. The objective orf this study was to identify the relationship of stunting with the incidence of TB in children aged 1-59 months. This case-control study in district of West Bandung (West Java). Cases were children aged 1-59 months who visited at clinic health center diagnosed TB by a doctor using a scoring system. Controls were the same age who visited the same clinic with the case, not diagnosed TB. Balita dengan TB dan bukan TB terdistribusi yang hampir sama menurut jenis kelamin dan ventilasi rumah. Stunting toddler, not immunized, and had TB contact at home had higher risk to be TB.

Abstract

AbstrakLatarbelakang : Memulai terapi Antiretroviral (ARV) lebih awal berisiko menimbulkan interaksi Obat Anti TB (OAT) dengan ARV, efek samping obat, keracunan akibat obat, tantangan kepatuhan minum obat dan terjadinya Immune Reconstitution Inflammatory Syndrome (IRIS).Metode : Penelitian ini menggunakan design penelitian kohort restrospektif dengan follow-up selama satu setengah tahun. Penelitian dilaksanakan di Rumah Sakit Penyakit Infeksi (RSPI) Prof. Dr. Sulianti Saroso Tahun 2016. Populasi studi adalah pasien Ko-infeksi TB-HIV yang naive ART dan tercatat pada rekam medis periode Januari 2010 - November 2014. Data pasien diperoleh dari rekam medis pasien dengan kriteria inklusi sampel adalah pasien usia ≥15 tahun, mendapat OAT minimal 2 minggu sebelum ART dimulai, dan memiliki data hasil pemeriksaan CD4 sebanyak dua kali dengan total sampel sebanyak 164 orang.Hasil : Probabilias kumulatif kegagalan perbaikan CD4 pasien ko-infeksi TB-HIV sebesar 14,43%. Hazard rate kegagalan perbaikan CD4 pada pasien yang memulai terapi ARV 2-8 minggu setelah OAT dibandingkan dengan yang menunda terapi ARV 8 minggu setelah OAT masing-masing 767 per 10.000 orang tahun dan 447 per 10.000 orang tahun (p=0,266).Kesimpulan : Hazard rate kegagalan perbaikan CD4 pada pasien yang memulai terapi ARV 2-8 minggu setelah OAT lebih tinggi dibandingkan dengan hazard rate pada pasien yang menunda terapi ARV 8 minggu setelah OAT. AbstractBackground : Starting Antiretroviral Treatment (ART) earlier was assosiated to pharmacologic interactions, side effect, high pill burden, treatment interruption, and Immune Reconstitution Inflammatory Syndrome (IRIS).Methods : This study used cohort restrospective design with one and half year time to follow up. This study was conducted from May to June 2016 at Infectious Disease Hospital Sulianti Saroso. Study population were TB-HIV coinfected patients, noted as a naive ART patient in medical records from january2010-november 2014. A total 164 patients ≥ 15 years old, had ATT 2 weeks before ART and had minimum 2 CD4 sell count laboratorium test results.Result : The cumulative probability of CD4 response failure among TB-HIV co-infected patients was 14,43%. Hazard rate of CD4 response failure was 767 per 10.000 person year in early ART (2-8 weeks after OAT) versus 474 per 10.000 person year in delayed ART (8 weeks after OAT) (p=0,266).Conclusion : Hazard CD4 repair failure rate in patients who started ARV therapy 2-8 weeks after OAT higher than the hazard rate in patients who deferred antiretroviral therapy 8 weeks after OAT.

Abstract

Abstract :Microbiota present in human intestinal are diverse, and imbalance in composition of intestinal flora may cause diarrhea.This study aimed to obtain a profile of intestinal bacteria in children with and without diarrhea and assess their presence with incidence of diarrhea. An analitical descriptive with cross sectional design study was carried out. A stool specimen was collected from each children of 2-12 years old with and without diarrhea who lived in North Jakarta. DNA extraction was performed prior to detection of microbes using Polymerase Chain Ceaction/Electrospray Ionization-Mass Spectrometry.Eighty stool specimens consisted of 33 and 47 specimens from children with and without diarrhea were included in the study. Thirty single and 6 multiple matches were detected in 30 specimens of the diarrhea group; 28 single and 8 multiple matches were found in 34 specimens of the non-diarrhea.Escherechiacoli and Klebsiella pneumonia were predominant in both groups. Firmicutes, Proteobacteria and Bacteroidetes were deteced in the diarrhea group, while Actinobacteria, Proteobacteria and Verrucomicrobia were in the non-diarrhea. The relationship of incidence of diarrhea and the present of enteropathogens in the stool was not significant, however, there was a strong correlation of the risk of suffering diarrhea due to the presence of enteropathogens (OR = 0.724 with 95%, CI: 0.237-2.215).In conclusion, most bacteria detected in both groups were similar, nonetheless, Actinobacteria was present only in the non-diarrhea. The chance to have diarrhea was higher when enteropathogen was detected in the stool.

Abstract

Abstrak. Daya tahan seorang perawat dalam menghadapi tuntutan pekerjaan dan tanggung jawabnya dalam merawat pasien di rumah sakit sangatlah diperlukan, agar seorang perawat dapat menyeimbangkan antara kinerja, masalah sehari-hari, dan beban pekerjaan yang dihadapi hampir setiap hari di dalam kehidupannya. Daya tahan yang dimaksud adalah resiliensi. Begitu pentingnya resiliensi bagi seorang perawat, namun, kajian maupun penelitian mengenai resiliensi pada perawat di RSPI-SS belum ada. Begitu juga dengan faktor-faktor yang mempengaruhi resiliensi tersebut.Tulisan ini bertujuan untuk melihat gambaran tingkat resiliensi perawat dan prediktor resiliensi pada perawat di Rumah Sakit Sulianti Saroso pada semua ruangan dan bangsal yang langsung menangani pasien, khususnya pada pasien HIV AIDS. Populasi penelitian adalah para perawat di rumah sakit Prof. Dr. Sulianti Saroso, sedangkan sampel penelitian adalah perawat–perawat yang secara langsung merawat pasien. Teknik pengambilan sampel dilakukan secara random. Untuk mengetahui tingkat resiliensi, self efficacy, hope, dan coping serta keterkaitan antara variabel, dilakukan pengumpulan data dengan menggunakan instrument penelitian yakni skala resiliensi, skala self efficacy, skala hope, dan skala strategi coping yang disusun sendiri oleh peneliti dan tim. Hasil penelitian menunjukkan tingkat resiliensi pada perawat di RSPI-SS berada pada level sedang. self efficacy dan hope terbukti merupakan prediktor terhadap resiliensi pada perawat,

Abstract

Abstrak : Campak merupakan penyakit yang sangat dimungkinkan untuk dilakukan eradikasi, dengan melalui tahapan reduksi dan eliminasi. Saat ini Indonesia telah mencanangkan tahapan eliminasi campak dengan menerapkan Case Base Measles Surveilance (CBMS) untuk memonitor perkembangan kasusnya. Studi ini bertujuan untuk memberikan gambaran perkembangan penyakit campak khususnya di wilayah Kabupaten Karawang. Desain studi deskriptif dengan analisis deskriptif data Case Base Measles Surveilans Dinas Kesehatan Kabupaten Karawang Tahun 2014. Sebanyak 109 kasus yang tercatat dalam CBMS, 48 kasus (43,6%) berhasil dikonfirmasi laboratorium. dengan 24 kasus (48%) positif campak dan 10 kasus (31%) positif rubella. Insiden Komulatif kasus campak sebesar 10 case / 1.000.000 penduduk dengan angka discarded kasus sebesar 4 case /1.000.000 penduduk. Kasus campak positif terdistribusi pada kelompok umur 1-4 tahun (45,8%) dan umur 5-9 tahun (37,5%). Kasus rubella positif terdistribusi pada kelompok umur 5-9 tahun (85,7%). 70% kasus campak positif bergabung dalam kelompok nol dosis dan tidak tahu status imunisasinya. Kabupaten Karawang belum mencapai taraf status eliminasi campak oleh karena angka insidens kasus yang masih tinggi, dengan sebagian besar kasus campak positif tidak diimunisasi. Selain itu, kasus rubella juga menunjukkan proporsi yang tidak sedikit. Kinerja surveilans CBMS dinilai masih belum optimal, mengingat discarded kasus campak masih dibawah 5/1.000.000 penduduk. Peningkatan kinerja surveilans campak harus terus dilakukan dengan melakukan konfirmasi laboratorium kasus campak klinis sebanyak mungkin. Abstract: Measles eradication is possible to be done, by going through the stages of reduction and elimination. Currently, Indonesia has launched a measles elimination phase by applying the Case Base Measles Surveillance (CBMS). This study aims to provide an overview of the development of measles in Karawang District of Indonesia.Descriptive studies using CBMS Data of Karawang District Health Office in 2014. A total of 109 patients recorded CBMS, 48/109 patient (43.6%) confirmed by the laboratory, with 24/48 patient (48%) positive measles and 10/48 patient (31%) positive rubella, Case Fatality Rate (CFR) = 0%. Cumulative Incidence of measles cases = 10 per 1,000,000 population, number of discarded = 4 per 1.000.000 population.Distributed positive measles patient in the age group 1-4 years (45.8%) and the age of 5-9 years (37.5%). Distributed positive rubella cases in the age group 5-9 years (85.7%). 70% positive measles patients joined the zero-dose group and do not know their immunization status. Karawang district has not reached the level of measles elimination status, with the majority of positive measles cases were not immunized. Rubella cases also showed an increased proportion. CBMS surveillance performance is still considered optimal, considering discarded measles cases still below 5 / 1,000,000 population. Improved performance of measles surveillance should be carried out by the discovery of cases and laboratory confirmation of clinical measles cases as much as possible.

Abstract

Abstrak :Pengaruh perubahan iklim terhadap demam berdarah dengue (DBD) bersifat tidak langsung. Hal ini karena terdapat faktor perantara penyebab yaitu nyamuk Aedes Aegypti. Perkembangbiakan dan siklus hidup nyamuk Aedes Aegypti inilah yang dipengaruhi langsung oleh kondisi iklim. Kesesuaian iklim dengan lingkungan hidup nyamuk aedes Aegypti ditandai dengan temperatur hangat dan curah hujan tinggi seperti Indonesia. Tujuan dari penelitian ini yaitu untuk mengetahui proyeksi peluang DBD secara rata-rata untuk periode 2014-2038 berdasarkan proyeksi curah hujan dan temperatur. Metode statistik yang digunakan untuk mengetahui pengaruh iklim terhadap kesehatan (demam berdarah) antara lain statistik downscaling, analisis komponen utama, dan regresi logistik ordinal. Hasil analisis menunjukan bahwa curah hujan yang sesuai dengan demam berdarah berkisar 100-300 mm. Untuk curah hujan relatif tinggi 120 – 317 mm yang terjadi pada bulan Januari-Februari, ancaman paling kuat adalah bahaya banjir dan DBD. Untuk temperatur udara, proyeksi ke depan (2014-2038) berkisar antara 26 – 30 oC, kondisi ini masih optimal untuk perkembangan nyamuk Aedes Aegypti. Proyeksi peluang demam berdarah berdasarkan proyeksi curah hujan dan temperatur menunjukan wilayah Jakarta masih berpeluang tinggi sebagai wilayah katagori resiko tinggi demam berdarah dengan nilai peluang 0,74 – 0,99. Abstract : Influence of climate change to Dengue hermologic fever(DHF) has indirect characteristic. There are other factor as medium DHF case, that is aedes Aegypti mosquito. Life cycle and breeding of aedes Aegypti mosquito has direct contact with climate condition. Suitability between climate and life environment of aedes Aegypti mosquito marked with warm temperature and heavy rainfall like Indonesia. The purpose of this study to know projection of DHF average probability period 2013-2038 base on rainfall and temperature projection. Statistical method, such as downscalling statistic, principal component analysis and ordinal logistic regression was applied to know impact of climate on health (dengue cases). Result of analysis shows suitability between rainfall with DHF case is 100-300 mm. January-February has heavy rainfall 120 – 317mm, so needed attention more and more stressing about flood disaster and DHF case. In the period of 2014-2038, interval temperatur occurred between 26 – 30 oC. The interval temperature like this is optimal condition for Aedes Aegypti breeding. The result of probability projection shows that Jakarta is still the region for high risk DHF, with probability value 0,74 – 0,99.

Kehadiran kuman TB menyebabkan progresivitas kasus ko-infeksi TB-HIV bertambah buruk sehingga mengancam jiwa penderitanya. Penelitian ini bertujuan untuk mengetahui gambaran kesintasan satu tahun pasien ko-infeksi TB-HIV berdasarkan waktu awal pengobatan ARV.Metode : Penelitian ini menggunakan desain kohort retrospektif di RSPI Prof. Dr. Sulianti Saroso tahun 2013-2015. Sumber data yang digunakan berasal dari penelusuran pada register pra ARV dan ARV, Form TB 01, buku monitoring ARV, monitoring farmasi ARV, pelacakan ikhtisar ARV dan status rekam medis. Pengumpulan data melibatkan petugas Pokja HIV/AIDS dan dokter (validasi diagnosa dan kovariat) yang di blind atas hipotesis penelitian.Hasil : Probabilitas ketahanan hidup kumulatif satu tahun pasien ko-infeksi TB-HIV yang mendapatkan awal pengobatan ARV di RSPI Prof. Dr. Sulianti Saroso periode Januari 2011-Mei 2014 adalah 81,5%. Probabilitas ketahanan hidup pasien TB-HIV berdasarkan waktu awal menunjukan bahwa ketahanan hidup satu tahun pada pasien yang mendapatkan pengobatan ARV pada fase intensif adalah 89,1% dan pada pasien yang mendapatkan pengobatan ARV pada fase lanjut adalah 74,5%.Kesimpulan : Pasien ko-infeksi TB-HIV yang mendapatkan ARV pada fase intensif cenderung memiliki probalitas ketahanan hidup yang lebih besar di tahun pertama dibandingkan pasien ko-infeksi TB-HIV yang mendapatkan ARV pada fase lanjut. Abstract Background : Human Immunodeficiency Virus (HIV) is a type of retrovirus that infects the human immune system that causes Acquired Immunodefiency Syndrome (AIDS). The presence of TB germs cause progression of cases of co-infection of TB-HIV getting worse so threatening sufferers. This study aims to reveal the one-year survival rate of patients co-infected TB-HIV based on time start of antiretroviral treatment.Methods : This study used a retrospective cohort design in RSPI Prof. Dr. Sulianti Saroso years 2013-2015. The data used comes from searches on the register of pre ARV and ARV form, TB Form, the book ARV monitoring, monitoring of pharmaceutical drugs, ARV overview and status tracking of medical records. The data collection involves the officer HIV / AIDS and the doctor (validation diagnosis and covariates) were in blind on the research hypothesis. Results : The cumulative probability of survival for one year patients co-infected TB-HIV get antiretroviral treatment early in RSPI Prof. Dr. Sulianti Saroso period January 2011-May 2014 was 81.5%. The probability of survival for patients of TB-HIV based on the initial time showed that one-year survival in patients receiving antiretroviral treatment in the intensive phase was 89.1% and in patients receiving antiretroviral treatment in advanced phases was 74.5%.Conclusion : Co-infected TB-HIV patients get antiretroviral drugs in the intensive phase tend to have a probability of survival is greater in the first year compared to co-infection TB-HIV patients get antiretroviral drugs in the advanced phase

Abstract

Abstrak : Penelitian ini bertujuan untuk memperoleh karakteristik spondilitis tuberculosis di RSUP NTB selama tahun 2012. Penelitian ini dilakukan dengan desain penelitian observasional yang diambil dari data rekam medik dengan klasifikasi diagnosis menurut ICD 10 A18.10 (tuberculosis of the spine). Sepanjang tahun 2012 terdapat 19 kasus spondilitis tuberculosis (8 perempuan, 11 laki-laki). Rentang usia dimulai dari usia 9-60 tahun. Seluruh subyek memiliki penyakit/gejala penyerta: TB paru (4/19), anemia (2/19), gizi buruk (3/19), sepsis (1/19), meningoensefalitis (1/19), fraktur kompresi vertebra (8/19). Tindakan operasi laminektomi dilakukan pada 15 subyek dan dilanjutkan pemberian obat anti tuberculosis. Kasus didominasi oleh laki-laki. Seluruh kasus spondylitis TB di NTB memiliki penyakit penyerta dengan kasus tertinggi fraktur kompresi vertebra. Sebagian besar kasus (15/19) memerlukan tindakan laminektomi. Abstract: The objective of the study is to describe clinical characteristic in West Nusa Tenggara Province Referral Hospital in 2012. Observational design was established using hospital’s record with ICD 10 codes A18.10 (tuberculosis of the spine). During 2012 found 19 cases of spondylitis tuberculosis (8 female, 11 male). Age interval between 9-60 years old. The accompanying conditions are: lung TB (4/19), anemia (2/19), severe malnutrition (3/19), sepsis (1/19), meningoencephalitis (1/19), compression fracture of the vertebrae (8/19). Laminectomy was done to 15 subject and antituberculosis drugs. The majority cases are male, all cases having accompanying conditions with compression fracture of vertebrae as most frequent condition. Majority of cases are need laminectomy.
