Abstract

Background: The prevalence of smoking in Indonesia is so high in all walks of life that the government has made several policies to control it. One of them is the inclusion of health warnings and health information on tobacco product packaging which aims to prevent novice smokers from starting smoking, as well as guaranteeing the public's right to clear, correct and honest information, as well as choosing based on clearer (visual) information that they understand (informed decision). The purpose of this study was to determine the extent of active smokers' perceptions of pictorial warnings of the dangers of smoking on cigarette packs. Subjects and Methods: This study uses a qualitative approach and the Extended Parallel Process Model (EPPM) method. The research subjects consisted of active smokers who were found in Pondok Gede District, Bekasi City. Data collection techniques in this study were in-depth interviews. The research instrument used was the researcher himself who was directly involved in the research. Then the technique of checking the validity of the data uses source triangulation. Results: The results showed that the informants' perceptions of the pictorial warnings of the dangers of smoking on cigarette packs varied quite a lot. Not all informants feel afraid and horrified by the scary pictures on cigarette packs. These results affect their beliefs to be able to reduce or quit smoking. Those who feel afraid and spooky of the pictures on cigarette packs have more confidence to be able to stop smoking than those who don't feel scared or spooky. Conclusion: Based on the results of the analysis it can be concluded that the pictorial warnings on cigarette packs has not been able to reduce the desire to quit smoking among informants.

Abstract

Pembatasan Sosial Berskala Besar (PSBB) is a policy needed by Bogor City's people to prevent the spread of COVID-19. In its implementation, Bogor City conducted 5-stage of PSBB. This policy provides an overview to stakeholders regarding evaluation, Analysis of strengths, weaknesses, opportunities, and threats, as well as recommendations for PSBB policies in Bogor City. This study uses a Rapid Analysis approach with Media Content Analysis on various policies made by its Government regarding PSBB published from April 13 - June 25, 2020. Data analysis uses a policy triangle and a SWOT model. Policy actors issued 18 health, socio-economy, education, transportation, employment, and religion regulations. At the same time, the content consists of guidelines for implementing PSBB in various sectors such as public places/facilities, educational institutions, workplaces, health facilities, places of worship, transportation facilities, markets/shopping centers, punishment for PSBB violations, as well as content regarding the extension of the PSBB itself. The response of the Bogor City Government to the COVID-19 pandemic was excellent and fast. However, many violations have occurred in the implementation even though there has been punishment. Lack of policy control, low community compliance, and lack of infrastructure can interfere with the implementation of PSBB. There is a need for the cooperation of all parties so that PSBB can run effectively.

Abstract

The Cardiovascular disease (CVD) prevalence continues to increase. This study aims to identify the CVD spending and utilization in the Indonesia National Health Insurance (JKN) scheme 2017-2021 using the JKN claims classified by Disease Accounts analyzed descriptively. The CVD spending increased by $125,4 million in 2019 compared to 2017 and mostly for the hospital (91% in 2019) with the highest spending occurring in the age above 60 years. Moreover, the hospital inpatient spending was dominated during these five years. The highest portion of spending by severity level was at mild level, followed by moderate and severe levels. Health facilities’ capacities across the regions remain the issue in providing health services, especially for the CVD that require further treatment using advanced technology. The CVD cost continues to increase annually; fortunately, the JKN plays a key role as financial protection for participants. But, there was a decrease in the number of visits and spending on CVD during the COVID-19 pandemic because of lockdown policies and health systems shifting to provide capacity to overcome COVID-19. The decrease in the number of visits to health facilities raises the potential of being diagnosed later with more severe. As policy recommendations, strengthening primary care screening and hospital capacity across regions through implementing the Health System Transformation are expected to reduce the impact of these problems.

Abstract

This study aims to identify marketing or socialization strategies for hospitals that provide Cardiac Surgery Services in Indonesia. The findings of this study will contribute to optimizing the services available in the regions as well as contributing to the transformation of health services, particularly in cardiac surgery services. This study is observational descriptive research. Data were collected from Cardiothoracic and Vascular surgeons and hospital websites with cardiac surgery services. Through the marketing analysis approach of 7P, this study identifies problems and policy recommendations that can be alternative solutions. The price of the service is high, but most of the patients used BPJS, and it is found that the hospital rate is higher than BPJS claims. The hospital should provide the costing and financial analysis that generate the hospital rate to BPJS revise the BPJS's claims. It can only be done if hospitals from many regions produce similar rates or slight variations. Jabodetabek still dominates as the region (place) with the most cardiovascular surgery services in Indonesia, with 29 of 35 hospitals. The recommendations to respond to this issue include ensuring data on demand for cardiac surgery services from various regions and encouraging the government through specialist expert associations and legislation to provide cardiovascular surgery services in various regions. Regarding the specialist (people), the government must provide scholarships for doctors to take BTKV specialists, especially those who can perform pediatric ones.

Abstract

The Lapor Vaksin COVID-19 Application was developed to support a vaccination coverage surveillance database sourced from community self-report. This application provides data on vaccination coverage and allows users to view updates to educational content about vaccines and visualization of COVID-19 vaccine data in real-time. This study aims to analyze  the perception among health worker about Lapor Vaksin COVID-19 application in the community. This study uses descriptive research with a case study design. This study was conducted in September 2021 through a virtual zoom meeting, in Ogan Ilir Regency, South Sumatra. The research sample was 42 health workers consisting of vaccine officers at the Health Office, RSUD and Puskesmas in Ogan Ilir Regency who were selected by purposive sampling. Data is presented descriptively. Based on the evaluation results of the application's use, more than half of the respondents were quite satisfied (52.4%) with the Lapor Vaksin COVID-19 application. This application can be used and developed as an alternative to the COVID-19 surveillance database.

Abstract

Ambulance is one of the hospital infrastructures that must meet service standards, security, and occupational safety and health in hospital operations. Therefore, it is the obligation of every hospital to ensure the standard of ambulance service. In the DKI Jakarta area, every ambulance, including hospital ambulances, must meet the standards in accordance with regulations and have an ambulance operating license. Based on data until the end of 2021, there were 82 (34.9%) hospital ambulances in Jakarta that carried out the recommendation process for an ambulance operation license from a total of 235 hospital ambulances, meaning that most hospital ambulances in Jakarta did not meet service standards. DKI Jakarta Governor Regulation number 120 of 2016 concerning Ambulance Services and Hearse is a reference for hospitals in Jakarta in order to meet ambulance service standards. In conclusion, there are 3 (three) standards that must be met in standardizing hospital ambulance services in Jakarta, namely standardization of vehicle units, standardization of equipment and standardization of human resources.

Abstract

To regulate the existence of Drinking Water Depots (DAM) in the city of Pariaman, the Pariaman City government issued Regional Regulation (Perda) Kota Pariaman No. 9 of 2015 concerning Business Permits for Drinking Water Depots. However, many depot owners do not implement these regulations. The purpose of this study was to analyze the implementation of Pariaman City Regional Regulation No. 9 of 2015 concerning Drinking Water Depot Business Permits. This research is a type of qualitative research. Data collection was carried out through in-depth interviews with informants involved in the implementation of Pariaman City Regional Regulation No. 9 of 2015 and focus group discussions (FGD) on drinking water depot owners. Selection of informants based on the principles of suitability and adequacy. The variables based on the framework of Edward III include communication, resources, disposition, and bureaucratic structure. The results of the research show that the implementation of the policy has not been implemented because the communication has not been optimal. The available resources and facilities are quite adequate, for sub-budget variables it has not gone well. Coordination has not been carried out properly, the sanctions set forth in the Regional Regulations have also not been realized optimally. Disposition related to the government's commitment to implementing policies has not been implemented. It is necessary to monitor the implementation of regional regulations and make derivative regulations related to supervision of drinking water depots, so that depots can carry out routine inspections according to what is stated in the regional regulations and make it easier for the Health Service to carry out supervision

Abstract

The study focused on the feminization of Sri Lankan generalist doctors in all major and selected minor specialties between 2000 and 2020. The study analyzed all relevant documents by exploring Sri Lanka's male-to-female doctor and specialist ratio over 20 years. This was determined by analyzing state medical faculty records from 1990 to 2020. For better clarity, the researchers analyzed the gender ratio changes in every five-year block from 1990 to 2020. Focus groups filled PGIM's gender data fields. Five-year medical students surged from 43% to 63% between 1990 and 2020. Linear trend analysis predicts 68% of female medical students by 2025 and 72% by 2030. This study simulated generalist doctors with medical students. Until recently, men dominated medicine, surgery, obstetrics, and pediatrics. Female specialists have increased in all fields over the past 20 years. The feminization of medical doctors challenges national human resource policies and health sector reforms. They emphasize gender-sensitive health workforce planning that considers a country's economic development and healthcare system. Sri Lankan health policymakers should consider gender dynamics in national health HR planning for the next decade.

Abstract

The purpose of this study was to describe the prevalence of stunting in children under five and the percentage of proper sanitation, as well as recommendations for priority areas for stunting control interventions in the Province of the Special Capital Region of Jakarta in 2021. This study used a descriptive approach to present an overview of the percentage of sanitation and prevalence of stunting in children under five by sub-district in 38 districts in DKI Jakarta. Data is displayed as a map to show distribution areas based on high, medium, and low categories. An overlay was made between the distribution map of the percentage of sanitation and the prevalence of stunting to determine the areas with the highest risk. Analysis was carried out on the area's characteristics obtained from the spatial mapping results to determine the recommended intervention program policies. Based on the results of the overlay on the distribution of stunting prevalence and sanitation percentage maps, the recommended areas for priority intervention are Tambora, Sawah Besar, Johar Baru, Senen, Menteng, Jatinegara, Tebet, and Mampang Prapatan sub-districts. The recommended program to be carried out is sensitive interventions consisting of improving sanitation facilities and increasing knowledge and perceptions about sanitation. Improving sanitation facilities means increasing access to gooseneck latrines with septic tanks and access to clean water. Increasing knowledge and perception of the importance of sanitation to prevent stunting is carried out intensively and routinely through community activities and optimizing the role of community leaders.

Abstract

The National Health System in Indonesia is developing and being discussed more seriously after entering the COVID-19 pandemic. With a national health system and several existing tools, Indonesia needed more time to be ready to face the challenges of that time. Upstream health policies should be optimized to mobilize resources and increase the nation's efforts to build a national resilience system. This paper was built using selected policy documents up-to-date and reliable literature. By reviewing empirical evidence, the author believes and shows the role of strengthening health policies in building a robust national health resilience system. Indonesia has several policies that have the potential to become a sub-system for national health resilience. The health resilience system is formed from the health system and health resilience components, which impact creating a Healthy Indonesia. Health system resilience can be seen from stable funding, good governance, flowing and integrated information systems, risk adaptation, adequate health workforce capacity, logistics, and robust health efforts. The interaction between the health system, health resilience, and the health resilience system formed is within the umbrella of health policy, the completeness of resilience policy, and the highest public health institutions. Strengthening the role of health policy in the formation of a health resilience system in a country is one proof of health policy reform. Health policy is no longer seen as the upstream side of health programs or efforts but as an essential side in functioning a system to create a healthy country. Reforming the national health system should followed by institutional reform of The Ministry of Health to become the Ministry of Public Health, and health policy reform by strengthening health resilience.
