Abstract

Background: The number of larva free index in Dwikora in 2016 was >43.5% which is still far below national standard, i.e., 95% with some dengue hemorrhagic fever (DHF) case obtained over the last two years was 16 cases. The study objective was to determine the relationship of community behavior in controlling the mosquito habitat with the presence of Aedes aegypti larvae in Dwikora. Methods: This research was an analytical survey with a cross-sectional design. The population in the present study was the patriarch or head of household residing at Dwikora sub-district, the total number of households was 452. By employing purposive sampling technique, 82 households were selected as samples. The data was collected by using the observation sheet and standardized questionnaire. Univariate and bivariate tests were performed for data analysis. Results: The results showed that there were 44 households which had less practice in mosquito habitat control, 48 households had a positive attitude, and 38 households had good knowledge. Bivariate analysis using chi-square test showed that there was a significant correlation between the household practice (p-value = 0.01) with the presence of Aedes aegypti larvae in Dwikora. Conclusion: It may indicate that the communities in Dwikora carry out less control over the existence of Aedes aegypti larvae at their houses. Therefore, it is suggested for the households to pay more attention to house cleanliness, to always use a closed bucket, apply abate to stagnant water and eradicate mosquito habitats periodically to avoid the occurrence of transmission of dengue hemorrhagic fever (DHF)

Abstract

Background: Nowadays, female workers are vulnerable to workplace hazards. One of the groups who are highly vulnerable to the exposure of workplace hazards is female waste pickers. This study focused on breathing problems in female waste collectors. The present study aimed to determine the effect of work behavior and work environment on respiratory complaints in female waste pickers at Terjun Landfill in Medan City. Methods: It was a cross sectional study in which the independent variables were work behavior and work environment, while the dependent variable was health complaints, respiratory complaints in particular. Work behavior was measured by collecting the data of protective mask use, while the data of work environment was collected by exploring the concentration level of CH4 and H2S at Terjun Landfill in Medan City. A total of 53 female waste pickers were participated as research sample. The data was analyzed by performing multiple linear regression calculation. Conclusion: It was obtained that there was an effect of work behavior (p=0.047) and concentration level of CH4 (p= 0.000) on respiratory complaints in female waste pickers at Terjun Landfill. However, it was found that H­2S concentration level did not have an effect on respiratory complaints in female waste pickers at Terjun Landfill in Medan City. This study suggests that Sanitary and Park Office of Medan City can work together with related stakeholders to cultivate methane emissions (C­­H4) to become energy source to reduce air pollution at waste landfill, whereas to female waste collectors, this study suggests that they can regularly use protective mask to minimize the risk of hazardous gas at workplace.

Abstract

The result of a preliminary survey by the writer found that Tanjung Pura Hospital not yet implemented and a team for Occupational Health and Safety. Whereas, that was stated in ministry regulation at number PERMENKES 66/2016 and has to prove it with the absence of report due to occupational health and safety cases in Tanjung Pura Hospital. Based on the background, this study aimed to analyze the management wisdom on occupational health and safety. This study used the qualitative method with case study design. To collect the data the writer used interviews with several informants, that is Director of Tanjung Pura Hospital, Head of Division, Quality Team, and Occupational Health and Safety’s Team. The result of the study found that the regulation of occupational health and safety in Tanjung Pura Hospital not formed yet. In another hand, the planning and also the implementation process of monitoring and evaluation of occupational health and safety not worked. In conclusion, all the wisdom of occupational health and safety at Tanjung Pura Hospital not operated, and this study should be the basis to implement the ministry regulation PERMENKES 66/2016.

Abstract

Background: The increasing population will influence the number of garbage. A pile of garbageincreases as many as 4% in every year, and in Sumatra has 8.7 billion ton a pile of garbage. The way to minimize the garbages by the managing or recycling. Aim: To comprehend the student in garbage managingat Pematangsiantar. Method: This study used cross-sectional with 228 students as a sample. Result: 53.3% of the respondent has a high behavioral intentionin garbage-managing with used the garbage in another time, while as many as 52.2% has a behavioral control, and subjective norm by the respondent be a high result of three-point in the analysis, represented with 72.4%. The effect of bivariate analysis found that behavior, subjective norm, and behavioral controlrelatedtothe behavior of student intention.Conclusion: This study showed that there is a correlation between behavior, subjective norm, andbehavioral control with behavior intention. The wishes ofthe student to caring formanaging or recycling because the garbage has many effectson the environment.

Abstract

According to Riskesdas data in 2013, the prevalence of URI (Upper Respiratory Tract Infection) in Indonesia was 25.0% and the period prevalence of URI in North Sumatra was 19.9%. Data from the Tanjung Balai Health Office reported that 902 people were suffering from URI in 2016. Based on the monthly report of P2 URI, most found in the Major Umar Damanik Health Center, namely 1,404 cases and 518 cases of URI found in infants. Important factors that can increase the occurrence of URI are the physical condition of the house (natural lighting, ventilation area, humidity in the house, air pollution in the house, and occupancy density). The results of Riskesdas in 2013 stated that according to occupancy density, there were 13.4% of houses with a residential density of more than or equal to 8m2 per person (solid). In the Major Umar Damanik Health Center, Tanjung Balai City, February-April 2018, the incidence of URI in infants was 518 cases (43.16%). So it is important to study as an effort to analyze the quality of home sanitation with the incidence of URI in infants in the working area of ​​the Major Umar Damanik Health Center in Tanjung Balai City in 2018. The study was designed using a cross-sectional approach in 69 respondents. There is a relationship between occupancy density, ventilation area, air pollution, humidity, temperature, natural lighting, with the incidence of URI in infants in the Puskesmas Mayor Umar Damanik working area of ​​Tanjung Balai City in 2018. Air pollution in the home is the most dominant factor affecting the incidence of URI in infants in the work area of ​​the Public Health Center of Major Umar Damanik, Tanjung Balai City in 2018. Abstract

Family Planning is an effort to control the childbirth in order to realize a quality family. Implants are a means of contraception that the couples of productive age (CPA) are not interested in. Based on data obtained at Medan Sunggal Health Center, implant is the lowest after IUD. This is because there are still many CPA who are afraid to use implants because of their lack of knowledge. The Purpose of this study was to determine the effect of counseling on thee behavior and motivation of CPA about contraceptive implants at Medan Sunggal Health Center. The sampling technique was determined by Accidental Sampling method with 66 respondents. Research type was quasi-experiment with one group pretest posttest design. The results of research obtained by using T test were the value of knowledge, attitude and motivation (P value <0.001). It can be concluded that there is influence of counseling to behavior and motivation of CPA on contraceptive implants.

Abstract

The quality of hospital care service is influenced by various factors, such as leadership style and organization culture those are developed and implemented by all nurses in a hospital. This study aims to analyze the relationship between leadership style and organization culture with the quality of care services at Tiara Hospital in Pematangsiantar. The subjects of the study were 45 nurses at Tiara Hospital as samples. Data collection was conducted using a questionnaire and analyzed statistically using data analysis that used bivariate statistical tests, named Chi Square and continued with multivariate statistical tests. Organization culture carried out at Tiara Hospital in medical services is developed based on the values ​​and norms adopted by the hospital. The results of statistical analysis showed the variables of organization culture of innovation and risk taking, detailed attention, results orientation, human orientation, team organization, aggressiveness, stability related to the quality of hospital care services. Variable participatory leadership style is the most dominant factor related to the quality of service at Tiara Hospital (OR = 13,333) meaning that the possibility (opportunity) of nurses is able to provide good quality care services 13 times if the participatory leadership style is well done compared to the nurse which stated that participatory leadership style is not good at Tiara Hospital. It is suggested to the head of Tiara Hospital in Pematangsiantar, especially the head of the room needs to apply a leadership style that leads to participatif style. The leadership applied by the head of the room is always adjusted to the needs of the organization and pay attention to the situation and conditions that exist. Nurses at the Tiara Hospital in Pematangsiantar are expected to improve the understanding of the leadership style that has been done by the head of the room and try to implement the organization culture into a good work culture

Abstract

Healthcare-associatedinfections (HAIs) are one of the causes of the increase in morbidity rate and mortality rate in a hospital. This infection becomes a new health problem. HAIs in a hospital is caused by bacteria, virus, fungi, or parasite which come from patients’ bodies and from exogenous factors, that is,from environment and polluted medical equipment. The objective of the research was to find out some factors which influenced the incidence of HAIs at RSUD dr. Pirngadi, Medan, in 2018. The research used observational analytic with case-control design. The samples were 69 patients with HAIs at RSUD dr. Pirngadi, Medan, in 2017. The result of multivariate analysis with logistic regression statistic test showed that some factors which influenced the incidence of HAIs were the use of antibiotics (p=0.032 < 0.05) and the number of treatment days (p=0.014 < 0.05). The factor which had the most dominant influence was the number of treatment days at OR-value = 5.516. The conclusion was that there as the influence of the use of antibiotics and the number of treatment days onthe incidence of HAIs. It is recommended that the hospital management pay attention to all aspects of infection in the hospital and prevent from infection which can endanger patients

Abstract

Stunting is a state of the nutritional status of a person based on the z-score height of age which lies in z-score <-2 SD. Stunting is a state of undernutrition that continues over time and happens in the long term. Prevalence of stunting in Subulussalam among children under 5 years old was is 64,91% which 48% occured in Simpang Kiri sub-district. This study was aimed to analyze whether micronutrient intake and exclusive breastfeeding were risk factors for stunting. The design was case-control unmatched, the subjects were children between 12-36 months with 53 stunted and 55 normal. The variables were the frequency of intake mineral source (Ca, Fe, and Zn), the frequency of intake vitamin A source, and exclusive breastfeeding. Data were analyzed using chi-square test and odds ratio.The results showed that frequency of intake Ca source (OR=4,11, 95%CI:1,83-9,25), frequency of intake Fe source (OR=4,31, 95%CI:1,70-10,9), frequency of intake Zn source (OR=7,46, 95%CI: 1,58-35,2), frequency of intake vitamin A source (OR=2,51, 95%CI: 1,15-5,49) and exclusive breastfeeding (OR=5,38, 95%CI:2,28-12,7) were the risk factor against stunting. Abstract

Background:Indonesia ranks fifth in the highest number of cigarette consuming countries and ranks third in the country with the highest number of smokers in the world. There is an increase in the number of smokers from age group under 15 years and the highest increase is in the age group 10-14 years. Objectives: This study aimed to determine the factors associated with smoking behavior among male students in SMP Negeri 1 Simpang Empat Asahan in 2018.Metode:This research is an analytic survey with cross sectional study design. Number of sample 128 respondents using total sampling. Data analysis was done using univariate analysis, bivariate with chi square test and multivariate with multiple logistic regression. Results: The result of the research showed that there was the correlation of knowledge (p=0.031<α=0.05), attitude(p=0.001<α=0.05), affordability of access (p=0.015<α=0.05), ad exposure (p=0.00<α= 0.05), parents’role (p=0.002<α0.050), peer role (p=0.001<α=0.05), school’s role (p=0.033<α=0.05). The variable which the most dominant correlation was ad exposure (p=0.003;OR=11.806 95% CI 2.345-47.541). Conclusion:It can be concluded that the ad exposure highly influences the development of smoking behavior in adolescents on the grounds they smoke to be more acceptable to people especially peers and to look more manly.
