Abstract

Introduction: Chronic obstructive pulmonary disease (COPD) is the leading cause of morbidity and mortality worldwide. Cigarette smoke and noxious agent result in oxidative stress and activate release of inflammatory mediators such as Interleukin-8 (IL-8). Quercetin is a flavonoid compound containing anti-inflammatory effects which can be used as an adjuvant therapy in stable COPD. Objective: To analyze the effect of quercetin on serum IL-8 levels, % VEP1, and CAT score of stable COPD patients. Methods: Experimental clinical trial with pre-test and pasca-test design was performed in 30 patients with stable COPD in Dr. Moewardi Surakarta between December 2017 and January 2018. The samples taken by using purposive sampling were divided into two groups treatment groups received standard therapy and quercetin 500mg/day for 28 days and control groups only received standard therapy. The decrease in inflammation was measured by serum IL-8 examination, improvement of obstruction measured by %FEV1 and clinical improvement measured by CAT score. Results: IL-8 serum level was significantly lower in treatment group than of in control group (p=0,001). The percentage of FEV1 was insignificant different between the two group (p=0,236). However CAT score was significantly lower in treatment group compared to that of in control group (p=0,001) Conclusions: Quercetin can decrease IL-8 serum level and decrease CAT score when given in combination with standard therapy for COPD patients. Abstract

Hypercoagulation with a clinical manifestation of Venous Thromboembolism (VTE) is a frequent complication of malignancy whose incidence has increased markedly these recent years. The occurrence of VTE is likely to be triggered by multiple pathways dominated by tissue factor (TF). The VTE itself can directly lead to patient mortality and becoming the cause of death in cancer patients. Furthermore, emerging data suggest that activation of coagulation in malignancy is integrally linked with tumor biology, host associated and cancer treatment particularly with anti-angiogenesis. The clinical hypercoagulable state is linked with prognosis in patients with lung cancer, including patients receiving systemic chemotherapy. (J Respir Indo 2019; 39(2): 130-9)

Abstract

Backgrounds: Tuberculosis controlling programme has become more complex with MDR-TB problem. Interleukin 10 (IL-10) 1082G/A gene polimorphism correlates with IL-10 secretion as anti-inflammatory cytokine which plays important role in pathogenesis of MDR-TB infection. The management of MDR-TB which used aminoglycosides could cause nephrotoxic effect to the patients. The protective role of IL-10 from IL-10 1082 G/A genotype to nephrotoxicity due to kanamycin still becomes a prolem nowadays. Methods: This study was a retorspective cohort study of MDR-TB patients who underwent treatment in Dr. Moewardi Hospital in 2011-2015. Results: Subjects of the study were 89 MDR-TB patients with IL-10 1082 G/A genotype polimorphism. The proportions of IL-10 1082 G/A genotype were AA genotype of 13.48%, GG of 4.49%, and GA of 82.2%. Statistic test showed that the onset of nephrotoxicity in GG genotype was faster than GA and AA genotype Conclusions: Interleukin 10 1082 G/A gene polymorphism had no significant correlation with nephrotoxicity onset in MDR-TB patients treated with kanamycin in Dr. Moewardi hospital. Abstract

Background: The treatment of drug resistance tuberculosis needs second line injection antituberculosis drug that associated with irreversible ototoxic. The aim of this study is to know the prevalence of ototoxicity in tuberculosis drug resistance patients and the contributing factors. Methods: This is a cross sectional study among drug resistance TB patients who receive kanamysin or capreomycin as a part of drug regimen during intensive phase in January to September 2017 at Persahabatan hospital. Ototoxic defined according to American Speech Language and Hearing Association (ASHA) 1994 criteria by comparing baseline audiometric examination before treatment with current result. Results: Seventy-two patients were included in this study. The prevalence of ototoxicity was found in 34 patients (47,2%). Ototoxic found in 5 subjects (14,7%) during the first month of treatment and 19 subjects (56%) without hearing disturbance complain. Ototoxic in kanamisin group (47,9%) is more frequent compared with capreomisin (36,8%). Ototoxicity was associated with age, the risk increases 5% every 1 year older p=0,029 aOR:1,050 IK95% (1,005-1,096). The prevalences of ototoxicity are higher in diabetes and increasing serum creatinin patients but statistically not significance. Sex, body mass index, the history of using injectable antiTB drug, HIV status and total dosis were not associated with ototoxicity. Conclusion: Ototoxicity is common in intensive phase of drug resistance tuberculosis treatment. Further study needed to determine the association of contributing factors. Abstract

Background: Some species of the Mycobacterium tuberculosis complex (MTBC) which can cause tuberculosis, particularly M. tuberculosis and M. bovis, may have different virulence property and therefore associated with various clinical severity in tuberculosis (TB) patients. The aim of this study was to determine the differences in clinical and demographic characteristics between M. tuberculosis and M. bovis infection among Indonesian TB patients. Methods: Thirty-one new and active TB patients were confirmed to have acid fast bacilli (AFB) sputum positive and/or Xpert MTB/RIF positive for M. tuberculosis from Dr. Soewandhie Hospital, Surabaya, Indonesia. Interviews were conducted to record the clinical and demographics required. The MTBC were isolated from bronchoalveolar lavage fluid (BALF) and determined by primer-specific PCR targeting TbD1 and RD9 region gene. The degree of lung tissue damage was classified using NICE Scoring System. Results: The MTBC were detected in all patients on whom 19 and 12 isolates were classified as M. tuberculosis and M. bovis respectively. Abstract

Background: Respiratory tract infections are the leading cause of acute exacerbation of COPD (AECOPD). The aim of this study was to investigate the relationship between bacterial pattern and the degree of airflow limitation (FEV1) in patients with acute exacerbation of COPD at Adam Malik General Hospital and Pirngadi Hospital, also the sensitive antibiotics according to the susceptibility test. Methods: This was a cross sectional study of all patients admitted to Adam Malik and Pirngadi Hospital with AECOPD from September 2015 until September 2016. In all 45 subjects who fulfilled the inclusion criteria, the spirometry was examination undergone to evaluate the degree of severity of COPD according to GOLD 2017. In each sample sputum expectoration, the gram smear was made. Barttlet criteria was used to calculate the amount of epithelial cells and PMN. The sample was then grown on blood agar medium. Bacterial susceptibility test to antibiotics was conducted using VITEC 2 methods. Results: From 45 patients, 30 patients (66,7%) had positive sputum cultures. The most frequently isolated pathogen was Streptococcus pneumonia (26,7%). The most sensitive antibiotic was Amikacin (100%). There were no correlation between the in degree of airflow limitation FEV1 and bacterial sputum culture result. There were no significant differences between gram positive or negative bacteria with the degree of airflow limitation. Conclusion: The incidence of bacterial infection based on positive sputum culture in AECOPD was about 66,7% Streptococcus pneumonia was the most common pathogen and Amikacin was the most sensitive antibiotic. There were no significant correlation between the degree of airflow limitation FEV1 with the positive or negative sputum culture and also with the result of gram staining. Abstract

Pneumoconiosis is disease that caused by deposition of dust in lung parenchymal with fibrosis was result of it’s reaction. Usually pneumoconiosis diagnose and classified by conventional chest x ray. The International Labour Organization (ILO) described standard system for classified radiograph abnormality of pneumoconiosis based on parenchymal and pleural abnormality. Parenchymal abnormality divided into 2 categories that is small opacities and large opacities. Description steps of chest X ray reading based on ILO classification was technical quality, parenchymal abnormality (shape and size, affected zone and profusion), pleural abnormality (localized, width, extent and severity of calcification) and other abnormality with recorded by symbols (J Respir Indo. 2019; 39: 272-9)

Abstract

Background: In tuberculosis (TB) endemic countries, the diagnosis delay in lung cancer is due to initially misdiagnosed as pulmonary tuberculosis. The major concern that rose since early diagnosis of lung cancer could improve survival by tumor resectability chance and chemo-radiotherapy modality options. This study objective was to find out the proportion of lung cancer diagnosis delay due initially to misdiagnosed as pulmonary TB. Method: The cross-sectional study was held in Persahabatan Hospital and the subjects were histopatologically proven lung cancer patients between September 2012 to February 2013 involving totally 100 patients. The diagnosis delay were determined as whether the patients had been diagnosed as pulmonary tuberculosis and received anti-tuberculosis treatment (ATT) more than one month since current symptoms onset. All patients were interviewed and all chest X-rays were documented. Results: Fourty one of 100 patients were diagnosed as pulmonary TB and 29 of 41 patients received ATT more than one month. It consisted of 21 men and 8 women with the mean age of 51.5 years old. The cytology and histopatological biopsy revealed 28 Non Small Cell Lung Cancer (NSCLC) cases, and One Small Cell Lung Cancer (SCLC) case with all case were in end stage condition (6 cases in stage III and 22 cases in stage IV). Pre-referral sputum Acid Fast Bacilli (AFB) was conducted in only 9 cases with all negative results. Mean duration of ATT taken was 4.5±0.4 months. The ATT were given by 13 general practitioners, 12 pulmonologists and 4 internists. Discussion: Similar radiological findings in highly incidence of pulmonary TB could cause a large number of diagnosis delay in lung cancer due to initially diagnosed as pulmonary tuberculosis. Without proper investigation based on International Standard of TB Care, starting ATT with inadequate evaluation leads to diagnosis delay and lung cancer progression. Abstract

Background: The levels of asthma control is an indicator of asthma management outcomes. A complex immunological mechanism influences it. We aimed to investigate the effect of Poly-Phytopharm on the asthma control tests (ACT) score, blood eosinophil levels, and miR-126 relative expressions in asthma patients. Methods: Quasi experimental method is used in 15 stable asthma patients who were not fully controlled at the pulmonary outpatient clinic of Dr. Saiful Anwar General Hospital Malang. Assessment of ACT score, blood eosinophil levels, and miR-126 relative expressions are carried out before and after supplementation of Poly-Phytopharm that consist of Nigella sativa 100 mg, Kleinhovia hospita 100 mg, Curcuma xanthorrhiza 75 mg, and Ophiocephalus striatus 100 mg extract, three times a day, two capsules respectively for 12 weeks. The ACT was scored by the investigator by direct questioning of patients, the blood eosinophil level was measured with blood analysis, and the relative expressions of miR-126 was detected with qPCR. Results: There were significant increase of ACT score (18.07±2.57 to 22.06±1.83, P=0.001), decrease in blood eosinophil levels (653.15±276.15 pg/mL to 460.66±202.04 pg/mL, P=0.038), and enhancement of serum miR-126 relative expressions (1.83±1.89 to 5.89±1.34, P=0.038). Conclusion: The administration of Poly-Phytopharm increases ACT score, decreases blood eosinophil levels and increases serum miR-126 relative expressions in not fully controlled stable asthma patients. Abstract

Background: The relationship between diabetes mellitus (DM) and tuberculosis (TB) is a significant health problem and more prominent in developing countries where TB is endemic and DM prevalence increases. The prevalence of pulmonary TB increases with increasing prevalence of DM. Uncontrolled diabetic patients with high hemoglobin (HbA1c) levels cause TB to become more severe and are associated with higher mortality and also have a significant effect on radiological manifestations of pulmonary TB. Methods: The was a case-control study conducted between January to December 2016 at RSUP H Adam Malik, Medan to 43 with TB-DM patients and 41 TB-without DM patients was studied from. Radiological examination with chest radiograph was done in both groups of samples. The HbA1c levels was examined in the TB with DM group.
