Abstract

Background Differential diagnosis of thyroid nodules can be problematic for pathologists and clinicians in routine examination. Especially, in differentiating hypercellular, hyperplastic nodule from follicular adenoma, as well as follicular adenoma from follicular carcinoma, because evidence of capsular and vascular invasion is necessary. Thus, ancilliary diagnostic marker may be useful along with histopathological examination of the nodules thyroid. Methods An analytic observational study with cross sectional approach was carried out. Study samples were patients with hyperplastic nodule, follicular adenoma and follicular carcinoma which were diagnosed histopathologically at the Laboratory of Anatomical Pathology of Dr. Soetomo Hospital during 2009-2011. Immunohistochemical examination was performed by using polyclonal antibody against MMP-9, a protein which has the ability to degrade extracellular matrix. Abstract

Background Gastric carcinoma is the most common malignant tumor in upper gastrointestinal. According to epidemiologic data, gastric adenocarcinoma intestinal type and diffuse type showed different carcinogenesis. Recently, targeting therapy for adenocarcinoma has established. The aim of the study to know the relationship between HER-2 expression and histopathologic type according to Lauren’s classification and grading of gastric adenocarcinoma. Methods This study has done in Department of Anatomical Pathology Faculty of Medicine University of Indonesia/Cipto Mangunkusumo National Hospital, Jakarta. HER-2 expression is analyzed to see the relationship with histopathologic type and differentiation of gastric adenocarcinoma. The specimens were from resection and biopsy (2007-2011). Histopathologic type evaluated with Hematoxylin Meyer’s staining. HER-2 expression evaluated with immunostaining with HER-2 antibody. Results We found 55 cases from 2007 until 2011 (76.4% men and 23.6% women). The mean age is 50.55 years (the range of age 29-73 years). HER-2 expression 3+,2+ and 1+ are 14.5%, 34.5%, and 25.5% sampling. 25% sample did not show HER-2 expression. We found overexpression HER-2 (3+) in 8 cases (14.5%) of adenocarcinoma gastric intestinal type. Conclusion There is a significant relation between HER-2 expression with histopathologic type (p=0.021). There is no significant relation between HER-2 with grading (p=0.253). Abstract

Background HER-2/neu is second family of epidermal growth factor receptor (EGFR). Amplification of HER-2/neu gene presented in some carcinoma, especially breast carcinoma. Due to increasing incidency of ovarian carcinoma and its poor prognosis because of chemoresistancy, a study on HER-2/neu expression on ovarian carcinoma is conducted. Methods An observasional analytic design with cross sectional approach. Population and samples were paraffin blocks of many type ovarian carcinoma from archives in the Laboratory of Anatomical Pathology of Dr. Soetomo General Hospital, during January-December 2012. Fourty seven samples were obtained and stained with HER-2/neu antibody. HER-2/neu expression assessed by semiquantitative gradation of intensity. Difference of HER-2/neu expression in many type ovarian carcinoma was analyzed with Chi-Square test by probabilitas exact . Results There were 47 cases with mean age 48.51 years (range 19-68 years). Fourteen cases (29.8%) showed HER-2/neu overexpression (score 3+), Statistical analysis showed significant difference of HER-2/neu expression in ovarian carcinoma (p=0.000; coefficient contingency = 0.606). It was showed musinous and clear cell type had more overexpression of HER-2/neu. Conclusion There was HER-2/neu overexpression in ovarian carcinoma especially in mucinous and clear cell type. Abstract

In solid tumor, conventional cytogenetic method usualy fails to detect chromosomal aberation due to failure in obtaining good quality metaphases. Therefore, special technique is required to get the karyotype of solid tumors. Whole chromosome painting (WCP) is a method to paint the chromosomes using Fluorescence In Situ Hybridization (FISH) technique. Application of WCP, both using one and multiple colors (mFISH or SKY) or their modifications can be used to identify chromosomal aberrations, which are valuable in helping to attain a diagnosis, to determine the prognosis and management, and to predict the response to therapy. Abstract

Background Lymph nodes metastases is the important major prognostic factor in predicting breast cancer mortality. HER-2/neu overexpression will make worse prognosis. MMP-9 is related to tumor invasion and metastases by their capacity to degrade basement membrane. Assessing HER-2/neu and MMP-9 expression before metastates occur is expected to be useful for prognostic and therapeutic assessment that can prevent patient deaths. Methods A retrospective study with cross sectional analytic approach was done. Assessment was performed on paraffin blocks of breast cancer patients who were diagnosed in Dr. Soetomo Hospitals during January 2011-December 2012 and met the criteria. MMP-9 expression was analyzed imunohistochemically. The relation between MMP-9, HER-2/neu, and lymph node metastases was investigated by Chi Square (2) tests. Results There were 26 out of 40 breast cancer cases with lymph nodes metastases, 22 of 26 cases with MMP-9 overexpression (2+ and 3+), and only 10 cases with HER-2/neu overexpression (3+). MMP-9 overexpression was 12 of 17 cases with positive HER-2/neu. There is statistically significant correlation between MMP-9 expression and lymph nodes metastases (p=0.011); whereas there is no statistical significant correlation between expression of HER-2/neu and MMP-9 (p=0.583) and between HER-2/neu expression and lymph nodes metastases (p=0.435). Conclusion MMP-9 is signifantly correlated with lymph nodes metastases. No significant relationship was found between HER-2/neu, MMP-9 and lymph nodes metastasis. Abstract

Background Colorectal carcinoma is one of the world's fourth most malignancies. Frequency of colorectal carcinoma malignancy most likely to occur in the gastrointestinal tract and causes the death of close to 15% of all cancers. Heterogeneous of the etiology is associated with many risk factors and molecular changes. Epidermal growth factor receptor is a 170 kDa transmembrane glycoprotein which contribute to carcinogenesis, prognostic impact and specific therapeutic targets. The aims this study to determine the relationship between EGFR expression and the clinicohistopathology characteristics in colorectal carcinoma. Methods This study is an observational study with cross sectional approach. The samples were specimens of the 57 samples that had been diagnosed as colorectal carcinoma from January 2010 to August 2012. Samples from histopathological preparations are reviewed and conducted outward EGFR immunohistochemistry with primary antibodies for the primary tumor. Results EGFR expression to histopathological characteristics are as follows pT3 of 45.6%, positive lymph node status was 52.6% and poorly differentiated histological degree of malignancy of 50.9%. Statistical calculations showed no significant correlation between EGFR expression and histopathologic characteristics of the pT in the TNM system (p=0.037), lymph node status (p=0.000), histological grading of malignancy (p=0.001), and high expression (61.4%). Conclusion EGFR expression affects the pT in TNM system, lymph node staging and histological grading of malignancy. Abstract

Background Endometrial cancer is the most prevalent gynecological cancer in the Western World. In Dr. Soetomo Hospital, endometrial carcinoma is the second commonest cancer affecting women. HER-2/neu is a receptor on the cell surface which activate cellular proliferation. Ki-67 protein is present in the nucleus of cells that are actively undergoing proliferation. The role of HER-2/neu and Ki-67 overexpression in carcinogenesis of endometrial carcinoma is not clear. This study is to determine the difference of HER-2/neu and Ki-67 expression in normal endometrium, simple hyperplasia without atypia, and endometrial carcinoma endometrioid. Methods This study used cross sectional design. Samples were retrieved from Pathology archives at Dr. Soetomo Hospital during January 2011-December 2011. Normal endometrium in proliferation phase, simple hyperplasia without atypia, and endometrial carcinoma endometrioid were collected and stained with immunohistochemistry using HER-2/neu and Ki-67. Statistical analysis with Kruskal-Wallis and Mann Whitney were performed. Result There were 9 samples with glandular proliferation, 18 samples with simple hyperplasia without atypia, and 18 samples with endometrial carcinoma endometrioid type. Statistical analysis showed no significant difference of HER-2/neu expression in proliferation phase, simple hyperplasia without atypia, and endometrial carcinoma endometrioid (p=0.054) but Ki-67 expression had significant difference in proliferation phase, simple hyperplasia without atypia, and endometrial carcinoma endometrioid (p=0.001). Conclusion There was not significant difference in HER-2/neu expression but significant difference in Ki-67 expression on glandular proliferation, simple hyperplasia without atypia, and endometrial carcinoma endometrioid. Abstract

Background Breast cancer is the most common malignancy and related death of female worldwide. The angiogenesis has an important role in the invasive ductal mammary carcinoma carcinogenesis, and vascular endothelial growth factor (VEGF)is a primary angiogenic factor. The aim of this study is to determine the relationship between the VEGF expression and clinicopatologic characteristics of the invasive ductal mammary carcinoma. Methods It was a cross-sectional study. Fifty-five parafin block samples of mastectomy tissue diagnosed as invasive mammary ductal carcinoma were obtained from the archieves of the Anatomical Pathology Department of RSMH, was stained by immunohistochemical method with anti-VEGF antibody. The result was analyzed in relation to age, tumor size, lymph node status, and histological grade. Results The positivity of VEGF expression was more common in the ≤49 years age of group, in the tumor of ≥9,6 cm size group, in the positive lymph node status, and in the high histological grade. There was a significant correlation between the VEGF expression and the category of lymph node status (p=0.047), as well as histological grade (p=0.020). There was unsignificant correlation between VEGF expression and the age category (p=0.781), as well as tumor size category (p=0.700). Conclusion There was correlation between VEGF expression and lymph node status and histological grade in invasive ductal mammary carcinoma.
